It’s sadly too common that people that have had a meniscectomy, meniscus repair, or even a meniscus transplant end up having chronic knee pain for months or even years after!
Most of the time, healthcare professionals do not have an explanation for this beyond “scar tissue build up” or “you’re tight and need to stretch more”...
The worst news to get is from the surgeon telling you “well you’re more likely to get knee osteoarthritis since you have less meniscus now.”
But there are ways to help the knee pain if you’ve had a failed meniscus surgery.

I had my torn meniscus surgically repaired. What the doctor didn't tell me is, that when you get this procedure, it's nearly a guarantee that you will need a total knee replacement years later. That's what happened to me, and believe me, a knee replacement is the WORST thing you will ever go through. So try to avoid this procedure if you can with physical therapy, etc.

Oh, I have to add this comment as well. I thought, let me do the bike to get stronger after TKR surgery. I had the EXACT experience you mention (around minute 44 in this video). Now I think I probably strengthened my quads so much (and had no clue I was actively creating a muscle imbalance and putting more pressure on my knee). I actuallyQUIT going to the gym altogether, got depressed that my new knee was hurting so much, gained weight (I’m sure the added weight didn’t help & a lot of the times the food I was eating caused more inflammation). Ugh…so glad I heard about you! Learning a lot. Starting to turn things around.

I wonder why they can't just replace the meniscus with some kind of cushion? Does it have to be living/vascular?

after today's physio session, my physiotherapist mentioned that I have improved in my leg angle, now at 102 degrees. and when i straighten my leg fully, it's now at -2 degrees, an improvement from -5 degrees. but its been a long journey

You are quite WONDERFUL, A TRUE BLESSING. I APPRECIATE your information. I'm going to be 72 this year, and I have both Osteo arthritis and being heavy set, I also have knee issues that have compressed my meniscus in both knees. Walking is difficult and eventually I'm sure I'll have to have surgery, although I've tolerated the psin and staved any radical solutions for 9 years. Love to know is there a procedure on the horizon that will just replace the meniscus so that it's not such an intrusive and alarming surgery? Has Medical Science done ANYTHING with stem cell research that will eventually be cost-effective to the average person?

I have tried rehab, rice and anti inflammatory but my knee is still painful. It has been 9 month. Do you eventually have to give in and have surgery to get some pain relief even though it is setting you up for early osteoarthritis?

What do you know about Stem Cell infection and PRP injection for knee arthritis? 2001 my meniscus folded and had surgery a week later. My PT kicked me out of physical therapy because I was in so much pain ... he said I wasn't cooperating!!! So, I never had PT after surgery. My knee is very painful and swollen ever since surgery. It feels full of fluid. Arthritis, obviously, become worse.

I have osteoarthritis in my hip I started your exercises this morning and I already can tell I don't want a replacement Ive had left one replaced it has done real good I just don't want to go through another surgery I have not been able to walk hardly at all thank you for you videos

Thanks for taking the time to explain and illustrate in depth how we can heal from specific knee joint injuries and surgeries. I wish I had gotten three professional opinions, as you said, before my arthroscopic partial meniscectomy. However, they say I had no choice because the tear was large and causing mechanical issues, and it might have continued to tear further up the horn if left untreated.

I’m 24 years old, Going on 10 months after my meniscus surgery and my pain is way worse than when I had the meniscus tear. My knee strength is worse, pain is worse, all around worse than before I got the surgery. I also been doing PT and it has not been helping. My knee has also been swollen since the surgery and it’s visible swelling. I just got another mri done just waiting on the results.

Hi Dr David, i had a meniscectomy on my left knee, it was a medial tear, i had this surgery over 12 months ago, before the injury i ran x4 per week, now i have pain every minute of the day every day, and the running that i loved so much i have been able to do, i compensate on my right leg while walking and really have to concentrate, i find i get knee pain on that right leg now to, i have been trying your squeeze the butt cheeks while walking but like you said walking a dog isn't the best time to do the glute exercises, i have had another MRI and due on the 04/08/2023 to get my results from my surgeon to see what action to take going forward. i do work out 4 times per week but have to be very careful with lunges/squats.

Hi. I would like to ask about meniscus surgery. I had right knee arhtroscopic partial medial menisectomy and cartilage repair of trochlear on 28 Sep 2022. 7 months have passed and my leg still swell. Though I am on and off crutches, I still have stiffness. I still cannot bend my legs. My physiotherapist said from the notes I also had BMAC. have you encountered patient like me who still have stiffness and not able to do full squats?

Augmentation of ACL and Meniscectomy of my left knee was done two months before. After sitting on a chair I can lift my leg up (full extension) but it lock when I move downwards. I can see something swelling just below patella. Thereafter I can move it downwards. What is the reason of this. That's why while walking I am not able to fold the leg properly. It stuck after almost 10/20 degree in every step.

Do you have any advice about the Popliteus muscle? My mom has been suffering and so many tests and doctors. They keep saying it's the knee and have her a shot, Xrays and MRI. Took her for acupuncture, and it helped, but not all the way.

I had arthroscopic surgery with meniscus repair on birth knee 9 weeks ago. Started PT and water therapy a few weeks ago, and I started having burning pain that woke me up in the night. The surgeon changed my pain medication, which doesn't work due to unfavorable side effects. Any suggestions?

Hi Dr David Ultrasound and x-ray of knee report read from GP, "bulging with the possibility of a meniscus tear". I was too have an Arthrogram and CT Scan done at Radiology this friday 15th March 2023, but cancelled procedure due to the thought of it. My question is : I have CONSTANT pain morning and night and at rest. Pain can be a continuous throb/ache or at times just an ache with no throbbing. Only very occasionally I may have a quick sharpe pain. The knee involved is my right knee - (pain on inside of knee), not outer side. Does this sound like a meniscus tear ??? Is constant pain (and at rest), something to be very concerned about ??. Does "bulging with the possibility of a meniscus tear" - (re report), mean that I have fluid on the knee ?? Not sure really if I should go forward with this. I am 65 years old (will be 66 y.o) this April. If is a meniscus tear - At my age would surgery be worth it. I had meniscus surgery May 3rd. I still have terrible pain while walking,is this normal? I also have a baker’s cyst behind my knee the orthopedist has had to drain twice since surgery. I wish I would’ve found you before I had surgery. now they want to give me gel shots. Which I don’t want to get really they were already trying to talk me into knee replacement. I’m 60 years old. I never had any issues with my knee before the surgery‘s except for the baker cyst. This has been the worst surgery I’ve ever had.

Hi David I have done a menisectomy 9 months ago. They just trimmed a little bit. I have had so much flares and problems with my knee afterwards. I have tried to follow PT's instructions but it flares up really easy. Now I have started your meniscus rehabilitation program online and the glute exercises, but my knee still flares up. My knee always feel better when I don't do any rehab exercises but I know it is so important to build muscles. I have tried to lessen the exercises but even the small exercises flares the knee up. When i don't do any I can feel my walking progress gradually, meaning I can walk without a limp and move faster and a little longer. Where do I go from here? I really appreciate if you could answer. 🙏🙏🙏

Hi! I’m 59 yrs old. I recently had meniscus repair exactly a month ago. and currently doing PT. I’ve been getting shooting pain on the inside part of my knee and shin. I’m so scared I’m tearing up the repair. I’m only 55 degrees on bending. My knee feels very tight. Are the pain normal? Should I be concerned?

I’ve had a meniscus tear over 20+ years ago. Dr. told me to have the surgery. I chose not to have the surgery & I am now 73 years old and I do have osteoarthritis then & now. I’ve been able to walk and exercise and do whatever I want. Moreover taking supplements to help the the bones helped a lot!! It’s the arthritis that bothers me if I eat Night Shade vegetable vegetables .

I fell a year ago shortly b4 my Mom died. I only had it checked out a month ago. I had a hard fall and hit my head and hurt my back and twisted my knee. My MRI they said I have a torn meniscus and Advanced osteoarthritis and three compartments of my knee. The doctor said I was bone on bone I know you don’t get much benefit out of meniscus surgery because of the poor blood supply. Anyway, of course I was recommended knee surgery. I tried following your tips.

I’m older and over weight, why does my left knee pops and click when I walk with pain behind center of knee? I had no surgery but had previously pain from my meniscus and therapy that helped. This is the first time it’s making a clicking and popping sounds when I walk.

I can go from straight leg to bending somewhat, which I do when I sleep. when I get up to go to the bathroom, I have to manually straighten my knee and it makes crunching noise and it really hurts now both my knees have catch me in bed.

I have a varus knee, I underwent partial meniscectomy of the medial meniscus 10 weeks ago and I have bone edema and subchondral fracture of the medial condyle of the femur and tibial plateau. I have pain when walking. I was instructed to go back to using a crutch and reduce the exercises, in addition to wearing an insole. After improving, then try to correct the biomechanics. Is that correct? Thank you very much!!

Hey Andre! We are saddened to hear that you are experiencing pain when walking due to bone edema and a subchondral fracture to both your femur and tibia. Yes, we do believe that minimizing the time on your feet is crucial to helping you get your pain and swelling under control - this can take some time. Once you feel you have healed enough then it might be okay to begin to exercise to potentially improve your knee issues. We do believe that correcting the biomechanics is important but perhaps what's equally as important is spending time on reverting the muscular imbalance that possibly contributed to your knee issue in the first place. When it comes to the meniscus and muscle imbalances, the hamstrings and quads tend to be more dominant, like the heavier side of the seesaw. This can put excess pressure on the knee joint, leading to discomfort and potential issues. Hi Joan, we are sorry to hear about your fall! Please be very gentle with yourself at this time. Try alternating heat or ice. Ice can help with swelling but sometimes the heat feels more soothing. If possible there is one exercise that we would recommend for now which is the tailgate swings. You can control the swing to your comfort level and it usually massages the joint but only swing gently and keep the movement small. We are going to share a video with you that demonstrates it. Hey Trusha, you should avoid squats for a while now. Strengthening your quads and hamstrings will only add more pressure to your knee joint. Instead, focus on building your glutes with exercises such as hip bridges. Hey Rhonda, great question! Cartilage actually can be regained, however, it is a very slow process and takes a long time. Proper muscular balance can help create a healthier joint so that the cartilage is able to heal. Keep up the good work on the glutes and send us an update on how you are doing after a little while!

Dear Dr. David. I love your podcasts. I have a strange knee problem. I can do couch stretches on my bed and most of the time against the wall. So my flexibility is pretty good. But after 2 hours of Pickleball it swells up. Then I have to ice it and wait a day. Wouldn’t a successful couch stretch indicate the knee is fine? I’m in Denver but I’d pay you to do a teleconference. Thanks, Marshall

I had a double tear with a flap/fold back. 1st surgeon just said do physio and come back in 4 weeks. Physio said "4 weeks! No way". So physio for 6 months. Pain with clicks and popping. Went different surgeon. He Looked at MRI and said this should have been operated on. Now 6 weeks later, no clicks and pops but still get pain walking. Getting the leg straight was mentioned at hospital physio as well my local physio. He did say 6-8 weeks.to heal. I suspect 12 weeks. Edit: 42:00 physio said to do stationary bike. I think in conjunction with the rubber band exercises and straight leg squeezing and push down to get leg straight.

Ohio watching later 😄 You mentioned that muscle imbalances are the most likely cause of knee issues. Even fallen arches. I have fallen arches, just had a TKR, and definitely glutes are weaker than my quads. But what causes the fallen arches? Is this a what came 1st, the chicken or the egg kind of situation? I’d really like to know what causes the arches in your feet to fall! One day (many years ago), I was fine and the next day, couldn’t walk bare feet on the beach (was on a vacation). From that point forward, arches were gone.

Thanks for talking about stationary bikes! I think, my worsening knee issues, 3 months after surgery, come from excessive use of the bike (I do some raining, not just relaxed pedaling). I'll repeat the MRI and change my training plan to focus more on glutes.

I've been using recumbent bike till last weekend (was afraid to carry down my 16 kilo bike from the 3rd floor) and can't remember using the glutes on the downstroke. I had a torn meniscus surgery in April. My leg is still swollen and having pain. Actually my left leg doesn't even look like my right leg. It also look like my left leg is bowing out more than the right leg 😢😢.
