Working with the rotator cuff muscles is easy if you familiarize yourself with the peculiar topology of the scapula. If this isn't your strong suit: Find the spine of the scapula and follow it all the way medially (it will dip down and disappear) and laterally (it becomes the acromion process and shacks up with the clavicle). Next, locate the medial border (you can pull it out laterally to expose the edge), follow it down to the inferior angle, and then find the lateral border. Follow that all the way up into the axilla.
Once you've found the spine and the lateral and medial borders, you've defined the triangle that contains infrapinatus. Hopefully you've also found how mobile the scapula is!
Some things to note:
The position of the arm can change how you interact with all of these muscles, and how intense the sensation is. Try working with the rotator cuff while the arm is draped down the side of the table, or placed on a chair near the face cradle.
You may not be able to get the scapula to lift up on some clients. This can mean a few different things: You may need to do more warm-up work, or work with the shoulder for a few more sessions before things are loose enough. You may need to use the hand-behind-the-back trick, and/or elevate the shoulder toward the ceiling more (try a towel under the glenohumeral joint). Some clients may be trying to "help," causing them to engage the rotator cuff as you try to mobilize the scapula. Finally, the darned thing is just too glued down on some people. Mobilize the fascia surrounding it and call it a day!

I recently had shoulder surgery and my scapula is so tight that it hampering my rang of motion. My physical therapist yanks on the medial border and pulls my scapula toward the ceiling. I wish they would get into the subscapularis the way you have showed here. I think it'd make a world of difference! I will definitely suggest this to them. I am also a massage therapist so thank you for this video so that I can help my clients better with their shoulder issues.

I agree as a physio...It is a pity that physiotherapy schools focus too much in exercising and little focus on this kind of manual treatment and electrotherapy, in this case I mean Norway...I have lots of patients with pain, and they come to me after having been at other physio centers, because the majority og pysios put the patients in gymroom , machines or other exercising stufff... Yet it is important, but first of all it is important to relieve and let the muscles be ready for that kind of intervention :)

Keep up the great work Ian! You are really helping a lot of you fellow therapist. I started watching you when I was a student. Now I look to you when I need ideas. Like today. This is the best video I've seen so far with shoulder work! I love how you're actually using anatomical terms, and actually pointing to the muscles in the body. Tutorials are easier to follow when you say supraspinatus, rather than just point and say "right here".

Amazing Amazing, like you said in the beginning, the pain is in the front, been trying to heal myself for couple of weeks with no luck eventually when i reached the back area where all the real tension was, it made big difference to the front pain, thanks for the great knowledge share ideed

Aloha Ian! I really like when you do rotator cuff mobilizations that you don't (always) place the arm on the back. Many of my clients don't like that (including me when I receive a massage). It can be very uncomfortable, even painful, if one has trigger points or restricted movement because of tight muscles. Lots of good stuff can happen with just laying it next to body. Thanks so much. Another great tutorial. :)))
