Don't miss or overlook the REAL source of your buttock pain. I talk with folks every day who think they have Piriformis Syndrome, but they don’t. They’re actually shocked to find out that their BACK is really the problem/cause of their buttock/glute pain.
Now, you might be thinking, "But Dr. Charlie, I don’t have ANY back pain… my pain is literally just right here in my butt cheek - I know it's got to be Piriformis Syndrome."
I’m just telling you, EVEN if you don’t have any back pain at all, stick around until the end of this video… because chances are still high that you’re missing something...
I walk you through some simple tests you can do at home to RULE OUT or make sure that your back isn’t the issue.
But first, here are a couple of key tips you should know:
1. Upstream Problems Cause Downstream Effects
Where you hurt isn’t always why you hurt. Just because your butt hurts, doesn’t mean you have Piriformis Syndrome or a problem there in your butt. It could be referred pain - which is when you feel pain in one part of your body, but it’s actually caused by a problem in another area of your body.
The most common place of referred pain from a problem in your back (such as a joint or disc that has been injured or aggravated in some way) is the buttock/glute region... followed by your hamstring, your calf, and even your ankle or foot. And what’s crazy is that you may feel no pain in your back (at the site of injury) and all of your pain in your butt or leg. If you miss this key piece, you could find yourself undergoing treatment for what you/they believe to be Piriformis Syndrome = injections in the butt, stretches for the buttock/glute, deep tissue massages- you name it… all aimed at the WRONG target.
Now, what do I mean by "upstream problems cause downstream effects?" Well, your back is the most upstream spot and it can effect ANYTHING downstream. But a downstream problem will not go against the current and impact something upstream.
To make this super simple- a back problem can refer pain to anything downstream below it. It can cause butt pain, hamstring pain, knee pains, calf pains, shin pains, heel pains, toe pains… anything. But a problem in the butt (injury to the tissue in the buttock/glute) does NOT refer pain upstream to the back. A hamstring tear does not refer pain upstream- to the middle of the butt or back. A calf strain does not refer pain upstream to the hamstring, the butt or the back. I think you get the point.
2. Consider A Common Denominator. It’s common for people who I chat with to have MANY different diagnosis. They feel confused, because they chat with multiple doctor who give them different labels - Piriformis Syndrome, sciatica, IT Band Syndrome, hip bursitis, then the foot doctor tells them they have plantar fasciitis, too. But what are the chances that everything falls apart at the same time? NOT high.
So, if you’re experiencing some form of butt or leg pains (pain in multiple areas), is it possible that ONE thing could be causing all of your pains? Absolutely. There could be ONE problem at the root of it all - a common denominator or single thread that unifies and explains everything- all the symptoms you’re feeling. To be clear, it’s usually upstream- the back causing referred pain into many different areas, or a sensitive nervous system (sciatic nerve or brain) that makes you feel pain in various body regions.

Hi Dr Johnson, I'm a big fan of your work and on behalf of all the people with back pain I want to thank you for your help. I just tried the exercises you have shown to determine where the pain is coming from, all of them were fine until the last one which you twist and bend backwards. Got a pain in the nerve straight away , running from my buttock then jumped to the side of my calf , also on some of my toes. Since it seems to be the back, what can I do to get this sorted? Any help would be much appreciated. Regards Antonio

Now, the goal is to live in action and follow a step-by-step process to eliminate your pain. Step 1- we must optimize your physical and mental healing environment- ID habits, routines, and inputs that are contributing to your symptoms and keeping things from improving. Also, beliefs, fears, worries etc that could be placing you on high alert. Step 2- Collect a baseline and begin measuring key metrics that will allow us to know what we need to focus on and will reveal if what we're doing is working or not. You have the best and most straightforward way of passing along information, and have steered me in the right direction. I was able to use your Diagnostic video to realize my hip pain IS referred pain, understand the possible causes, and seek an orthopedic eval from a back doctor rather than continue with pain mitigation. He was even impressed with my knowledge and I told him it was because of you.

Your channel is really helpful. Just started as entrepreneur in sports and common health field both in swim related things as primary focus. Joint pains and muscle pains are really common , like 90% of our customers come to swim with these problems and they do alot of water aerobics (like myself also) to heal, build up, hold healthy muscle strenght and do sports in pool or gym of the swimming hall.

LOVE all of your video's, you explain things that even the layman like myself can understand, your analysis or direction of how to break it down and lead one to the actual cause helped me recall something. I do have the butt pain, have had it for months, even noticed of late that it was felt in the other butt briefly. You see, I had a neck injury when I was younger (1960's) from a soccer game were I was knocked out, I recall having a big lady (Ursula) using red (heat lamps) and massaging my stiff neck to try and unlock my neck. I was away from home for a week away, i.e. a school excursion. I have always had a bit of a stiff neck since, but don't really notice it unless I turn my head side to side, which I can do, but it is stiff, so slowly as I go to get it to turn to the full side, but the core area I believe is next to my right shoulder blade, feels as if something in there has snapped, or like I was stabbed and the blade broke off in there and has remained. Time to get an X-ray and then go from there, because as you say, pain travels down stream and when I did the push back movement with the hips, I felt the knife in my right buttocks. @@drcharliejohnson No, THANK YOU, it's because of you I am pain free in my buttocks/hip/thigh area now, when I was previously thinking I can't take this pain much longer. Your different approach to others, i.e. look elsewhere sorted it for me. I am now back to where my original back/neck pain/stiffness, if you want to call it that. Some exercises and cracking helps to sort it for a while, so I can live with that, have done for decades. Got to pull my finger out and get that X-ray one day. Once again, THANK YOU.

In 2018 I hurt my hip and thigh ligaments et cetera by invertently doing the splits with one leg backwards. That laid me up for a year, followed by a year of Chiropractic which helped a whole lot. Then, a year with a physical therapist specializing in hip pain. Nothing changed. I'm now scheduled for an MRI . I almost don't care what the cause is I just need to know.

I did your workshop today enjoyed it very much when I did those 5 motions the one where u lean back and then lean back to right and to left those are very yucky movements caused a lot of pain instantly couldn’t do it very well so it did make me see what motions are not good thanks for the info and advice u give

I trust my gut more than my brain. When i felt pain I knew that although I'm physically active, its very specific movements and I know I had a weak core and upper body. I had lousy posture when I did sit at the computer and in general took my health for granted. I've kind of given up trying to figure out what the hell the pain is from, although an x ray is finally coming up just to make sure its not a tumour or something, but strength exercises are what has worked. Yesterday I did my first 'aerobic workout', which may have been a bit much and because I had to drive this morning and my lower back had me in enough discomfort that I was thinking about taking pain pills. Then I did that warrior move for about thirty seconds each leg three times each, and boom, pain gone. The body is an amazing healing machine-when you are trying to heal it rather than hurt it. I really feel for people who have to sit at work all day because I kind of think sitting is the root of all evil.

@@kathleensarmiento932 Yes, I also reformated my diet, and because I was almost ALWAYS in motion, I was also almost always eating or drinking. I amped up the exercise because as I was feeling better I wasn't doing as much, so SOME of my fat came back. Human nature is probably just as bad as either of those, as you feel better, you start to take things for granted, then maybe skip a day of exercise, which is never a good sign. But last night I slept ALL through the night, which is almost a first, and I could put on socks easier than ever before. Which probably means my brain will start saying "ok, your fine, no need to exercise anymore and might as well eat that cake!". THATS the real enemy!

I was pleased to find your old video describing exercise for piriformis problems. It helped quite a bit but didn’t completely help with the niggling knee and foot pain. A few days ago I noticed that when I got in from gardening chores - olive harvest time here in Italy - or if I’d been sitting a while, I tended to slouch. Partly because I was tired, partly because my back felt achy. I started to monitor my walking and made sure I was walking upright, shoulders down and back, even leaning backwards very slightly. After three days of this I’m completely pain-free. My problem was poor posture! Easily fixable. I imagine that lots of people fall into slouch mode when they’re tired or feeling a bit down. Does this make sense?

My pain is definitely caused by my back. I have moderate to severe pelvic pain, hip pain, lower back pain, pain down both legs(not sciatica type pain). No sleeping position is comfortable for me either. I had a spinal fusion in 2013 & it only made me worse plus caused a myriad of other pain for me.

Absolutely true, I ve had Buttock pain for nearly a year, turns out I gave herniated disc. No pain in back for 3 nonths, buttocks pain continues. And it throws my si joint out, constantly clicking when I stand up straight and move my pelvis

To be honest... thinking about past events... even doe I don't have a lower back pain, I think a disk might push against the nerve and cause my downstream pain, left left, just under the but line making me thing it's a Piriformis problem. When I lean over, north, in front... I get a little back pain on the left side of my spine, lower back, where most of the disks causes pain on the nerves. So... you made me think... thank you!

I had bilateral piriformis syndrome and my sciatic nerves were covered in scar tissue and the vasculature going to the nerves were also compressed and the nerves were severely damaged from my piriformis that had become fibrotic and crushed my sciatic. Surgery was the only thing that helped me get my life back. The longer I waited the more damaged my nerves became. I even had foot drop from severe piriformis syndrome. The severe compression of my sciatic at my buttock caused a compression lower down (double crush) at my common peroneal nerve which also needed to be surgically decompressed. I'm glad I didn't wait any longer after tons of people telling me it was my back. I'm a doctor myself and I wasn't being heard by anyone. If it feels like your sitting on something very hard and have nerve pain. Get evaluated by a peripheral nerve specialist for piriformis syndrome and get piriformis blocks and an MRN to confirm.

If you started having loss of nerve function (foot drop)… for sure that’s an indication for surgery.

I have had this for over 6 months. My right leg just isn’t right and both sides of buttocks hurt exactly at piriformis. I hit my left butt really really hard on a doorknob about 7 months ago. I can’t run anymore and it’s super depressing

Great info about so many muscles, tendons, joints and pains but so far I have not heard you mention the Psoas muscles. Is there a reason why it gets little, if any, attention in your videos?

Hey! So, to me, psoas muscle gets WAY too much attention. Several times a week I speak with folks who have tried the “hip hook” and “pro-rite” only to end up with a bruised belly and ticked off colon! I’m honestly not sure how much of a role the psoas muscle plays in these issues… but my experience/gut tells me- its not NEARLY as involved as people think it is. Appreciate your comment

Fell badly on right buttock 5 years ago and have right mid buttock pain. Seen chiropractors and physios but nothing has changed. Have tried your side bends front and back bends to see it it’s the back but they seem fine and don’t produce pain. Should I exercise through the butt pain I.e water walking or can you advise?

I have Muscle spasm because of Straightening of the spasm as per the digital xray. I have L4-L5 disc dessication, diffuse disc bulge. Superimposed disc protrusion. L3-L4 mild disc bulge for MRI result last april and everything has no pain at all. Just last august. When I drive for more than 2 hrs or 3 hrs then I lift my heavy bag more than 30 kg my back crack and pain is more on the buttok. This is going for 3 months now and tingling on my foot. I am doing now PT and dry needling with electrocution. Doing also excercises but sitll like nothing is changing. I dont have pain seating. I have pain when i walk too long and stand for long.

Totally agree cause from my lower back now it's affecting SI joint hip bursitis piriformis. I like to know can symptoms also cause Nausea Mirgraines and vomiting?

I have low back pain that the PT gave me exercises for which helped some but at the next visit she checked my upper back and found it to be way tighter than my low back and added exercises to increase the flexibility in my upper back where I have no pain, but it really helped my low back. Sounds like another down stream senario?

Ho waw. No one said there are two type of preforming syndrome. Stretching and compression. I did lot of exercise so I got priformis syndrome. I did lot of Stretching I got nothing better. With your advise I stopped streching now I am doing compression. Relaxation exercise .I am feeling better at 65. Cat walk reduce my pain. Thank you.

I’m in excruciating pain and already deal daily with pain from an accident. Metal hip, femur, wrist. I’m having extreme nerve pain. It’s debilitating 😢

I was in a bad accident in ‘20.Had a microdiscectomy for a 11mm herniation. Kept telling my doctors my butt/hip hurt so bad. Pain in back/butt/leg never eased up after surgery . Now three years in

Thank you for these informative videos I wish to ask you about the upstream problem can make the patient feel a downstream pain, but can this back problem cause tenderness as well in buttocks or in the thigh in addition to feeling of pain or the diagnosis will be different if there is a local tenderness in buttocks or thigh

Doctor llevo un par de semanas con un dolor a la altura del coxis, solo ahí no se radia hacia las piernas, ni hacia arriba. Lo curioso es que ocurre cuando duermo y me despierta el dolor, pero durante el día funcionó bien, ni siquiera me acuerdo que me dolía esa zona. Qué podría ser? Me gustaría su opinión por favor, aunque finalmente creo que deberé ir al traumatólogo....

I’ve had problems with lower back pain with hip and hamstring calf pain is bad that it is hard to stand walk or sit.. for 6 months after massage and 18 visits with chiro and doing stretches for hips and sciatic nerve.. I stoped everything and in 2 weeks I’m able to walk somewhat better and stand at work .. my question is why do I wake up I night with left thigh burning like hell and it both thigh muscles just above knee numb?

Hey! Need to do more detective work… could be for several reasons… either related to your daily/nighttime routine or sleep itself. Also, could be that the absence of external stimuli creates focus/preoccupation on the pain.

Can back pain or even sciatica cause morton's neuroma symptoms? I know I have decent mobility and my back always feels like it aches and is somewhat stiff throughout the day even though i do exercise a moderate amount and I am a active person. I am also 99% sure that i herniated a disc back in highschool while deadlifting, poor form I am sure. It felt like a squirt and liquid in my lower back.

Hey Andrew- good question. So, only way to prove it (with as much certainty as possible) is to establish a link between movement of your back and the "mortons neuroma" sensations. For example, if you move your back forward, back, left, right, twist etc... can you clearly change the symptoms in your foot for better or worse?

I think mine be part of the psoas but the pain literally starts at the butt crack on the left with pain coming around the hip to front. Im doing your exercises to help shorten it but still here 😢

@@drcharliejohnson yes im seeing a Gonstead method chiropractor because reg chiropractic wasn't helping. This has helped some. He said my SI is tight . Always cracks but no real relief yet. I did try a different acupuncturist today. Chinese and I feel that may have helped some. I've had a tummy tuck and the acupuncturist today told me she feels my pelvis is locked up. My friend did your exercises and her pain is gone. I do do them and I do feel some relief but it's still there, unfortunately

Hi Doc , your videos are really helpfull !!! . I have an herniated disk L5-S1 with a referred pain in butt and my right leg . I tried to do your movements but i don't find yet what increase my pain :) . Once i found it i suppose i should avoid it ? . For now i tried extension and flexion , but i can find a real difference due to pain. What make me crazy , is in a week , i can have less pain during 2 days and a flare up next day , and i can't identify why ??? . Could you give me some tips ? Hey Alejandro! Can be difficult for some folks to identify clear yucky motions. Not uncommon. Also common that symptoms come and go- without clear reason. I hope you can understand- honestly it’s difficult to give a specific recommendation without knowing more details. PAIN x8 in the piriformis muscle, no back pain. Pain also on side of knee and calf. Waiting for the ball to drop on the achilles. Can you tell I've also been diagnosed in the past? Ugh. I do a very physical job including some heavy lifts. I've been told my piriformis muscle was (now is again?) overdeveloped. This seems to be at odds w/what you said in another video re: lengthening verses shortening/contracting the piriformis. I'll try those exercises, though, 'cause it hurts like a mofo 😂😭

Did you get to the bottom of it? I'm the same as you pain--wise, but when I did the seated figure-of-four test, the leg on the affected side would go down noticeably further than the other side, so I was wondering if the piriformis is too weak as-per the video you're referring-to. That *is* my weak side generally, after all (biceps, quads, hams, all slightly less developed/ weaker than the other side).

Hello Dr Charlie, I am already aware I have l4 l5 disc prolapse with sciatica & buttock pain when seated, however you have never once spoken about nerve testing when they attach equipment to legs to test response - results stated I have l5 S1 problem indicating piriformis syndrome? Hey Riky! I believe you’re referencing EMG/NCV test… where they shock and poke needles to test nerve function! Very good tests… and so, I’m not sure if.how it was conducted- but question becomes is HOW does it change course of treatment? If you want surgery/injections- then possibly could give some guidance there. But if you’re still trying to resolve this naturally then it goes back to following a pain relief process and systematically testing to find what motions you can use as medicine. Make sense? Are you open to some help from me- or do you have a plan?

Thank you so much for the response Dr Charlie 👍 I'm waiting till I'm in a better financial situation before I can commit otherwise it would be a no brainer, in reference to your video...what I was trying to rule out was it's most likely piriformis syndrome for me as none of the lower back tests you suggest trigger my issues, it's as soon as I sit down or walk long before I go into buttock & sciatica pain - PS I do have L4/L5 disc prolapse on right side pressing on nerve - And yes you're right the needle leg nerve testing stated a L5/S1 problem piriformis Once again your clarification would be appreciated 🙂👍 Hoping early next year I can join your plan...Riky 🤗❤️

You are your best medicine (mind and movement). How you move your body, the thoughts you choose to accept regarding your pain... all things that can act as either an anti-inflammatory or pro-inflammatory factors.

@@370Zach good question. People with disc problems may respond to different things… some love forward bending… others like twisting… others have terrible pain standing, but love sitting. Presentation can vary. Your call- but I would NOT see an orthopedist if you mean that in the traditional sense of orthopedic spine doc. Unless you have loss of strength, feeling, or bowel/bladder loss- remember that they specialize in shots, pills, surgery. If you don’t want those things, then see an board specialized orthopedic physical therapist who can help you heal naturally. No pressure, but are you open to potentially working together? @@drcharliejohnson thank you so much for the diligent response. I’ve been dealing with this pain for nearly two years now. One day I was barbell squatting with my warmup weight and at the bottom of a rep as soon as I began to come back up I felt a “pop” in my right glute and have not had relief ever since. Sitting down is fine for me but after about 30 seconds of standing my pain starts to flare up. I will follow the link you sent me, thanks.

YES Jen! Disc problems often cause referred pain to butt and “SI joint” (but it’s not really the SI Joint) as well as hip and anything below. 👍 What are you currently doing to solve it?

What about a side Bend I can stand straight up and side Bend to my left no issue side Bend to the right where I have the pain and it shoots pain into my buttock and calf. I guess that's positive for back

@@gri7 all good man. So nerve problems may cause burning… but also other feelings- like tingling/numbness. If symptoms all the way to calf- likely some degree of nerve involvement- but not necessarily. If you have what you’ve described- pain moving back… AND a + straight leg raise test (sit and raise bad leg carefully- ouch!) then sciatic nerve is involved.

Hey Charlie, I got my mri results back to day nothing is wrong with my disks. When doing these movements my right sided sciatica pain gets really bad while leaning to the right side/leaning forward to the right. I have noticed in the past that there was some curvature in my spine(like my left shoulder being higher when I try to stand straight. Also when I try to relax my right hips wants to lean to the right making me stand very curved. Hope that makes sense, any tips?

Can pudendal nerve pain be caused by a herniated L4/5? Butt/pelvic floor burning to be specific.

Good question Angela- so, quick thoughts. The Pudendal nerve is made up of branches from the S2-S4 levels... So, technically not a super clear correlation there. Unless the herniation is broad enough to impact things at the cord level? Now, disc problems at this level can cause pain in buttock... Hope this makes sense. Are you currently doing anything for treatment?

@@drcharliejohnson thanks. Upon mri i have S1&S2 affected but not as bad as L4/5. My pain is sitting/standing, but once it starts it doesn’t go away with movement easily. It may go away or may not. I have had lower back pain on and off throughout my whole life even as a child. Went to chiro throughout my life. Then in my thirties started having bladder /pelvic floor pain. Seemed to start after i started power lifting. I am of a bit more lanky build, not really typical powerlifter build. I have tried pelvic floor therapy for years, massage, chiro- nothing. I have been seeing a sports medicine doc that has been injecting my SI joints with fluid as they were completely dry. That has been about 4 times now. I will be seeing someone else for L4/5 steroid injection in a few mnths. I live in Canada, so ya there is that. I have been currently wearing an SI joint belt which helps a bit. I have been doing some clam shells, bridges, fire hydrants. But sitting will start the but pain and then it will radiate into my crotch/pelvis. Like a cascade of hot water. Much burning sensation. I just started back at the gym a few mnths ago after taking 13 years off because of pain made worse by exercise, yoga did not help. Infact an instructor had asked me once if my back was fused together because it looked so stiff and rigged. I am doing upper body stuff and some of the lower body i mentioned above.

I would think that most orthopedists would be able to perform simple tests and determine if the pain is being caused by a back problem, like herniated disc, even though there isn't any back pain involved and the problems are "downstream" only. My ortho certainly could tell me. Do other people not have the same experience? He couldn't tell me how bad the herniation was or exactly how long it would take to get better. In my experience, there didn't seem to be any way to promote healing from a herniated disc, which was challenging since I'm an active person. It was mostly just a long and painful wait for me. I moved as much as possible, but needed a lot of patience.

Fell badly on right buttock 5 years ago and have right mid buttock pain. Seen chiropractors and physios but nothing has changed. Have tried your side bends front and back bends to see it it’s the back but they seem fine and don’t produce pain. Should I exercise through the butt pain I.e water walking or can you advise?

About 3 months before the fall I was walking at quite a pace with my husband and there was a loud popping sound in the hip area (which my husband heard) and then I suffered sciatica immediately. The sciatica settled after about a month. Now after 5 years later it seems to be a bit all over the place - sometimes in the SI joint then I will feel it in mid butt then it feels like it’s high hamstring. We have a small indoor pool so I do water walking in it. It does still give pain after I walk in the water but maybe doing too many steps! I am sleeping on left side only as it is painful on the right side. If I am weight bearing I feel it predominantly on the SI joint. The chiropractor said the SI joint still had movement. We had huge emotional stress 6 years ago when we lost our daughter through medical negligence (which happened 9 months before the fall) I understand that the SI joint is the emotional joint could this possibly be connected? I am totally at a loss how to fix this.
