Thanks for the info. For two weeks now I’ve not been sure if I’d developed a bunion, or gout. Now I feel it’s neither as your description matches my conditions. Appreciated. Will look into this further.

I'm a recent adopter of Hoka running and walking shoes for this problem (hallux limitus). It is wonderful to walk without pain for a change!

Hoka didn't do anything for my hallux limitus.and I was hopeful after submitting to their price tag. Glad u had luck though. Going to get a silicone joint in a few months.

@@sarahw.7668 I have to say that 8 months on I'm slightly less enthusiastic. They do work but I've given up running again & don't walk more than about an hour on smooth surfaces. Best of luck for your operation!

Further update: 13 months after I bought my first Hokas I've bought more. They are not the most robust of shoes & the soles wore down fast in 600km of walking (the running shoes have maybe 300km on them & are still ok - I've not been running for other reasons) but the fact that I can still walk up to 10km/6 miles at a time without crippling pain is fantastic.

@@HughDWallace I was recommended HOKA but also Brooks Adrenaline GTS 23 or Brooks Ghost Max for my hallux limitus. Bought the Adrenalines, comfortable so far but haven’t tried running yet. Do you expect the problem to remain permanently? Or has it eased up at all ?

I found out a few months ago I have 100% arthritis in the big toe. The best advice I got was a rigid shoe. My job is walking all day on inpatient wards and at my age will not find another job. After lots of research and a chat with the podiatrist I found the Hoka shoe is the best. They are quite expensive but do give relief. I found the range quite good, apart from work shoes they also cater for sport, sandals for summer too. Worth a try I promise.

Yes I absolutely agree with you. I love Hoka’s! They are great for several different foot problems and definitely good for protecting an arthritic toe joint.

Until other joints start to hurt like knees, hips, shoulders, neck, back... Stiffness causes more problems in body.

Thank you for your great information to explain the problem and helps. I climbed Mt Timpanogos in 2011 and ended up with lots of inflammation and jamming to my toe. Massaged in Christophers Complete Tissue massage oil every night for six months, which nearly cured it. Been on my feet a lot lately, so it seems it's time to get back to this massaging. I'm convinced massaging is one of the best ways to get toxins out of any area--plus great rehydration, like what Dr. Batman teaches. Thanks so much for being a part of the solutions.

does putting a pad in middle of metatarsal area also help to keep pressure off big toe and does a spring plate still allow you to bend your toes just less

Hello the metatarsal pad does not really keep much pressure off the big toe and it’s more intended to restore the transverse arch. Spring plate does have a slight flex to it, so yes your toes will bend a little bit but not much. Getting a stiff soled shoe with a nice thick lug sole is a great way to protect your turf toe.

@@Biomechanicphysicaltherapy Thanks, I am a tennis player in south florida been sooo hot, finally cooled down, must play, my favorite shoes probably too flexible, dont feel any big toe issues with my stiffer tennis shoe after I play but not as comfortable, heel too stiff, put in a softer heel cup real thin though, want to try stiff insole in shoe I like more. I dont drink alcohol, if it was gout it would not just flare up after exercise?

Is the limited range of motion permanent? How can one increase their range of motion of the big toe?

It does not necessarily have to be permanent it depends on how much arthritis and bony overgrowth you have. You can stretch it, see the video on bunion recovery and it is the same stretch as is there. You only want to feel a stretch in the foot you do not want to feel any pain or pinching at the toe joint. If you feel pain in pinching it could be bony abutment.

I saw I had early signs of this & I stopped wearing small box shoes since they compress my left foot so bad

A stiff soled shoe with good shock absorption will help protect your toe as well.

Hello, thank you for this very helpful video. I injured my big toe about 4 months ago in sort of a turf-toe manner, following a skating accident. It does improve and I'm able to run and skate again, however the shape of the toe is deformed and it seems like the bone is sticking out a bit, like a bunion. I feel like the range is a bit more limited than used to be before the injury, and at certain angles it is a bit hurting still. Even though as a whole it does improve I'm afraid the it won't heal completely. I have an appointment for a physical therapist who specialises in feet, and hope they can help me there. Is there any advice you can give me? thanks in advance!

After thinking about it overnight I do have a little more advice. Beware of exercises. Do not let them give you heel rises and such. Hill rises and plyometrics would not be valuable and could actually make things worse at this point. Exercises really are not indicated most likely in this case but physical therapist just seem to not be able to help them self sometimes! 😂

@@Biomechanicphysicaltherapy For the first two-three months I did some exercises as advised by my physiotherapist and I think they helped, to an extent. But now I'm already four months deep and in the last few days it began to hurt a bit again. I already thought I overcame it (except for the deformation that still exists). But now looks like I haven't.. I really hope I won't need a surgery :( what do you think the feet specialists will do? would insoles might help? (I also have flat feet, but as I said the hallux valgus-deformation was caused by a sudden trauma and not due a gradual deformation.)

1. Gout is a crystalline deposit in the synovial fluid of the joint (diet related). It is painful, red, swollen and episodic, usually no known injury set it off. The toe position is not changed from baseline with the onset of this pain. 2. Turf toe = hallux limitus or rigidus and refers to loss of range of motion to differing degrees. Usually the person knows when and where the damage happened. It will be red and swollen at the initial injury but later on will just have limited range of motion and be sore if it is made to work. It is due to repeated motion or impacts and can be alleviated with wearing a stiff soled she. 3. A bunion is an angulation of two bones in the foot and toe and although it can be painful, red and swollen, it has an obvious angulation. It is due to the foot structure you were born with and can be corrected with treatment.

@peekaboots01 yes podiatrists know what to do with this problem but podiatrist have different surgical allowances in different states. So it depends on where you live whether a podiatrist is allowed to operate it on on it. You could also seek out an MD who is a foot and ankle specialist and they will be able to correct it for you. It sounds like yours is severe enough based on your description that you would be a surgical candidate. Although it would hurt for a little while you would be much happier when it was all over and no more bright red ball on your toe. Orthotics can help a little bit right now but since it sounds like it's very severe. I'm not sure how much. The orthotic must have a forefoot post and a first Ray cut out with fill in and you can tell your practitioner that. Orthotics will help with recovery afterwards and will give you many years on your new toe. If you don't wear orthotics after the surgery, you risk it gradually returning because the reason for the bunion is your foot mechanics. The orthotic fixes the reason why you developed the bunion in the first place, but it would take five or 10 years for it to begin to reform.

@moojeni it can be managed. First two things, decrease inflammation (NSAID of choice, by mouth, topicals, ice) and decrease the mechanical irritant (wear very stiff soled shoes all the time, no barefoot, try HOKA sneakers for running, pick the stiffest sole you can and something like Birkenstock's when you want to be barefoot). If you can get to a physical therapist who does cold laser and iontophoresis with Dexamethasone that will work dramatically. If your achilles is tight it can put more force through the forefoot, so stretch your achilles if it is tight.

@@Biomechanicphysicaltherapy Thank you very much for your answer! So even in the hallux limitus or first stages of hallux rigidus it's advised to limit the joint movement? I thought that one should work in maintaining the range of motion as much as possible through PT exercises and maybe a custom insole with reverse Morton's extension. I've tried Hoka Bondi shoes but they didn't relieve my hallux pain and they gave me all kind of pain everywhere else in my feet because they felt very unstable. Maybe I should try a shoe with a carbon fiber plate?

@@moojeni Yes the carbon fiber plate is a great idea. The problem with your hallux limitus is that unless you just have inflammation you also have bony overgrowth. I have a little bit of it and I’m perfectly happy and run and can even go barefoot now because I wear stiff sold shoes when I’m wearing shoes. Try just getting the inflammation down first. Maybe you don’t have any bony problems. The Morton’s extension is to protect the big toe. I’ve never heard of a river is Morton’s extension. But no exercises are recommended.

@@Biomechanicphysicaltherapy Thank you very much again. I have been suffering from pain in both of my big toes for the last 4-5 years. Until a year ago I didn't have any bone spur at all, but I have developed one on my left toe (very small). The X Rays don't show any significant cartilage loss (they don't even mention in on the X Rays report) and I also got MRIs, which only show mild inflammation on the first MTP joints and surrounding tissues. The thing is that I have constant pain, on every step I take, and have been like this for at least the last 2 years. So far I've tried Hoka shoes, a cortisone injection on each toe, custom orthotics, 6+ months of PT (exercises + magneto therapy), chiropractic, foot shaped shoes (Altras) + Correct Toes, and nothing worked. Now I've been trying spica taping with KT tape which somewhat helps to stabilize the joint, and I only have left to try shoes with a carbon fiber plate, or another stiffer sole shoe that isn't a Hoka. I feel very depressed because until 2019 I was still able to run marathons and now I can barely walk, and with horrible pain. My gait has also suffered and I feel much more unstable because subconsciously I try to avoid putting all the weight on my toes. Having said that, I still have normal range of motion on both toes, and no doctor has talked to me about any kind of surgery (I guess there's still nothing to operate).

@@moojeni I notice however that you have not done the two things that are the most powerful treatments that I feel work the best for this. Cold laser not just a light bright. Must be nonvisible red spectrum cold laser. Preferably class 4. And Iontophoresis with Dex 2-3 x wk. Not the injection. If none of this works then toe fusion is the next phase. You can run on a toe fusion after a year or two.

Nice presentation but Kenalog (triamcinolone) is indeed a steroid. We use it often for joint and dermatology injections.

No one ever addresses how unsightly this issue is. It's embarrassing. Who wants a deformed foot? Even when it's not totally painful because sometimes I I go for long without it flaring up but it still looks like I have a red ball on top of my big toe joint and it feels so bony when I touch it. it's really ugly. ☹️
