My hips are beginning to go I think especially the right one. But I'm already doing knee exercises and strengthening my glutes thanks to Dr D and am having huge relief and a very noticeable improvement. I recently turned 60 ! Since I was about 50 every time I go to my doc I get the 'wear and tear due to age' diagnosis which ive never really agreed with. I'm also not interested in popping pills forever more. That's why I love this channel and Dr D's approach. It's music to my ears folks 🙂

This was absolutely the most thorough & clear explanation about hip osteoarthritis. Even my doctor and physical therapists did not explain the situation anywhere near the way you did. I understand now why the glutes strengthening exercises are so important and why age is not really a factor. Thank you so much for this!

I have all of these symptoms. They have happened gradually over many years. In the last year it accelerated to the point where the pain is now constant. I rate my pain as resting at 2 to peaking at 8. I was diagnosed bone on bone after Xrays in the fall. I'm a carpenter with a decade to go before retirement and I want the replacements so I can function. I have always been active in weight training, snowboarding, and back country canoe camping. I was even once a semi professional skydiver. I'm now at the point where even though my job is less demanding as a senior level employee (they are trying to accommodate me at work) I'm barely making it through each day. My morning workout has become a morning physio routine and my evenings are spent repositioning a heating pad around my pelvis lower back and quads. My situation is different in the sense that I'm Canadian and surgery will cost me nothing so my US comrades have my sympathy. My biggest gripe is with COVID everything has ground to a halt. I'm told that my first surgery would normally be 4 months after approval and is now possibly 2 years. Please, everyone get vaccinated, be careful around others. Don't get sick. Be aware that unnecessary patients in hospitals are slowing down treatments for everything including life threatening issues. Do your part!

I’m 44 with torn labrum and hip arthritis that flared up when I increased hiking a couple of weeks ago. Made worse by a cortisone shot. I’ve been seeing a chiropractor for pelvis adjustments, but it always goes back within a few weeks. I was told a replacement is my only choice and I’ve been getting weaker and feeling out of options. I feel hopeful for the first time in a while and started the glute exercises you suggested. Thank you for your detailed explanations - they really help!

You give so much more information than doctors do. My doctor basically told me it's nothing, but the next time I was seen by his associate and he told me that I have hip osteoarthritis and gave me a cortisone. Which did not work at all. Prp injections are still in the early stages, and because of this, insurance won't cover the prp injuction.

Hi Dr. D. So glad I found your site and wish I lived in El Paso! Your videos are so thorough and easy to understand. I especially am liking your series on Hip Arthritis. Back in 2017 I had an anterior hip replacement on my left hip. The surgeon did tell me I would eventually need the right hip replaced. For almost 6 months now I have gradually been having hip pain on the right hip which has now gotten to the point of a very similar pain that I experienced when my left hip was a problem. I am having all of the symptoms that you outline except for the cracking sounds. I am at the crossroad of do I get the right hip replaced or do I try a more holistic approach. I will start your recommended exercises...I was doing almost every exercise that you had recommended NOT to do...no wonder my pain kept getting worse. Thank You for all your advice!

Dear Dr David, firstly, wishing you a Merry Christmas and a Happy New Year of good health and prosperity !!!. Thank you for your wonderful work you are helping so many people. I have diagnosed bob osteo hip, and since taking your advice about correcting movement and exercising glutes, the joint pain and clicking is much less since moving focus to glutes. Often, merely squeezing the glute can stop joint soreness. I can walk non-stop for one and a half hours. This used to make joint pain but now I get sore and stiff gluteal fascia instead. Would this indicate that the glutes need further building up?

Hello again, I’m in my early 60’s Diagnosed with Osteoarthritis Arthritis in the hip, I ran into ur channel recently and have been binge watching. I have been sleeping better with the pillow between my legs, tried doing some of the exercises, but the stiffness is terrible. Can’t get my boots on, and yes, sitting on the toilet. Have been icing the hip and it helps, was on anti inflammatory for a few months had to stop, but need pain killers each day. I try doing my stationary bike I was a speed walker that use to work out six days a week, always on the go. Anyway I’m curious as to how to help the hip or should I get the cortisone shot to avoid the surgery ? My brothers and father had the same n had surgery successfully. Family heredity I’m guessing. Thanks a lot. From Montreal. Love ur vids.

Can a tilted pelvis cause or worsen the hip joint problems?

I'm 59 and 175 lbs very active with hip arthritis ( had a xray). Is there a piece of cardio equipment I could use that would be good for my hips? I'm thinking of possibly an air bike, row machine or cross country ski machine like the old nordic track. I have exactly what you describe that the ball needs to be pulled back in the socket by strengthening the glutes. Great ( maybe life changing advice about always moving my knees back and forth when sitting. Thanks for all the great videos, I plan to watch all of them in the next few days.

Hey BWCA 1981! We're glad you are implementing our advice! Best of luck to you! As far as using a piece of cardio equipment, it depends on how you're using it. Because if you are using your glutes more on the machine, then it will likely help you. If you are using your thigh muscles like the quads and hamstrings, then it will probably make your hip arthritis worse.

My diagnose is moderate to severe right hip arthritis by x-ray, but i never feel stiffness, the pain ranges from 0 most of the time to maybe a 5 on a scale from 0-10. I stopped doing high impact exercises after the diagnoses but i had 0 pain when i did them, but i stopped doing them anyway. I don't want it to worsen so i'm doing what you're recommending in you videos about hip oa.

Hey Susan! In the US, you can find a physical therapist trained similarly to Dr. David at this website: aaompt.org/Main/Public_Resources/Find-A-Fellow/Main/Member_Resources/Find-A-Fellow.aspx?hkey=108c9a56-f793-45e4-b3ed-cefe5d871a94 Dr. David is a fellow of the American Academy of Manual Physical Therapists. His specialty is orthopedic manual physical therapy, which is the ideal specialty for helping someone avoid surgery, injections, and medications. Normal physical therapists are best trained to help people right after surgery.

Thank you for this detailed video, I learned much from it., I have all the bad symptompts you explained , with my right hip joint, I have had physical therapy, cortison injection and prp injection. they did not make much difference. The cause is also just as you said, muscle imbalance, I have strong legs and weak glutes. As a last source my doctor mentioned stem cell injection, he also explained it to be an experimental cure . What is your opinion on stem cell injection pls.
@epmanualphysicaltherapy
@epmanualphysicaltherapy Жыл бұрын
Hey Ahmet, sorry to hear about your frustrations so far with healing your hip. Stem cells might offer temporary relief and they also might not work at all. It would not be good to only rely on stem cells. You can use it to get temporary relief while you work on something else to fix the root muscle imbalance. You've got to address the muscle imbalance so that you can depressurize the hip joint. It starts with working on your glute strength in a way that doesn't make it worse. Take as long as you need to be able to get to your desired goal. Gaining glute strength can take many months!

What do you think of red light therapy?
@epmanualphysicaltherapy
@epmanualphysicaltherapy Жыл бұрын
Hi Marilyn! 🧡 Red light therapy has shown potential benefits for pain relief and tissue healing. It can offer temporary relief as it primarily targets symptoms rather than the root cause of the issue. To achieve long-term results it's important to address the underlying factors contributing to the pain. Combining therapies that target both symptoms and the root cause can offer lasting results. In case you are interested we do have additional information and exercises in a playlist we have compiled for hip arthritis help. You can check it out here: “Hip Arthritis Help” kzfaq.info/sun/PLJVIPiFvv1mbICbP6qlXUZwMyAJlKD1dt We wish you all the best on your road to recovery!🧡

Hey William! Sorry to hear about your OA and heart attack. Have you ever done cardiac rehabilitation? It’s usually done in a hospital or heart clinic. You should consider doing that first before you pursue any other exercise so that you can have more confidence in doing other exercises. Our exercises are rarely very tiring for the heart, but we can’t tell you to move forward if you haven’t properly recovered from your heart attack.

Thank you for your videos. You talk a lot about muscle imbalance, specifically under developed gluts, but is under developed quads also possible?
@epmanualphysicaltherapy
@epmanualphysicaltherapy 2 жыл бұрын
Hey Steve! Great question! Yes under developed quads is possible, but rare. In someone who is overall weak, they may need to strengthen all over even the quads. The most common situation where quads are weak is right after a knee surgery. Anything that injures the quads will generally make them weak. Focusing on quad strengthening in that situation would be appropriate. But if there has been no direct injury to the quad and there isn't an overall debility (like being bed ridden from an illness or chronic injury), then there is usually a glute weakness that needs to be addressed for the knees and hips to be healthier.

Hey Vivien, it's hard to say without looking at imaging studies. Groin pain can happen due to many different issues, but from reading your previous comments we understand you have already been dealing with knee arthritis. These could be related, glutes that are significantly weaker than your hamstrings (and quads) can lead to worsening symptoms when a person has OA. Yes, exercise is good for over all reason of the body but let’s not fool ourselves here…once the cartilage is gone there is no way of getting it back that’s why it’s degenerative disease it gradually eats away the cartilage…yes, exercise does help to strengthen the muscles, cores, glutes eating the correct anti inflammatory foods helps with inflammation very true 👏🏻!! All these exercises helps to ease the discomfort, but facing reality cartilage is gone!!

The aim is to provide patients like yourself with effective solutions and a better quality of life. We understand that seeking non-conventional approaches can be met with skepticism, but we believe that exploring alternative treatment options can sometimes lead to breakthroughs and improved outcomes.
