Feller and Bloxham Hair Transplants

Күн бұрын

Dr. Blake Bloxham of Feller & Bloxham Medical (www.fellermedical.com) discusses an all too common consultation request he receives: patients who underwent an FUE procedure and are now worried and seeking repair. Dr. Bloxham breaks down this patient's case and points out the "red flags" and pitfalls so YOU, as a patient, can avoid these problems and, most importantly, avoid repair after an FUE surgery.

I went to the “Bosely Group” up in Seattle in 1990. I had a hairline just like that guy. To their credit they told me “You are no longer in High School, you are a 27 year old young man with an appropriate hairline for your age. Frankly, I don’t even know why you are here”. Unbeknownst to me, my hairline had already stabilized and it and remained the same until today. I’m now 58.

Great video Dr Bloxham... I'm normally not one to comment on anyone's video even if I love it but you brought up so many great points in this video that I unfortunately had to learn the hard way ... and if I could pay you only 1 compliment, I would say that after watching this entire video... If your talent and ability as a transplant physician was only half that of the code of ethics and ethical behavior that you project and convey throughout your video, I have a feeling that would still be more than plenty to put these other doctors to shame... I respect you keeping the video focused on educating and what to be cautious of in the most professional manner as I'm sure that just for the very fact that you had to make this video, I'm sure there are plenty of companies/doctors that you would just love to cut into and tear apart for casting a negative shadow over the field of medicine that you have dedicated yourself to and made your livelihood... I'm sure anyone else who has actually gone to a few consultations will agree with me when I say that it was extremely refreshing to sit through this video but also stings a little at the same time as anyone who has done a few consults and then ultimately had the procedure and paid $10,000 - $15,000+ looks back and realizes that most of or all the doctors that they saw had a code of ethics that would fall somewhere between a Lemon Lot Used Car Salesman and a self-righteous, glorified, overcharging tattoo artist ... Only instead, they have a doctorate in Medicine instead... I've never heard more doctors tell people only what the patient wants to hear in my life... sprinkled in with only a few carefully and extremely well-worded reality checks throughout the entire process... my personal favorite was being told that 8,000 grafts would be just a little short of what I wanted and that I would be back to the little bit that I lost if I went the next step up to 13-1400 grafts so of course I did it and then it wasn't until my post surgery visit that they told me that I will probably need to do 1 more round of that same procedure to be where I wanted to be... I was so livid... they seemed so reputable at first and then they pulled a complete 180 on me... Maybe you won't bad mouth any doctors or companies but I will... I had my procedure done at Bosley in 2012 ... I always had insanely thick hair but I noticed my hairline falling back just a little bit ( about 1/3rd to 1/2 the amount of the guy in your video ) ... I was more convinced that it had to do with stress as it started to happen just after coming home for good after serving in Iraq as a Special Operations Marine Scout Sniper... (not the stress of my experiences there, but coming home and adjusting to civilian life after having such a high profile job and no comparable opportunities here at home on top of other personal relationships ending) ... I looked into a few options and Bosley seemed to be the best at the time... I can definitely also say that I was a bit blinded by the fact that this wasn't done at Bosley, Anywhere USA ... I had my consult and procedure done in NYC at the Bosley Office in Manhattan and my transplant surgeon was Dr. Alpeter who just happened to be one of the Doctors who spoke the most in the Bosley TV commercials that were airing at the time (Besides Dr. Bosley himself) ... I was even given the option to schedule my Procedure with Dr. Bosley himself since it was Manhattan but having him do it would have made my same $11/12,000 dollar procedure that I paid be a medical bill of $30,000 just for him to do it (which was never going to happen)... I def had a false sense of security as I thought to myself " well, I'm sure I'm in good hands as I made sense of it in my head that of all their doctors, I'm sure they would choose one of their most capable ones to represent their brand and company on TV"... I should have walked out when he wouldn't listen to any of my input at all... I showed him a picture of me that was only 2 years old and showed him that my hairline had moved back maybe about 1/4" or so... it definitely wasn't even at 1/2" back... I always had an extremely straight hairline and it receded straight back so it wasn't like a "V" like the guy in the video... there was only extremely minor thinning by my temple but not noticeable... the only reason why I even notice that my hair moved back is because I still had a few baby hairs where the line used to be and my hairline was perfectly framed and exactly where my forehead starts to curve up to my skull ... I had lost a line there that was basically as thick as my pointer finger... it was the only reason that I came there and he kept arguing with me that it would look too juvenile and he would keep saying to me " you think you want that now but you have no idea what kind of hair style you will be wanting 10 or 15 years from now... I looked at him and said " do you not see the tattoos on my arms? They don't come off... I can sit here and tell you right now that how I wear my hair is never going to change.... I'm a U.S. Marine Doc... I fade my hair by my ears and the back of my neck and keep it 3-4 inches on top... thats the way its going to stay until they drop my casket in the ground and hand someone an American Flag... so can you please do what I'm asking for the $12,000 that I'm paying you and put my hair back to the way I've been used to wearing it for the last 13 years.... He finally halfway gives up and says " I come as close as possible... He completely disregarded me, he put in multiple strands in spots around my hairline like you were talking about... and my hair has been straight as an arrow my entire life... before those 2 years of heavy stress my hairline was flawless... not only did he not bring my line to exactly where I instructed, but he sparsely filled it out and you can tell it's weaker up front now ... when I said "that doesn't look like enough hair up front, he said " well, you had a spot on the top of your head that was a little smaller than a Quarter ($0.25) and it was right on your part so we thickened that up... I said " I never part my hair so why would you do that when all I wanted was the front fixed with 1400 graphs within a 5 inch x 0.25/0.40 inch area... and then if I wasn't angry enough for some guy doing whatever he felt like after being paid 11/12,000 bucks, the most aggravating was yet to come but I wouldn't know until my hair grew in... as I said, my hair was always straight and curved up and down towards my face my entire life... they must not have been careful or paying close attention when placing the donor strands because they placed the follicles with the hair curving to the side on the one side of my hairline so now (thank God it's not a big one but it's enough to make my blood boil) ... if it wasn't bad enough, they basically installed a tiny Cowlick up by my hairline so now everytime the hair grows out, I have to go through my hair with a fine tooth comb and separate the straight strands of my hair from the small patch of sideways curving hair and then I have to take a sharp pair of scissors and cut that small patch of hair down to the base of my scalp .. Honestly, if I could sue them for my money back I would.... and if I tried and they said no BUT they offered to fix what they fucked up for free, I don't think I would allow them to touch my head again... and it took years of rubbing Moderma Scar Cream into the side and back of my head to get the scar they left when they butchered my scalp to fade enough where it no longer looks like I had my skull cracked open in 2 places ... it would have been so much unnecessary trauma avoided had I found you as my doctor instead of walking into Bosley's Butcher Shop ... I would never suggest anyone go there... modern day snake oil salesman with scalpels and only want your money ... it was my first great lesson in business though... " Just because someone can afford to advertise the business on a grand scale on TV doesn't mean they're good... doesn't mean they're the best... it just means they have enough capital to pedal their garbage to the masses"... ...The industry needs more doctors like you... if you ever need a guinea pig to film and market yourself fixing the Big Dogs mistake, let me know Dr. B ... hahaha

Dr Blake, Thank you taking your time for helping all of us in the hair loss situation. I learned a lot and wanted to know can it really be determined in a person what the underlying cause of hair loss is in that one patient, before it comes to a transplant. I tried Rogain some years ago and it did NOTHING. Does PRP, or other procedures work and do you do any type for people with different types of hair loss? I feel that my hair loss is mainly stress caused by an accident in 2007 being told at the local hospital that I could go back to work and then finding out I had a meniscus roll in the right knee and the tendons torn loose in my hand and today in 2021 I still need more surgery. Then my attorney never took action so my W.C.case has set for years now. Way too much stress that no one should ever have. Do you think I am right? Thank you again for doing this video !!!

But then also ive have see the reverse in pimple pitting , like follicles coming out of a milk spots , is it a depth issue on the later ?

The guy went to the gas station for sushi. If you are under 25 get on medication ( finasteride/minoxidil ) get a laser cap, derma roll, do what you can to delay surgery for as long as you can. Save your money and research extensively for the right clinic when the time comes. Hair loss is progressive and it’s a life long fight. If you must have surgery in your twenties do FUT. This informative doctor is right about donor supply and management. At such a young age exhaust your FUT before going FUE. Thank you Feller and Bloxham for this video.

Many years ago , hair transplant procedures were done with rounded hair punch and the end result look like tuffs of dolls hair because of the rounded appearance of the grafts ' When turf is transplanted to grow a garden lawn , the turf is cut into squares and not rounded shapes , In the past , it would have been better if the punches cut square shape for implants because squares shapes are easy for the edges to meet up and to look more natural . I think there is too much over reaction in this video because the hair grafts have grown which is the most important thing as hair density is very important for the final results. Once the hair grows longer and depending if the hair is combed backwards or forward would also make a difference to the final result . By looking at this video , the patients might only need to have some refinement surgery to the edges of the transplant but overall he is lucky he has got so much density from the transplant and he has not had a failure where the grafts have failed to grow at all .

My transplant looks a lot lol this one. A shit ton of multis in the hairline, black dots, and a lack of density, I’m at 6.5 months I I still see growth, so it may improve on that a bit, and it is not as low and the design of the hairline is very good, it looks straight but is subtly irregular both in overall shape and in microirregularities, so it’s not too bad, as well as my hair being very fine, the multies are not as coarse and pluggy looking. But if it turns out not dense enough I’m definitely going for a repair aiming to soften the hairline and packing more overall, and I think it’s gonna be perfectly fine.

thank you very much for the video. When I chose a hair transplant clinic (in Turkey), I tried to avoid clinics that did an aggressive front line (which looks like a zigzag). And of course the front line is very important for the patient. I was offered to make the line 1 centimeter lower, but I asked to make it higher, and the doctor agreed. It is very important to discuss the front line with your doctor. Thanks!

Doc, I've managed to get pretty much 100% survival rates with FUE with 2 top surgeons in Spain. This conception of only 90% yield on FUE is a little outdated to me. I even got full growth on BHT (did two different 100 graft BHT tests) when some surgeons claim on 50% will take. With today's protocols, storing solutions (ATP) and extraction tools there is no reason for FUE to yield over 95% on the right hands on a virgin scalp with no scar tissue and proper blood supply. We have hundreds of cases on the Spanish forum by these surgeons where it looks like complete regrowth. I am not stating it will be the case with every patient but it is the norm with their work among us forum users.

14:36 why would you advise a strip (fut) to be more agressive with it ?
@fellerandbloxham
@fellerandbloxham 4 жыл бұрын
Hi Francesco. Several reasons: 1) It's a more effective use of the donor region and provides more lifetime grafts. This means you will have a higher number of grafts to use to potentially fill behind the hairline if necessary. 2) FUT has a more consistent growth yield, so we feel more confident creating aggressive plans in more visible regions. Hope this helps. -- Dr Bloxham

excellent advice given here, FUT strip is still the gold standard and young patients should have it to start with, I had two FUTs over a period of 5 years and by the time I came to having a third and final touch in I had plenty hair to harvest for an FUE, and crucially my grafts were prepped under a microscope, that is very important because if your grafts are not prepped under a microscope you hair will end up looking like the dreaded dolls hair plugs, like a 1970s pluggy transplant, and you don't want that at all!!
