Heparin APTT Monitoring (MADE EASY)

Heparin is one of the most common anticoagulants in clinical practice. Even though it is high effective, it still has the risk of bleeding if not used appropriately. For patients receiving heparin infusions, the APTT of the heparin must be monitored to ensure efficacy with reduced risk of bleeding. In this video we will discuss how the APTT should be monitored and how to adjust the heparin rate based on that.

Thank you for the video! Do you alway have to have baseline aPTT result before giving heparin bolus+drip? Would you not give heparin if the baseline aPTT is in abnormal range?

Yes you need baseline aPTT and depending on the result, the starting rate can change.

@@thepharmacistacademy thank you for the reply. I just started working at hospital. Md wanted to start heparin drip w/o the bolus dose. The initial rate was calculated according to pt’s weight, and the aPTT lvl was not available.. should I have told the nurse to draw the level before starting the drip?

Yeah it’s okay to draw up the level and proceed if it’s emergent.
