In this video, I discuss my decision to leave management consulting (focused in the healthcare industry) to pursue my residency training as a surgeon. There were a lot of considerations into this decision, and hopefully this framework can be helpful for you as you think about your career.
What is management consulting? Consultants are hired to help businesses and organizations achieve their goals. These businesses and organizations have a problem they are trying to solve, and consultants use their expertise in the industry provide a set of options and paths forward.

Young man, you are an incredibly talented communicator & able to present life decisions in a thoroughly thoughtful & competent way. Your parents must be so proud of you. One of the men I most admire, Dennis Prager, often says: "Teaching your children to be good kind people is the most important life skill." You not only are gifted with many talents & abilities but appear to meet the "good kind person" criteria as well. Thank you for sharing your life journeys.

Thank you for this video. I'm a high schooler and obviously I'm in no position to be deciding between being a surgeon or management consultant, but as I'm starting to think about potential careers and majors I found your list very helpful!! You seem like a very genuine passionate person and best of luck down the road :)

I think its very clear this dude is competent enough to do anything. There are some people who are lucky to be doing well in finance or medicine, this person is clearly someone finance and medicine is lucky to have, I mean even the way he breaks down his decision on how to spend life shows some really deep and original thought. Hey Richard, thank you for the video! I really enjoyed watching it and hearing about your thought processes. With regards to finding your North Star, how would you recommend for one to go about constructing those hypotheses in the first place, especially since there are so many possibilities and options on what one can do in their life? Also, how did you go about discovering your interests in medicine and consulting? As these fields may seem quite different from each other, how did it occur to you that you could go about combining both of these industries? Thank you! :)

Thank you for watching! My advice for this is to be hypothesis-driven early on. Define something, find ways to explore it (internships, networking, etc.), and see if it sticks. If it doesn’t, assess why and you recalibrate to get closer to revealing the big picture you want. My North Star do to speak was not clear to me until this year (after many years thinking about what I wanted to do!) so just start by taking one possibility and explore. I took this same approach to discern between medicine and consulting, and as I explored each path I found mentors who did both and that gave me an increasing sense of how they can be combined. This was a great video. Very insightful. This made me think about options that I never thought about when deciding to do an MD/MBA. Thanks again for posting this. Maybe I missed it, but how do you plan on incorporating management consulting in your north star? because, and pardon me if I'm wrong, it sounds to me like you're shifting focus to ENT/Facial Plastics...great choice btw. Either way, I think you will be successful no matter what you do. I have faith in you. Thanks again for posting this video. It was very helpful

For me, I think of what I learned in management consulting more as a toolkit I can take anywhere else, as opposed to a function I will do day to day. The toolkit includes things like how I manage people, how I communicate, how I solve problems. Those I will always carry with me even as an ENT surgeon. Any other consulting “work” I’m putting on hold for now!

This was an amazing, informative video, you definitely earned a new subscriber. I was just wondering, as an upcoming freshman in college this fall, what would you say are the most important things to do as a freshman and sophomore in college? Should the emphasis be on joining extracurriculars, becoming involved in research, etc. ? Or solely on academics? I was wondering, how did you use your time during your freshman and sophomore year of college to accomplish your professional and personal goals?

I would say all the above, but I’d prioritize academics and relationships/friendships. For extracurriculars, you can cast a wide net early on but then after a year be judicious about which things you continue so you prioritize and invest your time in only the things most important to you. My freshman and sophomore year I focused on classes first, and for extracurriculars I was in student government, orchestra, and helped plan a civic engagement trip on social determinants of health through our civic engagement center. I dialed back on my orchestra involvement and leaned more into student government and social life as years went on. Hope this helps and best of luck on your journey!

Hey Richard, great video. Do you think that in addition to building a practice, you will also do work in healthcare systems sometime in the future? If so, what problems or fields interest you most?

I hope so, though I think it will be supplementary to my practice. Within ENT, intersections with public health that I’m most interested in are vaccine development and delivery (such as vaccines for HPV, which is a common risk factor of head and neck cancers that ENT surgeons treat)

Awesome question - I see identity as how you relate to your self/self-concept in a particular role, whereas purpose is how you relate to the work you are doing. They are closely related, and the work you are doing can certainly shape the way you see yourself, though it doesn’t have to.

bro 80k a year is still great. I know alot less than 350k but compared to other countries its huge. In the first job right after med school you get pays 20k... thats the NHS for you

I’ve found that the more I love myself and take genuine pride in my decisions, the more confident I am! Medical school can really wear you down and being your own biggest supporter and advocate is really powerful. Best of luck on your journey 🙏🏼

This guy will be back in" management consulting ", whatever that " racket " is in no time. Medicine is a very sacrificial profession and not just during residency. This guy was making 350k, traveling, and dealing with mostly more sophisticated types. In medicine, he will have to deal with all types of people, especially during his residency days( 5 -6 years is a LONG TIME) and he will have to deal with so much BS from his attendings, fellow residents, the nurses, and last but not least his patients, that he will not know what to do except what he has excelled at, being fickle and thinking he can do it all. Little does he know how much different it is to be a resident, from being a medical student. The part I am upset about is that he took up a residency spot from someone who would likely follow through and be a practicing physician. This guy will never finish ENT. Others have enabled him and he seems fortunate to possibly be able to return to be " a management consultant".

How many research publications did you have to do in order to match into ent?

I personally had 3 non-ENT peer reviewed pubs and 3 ongoing ENT projects when I applied. There’s no “standard”, but folks applying to ENT generally do have research projects in the field to demonstrate interest
