Hi Dr., do we know which flavor of annular tear this patient has: ie, radial, concentric ? and does this change your method of repair? thank you

Love these videos. I have been dealing with what I think is an L5/S1 Annular tear for years that has never been addressed. I had a discectomy at L4/L5 that re-ruptured and had a fusion, that did not help in 2020, had a revision at L4/L5 in April 2023 that seems a little better (I can sneeze without back pain now) but anytime I lift anything over 20 or so pounds I get numbness in one or both legs, low back pain and some times hip pain. Just sitting and moving my spine I can feel burning in that same area. I know something is not right and tired of just dealing with it. I am going to get another MRI soon. It is funny back in 2018 and 2020 separate MRI's said annular tear at L5/S1, but now the MRI's don't say that? I also had a lidocaine injection in L5/S1 for diagnostic procedure that I thought confirmed L5/S1 disc issues but my surgeon did not want to address that disc.

Read the back mechanic by Dr. Stuart McGill. Also, Brian Carroll’s “The Gift of Injury”. Annual tears take 2 years to heal up but with subliminal healing. I dont think we have any biologics known to us that sew up or heal up annual tears to its original pre-injury level since the collagen fibers have delaminated. Anyways read those two books and watch their videos. Give your body time to heal, do core stabilizing excercises. With annual tears and disc degeneration and disc ruptures, spondy on spine, essentially, the spine has reduced stiffness. So build that stiffness by adjacent muscles around such as core and erectors. Read those two books and look those two gentlemens up. From a non-surgery perspective, thats the best advice. Walking helps on a regular basis throughout the day. I walk 3x a day to keep blood flow going. Feel better and pray for me and you both. Regardless you get surgery or not, you have to rehab exactly as those two gentleman (Brian and Stuart) have laid out in their resources.

Does transforminal approach cause disc height loss . Was told endoscopic interlaminat better approach to save disc height

Disc height loss can occur due to various reasons and is not solely attributed to an approach utilized.

Very interested for your intention to show your intervención les on pacient. 3:35 but mostly your voces has transmisión. Other treatment showing surgery not so easy to follón if drilling etc etc. Sorry
