That was no shotgun it's a double barrel rifle usually 500 nitro express.50 cal double rifle.

Yeah Bee...chances are he just pulled the wrong trigger. I think they said that one trigger causes one bullet to fire and the other causes both to fire...so you're supposed to pull the one that causes a single bullet to fire first. He didn't do that.
