Ketogenic diet has become a center of attention for strategies to improve symptoms of Alzheimer's dementia, with some individuals claiming reversal of the disease. But what does the science really show? Is it time for us to implement a ketogenic diet as a potential "treatment"?

There is a tsunami of diseases of the brain such as Alzheimer’s, stroke and Parkinson’s disease permeating our culture. In our own communities and families, we all have known at least one person suffering from these illnesses and in many cases seen the fallout first-hand. There is no treatment for these diseases, and the emotional, financial and social burden is immense. These diseases are thieves, stealing time, money and ravaging the minds of our loved ones. Lifestyle change is the only answer.

I do not know about everybody, but I am thriving on keto. No it was not about weight. I always been of normal weight. I beat pre diabetes. I have been on it since 2021, my 67 years old body feels good not many pains and aches. So far, so good, and I intend to be on it for the rest of my life. I do not care about ketosis good if I reach it, I do not track anything, but my body is appreciating the way I am eating, and I also do intermittent feeding with twice a day because my weight can drop too much. Besides vit d and b12, I do not do supplements. It is all good, real, wholesome food from nature, no internet keto. II do not care about science anymore. I care about my health, and have finally learned to listen to my body, and my body is not complaining it actually loves it. Actually, occasionally I deviate a bit, and my body right away lets me know, I need to stay put. Cholesterol LDL has always been high since I remember, but my HDL/tri are very good and CAC of zero. I do not know about everybody else, but my experience has been very positive on keto. When I was on the SAD diet, my body was breaking down faster. I was mostly eating greens (I love my greens and eat them still as part of my keto), low fats, low everything, and I was sicker.

Excellent post. My experience as well. Used to be almost vegan but was getting sicker and sicker. Went partial keto and became a lot better. Went full keto 6 weeks ago. My body thanks me daily. I’m 70. My arthritis went completely into reverse, now pain free. That was unexpected and well welcome. I’m eating this way for rest of my life.

One striking point about people in south Indian states of Kerala is that Alzheimer's is not at all prevalent in its senior citizens. The people here use a lot of coconut oil and products. It seems a case study in the people there may bring out amazing facts about coconut oil's efficacy in mitigating Alzheimer's.

I admit to being caught up in the very powerful trend of demonizing carbs of which the ketogenic diet is the most extreme example. However, I really appreciate that you both announced your biases and I believe you are committed to evaluating the data accurately. That helps me listen to information that disconfirms my current beliefs, and that's quite significant. Well done. I find you both to be compelling, benign and ethical.

Your discussion combines two very different things: 1)The general population and 2)The population that already has Alzheimer's. The study on Alzheimer's patients comes back and says the reported quality of life is better for patients... and you say that doesn't matter. Their reported feelings are insignificant The study is too small ... Really? Alzheimer's is a terminal illness. What does one have to loose!

I cannot help but believe that a diet that requires supplements in order to to combat the bad side effects of said diet, is a diet that is not balanced. And most would agree an unbalanced diet is not a healthy diet, long term. With this said, I do think there are variation between individuals that would determin what a balanced diet is for them. And even that can vary to the individual, depending on their age and lifestyle and the environment that they live in. In winter, I eat more animal products and fats because it keeps me warm. But come spring and summer I dive into the fruits and vegetables. They are in season and fresh and I crave them like nobodies business. Lol I do eat fruit and veggies all fall and winter too. Just not as much as spring and summer.I could not survive on keto full time. And I cannot survive on just plants either. I tried, and it was awful. I must have some of all of the foods to feel like a healthy humans. Stopping fast foods and processed foods has helped me the most.

Wouldn't the simplest way to study whether a ketogenic diet helps prevent Alzheimer's Disease be to study the incidence of Alzheimer's Disease in epileptics who have been on a ketogenic diet? And to compare that incidence rate with the incidence rate in epileptics who have not been on the diet? Aren't epileptics a ready-made population, that even comes with a control group, that could be studied on the question?

Why is Alzheimer’s referred to as type 3 diabetes? Is diabetes not a result of high blood glucose/high insulin levels? By using my glucose meter for three months I have isolated the foods that caused insulin spikes. For me, it is sugar, all grains ( whole or not), pulses, and rice. Don’t consume those anymore. I heard in this podcast that you encourage whole grains. Consequently, type 2 diabetes, at least, would still be my biggest metabolic challenge if I followed your guidelines. I do know that glucose/insulin spikes are very individual. Four people, each eating the same size and kind of bread, 4 different glucose readings.

It's the animal fat that clogs the cells that then makes these cells not react to insulin. If you consume whole food plant based unprocessed carbs (rice, beans, etc.), this is completely different & you should be ok.

I totally agree with you. That animal fats clogs the cells is ridiculous even JACC Jornal of American College of Cardiology has published a white paper saying basically the association of Saturated Fat with CVD was wrong and is outdated. Telling people with T2D or TD3 to eat legumes or grains is pretty much inviting their diseases to progress and up the need for medications. I’m quite frankly shocked at this recommendation.

Please forgive if my opinion is intrusive. I have done so much research on Keto for both healing AND weight loss, and am far from expert. However, there are so many experts on KZfaq (doctors and nutritionists and people who have had success) who believe that the body will heal first what most needs to be healed. The fact that you have reversed cancer and kidney disease is a HUGE success! I do think that if you can be patient and stick with this way of eating, and perhaps continue your own research to find ways to tweak the diet a bit, you will eventually lose the weight. Don’t be discouraged! I wish you continued health and improvement. ☺️

Interesting questions about long-term effects, sustainability etc. I am trying to locate the research you are quoting on atherosclerosis and risk of cardiovascular disease in eskimo population - could you help with that reference, please? I disagree with the comparison with chemotherapy: unlike potentially deadly chemotherapy, nutritional ketosis is a naturally occurring physiological (pregnancy, new=born babies, suppression of appetite as defensive/coping mechanism) or survival (lack of food) mechanism. Dr Philips's study shows great promise but, unfortunately, long=term studies would be difficult for many reasons; wish we could get to that point! I am on a ketogenic diet (the paleomedicina protocol), and I wouldn't say it is difficult - the most challenging part is doing it on a budget (UK). I started it for general health benefits and also as an experiment to see if I can improve my lipedema (with veinous insufficiency). However, I do notice improvement in multiple domains,

Would a ketogenic diet be advised for bipolar mood disorder? My daughter was put on lamotrigine which is also for epilepsy and I have read that researchers don't exactly know why it seems to work for bipolar. So, is that something you have any knowledge about? The side effects of this medication are very serious and down right scary, but so is having our daughter in the pit of depression.

Doctor's now may not agree. But some decades ago a ketogenic diet was the only thing that could control epilepsy. Find an open minded dr and consult. We have too many biases in this society. We are so obsessed as to disagree with everything that does not follow our views and beliefs. For me a ketogenic diet of 50 or less carbs works well to maintain my diabetic genes under control, and I find it very doable. It may not be the right diet for everyone, but it is for me. That's all the matters!

The brain is made of 60% fat. The brain works perfectly fine in ketosis via the process of gluconeogenesis and you can in fact halt many forms of dementia. The truck is catching it early enough. Getting into ketosis isn’t hard, it’s actually quite simple. A simple 24-36 hour water fast will get you into ketosis and eating low carb vegetables like cabbage, cauliflower, peppers, and broccoli will keep you there. Getting someone that exhibits the early stages of dementia will obviously need someone to look after them 24/7 during attempts at fasting but it can be done. Potassium, magnesium, and sodium should be added to water during the fast due to rapid depletion until eating has resumed. There’s a big reason that of the 12 major religions fasting is mentioned over 400 times. They knew something we seemed to have lost. As for urges for carbohydrates it common the first two weeks of the keto diet. Your body is starving for the crap you have fed it for years or decades. This too shall pass. A few dark chocolate squares with ground peanuts with salt will satiate most carb addicts. While I agree that a whole food or Mediterranean diet is probably the way foreword for someone with dementia I would bet dollars to pesos it slows mental decline faster. The problem is keeping the persons with dementia from cheating/forgetting that they shouldn’t eat certain things.

It's true that we are plant-based, but what we feel has nothing to do with what's dictated by science and data. We don't eat fish and it makes us uncomfortable to say it but there is no data against its consumption. There's no strong evidence against dairy either. The MIND diet shows that eating fish and maybe even chicken has been associated with lower risk of Alzheimer's, and that's what we have to accept. But we can't propose keto because of the reasons we discussed in the podcast. We keep our biases in check, as should you. Thank you.

I’m 74, I’ve been LCHF Keto with Intermittent Fasting for 4 years, reversed my T2D as have thousands of others, got off 7 of 9 meds, lost 92lbs, lowered my Triglycerides, reversed my psoriasis, my eFGR went from 59 to 88. I do believe that you can do Ovo Lacto Vegetarian Keto, but not absolutely not possible to do Vegan Keto without animal supplements. Mediterranean or Pioppi Keto is perfectly feasible as is Pescatarian Keto or Carnivore. I’m personally Omnivore Keto with none of the side effects you mentioned. My TRIG/HDL Ratio prior to Keto, was 4.78 now for last 3.5 years it’s below 1. Yes my TC and LDL are now considered high, but my HDL also doubled. But I do believe the current TC & LDL levels are artificially set by Big Pharma misinformation to sell more drugs. Even so the TRIG/HDL Ratio corresponds with low CVD risk. This confirmed by a recent NMR Lipid test that gave me a low risk of CVD as my LDL particles are mainly the large fluffy good type not the glycated /oxidized small dense particles. FYI : I was diagnosed with T2D in 1999, and followed my Endos standard T2D therapy for 19 years with disastrous results. I bear no ill will to my Doctor because he’s was just following the US standard of care guidelines. I do find your saying you’re open to discussion, but in reality your inaccurate comments on LCHF Keto and pushing Vegan diet totally negate that. PS: I have ten years of my quarterly Lipid Panel and CMP Labwork results.
