68 y/o f, lchf/keto/carn 3 yrs. 1st episode gout 1.5 weeks ago. No alcohol 6 mo. Ate 2 meals that was gr beef and pepperoni 2 days in a row is what I think might have triggered. Last time I had pepperoni 1 meal, not affected several mo ago. Self diagnosed and homecare only. Left great toe area. No bunion but in that area. HTN, IR, NOT DIABETES, 5'4" 183 lbs. Hx spondyloarthropathy 2010 acute large joints affected. Now told I have osteoarthritis.

The hot water worked for me, and the hypothesis makes sense with my personal experience. I'm going to start allopurinol soon as well, as ketovore hasn't been quite enough to get the results I want (still feel some pain in joints where I've had gout flareups), just waiting to get the latest bloodwork done to check my levels first, but almost certain they're still high. Thank you for your continued research and quality presentations on gout! You are helping educate and hopefully advance the field.

Is dry heat to knee like pad going to do the same as wet heat? I was encouraged to read the effect of apnea as possible explanation for my out of the blue gout flare that I am in. I had stopped using my apnea machine because hubby said my breathing was better. But I think there are signs My apnea is back. I have been on strict dietary guidelines as well as experimenting with fasting 20/4 daily when this flare hit. I take the Quercetin, tart cherry and have prescription for allopurinol. i am testing daily and my normal UA range in 8’s is down mid 6’s (even during the flare), my blood sugar 98-111 down from 125, my ketones .5-.8 daily unless I eat any processed food. …even keto breads turns out. Anyway, thank you again.

I have Gout but have not had an attack in 10 years, I don't smoke, or drink I do take allpurinol

Then maybe you are good to go. But I would look at the other drivers and make sure that they are limited.

great video so one may find that they start to lose calcium in the cells and need to increase/supplements, and one would watch there calcium to magnesium ratio, as well as your sodium to potassium ratio wound tend to high sodium where one would limit salt and increase potassium supplements? Also you would think glutathione supplementation for inflammation would be mandatory?

Best video on gout I’ve ever watched, comprehensive, and informative. Are you still actively researching this? Longtime gout sufferer here, just had my first flare in 4years 3 months, diet mostly achieved this. And a lot of my trial and error with diet really came to sugar as being my only trigger since I don’t drink alcohol. Gluten free, then keto, then carnivore, back to keto over the last 7 years and 16/8 intermittent fasting over a year. I had a massive flare, knee then foot then foot in other leg. Worst attack I’ve ever had. Had to take a midigar, stopped all of it except for original knee flare.

I think this flare, which was my first in over 4 years was onset by a culmination of factors. I got sick, previous 3 weeks I was eating a lot of chocolate, also I did 3 days of 21 hr intermittent fasting, and was getting poor sleep.

Usually flares happen when a change is made that causes a large flux in UA which can be up or down. Chocolate and IF would cause large swings in UA. @@motojedi6580

Yes. I am researching hard which is why I'm not uploading videos at the moment. Writing a paper and preparing for a conference: Citizens Science in Las Vegas in March.

Brilliant and informative as ever, thank you. I was discharged from Rheumatology clinic but we cannot be complacent, lowering sugars/carbs is in my opinion a complete lifestyle change not a temporary measure, also if I may add, I have regained mobility in my big toe by increasing activity i.e. having a walk after every meal, especially after the evening meal, you can easily exceed 5000 steps and doing resistence training. Stay hydrated, I drink sparkling water (never from plastic bottles) and filtered water, I have noticed slight twinges if I have been a little dehydrated for a couple of days.

I wonder how many people without gout and with normal uric acid levels would show crystals in their joint if fluid was taken. Interesting to think about.

Sorry I was slow to respond. I don't know. However, any drug that alters the concentration of uric acid can drive a flare.

@@DrPetesKetoKlub Pete - do you think there is any merit in acid alkaline balance - I started drinking a green smoothy daily consistently and didn’t have a flare for a long time - also taking bicarbonate of soda- would love to hear your thoughts 😊
