This video is on CCCA, or Central Centrifugal Cicatricial Alopecia. It's my most requested topic because it's a condition that isn't well understood or well researched. Until now, that is.
I've been able to find a lot of new information on the condition and in this video I share some of that with you.
For those of you just getting familiar: CCCA is an incredibly frustrating condition because it's so often misdiagnosed.
CCCA is a progressive, scarring form of hair loss that primarily affects African American women. It's characterized by patchy areas of baldness on the scalp that are usually accompanied by scarring and inflammation.
It can be extremely distressing to have this condition.
This video will help you understand what CCCA is, how it presents itself, what sort of treatments are available for it and all the latest updates and research, so if you're curious about CCCA, have questions, or know someone living with it, then this is the video for you.

I dont agree with knotless braids unless the stylist does not braid tight, does not use a lot of hair (extremely long ) and only use REAL human hair. THOSE SYNTHETIC HAIR FROM BSS STORE HAVE CHEMICALS ON THEM AND IRRITATES THE SCALP. It eats out the hair from the root. I have CCCA. my middle and edges were completely bald but its growing back. I did my research and did exactly what was mentioned in this video in terms of diet and vitamins, exercise, meditation, sunshine, sleep, low stress environment. I used oils and massage the area twice daily. In the summer I sat outside so sunlight can reach my scalp or i go for a walk in the park without my wig or hat. Do not cover the area all the time with hats. infact I get inflamed when i wear hats. I stopped chemical relaxers, I only use natural products to cover my grey hair on my edges. I am a weave lover and miss my weaves but I dont want to lose all my hair so for now i am rocking a nice removable Beyonce wig lol but the moment i get home its off my head and I only wear it 4 days per week at work. Correct is knotless braids is done the correct way there should be know tension and I always suggest clients to shampoo the product off the scalp that’s used to braid the hair or clean the scalp with witch hazel

A year after this video , been diagnosed with ccca alopecia since 2016: . Left the country for a while . Had a big flair up, lost a bunch of hair . Came back to the states , and my dermatologist offered topical metformin/ rogain compound serum , along with oral rogain . I seen almost just as much growth as my firt couple of months of corticosteroid injections without having corticosteroid injections. Back when I got diagnosed . The only treatments available was corticosteroid injections , doxycycline and clebetesol. Now there are a lot of new things . I cannot wait for the day where I just have all my hair in my head and I don't have to think that deep into maintenance, medications , treatments etc

Not one but ALL OF MY DOCTORS, General practitioner, dermatologist, and nurse practitioner who treated my hair loss were completely INDIFFERENT. They just said, "You have ... alopecia".

I had it. It was treated with steroid injections. Now I wear sista locks… very happy with my results.

I’ve been struggling with it for 13 years! I long for the day where I can wear knotless braids again without my patch of missing hair being noticeable! So excited for this information! I’m willing to do what it takes to get my crown back!!

OMG I've gotten crochet a few times and every time it made my hair fall out more. It's not a protective style for me. I just got diagnosed with CCCA yesterday, so it all makes sense. Going to see the dermatologist in 2 days and will discuss some of these things. Thanks so much for your video ❤

I wished I had your expertise back when I was diagnosed of ccca years ago. Every dermatologist was white that I was scheduled with through my providers and their knowledge was just not comprehensive enough to be helpful

Thank you so much! I was messed up when my hair came out within a matter of months. I had no symptoms at all, but I do have arthritis and my eye doctor once told me I had inflammation in my eyes. I'm going to cut sugar, glucose, dairy, and red meat from my diet. I'm already taking Nutrofol, and I have a steroid cream for my scalp. The dermatologist gave me propecia but I didn't like what I read about it and I didn't feel she was giving me enough information. I received an print out with my diagnosis after I asked the nurse who removed my biopsy stitches and didn't have it explained to me. I'm looking for another dermatologist, to say the least. You have given me more information than I received. I wish you were in Delaware. I would definitely come to see you.

I had COVID-19 in late March and early April 2023. I also have CC alopecia. In mid-June 2023 my hair began to shed greater than usual, by the of June, ~1 1\2 weeks, I was losing a alot of hair.I did see my dermatologist in ealy July. I was expecting to hear Ccc was the culprit. When I was diagnosed with Hair loss secondary to COVID-19 I was flooded.. The treatment regime worked well, shedding started to decrease in a few days by one week i had no shedding.I was pleased, but my blood pressure was suffering. Hypotension was pretty bad in the mornings and even when sitting and getting up to go made me light headed. So I stopped the regimen for a few days and low BP improved. Unfortunately the low BP returned, so i stopped. I have a f\u appointment in the near future. COVID-19 was horrible. I want to tell everyone shedding copious amounts of hair is not normal. Run to your doctor or dermatologist,you will not regret being proactive. 💜🙏

Thank you! I was recently diagnosed with this condition, and so glad I found you! I recently started going to a salon that used extreme heat, now this. So I stopped and using a topical treatment from Dr called Halobetasol Propionate. I don't have as much tenderness, but I sense it spreading. Any advice or insight? I'm not in ATL area..

@drnina Hello Dr Nina. Thanks for this information. I have been diagnosed with ccca, i used to have shoulder length hair and now its short in skne areas and a little long in others. But worst than the hair loss is the itching and burning of the scalp. So o have 2 questions for you: (1) Can the topical metfomin help to sooth itching and burning of the scalp? If not what other product would you recommend to sooth the itching and burning. (2) What are the side effects of using the topical metformin especially over a long period?

Thanks so much for this video Dr. Ross. I was diagnosed with CCCA by a black female dermatologist here in NY back in 2015. Unfortunately She only takes cash now and not insurance. My hair grows in the warmer months and totally breaks off in the winter- I mean down to two inches from 4 or 4 . I lose all my growth every year. Have you heard of CCCA progressing in the colder months and not in the warmer?

I was diagnosed a few weeks ago and I’m so blessed that even though my dermatologist is white she was knowledgeable and kind. She gave me the injections, topical steroids and antibiotics. It’s only been 3 weeks but my scalp which used to be soooo itchy does not itch AT ALL anymore

I am on Spiro but only for acne and the dosage low is 50mg per day.....I only take it 3x per week even though I am suppose to take it twice per day every day. Not sure if it helped my hair growth since the dosage is so low and i am not consistent but it certainly cleared up my face. I dont like the side effects hence why i only take so little.

I have to wear wigs, and yes, I remove them as soon as I get home. I wet n massage my scalp. Self confidence is necessary; hence, my wig. Relaxer burned my scalp; scarring alopecia.

Hello Dr. Ross. What about hydrocloquine for the alopecia inflammation?. I was dx with ccca alopecia and then recently sjögren's syndrome which is an auto immunedisorder. Was given the hydrocloquine medicine ,for the sjögren's syndrome, and noticed a decrease in the pain and tenderness of my scalp. Could CCCA actually be more of a systemic inflammation that is in the body overall, not just the scalp? Where would I get the topical metformin.? I'm interested.

I can definitely see how the Hydrocloquine was beneficial. It helps with suppressing inflammation in the body. Systematically if there is inflammation in the body it can definitely have an effect on your CCCA. In our clinic we keep our clients on a anti inflammatory diet, especially those that are suffering with CCCA or any other forms of inflammatory hair loss.

Hmm. I was taking hydroxychloroquine briefly for UCTD and once I stopped I had a flare in my scalp. I had stopped doxycycline, injections, and topical steroid solution months ago and even relaxed my hair. I saw a tremendous improvement until I got some box braids. Ugh!

thoughts on relaxer if you have CCCA? I got CCCA when i was natural and wore braids and wigs for years. relaxer also allows me to do tension-free and heat-free types.

The best option is to just be completely natural with CCCA as hard as that is. My confidence has been ruined from having to rock my afro with the 1 inch hairs around my crown, but I know it’s helped my hair restore itself. CCCA mutâtes your hair shaft; perms already mutate your hair shaft to refigure the shape of the hair follicles. It’s a bad idea to use chemicals on your hair when chemicals are considered a secondary cause of CCCA as is.

I’m currently on the topical Metforman hcl 10%. It is not covered by insurance so I pay a pharmacy who does compound and pay out of pocket approximately $68 per month. I will say I’m seeing improvements in the last 3 months but I’m also doing oils and creams to keep my hair moisturized as I find it dries out my scalp. So I’m massaging my scalp daily. I also take Nutrafol and the doc has me still doing Clobetasol on 14 days and off 14. I will try the diet change though. Has your patients seen results with the topical Metformin?

We have a few clients who have tried topical metformin. It works for some and not for others. A suggestion i would make is to put the oils on your hair instead of your scalp, because it can slow down the process of getting rid of the inflammation.

@@NinaRossAtl hello. How long can you have had ccca and derived benefits from metformin topical? Also when a physician assumes that you have a relaxer that caused the condition, aren't there other things that might have caused scarring such as being hit on the top of your head?

I was just diagnosed with ccca and was prescribed Clobetasol topical solution and doxycycline tab. The dermatologist looked at the top of my head and basically said that she don’t see any hair growing there because of scarring. But I’m hopeful very hopeful. Are these medications good for me is the question

Dr. Nina I finally went to a dermatologist a year ago who said I have ccca without a biopsy. I've had occasional tenderness but no other symptoms of ccca. I experience more loss 5 months into the treatment that was clobatasol, steroid injections and nutafol. The Dr says I should have more growth & added minoxidil and said I'd hv to do this for life! My blood work in the last year has showed no deficiencies. I eat clean whole food, got off Metformin due to side effects and hv been on Mounjaro ( just the 1st & 2nd strength) for the last year and will be ending it next month.Do you think topical Metformin will work or a DHT Blocker?

Hi there, so there would need to be more treatment needed for CCCA because it is such a complex condition. But, DHT blocker sprays will assist with an irritation and inflammation caused by the condition.

Hi, We don't recommend them because they are high tension hair styles and can accelerate hair loss. It's important to stray away from anything that puts tension on the scalp and hair follicles.

I would try clove water as it is a anti bacterial. I had the same thing from sweating and oiling the scalp too much. That help clear it up as well as ors uplifting shampoo to keep the scalp clean. No oils in the scalp

It is important to keep your blood work up to date. Our bodies change throughout the years so once a year is a general idea for most people. Only under severe circumstances would you have to get multiple test throughout the year.

@@maddy3562 praying your hair will grow back!! mine started in the middle of my head huge bald spot but it grew back about 2 inches but won’t grow any longer than that so I just had longer hair around it so I just cut it all off but now it’s balding on the right and left side kind of like a m shaped hairline . I just can’t win 🥺

@@nehashetty9394 For me, the thing that worked the most has always been getting the corticosteroid injections by a doctor every 4-6 weeks , along with doxycycline antibiotics ( which also act as an anti inflammatory), topical rogaine twice a day, clebetosol cream or shampoo ( about 2 /3 times a week) … also I don’t eat meat.and I take a lot of supplements multivitamins, vitamin c and D3, fish oil, and folic acid . I think all of these things play their role in helping my hair grow back. But I do believe the most active and powerful things is the corticosteroid injections , along with the rogain. It just reactivates all the hair follicles and you start to see the hair growth quite fast

@@MASTER3371 yes my hair grew back but it doesn’t take one injection. I start noticing growth between the 3rd or 4th month. I’m actually kinda shocked your dermatologist didn’t tell you that. But again, keep doing it. If there are hair follicles there it will grow back.

Hi as long as they are not tight or crocheted in that is fine. I recommend using a lightweight brand of hair because if it's too heavy that can cause a lot of tension on the scalp.

Try rinsing with vinegar. One part vinegar and 2 parts water leave on for 5-10 minutes 2 to three times a week. Also spray with water mix with teatree essential oil and rosemary. Do not put oils on the scalp also. God bless you.

Your scalp is inflamed. That’s what made me go to the dermatologist and get diagnosed with ccca. I was given an ant inflammatory and a topical steroid. Once your inflammation is controlled, you can focus on growing back your hair. But don’t wait too long, that’s how the hair loss spreads quickly! Wishing you luck ❤
