The main cause of gum disease is the presence of plaque on our teeth. This plaque is consumed by bacteria in our mouth, which turn it into harmful chemicals. This leads to inflammation of the gums, bleeding gums, destruction of the underlying tissues, and eventually bone loss.
Therefore, our goal is to remove this plaque!

Thank you for this, not everyone can afford to go to the dentist. I'm in the UK and we used to have a good NHS but nowadays not many dentists are accepting NHS patients. The British are known for having bad teeth and they are poorly educated in this subject. I found out I had Stage 3 gum disease in January and it must've been progressing for years, but I wasn't told. Many people are not told by their dentists that they have gum disease, I don't know why, maybe because the dentist knows they cannot afford the treatments, or on the other hand they are waiting for it to progress so they can make more money. I am trying to find ways of treating it at home, particularly as I suffer from Agoraphobia !

I had severe gum disease and a deep pocket on one of my teeth, went to the dentist had a deep clean and trying to save that one tooth. Apart from brushing i also massage my gums once every day with turmeric. I don't know why it helps but believe me i have seen amazing results and have saved my tooth.

Thank you for this video, Dr. Paul. It's the first video of yours that I've seen. My gums are in much worse condition after receiving Chemotherapy for a few months earlier this year (I stopped 1 month ago, in early may of this year). I was getting it from Mid-January-early May 2022. I was astounded to see on the Cancer Council website that some people might experience such gum aggravation/soreness that maybe they shouldn't even brush at all, using only mouthwashes. Well, my gums became so sore that I could barely even eat & had to have a lot of soups, even eating a soft banana hurt them. So, I thought that maybe I was one of those cases who shouldn't brush (it was incredibly counter intuitive to my thinking, but the soreness really was THAT bad). Brushing only added to the pain & I worried that my gums had become so soft that brushing might wear them away to a really bad degree. So, there were times when I didn't brush for 2 days, until I couldn't stand it anymore (I couldn't stand the dirtiness of them). Now that I've stopped Chemo, I am back to brushing every day but I see a massive gum recession on the front of my bottom teeth (the 2 front teeth). My God, the bleeding was horrendous a few nights ago but it's getting less & less with each day that passes, Thank God. But the recession is something that doesn't correct itself, as you know - not from brushing & flossing regularly. I stumbled across a video talking about a technique called "PinHole" technique which helps enormously to bring the gums back to being less recessed. Have you heard of this technique? I would really love to get it but there are only 2 dentists in Australia who do it. 1 is in Melbourne (my home city), the other is in Sydney. Do you know about it? If not, would you be interested in learning it? And... which city are you in? Thanks so much for your channel :)

Hi there! Sorry to hear about your situation! The pinhole technique is one of many that can replace lost gum due to recession - there are others that are as effective and have been around for longer (not to say one is better than the other). As a general dentist this isn't a field that I have learnt (yet). After assessing your teeth and gums, I would send you to a gum specialist.

In addition to the steps mentioned, I suggest that you also switch to using a soft-bristled toothbrush. In addition to removing plaque, this can also help prevent gum recession as per kriss ai and my dentist. 😉

Thanks for explaining about the mouthwash use for those of us with gum disease, and giving options available in Australia. Any recommendations for super sensitive teeth?

You're welcome. For sensitive teeth: try a sensitive toothpaste. Don't rinse after brushing, to help the paste be more effective. After the night time brush, smear some of the paste on the teeth most affected. If that doesn't work and you want it fixed, you may have to go to the dentist for them to seal it off.

There are many crooked dentists out there. They purposely crack your teeth to get more work. Beware people! My dentist cracked my tooth then extracted it. They ruin teeth without you knowing. Just like bad mechanics. Not saying this dentist is bad. He seems to be a nice guy. But... always be cautious!

Sounds good! Recession can also be caused by over brushing with your toothbrush so be careful about that too

Not always. In those cases it could be tooth decay (solution - fix tooth decay) or it could be from gut problems (solution - see a doctor about it). In cases that it does cause bad breath, you need a deep cleaning with a dentist and then good at me care

Yes it should be fine to go, very very low transmission of covid in a dental practice. Better to get your gums healthy 👍

Sensitivity is related to the tooth being sensitive, not really the gum. Sometimes it is caused by recession of the gum. Sensitive toothpaste first, but if no success, then sometimes sealants can be done on the exposed surface Gum infection needs deep cleaning from a dentist then good at home care.

Hi there sorry to hear about your situation but I am glad that you liked the video. I don't think that the driver for not telling patients is to make more money - most dentist don't like doing the procedures to fix gum disease tbh. I think the crux of the problem is that not enough dentist check thoroughly for gum disease and they don't have time to discuss the findings and the implications

If you have it, you really need to be going at least twice a year for a professional cleaning. You can't treat it at home, you can only help slow it down. The twice a year cleaning will help you stop it

💗I stopped smoking( ❄️ 🦃) but I see dry blood in the morning red swollen gums I haven't used listerine I don't want dry mouth issues like my elderly mom deals with I will try apple cider vinegar

The dry blood and the red swollen gums means that the gums are inflamed, due to the presence of plaque, calculus and bacteria. Do you have regular teeth cleanings at the dentist?

I had a filling in a molar and after 6months I had a pus that doesn't want to go after the doctor gave antibiotics. I don't won't to lose my tooth due to depression. Thanks for the video.

It sounds like the nerve in the tooth is dead so the tooth either needs to be removed or you have root canal therapy. The antibiotics won't remove the infection properly

What % Curasept mouthwash do you recommend?? And how long to use it? So you recommend using the wash how long after brushing? Just once a day?

Hi there, 0.12% or 0.2% are both OK. All of those questions are answered in the video: use for 1min, at least 1hr before or after brushing, 1x/day

Thank you very much for your excellent video sir. Apart from the advice, what I liked about it was your sympathetic approach and voice. You were on the side of the patient which is not always the case with some dentists I feel. I have had a sore gum for many, many months, not helped by the coronavirus restrictions of course. The problem seems better or worse at times though probably it is not helped by my contant prodding to see if it still hurts! I have heard that hydrogen peroxide mouthwashes are preferable to ones containing chlorhexidine so I am a bit confused! Another one here in UK is called Corsodyl but again I have heard it should be used with caution. Meanwhile I am exercising to gain strength for my next ear bashing from my dentist! Hi George, thank you for the comments! Yes hydrogen peroxide does also work well. It has been shown to have good results with gum disease. I mostly recommend it for people with gum issues around wisdom teeth though. Maybe try it? The cordosyl that you mentioned has chlorhexidine, so it should work well, but try and find out if it will result in staining. Hopefully you can get to the dentist soon and they don't ear bash too much 😅

@@DrPaulsDentalWorld Thank you for taking time to reply sir. In fact, at my routine six monthly appointment, I saw a partner of my usual dentist who was undergoing minor surgery. She was a bit more gentle with the ears though she was a little less so with the drill she was using to do a filling! The tooth root/gum affected is towards the back so presumably it's in hiding! Anyway I will splash some Corsodyl on it until I discover the name of a hp type wash. Thank you again. You have made an old (ish!)man happier. Blessings and peace.

Hi George, try and use the Mouthrinse a few times a week. Also put some on your toothbrush in the area that is affected the most. I will also link you a toothbrushing video which you might find helpful. I've never liked the idea of giving my patients ear bashings, I don't think it's the most effective way to make them way for them to want to improve their situation 😅

Hi Cathy, did you have periodontal disease prior to visiting for 10 years? If someone was having their teeth cleaned *effectively and they were doing *effective (technique, duration, frequency) cleaning at home, I would say it is almost impossible to develop serious periodontal disease. If this was the case, I would be looking at seeing if there are any underlying medical concerns that could be contributing.

Hi Cathy, it sounds like you have made a great effort to slow the progression down! If you had already lost some teeth, I imagine the periodontal disease was quite bad, so to not have lost any other teeth is great. Were you ever seeing a periodontal specialist or just the hygienist?

@@DrPaulsDentalWorld I lost my back teeth due to receding gums not peridonal disease. I was never referred to a specialist just the hygenist cleaning my teeth every 3 months for 10 years not sure why my gingivitis progressed to peridonal when I was receiving treatment evert 3 months. On one visit to the hygenist she pierced the gum on my front tooth i jumped out the seat it was very painful and my front tooth was always painful after this happened. About 12 months later I went for my routine clean and she called the dentist in for a second opinion and he said you have peridonal disease your teeth need to come out. Do you think she damaged the gum which aloud bacteria to enter the deep pocket? I was never given an x-ray or referred to peridonal specialist even after complaining about the pain in my front tooth.

Hi Cathy, the pain that you had on the front tooth wouldn't have affected bacteria being able to get in further into the pocket. In regards to the recession and how everything progressed into periodontal disease, it is very hard to comment over the internet. It sounds like you went to great effort to maintain your teeth, which I'm sure increased their life expectancy.

Is chlorohexedine really any better than other antiseptic antibacterial mouthwashes like listerine for inflamed sore gums? If so, why? Also, can you brush after mouthwashing with chlorohexedune to prevent staining?

Yes, miles better. It targets the specific bacteria that cause gum disease. Listerine mouthwashes don't do anything, and I don't recommend them except if you want to feel like fresh breath. They say that they kill bacteria, but that isn't a good thing because the mouth is a complex environment with bacteria that you don't want to disturb (unless it is specific like chlorhexidine). You don't want to brush afterwards because it will reduce the effectiveness of the chlorhexidine (one - because toothpastes inhibit its action, two - you want it to stay around the gums after using). My advice is to get one that doesn't stain.

Dr. Paul, wish you were my dentist! I have horrible dental anxiety. Amongst many things that have scarred me, had a scaling performed once and the EXTREME sensitivity in my upper incisors created so much pain and zaps down my spine, I jerked up out of the chair! I have chills even revisiting the thought. I know this sounds extreme, but it's led to me avoiding the dentist and now I have pretty advanced gum disease. I asked my previous dentist for nitrous or sedation for these procedures (even offered to pay cash if insurance wouldn't cover), I was always told no and feel my anxiety wasn't addressed very compassionately. The dental practice administered nitrous on a regular basis for peds patients (extractions), so I didn't see the issue with requesting it for myself. Please offer your advice or suggestions... how would you proceed with a patient such as myself?

Sorry to hear the situation! You definitely need a dental cleaning and we do have services for anxious patients like you: - happy gas - oral sedation (medications like valium) - and we can make your teeth numb with local anaesthetic If they say no to these, find someone else

It's not the best idea to get implants unless the periodontal disease is completely under control, because an implant can get periodontal disease, often easier than a tooth can.

Normally we clean the teeth thoroughly, and then re assess it at 2-3 months. Often it requires 3 monthly cleaning to improve. How long it takes depends on how bad it is to start with, and how good you are at home cleaning

I'm not sure of a best mouth rinse in the UK. Any that have fluoride will do the job. For gum disease, any that have chlorhexidine work. For wisdom teeth pain, any that have hydrogen peroxide will work

Dentist says I now have periodontal 😞. She said I’ll have to be taken this chloroxedin fluid twice a day for the next 6 weeks. The plaque and tartar was removed just two weeks ago.

i’ve noticed that after i got a cavity out, my gums have been red and bleeding but it stopped as time passed by. i have a cracked tooth and i need to get it fixed but it builds up brown stuff in my tooth and every time i brush my teeth i rinse my mouth out with a lot of water to get it out. but ever since that doctor appointment my gums have gotten whiter and red at the very top. i don’t know what i have. i think i have gingivitis but i don’t know if it’s something more serious.

Hi there, you shouldn't use every day with the chlorhexidine. Normally the teeth start to stain after two weeks, not a few days. How often are you using it through the day? Normally use it for a week, then don't use for a week. That toothpaste should be fine, especially if it has fluoride in it.

Dr. Paul, I have gingivitis. I brush my teeth twice a day. I start off with 1) A rinse with sodium bicarbonate/sodium chloride solution. 2. brush my with gentle strokes from the base of my gum line towards the tooth (upper and lower jaw). 3. rinse repeatedly with the sodium solution in step 1. Step 4. I rinse with 1.5% hydrogen peroxide with mint flavor from colgate. Do you think this should reverse my gum infection? Thank you.

Dr. Paul dental world, am from UK, my gums count is 3mm and I got aggressive gingivitis am told and swallow my roof of my mouth, just had deep scaling and I had tablets bring it down, based on the gum count would this be reversible at this stage? If it is 3mm, then that is just inflamed gums (gingivitis) and not gum disease. This means that after your deep scaling, if you can maintain things with good brushing and flossing at home, and regular cleaning at the dentist, then you won't get any worse 👌

- Completely cured, yes. You just have gingivitis. If you have gum disease (where you have pockets more than 3mm), then this is technically non curable and can only be stopped. Not too much of a concern unless the pockets are 6mm or more though - Coconut oil pulling can help gingivitis, but needs to be done once a day for about 20mins - Mouthrinse isn't actually effective. Just make sure you are flossing first, then brushing correctly afterwards, and cleans every 4-6 months - Don't make the gums bleed. If they are bleeding it means that you have gingivitis (gum inflammation) or you are flossing too hard which is bad for your gums in the long-term

Hi there. Yes, redness around the teeth is a good indicator of gum disease, because your gums are inflamed. It may be the early stages, but it may also be more advanced stages as well. Best thing would be to have the teeth professionally cleaned to remove the hard build up (calculus), and then really good cleaning if the teeth/gums at home.

Hi Joyce, if it is swollen, that is not a good thing so I would suggest getting this checked out if it doesn't go now. It could be an infection, and the infection won't go away by itself.

Hey Dylan, thanks for reaching out. Braces are difficult because plaque collects on the teeth really easily. If the gums are swollen, sounds like you'd really benefit from a thorough clean from your dentist, and then meticulous home care. I would also recommend getting yourself some curasept mouthwash as it kills the bacteria associated with him problems. Let me know if you d save any other questions.

Ok fair enough. Just make sure you're brushing three times a day, and use some curasept in the afternoon (not within an hour of brushing). Pour some on your brush and brush it around the gum that is most swollen.

Impossible to give you a diagnosis and treatment plan over a KZfaq comment, but moving teeth isn't good long term. If they say not to remove them, then maybe you shouldn't. The replacement options are either cheap and not that good (removable denture) or very good looking, comfortable but expensive (all on 4) Bone loss won't recover, but with good at home care and frequent visits hopefully you can improve the gum and bone attachment and they will move less. If they are yellow, you can do teeth whitening?

I just tried to search about why my gum was peeling off. One of my tooth's gum at front was peeling off. It started about a week ago. It may seem like it's a Mild Periodontitis. I remembered my dentist diagnosed me with Gingivitis. But I neglected her warnings. And now, it came to this. I am currently planning to visit a dentist about my condition. Is there hope for me? Can this be cured or is this a life time?

There is definitely help. If you have periodontitis, you have bone loss. The bone won't come back, but your goal is to get re attachment of the gum on to the tooth and to keep it healthy and strong. This happens with the tips I give in this video

If you have gum loss near the frenectomy, then having it removed can help.

@@DrPaulsDentalWorld No I have gum loss cause my frenulum is high and my doc recommend frencetomy so after that will my gums get back to the original position? or maybe the pockets will improve?

It probably won't go back to the original position, but it should improve. It will also help you keep the area clean which can improve the pockets

Peridex caused staining. I was told it was the chlorexidine. The one you said does not cause staining. I’m confused.

have petiodontal disesse(I don't have moving teeth or loose teeth).l'm after deep cleaning and surgery with prf.Now ewrything seems ok. Can I stop the progress? or only what i can do it, is slowing down? I do all 5 step . The pocket depht after surgery is 2-3-4 mm( 6 month control)My periodontolog said I can stop it.I confused now 😢

Hi there! If you have 2-4mm pockets 6mo after that is great. Healthy is 3mm or less so 4mm is not that bad. The periodontist likely wouldn't have said you can't stop it, but they would have said that once you have gum disease it will be there forever. Because once you have lost the bone, the bone will not grow back. The definition of gum disease is a pocket of 4 mm or more. That does not mean you cannot stop it's progression, which is what your goal is now. You want to make sure that the pocket does not get any deeper and that is via the steps I have mentioned in this video. Does that make sense?

Can we ( petiodontal disease patient) consum vitamins, supliments for a helthier gum and teth?What kind of vitamins can we choose? I'd cut all the grain and sugar from my diet after i was diagnosticated with periodontal disease. What are you thinking about keto or paleo diet in periodisease?( Sorry for my english I don't really speak the language 😅, but i undertand almost everything) Have a nice day!

To be honest there isn't hugely strong evidence about diet and gum disease, so there aren't specific vitamins etc to have. The most important thing is removing the plaque and the biofilm (using these tips)

I dont have a dentist but just got an emergency appointment this morning. The dentist said I have an gum infection and the gums are starting to recede.I was given antibiotics. Will they stop it? Also is an infection the same as gum disease? I am so upset I am now flossing and using various otc treatments. Is there a chance I can reverse this? They only started bleeding 2 weeks ago.

Hi there, to be honest it's hard to say based on your description. Antibiotics can help for the time being if there is a gun infection. It won't really stop him disease. Gun disease is different to infection. Receding guns is, however, a sign of gum disease. So maybe there is gum disease and infection?

@@DrPaulsDentalWorld Ah thanks for your reply I am beside myself with worry. I thought gum infection was gum disease. She said I had the start of receding in one tooth. If it is gum disease can it be reversed with extreme otc remedies and excellent hygiene? I really appreciate your help I have been crying all day.

Hi there. Gum disease can't be reversed but it can be stopped. Receding gums are also unlikely to be able to be reversed. Did she mention pocket depths? It might have been some numbers she would've said to her nurse. If yes, the idea is that we reduce the pocket depth

She didn't mention pocket depth or measure anything could a gum infection be stopped ? she isn`t going to see me again for a follow-up. don't have a dentist and can't get one in the UK. It depends what it is for. If it contains chlorhexidine, make sure it is not within 1hr of brushing. If it is just for fresh breath, any time. If it is for fluoride, then you should use it during the day sometime.

My gingivitis is as bad as it looks almost as bad as a small cut but I have tried for four weeks to get rid of it and I tried the 45 degrees and it is still bleeding and it is getting better, but it is still tough to remove

When was the last time you had a clean at the dentist? If you don't do that, the build-up of calculus will never allow you to get rid of the gingivitis. Once you've had the cleaning done, it's a matter of keeping it clean with the tips in this video.
