As birth doulas we can remind mom to come off her back or tailbone to allow a combination of bones in the lower back/sacrum the freedom to move which creates more room in the pelvis and less pressure on the tailbone. Medical professionals often achieve vaginal birth with force by using things like Pitocin, vacuum or forceps to increase the strength of contractions or to pull/push baby out. BUT why don't we look at birth physiology first and use positions that widen the diameter of the pelvic outlet to make room for baby's head for a smoother pushing phase!
