People are not the only ones that need rehab. If you love your dogs as much as I do, then you might need to give them dog therapy one day. One of the most common dog injuries involves the cranial cruciate ligament (CCL). This is similar to a human ACL. Make sure to ask your vet if it is okay to start rehab on your dog because just like humans, there are many different types of surgeries with different rehab levels.
The first thing you want to do is massage and warm up the muscles in the leg because they can't run around to warm it up themselves. Do this for about 3 minutes. Once it is warmed up, you can start distally (the furthest away from the body). The furthest joint away is like the ankle joint. Hold above the joint and below the joint to stabilize it. Gently push forward and backwards at the distal (furthest) end about 10 times. Then move to the knee joint. This is where the surgery is most of the time, so be gentle but firm. Again hold above the joint and below the joint to stabilize it, and make the movement at the distal end 10 times. If they cry out, you might be pushing too hard.
If they have a scar, you will also want to do a scar massage after it has healed and there is no more scabbing. Just roll the skin around between your fingers for a few minutes. Then you want to move to the hip joint. Again hold above the joint and below the joint to stabilize it, and make the movement at the distal end 10 times. Finally, rotate the hip in a half circle 10 times. Remember, this is just like in humans, this might be uncomfortable, but it shouldn't hurt!
You might need to help your dog put more weight through the leg. After a surgery or injury, they might not want to put all their weight on the injured leg, and we can't tell them they need to do it to get stronger. The best way to do this is stand over them and lift up their front paws forcing them to stand on their hind legs. Make sure you don't injure your back doing this. Use proper lifting techniques! If you have a heavy dog, you can try placing their front paws on a couch while they are standing on their hind paws.

You're welcome. One of my pups, Kali, had her surgery last year. I was a nervous wreck too! She did great though. No need to push the healing, just helping with gentle motion once your vet says it's okay, helps encourage them to use the leg. Dogs are pretty tough :-) Good luck to Grace and you!!

Thanks. My Tibetan Terrier tore her knee ligament a couple of months ago. She doesn't like putting weight on it especially after she's laid down for a bit. This will really help.

You're welcome. As long as you are doing gentle pain free motions, they should be okay. Your vet should give you a timeline of how to progress the intensity depending on what specific surgery was done.

Curious how your dog did after having TPLO surgery? any issues arise later in life? do you feel it enhanced quality of life for the remainder of their life after the surgery? How do you feel about the risk of complications with this surgery?

Thanks for the demonstration video! Our Golden Retriever just got surgery for his cruciate ligament two days ago but the instructions for the post-op range of motion exercises were in writing with no demonstration. This helps a lot!

CCL stands for cranial cruciate ligament, not caudal. It's named based on where it attaches to the shin bone. Cranial (same as anterior) is toward the head, caudal (same as posterior) is toward the rear.

Great dogs. Unfortunately, my dog is no longer with us but she had this exact problem and needed surgery. My main problem was trying to keep her still while it healed, she was amazing.

Hi, my young pup had surgery on her cruciate about 5 months ago, it was an acute injury, things were going well, but then after about 3 weeks she tore open her crate, rampaged around the house when I was at work and tore her patella tendon in the sane knee. She then had a second surgery on that. She is still limping and it’s been almost 7 months since the initial injury. My vet thinks it may be scar tissue in the knee causing the pain, we do not want to do another surgery, so I am doing exercises to try and increase her range of movement and hopefully break the scar tissue up. Will these exercises help with that?

Hey Doctor Jo, I'm a certified VT at a surgical referral practice and I just wanted to correct something in your video. CCL stands for Cranial Cruciate Ligament in animals, ACL refers to Anterior Cruciate Ligament in humans. The ligaments are the same but the terms are used to designate the same planes in different species. We refer to those planes as Anterior/Posterior in humans and Cranial/Caudal in animals. There is also a Caudal Cruciate Ligament associated with the stifle joint, but it's initials are CrCl in dogs. The CrCl can also rupture, but it is not very common. Just wanted to clear that up.

i have a 175 lb dane who just had his 1st knee done - he has torn both legs acl AND miniscus. it is week 3, and he is still having a hard time with getting up and putting weight. due to his other leg being bad, he is a rescue, and i cant do a lot of these, since he is so big. any other suggestions?? he is using his front leg and back muscles to even help him sit... he just had a chiro adjustment to try to help as well..... thank you.

I'm curious about CCL injuries. My boy had a lateral suture surgery to repair his year about six months ago. He just started limping suddenly last night on that same leg, seemingly out of nowhere. Is this just a fluke or could something have happened?

Hi Dr. Jo, Our 9 year old lab mix has a CCL injury and we wanted to try going the no surgery route first. We are using a brace right now, just started using it and so too soon to determine if it is helping. My question is can we do these exercises with him even though he has not had surgery. Thank you

Thanks for showing this. Our dog, Oreo, has a CCL tear / Ligament injury on back left around 5-15-2020. We are going to try conservative treatment and no TPLO or rope surgery yet. We are taping, limiting movement, supplements, and lots of rest. I hope that we can heal the CCL by ourselves. The tear must be minor, because she is not showing any real indications that it hurts or even exists. But, Vet xrayed and said that it is POSSIBLE is a CCL tear / ligament injury. So, I hope that it is a small one. I had knee issue before, and I rested for good while. Now, doesn't bother me at all. So, I hope that I can do the same. Maybe, if you don't have videos on conservative treatments, maybe you could create some. Thanks, Poe Lemic

My Golden Retriever had CCL3 weeks ago. Can anyone tell me if she can take a short bath yet? (in Shower) sutures are still visible but only near one end. She's itching a lot in summer heat. Causing hot spots and smelliness. one week until vet check. Thankyou!

My dogs legs and ankles crack when i do this. Doesn’t hurt her. She will do it naturally too when she gets up after lying down for a long time.

My little guy had this surgery about sixth months ago and he's just now starting to get active again. This has been the toughest six months ever for both of us. I'm very nervous about integrating him back into off leash play. Today I had him in the yard and he over stretched his collateral ligament on opposite side by standing on his back legs with paws against the base of a tree and putting a little weight on back legs. (Trying to gradually build back strength) Do you think this is due to lack of activity? I rushed him up to the vet because I was terrified and thought he tore his other cruciate and thankfully they told me that he overstretched his collateral ligament but didn't tear it. Im thinking of signing him up for physical therapy because I want to make sure he has plenty of strength and stability in his legs to prevent this from happening again. Any advice or input would be much appreciated.

The exercise in the end of the video where you're holding your dog up is essentially what I tried doing with my dog but he had his front paws on a tree and back legs extended when he over stretched his collateral. I thought I was helping him since he's sixth months post acl. Is it possible he just over extended his leg and has to regain his range of motion over time?

Not "caudal because it's the back leg", i t'scranial because it attaches to the tibia on the cranial (anterior in humans) side.

Ice can be used as long as the vet okays it (usually they give instructions, and icing is included). It would be just like humans, so no direct ice, use a towel for a barrier, or an icepack that has a covering, and it can be place over the surgery site.

Dr. Jo, my BC just had CCL surgery on Nov 12, 2020. This is the second time, her left was done 3 yrs ago and now the right one last week. For the stand on rear legs, can she do that the second week after surgery. She is pretty much hopping on the left when she goes out to potty and occasionally will toe touch the ground with the right. I am so afraid I will hurt her by making her stand on her rear legs.

No, that's pretty early for something like that. It really depends on the specific surgery done, so that's really a better question for the vet, but in general, it's best not to force them to put weight on it.

The incision should be completely healed for the massaging (so no open areas or scabbing). As far as the stretching, your vet should let you know, but just base it on how your pup feels. If they are pulling away with pain, they might not quite be ready, but gentle movement should be started pretty quickly.

Hi, 2 weeks ago my dog started limping on his hind leg after some very acrobatic movements at the park. I’m pretty sure he tore his ccl but don’t know if it’s a partial or complete tear. He is only about 15lbs so I want to avoid surgery if possible. I’m not sure how soon I can start using your exercises in the video. Should I wait a few weeks to start them so scar tissue can develop? I also want to know if you had any ideas on hydrotherapy. I would be using a small kiddie pool in my backyard for that. Any advice would be greatly appreciated. Thank you!

You can start them right away. It will help the scar tissue lay down correctly to keep proper motion. I would have your vet check it out just in case. They can do a simple test (in humans, it's called a Lachman's test), with their hands (no expensive tests needed). Just make sure your dog is not in pain while you do them. I think hydrotherapy is a great idea! It will take gravity out of the exercises so they won't be as painful. Good luck, I hope he feels better soon!

The surgeon should give you a hand out of when to start. Usually gentle range of motion can be started right away as long as your dog will let you.

Thank you for the video! Ours yellow lab would not bare weight on his leg after surgery. We were not sure if his weight was a factor or just playing the sympathy card. We will try this. Do you think he can safely go for half a mile walk every other day? I thought the distance may force him to use the leg to bare weight. He's a robust 4 year old yellow lab. During our training days, he walked up to 4 miles every other day. Approximately about a year ago for a 2 year period.

I think it is okay for him to start walking again, but you might want to start even shorter like a fourth of a mile to see how he does. Most of the time they don't put weight on the leg because they don't know they are suppose to. It hurts, so they don't do it. But once they start, they usually do pretty well! Good luck! My 4 month old puppy fractured his elbow and went through surgery to screw it back together. He just came back to us yesterday. Is it too soon to start these rehab movements since his injury is so fresh out of surgery? I want to make sure we set him up for a speedy healthy recovery! Thanks!

Thank you Dr. Jo. My Bandit just had lateral suture surgery 8 days ago. We are massaging and attempting some range of motion. We just started 10 minute slow walks, but he only puts his leg down about 10% of the time. Is that OK? What should my expectations be?

You're welcome. That's pretty normal. Dogs usually heal extremely well at their own pace, so I would just let him go at his own pace, and gently help him along the way. Good luck!

Your vet should have or should give you some guidelines for when to start. But in general, early on, usually just short walks and gentle range of motion if your dog will let you.

Hi I have a question, I was doing range of motion on my corgi and when I extended his leg I felt a sort of popping noise. Should I be concerned? Did I just make it worse? I hate having the thought that I could’ve hurt him. He had his surgery about 7/8 weeks ago but doctor said he has slight signs of patellar luxation.

You probably didn't do anything bad that far out, and if he didn't seem like it hurt, it was more likely scar tissue breaking up, or maybe the tendon rolling over a bony area.

Hi there Dr.Jo, i am desperate to find out what to do to prevent the other leg from rupturing as everywhere ive read and in conjunction with what my vet tells me, almost always they will have to come in within a year because the other leg will tear, and will have to go through all of it again..at 9 years old and this being her first incident, i dont want to put her through the trauma a year later ..spending the last bit of her time in surgery and recovery. She lost 4 lbs before her accident (high jump) and regained so much youth, energy level on HIGH and now this ..im heart broken for her. Are they correct ? No matter what i do for her , will the other knee tear? She also has luxating patellas in both knees.....

Not necessarily. The key is to make sure she stays strong and a healthy weight so she doesn't compensate and overuse the good side.

They would be similar, and you can discuss non-surgery options with your vet. Depending on how active your dog is, it is possible to treat it conservatively.

I have been doing some rehab exercises with my Boxer put she is still very guarded when flexing her knees (as in the type of range of motion that would be needed to do a proper sit.) Sorry to hear that. I think a lot of it depends on how active they are and their age. Depending on where you live, there are some vets who have physical therapists to help (or at least know of some you could go to). Some vet/therapists can do alternative treatments, just like with humans, so acupuncture, electrical stimulation, ultrasounds, etc. You just have to search and ask around for someone who does it. Also, it might be hard to find, but a pool or lake (when it's warm) for aquatic therapy/swimming is very helpful too. The key is to be gentle and encourage the motion, but not force it.

I'm from a small town, which means I cannot find a professional for my dog. Do you know of some videos that can guide me as to what to do with his hind legs?

+Incog Nito You're welcome! Do you have access to a pool or water? Water therapy for dogs is great to get their legs moving, and usually (if they like the water) they have fun doing it.

Thank you again for the quick response. Since mine is a small town, there are no pools for dogs and I certainly have no access to a private one. Also, since he's barely able to stand (and he's quite heavy), hydro-therapy at this stage will not be a viable idea. I would be very grateful if you could point me towards (in your opinion) good videos for dogs with barely functional hind legs.

+Incog Nito Unfortunately, there's not much out there. If he is barely able to stand, the water would be a great place. The buoyancy takes gravity out of it. Imagine when you are standing in water, and you feel lighter and it's easier to move. Do you think you would be able to get him in a bathtub? I still think it would be helpful. As far as other videos, I really don't know of any. I think the important part now is to try to keep his legs moving even if you have to do it for him.

So sorry to hear. It really depends on what kind of surgery your dog had (there are different procedures just like humans). To be on the safe side, I would call and ask the vet who did the operation. Good luck.

I'm seeing mixed things about torn ccl/acls dont know to do the surgery or not seems like alot of people saying it will heal if u keep the calm and less active my vet told me it dont heal and she needs surgery the surgery is alot of money and going to be hard on me but I'm confused on what to do

If it's completely torn, unfortunately it will not heal. Just like in humans, ligaments cannot heal on their own. It does depend on how active your dog is and how old they are though. Some might do okay with resting and physical therapy, but they will not be able to run and jump as they did before. Some vets will do the surgery for a much cheaper price, especially in a more rural areas. So I would suggest calling different vets within your area or search online. It would probably be cheaper even if you had to drive a few hours away to get a better deal. Hope that helps.

My little dog tore her acl/ccl and I was doing therapy with her and keeping her from running, no jumping. I gated my upstairs and she could jump on any furniture anymore, strong rules were set too because she needed to lose 3lbs. That's like us trying to lose 30lbs without execersise. She was getting stronger and started putting pressure on it and soon walking on it again. I'm giving her vita treats 15 in 1 multivitamin for joints as well. Unfortunately almost 3 months later, she completely torn it and it was a huge difference from a tore & completely torn acl. I begin working with her all over just so her leg would come down before surgery. This Wednesday its scheduled, she lost only 1lb I'm just worried about the other legs giving out. 😔

Sorry to hear. They heal surprisingly well after surgery. Just make sure you are doing everything the vet recommends, and she will hopefully heal quickly.

Don't forget if your dog is on anti-inflammatories or pain medication, giving them the anti-inflammatory & pain meds will make this less painful.

Thanks for the vid! I have a 90lbs pitbull very active and he just had the TTA surgery on his left knee, the staples are still in but should we start the range of motion now? its been 3 days now and hes still not really putting any weight on it. thank you!

You're welcome. If he's willing to tolerate some gentle motion now, it should be fine, but definitely don't push it. He will let you know if he isn't ready for it.

Thanks so much for this! We are doing recovery from a CCL surgery. He just came home today and our vet said to get started ASAP on the ROM stuff an "balance exercises". They didn't say what the balance exercises are but I believe they're weight bearing like what you did with having him stand on the hind legs. Anyways, just wanted to say thanks. Today is the first day after the surgery. He let me do the ankle ROM but the knee ROM seemed pretty tender. I got two stretches in and he was over it. I didn't want to push it any further. One question, do you think a little 5-10 minute of heat before massage before the exercises would be good? Or should that only be done after the stitches are out?

+Sad Chihuahua You're welcome! I hope he heals quickly! I wouldn't do heat just yet because it might increase some of the swelling/inflammation. I would wait until the stitches come out, and just very light gentle massage to help promote the healing, but not hard enough to irritate the incision site. Also, when it gets warmer if you have a place where he can get in the water, aquatic therapy/swimming is great for them! Good luck!

+AskDoctorJo My mom has a pool that we will be visiting as soon as we can and we also are going to see what we can do in the bathtub. We will probably start the bathtub as soon as the stitches are out. His legs are short so its not hard to make a tub deep enough to make him swim or to create buoyancy. If you have any suggestions for rehab in a tub I am more than open to hear some suggestions.

+Lindsay Branscombe That's great! I would wait until there isn't any scabbing left even if the stitches are already out to make sure it doesn't open back up when soaking in the water. Using a treat to get him to swim around is probably the best way to do it. Also, sometimes just holding them up lightly underneath their belly triggers a swimming motion/kicking response. You could also do the stretching in the water where it might be more comfortable for him too. I hope he feels better soon!!

+AskDoctorJo So we started his PT and it seems to be going very well. He gets his stitches out tomorrow and we will get to meet with the vet to make sure we are doing everything correctly. I have to say though, there seem to be VERY few resources and examples out there showing how to do this type of stuff on a small dog. It is much harder trying to support the upper leg on a tiny little leg. Here's a video of one of his first PT sessions. He was more open to PT at the beginning. +Lindsay Branscombe He did look pretty calm in that video! You did a great job! I thought you had peanut butter on your arm the way he was so content with just licking your arm. I hope his follow up goes well, and I hope he can get in the water to do some "aquatic therapy!" He might be more willing to do that :-) Please keep me posted on how he's doing!!
