In this video I embarrass myself, yet again, by taking one of the harder examinations throughout medical training - The United States Medical Licensing Exam - Step 1 -- aka the USMLE Step 1, aka Step 1. This exam is full of nuance information and subtle things ranging from embryology, biology, physics, anatomy and physiology, medicine, pharmacology, microbiology, etc etc. There is a ton of basic science on this test...and to be frank...I haven't thought or studied this stuff in a LONG time.
The best part about today's video is that Andriana and I SWAPPED board examinations, so hopefully she gets to feel some of my pain! On today's video, I will be taking PANRE (Physician Assistant Boards) questions and Andriana will be taking USMLE Step 1 questions!
So hopefully, some of you all who are currently in pre-clinical medical school can get a laugh at this video. We had a ton of fun making it, so hope you enjoy!

the USMLE questions shown here are FAR too easy compared to actual questions. Thats why almost all the quesitons shown, over 90% got correct.

You all are tripping. Those questions are cake and gimmes compared to the real USMLE. He practically let her win. He got into a radiology residency, you need a really high step score. If he gave her real questions she would have for sure gotten a 0%

Love your videos as a resident but you really should be speaking up regarding your wife's claim that she can do everything a physician can do as a PA without supervision. This is irresponsible not only for devaluation of a medical degree but the entire profession of being a physician as a whole. The amount of basic sciences training, clinical medicine training, residency training, and subspecialty fellowship training physicians go through can't be compared with a PA. We need more physicians with a large social media following to speak up on #scopecreep from NPs, PAs, and CRNAs and publicly supporting the Physicians for Patient Protection (PPP). Propagating the idea that a PA can or is qualified do everything a physician can without supervision is simply irresponsible and harmful to patients in the long-run.

Lol definitely not “propagating the idea” that they are equal. Medicine is a team sport...physicians need PAs, NPs, and CRNAs and if you are not an egomaniac, they can make your life exponentially better. But you are prob just a premed or med student that has never stepped foot in the hospital...or worked as part of a healthcare team.

YOOOWWW Literally learnt her last question yesterday in my neuroscience class. Broca's motor speech area problem would present with a difficulty in speech (motor aphasia) but here the patient is showing sign's of receptive aphasia thus Wernicke's area. wow this is so cool

I know this is an older video, but I love how it demonstrates the value of a doctor/PA team and how it leads to the best outcomes for patients. The way you and Andriana mull over symptoms and diagnosis, catching each other's oversights or false conclusions instead of competing (despite it being a "versus" video) is so cool. As an aspiring PA this is definitely they type of content I'd love to see more of and the type of work I just can't wait to do.

Just took my PANCE the end of September (and may have a family medicine job soon :p ) The reason it is supportive measures is because in non-clinical, book world.. clear rhinorrhea is viral whereas purulent rhinorrhea is bacterial. I honestly feel like that's the main clue. In clinical practice of course, that isn't always the case. The other reason pointing to viral is it has only been two days. Usually start with supportive and then if it doesn't get better in 7-10 days (unless you know its bacterial), then you would assume bacterial and prescribe amox. Haha yes as soon as central umbilication = molluscum :p easy key word Wernicke's word salad, thats how I always think of Wernicke's vs Broca Also.. imaging (all types) and ortho are my two downfalls. I did know the one about duodenal atresia because I could basically guess it from the answers without the x-ray. The pulmonary.. got that wrong as it was hard to see. First I thought there were broken ribs showing so I was thinking flail chest, but too far away for me to look accurately.

In addition to the clear rhinorrhea, I would also think that you would not prescribe any antibiotic until first getting cultures sent to the lab and since that was not an answer choice then supportive measures would have to be the next best answer

Clear, green or yellow can be viral. The color is from stasis and release of immune proteins like myeloperoxidase in the mucus. (12 yr IM PA)

99% of young child respiratory infections are viral. the Fever, saturations, and rales have nothing to do with differentiating a viral vs bacterial illness. Board certified Pediatrician. IF a 4 year old had bacterial pneumonia they'd be sick enough to admit directly to hospital. The active and eating well is a give away that its viral.
