Thank you for the in-depth analysis. It's refreshing to hear from someone hypothesizing on the molecular process behind gout, rather than hearing the standard "don't eat purine rich food" and "take allopurinol" from your doctor. I don't pretend to understand evey detail of your hypothesis - it's very scientific - but the essence of your argument is very compelling. As an aside, I'm 65 and have had gout for 15 years. Only since last year, after going on a keto diet and restricting alcohol have I had relief. I am almost convinced that gout attack occur when there's a "sudden rise" or a "sudden fall" of uric acid. Why the "sudden fall" scenario? Well, I remember one time when I had a gout attack in my toe, I took baking soda, based on a theory of "alkalizing" the body to neutralize the uric acid. Within a day, the pain almost disappeared in my toe, but while continuing with the baking soda for a few days, the pain not only resurfaced in my toe, but also in my wrist for the very first time. This gout attack lasted almost a month. The worst pain ever. Interesting to understand your hypothesis behind such a scenario.

Excellent work. Not many are working on gout. I solve my inflamed knee pain with electrolytes full of magnesium and potassium and a low-carb diet. But a glass of wine at a meal is unnegotiable.

Did I miss it? Are there any recommendations or opinions about the timing of when you eat…any feeling about fasting…I know you say this is not a calorie restrictive diet, and ketosis is important….thx

Maybe a video in this area is coming up. Basically, if you are doing a good job with the ketogenic diet most people evolve over time to eating two meals a day depending on their activity level. If you workout many days a week then sometimes three meals. Usually those meals are between late morning (10-11 am) and dinner usually before 6 pm. This provides for a large fasting window without formally fasting. I don't recommend IF, especially many days. The reason is because of the relationship between UA and ketones. I have found that by keeping my BHB between 0.5 and 1.0 mM maintains the lowest UA. If I let my ketones get up above 1 mM, then my UA rises above 6 mg/dL. Remember that ketones are preferentially excreted by the kidney over UA, so UA backs up in the circulatory system.
