The Simpsons aren't the only ones who can predict the future. The puppeteer for Elmo came under fire for sexual assault allegations in 2014. He was cleared of all charges. But his career didn't survive. This particular episode of south park predated the charges by at least a couple years.

In a later episode it's explained his name is not Token. It's Tolkien. Because he was named after J.R.R. Tolkien who wrote the Hobbit and Lord of the Rings. As usual, it was pretty funny how they revealed that. You need to watch "Butter's bottom Bitch." That episode is crazy.
