You, good Doctor, are a genius. I am a neonatal ICU MD with debilitating Long CoViD and dysautonomia, and none of my MD colleagues can help me. Neurologists won’t see me. Cardiologists won’t see me. It’s been 15 months, and I still can’t even get a tilt table test. I am so grateful that I found your channel!! My mind is blown, so to speak… It has always bothered me that MD specialists tend to approach medicine in an organ-centric manner and not often in a fully integrative way. You have a gift in teaching and healing. Enjoying your content as an academic and importantly, finally feeling real hope as a patient. I wish that you were my doctor! Vestibular Autonomic Regulation is probably my favourite book I've ever read, it was what got me really interested in the topic of functional neurology; it mentions a lot about Spaceflight deconditioning and the vestibular/otolithic system and why many astronauts' bodies struggled to acclimatise properly when returning to Earth. They'd regularly end up with elevated heart rates when standing and other strange disabling symptoms. Thank you for addressing this Doc

Great book! He went on to write another follow up paper in 2014 that you would enjoy. You can also check out Ogoh's work which is great. 😊

I'm just going to paste my response to some other posts of yours @ POTS.... "This all relates exactly to my recent experience, including how sensitive the response is to every sort of factor....energy level, last meal, etc....I have learned to manage disequilibrium quite a bit by carefully planning calorie intake, monitoring my energy envelope, devotion to sufficient hydration and sleep, learning how to rest deeply during the day, learning how to disconnect anxiety with meditation, breathing, thought direction, somatic tracking, etc. Your talk was never in the least monotonous, I was hanging on every word and idea. I only wish I could find find any healthcare provider nearby that understood a word of this. I have been completely on my own sorting through what my mysterious affliction is."

You’re putting so many of my personal experience hypothesis into better words and context. You really seem to be making the connections that so many patients have that so many doctors seem to refuse to acknowledge. I’m skeptical about most social media “experts” but I’m really intrigued by the connections you’re making here and things you’re pointing out. I do think you could benefit from clearer explanations and scripts for the sake of our brain fog lol, as well as you don’t seem to have a comprehensive video on your “treatment” recommendations. I keep seeing talk of eye exercises but could you do a video detailing this and other alternate treatment methods directly? Thank you in advance for your work 👍🏻

This is me, in part. Over time, it's gotten totally disabling which creates deconditioning which, in turn, exacerbates the issue.

Yep, exactly, same here. I used to run regularly, and now some days I can’t even get out of bed without help.

Such valuable information I haven't seen anywhere else. I suspect this is part of CFS' puzzle with many patients reporting POTS and brainfog as part of their constellation of symptoms. I also wonder about effects of poor posture, either by way of bloodflow restriction or through compromized proprioception. Food for thoughts for sure.

For central vestibulopathy resources, you could start investigating velocity storage as a concept with whole body rotation. Cohen and Raphael are a great place to get started. Dig in and enjoy!!

Is it possible that the vestibular system is affected by a neurotropic infection (covid, lyme, dengue, Zika, enteroviruses) ? Can you rehabilitate a system with an active infectious disease process underway? What are the leading theories of how the vestibular system would come to be compromised in the first place? As a POTS patient who went from super active 20,000k steps a day to bedridden within a one week time frame that did not include bed rest, I have a tendency to bristle when deconditioning a la NASA astronaut in space is mentioned as a plausible cause as both myself and many me/cfs patients have onset and worsening of orthostatic intolerance with an infectious trigge. sustained or increased physical activity during infection seems to worsen symptoms or act as a trigger on its own as well. Sorry for the barrage of questions and thank you for sharing your perspective! Tone is not meant to be cynical/pessimistic. Just genuinely curious! Thanks!

Hi Thomas, I appreciate this question and your curiosity 😊. I’ll do my best to give you a sense of my thoughts. I think there is a high likelihood that either peripheral or central vestibular systems can be vulnerable to infection. We see this in other situations as well. The idea of an inner ear infection, labyrinthitis or neuritis is a relatively common phenomenon. Whether it is uniquely triggering or exposing an underlying weakness likely depends case by case. The second question of rehab is more subjective and without clear consensus. The type of infection, previous treatments, duration and symptom burden may influence the decision as to when to begin the rehab process. I agree with you that deconditioning as a cause can be puzzling. That said, over long stretches of time it’s reasonable to consider that people will be a little out of shape and may have trepidation around returning to activity. For good reason, activity is often the place where people have the highest symptom burden. This is just part of the process of returning to life, but in my experience is not the cause of the underlying problem. You are right most people can’t start from a place of physical exertion and they shouldn’t. I’m sorry to hear about what you’ve been going through, it is a truly frustrating experience. Covid-19 and February of last year and I had this problem for a long time but I noticed I'm getting a lot better meditation has helped me tremendously

I sometimes wonder about this connection. I am having the opposite happen, as far as POTS, where my BP raises upon standing, which leads to lightheadedness, shakiness & fatigue. I feel that there is also something that happens in my vestibular or proprioception systems too because driving my car & watching traffic around me or frequently turning my head will trigger an episode of lightheadedness or feeling like I'm standing on a boat. This can also happen to me when I'm scrolling through IG or YT.

Thank you Dr Keiser. I suddenly developed POTS with CFS after covid. I have previous bad injury from car and horse accidents. I am now considering this to be a possible cause along with deconditioning from months of lying down. 9 Doctors have not been able to suggest anything. Is a neurologist the best person to see? And is there anyone in South Africa you would know of? Most grateful for your insights!

About a year ago I started having dizzy off balance spells. That slowly turned into chest pain shaken hands and legs along with brain fog. All of my symptoms come while standing,moving or turning my head. I was diagnosed with pita but the treatment hasn’t helped. I feel better today than I did 12 months ago but I went from owning a business walking 30k steps a day to barely being able to go to grocery store or walk for 30 mins without feeling the need to lay down. I’ve been in and out of drs not sure what’s left to do.

Do you know Dr Minh Tran or Dr Joseph Krankanian, in California? I got to their clinic, but I’m not sure if they use all the nuanced treatment techniques you mention in your videos (they don’t usually explain the details unless I ask)
