The full phrase is "spaghetti falls out of pocket", which is from 4chan greentext stories where the awkward guy embarrasses himself in social situations by randomly dropping pocket spaghetti and trying to scoop it back up (by hand, no less). It's both absurdist humor and a metaphor for social awkwardness people have that screws up their chance to look normal to others.
