Bob interviews Steve August Physiotherapist from New Zealand about iHunch. Steve is the creator of the Backpod. The Backpod is a practical New Zealand innovation developed to address a problem which was either ignored or misunderstood for far too long. It has been developed by New Zealander Steve August, a physiotherapist for over 30 years', with extensive experience in treating back and neck pain issues. The tool has had an extremely positive reception in the professional community and from customers who have genuinely found a solution to a painful and debilitating physical issue. It is especially effective for the very common hunched back and/or tightened upper back that drives most neck pain and headache. Statistically, most people will experience this problem at some point; one in six people will be in acute pain right now; and everyone knows someone who is stooped and sore - usually hunching over a laptop, tablet or smartphone has an element of cause.

You have helped me so much. I was prepared to have surgery. Mine includes a bad shoulder from a fall that included a broken color bone. Nothing got handled right from the beginning, and with the use of the lap top, and cell phone, things have deteriorated. You helped me put 2 and 2 together, and possibly saved this old woman from going under the knife. I feel so much better, and combined with your shoulder health exercises, I think I'm going to be ok. Can't thank you enough.

The back pod costs more than I wanted to pay, but I was in a desperate state and bought it anyway. SO glad I did. It was the final piece of the puzzle in getting me out of ongoing, unbearable pain. I now have great mobility in my thoracic spine and ribs. I can hold my shoulders back now in the right position. Now I just have to remember to do it-building a good posture habit is my next challenge! Good luck everyone out there who are in pain-there is relief.

Steve is a legend. He helped me so much to understand my costo. He is absolutely spot on. My costo was the outcome of my broken ankle from school times. Lack of dorsiflexion led to unstable pelvis, which led further to scoliosis and costo. That had a huge effect on my mental health. Yes, the biomechanics of you musculoskeletal system has a huge effect on your mental well-being. And, yes. I can attest there is no better device than Backpod. And it is very painful to stretch this collagen. Oh boy. You just need to be consistent. Your spine is not only about strength but also about flexion. Listen to Steve and follow it to a T. And you will be fine. Be patient. Would be great to see Steve doing a podcast with John Bergman. He has a few videos on this. Thank you Steve for sharing your knowledge. It's priceless. You have always been so patient with explaining things. Your humbleness and knowledge are inspirational. Forever your fan. Such a great podcast.

I also 100% agree about massage being able to break up adhesions and scar tissue. I used to be a massage therapist and my specialty was in exactly that. I can say it hurts like a bugger to have it done if the process is rushed. (It hurts less if done slowly.) But the results are tremendous. And they are long lasting, too. The muscles will tighten right back up without the fascia released. But myofascial work really lasts. PS, that back pod will stretch out the connective tissue, too. It won't break up scar tissue or serious adhesions. But it'll release all but the worst if given enough time. (Fascia stretches much more slowly than muscle.)

I have probably the most perfect case of this issue. My life is ruled by daily pain and headaches. Pain and inflammation in the back of the neck, back of the skull, the textbook neck headache, upper traps, neck flexors, scapula, clavicle, shoulder, even down into my triceps and forearms, as well as daily back pain in the rhomboids and lats/ribs. Also costochondroitis. Winged scaps too. The whole enchilada. It is very rare that I can sleep without waking up with pain and have to break out the aspercreme and lidocaine and run my shiatsu, and use rice heating pads. Can't leave home without all that stuff. I have went to PT and Chiro, and it was EXACTLY like you said - it gave temporary relief but it didn't last. I've tried exercises but they only seem to make the pain worse because I don't have the mobility or range of motion (is that the same thing? lol). I have a foam roller that is great for pain relief in the back, but doesn't help the shoulders/neck much. I'm going to try one of these pods as well as exercises. I'm going to really try to do everything the book says. I'm so ready to have even a little relief. I'm used to it by now, and I've learned to avoid self-pity and pointless questioning which is what really makes you miserable, not the pain itself, so I'm still happy and not depressed or anything. But that doesn't mean I wouldn't very much like to experience a remission of symptoms! Thanks for this video, very informative. If I have success I will tell everyone I meet who has these problems to try it out.

Hi Colin. Good - well done on thinking for yourself, and also not giving up. Yep, it does sound like you've collected the whole bundle - in the UK they'd call it the full monty. So, costo on top of the iHunch when the rib joints round the hunched back got tight enough to start the rib joints on your breastbone straining to compensate. Well, that's exactly what we built the Backpod and its small home programme to counter. It all happens completely logically - which means you can build a simple device and programme to counter all the bits. It's not actually difficult, but will take time and effort. It's incredibly worthwhile doing, because otherwise it just keeps on getting worse and more hunched - and the neck headaches are horrible! I suggest you look over the Backpod's iHunch page - it's got a good overview, plus the home programme in video form: you will need all of it. Link is www.bodystance.co.nz/en/ihunch/ Good luck with the work! Cheers, Steve August.

The back pod although pricey, works great. After a week 1/2 I became comfortable using the pod. I was hobbling around with pain in my hip from running hills. After using the pod on my back for about 1 week, I had a nice release of tightness in my back. I foam rolled my hip flexor & was feeling much better but I think the hip pain was referral pain.

@@debbyesche1257 Hi Debbie. We do state in the Backpod's user guide that it shouldn't be used if you have osteoporosis. That is largely to cover ourselves - who knows how sensible anyone buying a product is? In practical terms, the Backpod is just delivering a stretch to part of your back and rib cage. It's entirely under your control - not just how long you do it for but also how strongly you do it, and you can simply get off the Backpod if there are any problems. This is really clear in the instructions - start off with up to three pillows under your head, and if necessary a layer or two of fluffy towel over the Backpod itself, so that it's just a mild stretch initially - definitely not painful. It should be a bit uncomfortable to start with - the Backpod is a real treatment device, not a magic gimmick, and if you can't feel it doing something then it won't be. Just like stretching a hamstring muscle - feel the stretch but don't go hard enough to be painful. As with a hamstring, it gets easier as you keep the stretch on and things free up. This should be okay with mild osteoporosis, but it is at your own risk. There is common sense involved - you just do a bit quietly and see how it feels. There are degrees of osteoporosis anyway, and it can be quite mild - you should simply ask your doctor, who should know about your particular case. It's not like there are any sudden sharp jolts or impacts like you get with a manipulation, say - it's far safer than that in any case. We've certainly had patients in their 80's where the Backpod has helped lots with no problems whatsoever. I hope that makes sense. It's at your own risk, but the risks aren't great anyway, and they are completely under your own control. But do ask your doctor first. Cheers, Steve August (B.A.,Dip.Physio.).

All this being said, is it also important to strengthen the weak upper back muscles BEFORE stretching them? My PT says they first want to strengthen my weak upper back and then start unjamming all my tight spots so that when the tight stuff is released, I have muscle strength to hold it all up. (If that makes sense) I continue to get brick like painful upper traps that start to cause a tingling affect into my neck and cheeks some days. Is there possibly something else causing super tense upper traps that I am not addressing? Just seems so sudden of an onset that won’t calm down fast enough. (Almost 3 months in)
@stevenzphysio4203
@stevenzphysio4203 2 жыл бұрын
Hello Stephanie. The standard common pattern is strong, tight upper trapezius muscles along with weak, stretched out middle and lower traps. Your body's got into the habit of overusing the upper traps, so the neglected middle and lower traps weaken and stretch out. It's really common. You can stand behind someone like this and you'll see their shoulder blades (scapulae) pointing in and close together at the top, and pointing out and wide apart at the bottom. I don't know about strengthening the weak middle and lower traps BEFORE stretching the upper ones. They both need doing so I just get the patient doing everything at once. It takes longer to strengthen the middle and lower traps (and other muscles including the lats, rhomboids) than it does to stretch and massage the upper traps, though. Massage will help heaps - it's much faster if you do that as well, rather than just stretching on its own. The two home massages i find most useful are shown as videos on the Backpod's iHunch page - link is www.bodystance.co.nz/en/ihunch/ You need to talk, bargain, or bribe someone into doing them on you, though - say every 3-4 days for a few or several times.

@@stevenzphysio4203 I didn’t realize how common my next question was until I tried to research it and still have found no answers. Steve, do you know why it is that pain from Costochondritis kind of moves around? Like this morning I woke up with rib pain behind the both side breast tissue and off to the right side of my rib cage and along my right lats. Whereas yesterday it was mostly my upper left rib cage and sternum. The day before that it was mainly the bottom portion of my front ribs. Some days it’s only irritating pain next to my right clavicle and scalenes. I don’t recall doing anything different on any of these days I’m always doing the same stretches and the same minor workout of the upper back. Along with the back part of course.
@stevenzphysio4203
@stevenzphysio4203 2 жыл бұрын
@@stephaniegutierrez6222 Hello, Stephanie. You can get that. Frequently, the whole rib cage is tight, the whole middle and upper back tight and hunched, and even the collar bone and neck tight. You're working away at remodelling your whole upper body - getting it back to being strong in the bits it had become weak in, and erect and balanced like that. It takes time. Often one bit of the whole thing will have freed up a bit more, so you feel it more at the bits that are relatively tight. One general thing that happens often is that as you start to free up in the rib and spinal joints round the back, and sit straighter and move more, you get a pulling on the still tight muscle and soft tissue around the front. So this can get sorer, temporarily. (Pec stretches help this bit - see Bob and Brad's video on this: kzfaq.info/get/bejne/iJtmm6x-3t3IpWQ.html) In the same way, as the joints round the back free up on the Backpod, you'll breath in more and move more, and often this means you get a bit sorer in the shortened muscles between the rib, temporarily. So - just keep going, with the Backpod and also all the bits in the Backpod's simple home programme in its user guide. Don't worry about twinges here and there - it's all part of the whole unit working itself back to the full range, strong, erect posture it used to have. It'll get there.

@@stevenzphysio4203 Are the upper traps in pain from being short & tight or in pain from being weak even if they feel tense & tight? I have conflicting answers with my PT and chiro. So I have been going to PT for a few weeks. They stretch my upper traps and attempt to strengthen my upper back muscles however with nearly all the rhomboid ( & that area) strengthening exercises, my upper traps and neck start to burn. They have a hard time zeroing in on my upper back without the upper traps burning. Does this indicate my upper traps are tight or are they actually just long and weak instead and thus need strengthening as well? How can we tell if a activated brick like upper trap is “tight” or if it’s actually weak instead? Is there a test for this? I’ve had a couple of physical therapists mention that traps are rarely tight and usually weak instead and need to be worked instead of stretched yet my PT stretches mine and never has me strengthen them as of yet.

Hi Jan. Costochondritis. It's a dull or stabbing pain at the front of your chest, where your ribs join onto your breastbone. It's not the heart, lungs or anything else dire. It's the front rib joints straining and giving because the rib joints round the back are frozen up and can't move. The Backpod's relevant to treating it because it's about the only thing around that will do an effective stretch on the tight rib joints round the back. It's often explained as a "mysterious inflammation" at those front rib joints; however the actual medical research says it's not.
