Post Op Nasal / Sinus Surgery - overview, instructions, what we typically recommend to do and not to do
Note: these instructions do not apply for nose jobs (rhinoplasty)
these are our general recommendations - these may not apply in all cases, and other surgeons may have different recommendations.
Check with your surgeon regarding their specific instructions
Pain may be managed with Tylenol and/or Motrin
These can be taken together every 6 hours
or can be taken alternating every 3 hours
Narcotics may be prescribed for more severe pain
If so, follow the instructions
Many narcotics contain Tylenol - if so, don't take them with over-the-counter Tylenol at the same time
Afrin (oxymetazoline) over-the-counter nasal spray can be used to slow, or potentially even stop, bleeding
Neil Med Sinus Rinse should be used starting the day after surgery
use it at least 3-4 times per day...and there is no upper limit
Rinses should be high volume, and low pressure
Avoid strenuous activity post op. Anything that gets your blood pressure up can increase the risk of bleeding (exercising, sex, etc)
But don't just lounge around. Light activity like waking prevents blood clots in your legs.
Most people only miss a few days of work unless it requires strenuous activity.
Don't fly for about 7-10 days after surgery. The altitude and pressure changes can be uncomfortable and dangerous.
Don't swim or get water in your nose - the only liquid going into the nose should be the Sinus Rinses.
Don't take Sudafed or other oral decongestant as they can increase the blood pressure and risk of bleeding. Elevating the head and using ice over the face/nose and in the mouth can help decrease swelling and congestion.
Don't aggressively blow your nose. Light blows and snorting /sniffing is ok.
A little bleeding afterwards is normal. A "moustache dressing" can be used.
If the bleeding is brisk, using Afrin and pinching your nose for 10 minutes may stop the bleeding. Ice can also help decrease bleeding.
If you experience any facial/eye swelling or bruising, and especially if there are any vision changes, call your doctor immediately.

I just had sinus surgery, yesterday. Surprisingly: I’m not in too much pain (in itself) but my main complaint is that I can’t breathe through my nose, which is the worst feeling in the world. My nose keeps bleeding (which is a common symptom, and I had to change my gauze 4 times, since I got home) but thankfully, I have a follow-up appointment, next week. I pray they remove the splints in my nose, next week, but I watched your video, a few days prior to my surgery, and wanted to thank you for taking the time to share this information with us. It helped me learn what questions I should ask, and what to expect. I even asked for Afrin nasal spray, per your advice in this video, and they gave it to me! (*whispers* Thankkkkkk youuuuuuu 😆) Many people on the Internet love to exaggerate, scare people, and make things sound worse than what they really are. While everyone’s experience is different: you motivated me to proceed with having surgery, despite the large amount of anxiety that I had, beforehand, so I wanted to show my appreciation to you for that. :)

Had all three surgeries that he listed (septoplasty, turbonates, and the sinus roto treatment). There’s definitely mucho bleed-o. I also have a metal splint in there, but I can’t even feel it to be honest.

I know they probably won’t see this, but I have a question. I just had sinus surgery and it went great. I can breathe pretty much perfectly. But I haven’t used a sinus rinse. I don’t like using them. Do I need to do that? What’s the risk if I don’t

Thanks guys a big help, had surgery today and have a slow trickle of blood and fluid and a very sore throat, surgeon recommends irrigation after 48 hrs. Definitely hard to sleep with a sore throat and plugged solid. Thanks for the informative videp

Perfect!!! I has sinus surgery today and you answered every question I wanted addressed. The one thing no one addressed was the Afrin spray and that’s exactly what I wanted to know as well as how soon it was ok to start rinsing. Simple and straightforward

@@DavidHernandez-sn1kk … yes. Right after surgery, I had issues from what would later drain. It’s been two months now with no issues breathing. I had constant sinus infections before surgery.

@@DavidHernandez-sn1kk … the first two weeks was rough. Lots of drainage. Honestly, pretty nasty drainage for 24 hours straight. That was about day 7. Then every day got better and better. By week three, barely anything at all. After a month, I was completely healed. That first couple days, Afrin will really help. I can’t emphasize enough to use the saline rinse .. it’s a lifesaver. I still everyday when I shower do the saline rinse.

Thank you so much for this video! I had my septoplasty Inferior turbinoplasties closure septal perferation surgery on 21st December, last surgery of the year for my surgeon. I thought it would ne a great time to have the surgery, sinceI was off work anyway and could use an excuse to avoid certain family members over the holidays! However I didn't count on excessive bleeding, caused by my rinses, I just wasn't healing at 3 - 5 times a day i was doing it. I watched the video and have cut back my rinses and already the bleeding has stopped for the first time in a week, and I'm much less congested. I would recommend surgery this time of year, however when there are even small complications it can be impossible to get advice outside of spending hours in the emergency room, so thank you for putting my mind to rest, and happy holidays from Australia! ❤❤❤

I had sinuplasty and turbinate reduction three weeks ago. First two weeks I was very stuffy and only the sinus rinses offered some relief. Into the second week I was blowing out huge long chunks of hardened mucous that once expelled gave much relief. This I was told is normal shredding of scars from the turbinate reduction. Now as I start my fourth week I have very little stuffiness and a much clearer nose. I had little need for pain killers. So it's a gradual healing process but very much worth it.

Really bad I would say 16 hours and now things are getting better so just hang in there. Be sure you have Afrin when you come home from the hospital, I don’t pick mine up until a few hours from now which is the day after of my surgery. Afrin will reduce the size of the blood vessel so you don’t bleed so much also get one of those Nettie pots and rinse yourself out as soon as you can I just did mine three hours ago that would’ve been 13 hours after my surgery and it made a big difference

Concise, clinical, professional and to the point. Highest marks, unlike the useless, narcissistic videos on this topic. I just had this procedure done and although enduring a lot of bleeding, congestion, sleep deprivation and even some esophageal discomfort (hiccups, burping can be a side effect of the breathing tube) have yet to take a pain pill. I'm 66 and I'm shocked how many young people have had this done, I thought only old timers required this.

Same here. They quickly told me a bunch of stuff at the hospital right before surgery & after when I was still under sedation. Little to nothing handed out, except when to go to emergency. 🙄 Random things pop into my head like, don't eat anything w Red dye. ?? There should be an extensive clear list of discharge instructions & aftercare. With pictures!

Good evening Doctors Liu and Doctor Waglama! Excellent excellent video! I may be having functional endoscopic sinus surgery for complete opacification of the left frontal sinus. Will this result in loss of sense of smell, either permanently or temporarily? How long is the recovery and what is recovery like for complete opacification of the left frontal sinus?

I have watched so many videos on this subject, this video is absolutely the best. The part on blowing your nose was especially helpful to me. Obviously after irrigation you have to do some blowing! 👍🏼🤧👍🏼

Had turbinate reduction surgery 2 days ago. Doing the nasal irrigations several times a day. Been driving me crazy not being able to blow my nose. And now my eye won't stop watering. Did you have this issue

Please make sure you tell people to use DISTILLED WATER.. NOT JUST WARM WATER. Sink water can cause you to die of a bacterial infection.

I had a baloon sinusplasty a few days ago and would like to know, if I should be concerned with the burning in my nose and head when I use the saline rinse? Also m, I have been having a throbbing head without pain is that common after the surgery?

i'm day 1.... i've been using neil med and its derivatives for a while, i just went to town the day of surgery instead of the the day after surgery.... oooooooooh... okay. so i'll stop now. i'm curious if you're given a narcotic+tylenol, and doc said don't overlay with more OTC tylenol, how about switching between the two? is that okay?

Just had septoplasty and turbinate reduction surgery. My instructions for nasal irrigation weren't exactly clear. I've seen to wait until my nasal stents have been removed and to use saline mist spray instead. Then as you say I should be using the Neil Med irrigation the next day. With the stents in my nose I'm not able to get the bottle tip in to my nostrils. Any advice?

Is walking on the treadmill ok 2 days after surgery if I take it easy? When can I walk fast enough to get my heart rate up and the when can I run again? Also my stitches in one side is coming out! Is that ok to ignore until I see the dr for follow up? I also wish and wonder if I could put vassalene or neosporin or something lubricant on the stitches inside because they feel so dry and tight?

Thank you doctors. The video is very informative. I am a 69 yo man who just finished a 3-1/2 hrs surgery 4 days ago. I am in so much pain the 3rd day post-op when everything started to show. I have my left nostril enlarged by about 2x the regular opening. It's full of packing that started to come out with rinsing. My problem is with the Norco I was given. It gave me high blood pressure that wouldn't go away. My upper teeth gums are hurting. Tylenol and ibuprofen as you mentioned are much better choices. I wished I knew you guys before. Btw, I am still bleeding on the 4th day.

@@GeneLiuMD Thank you doctor. I finally was able to sleep after 8 days of sleepless nights. Blood pressure went away without further treatment. Doctor said it was reaction from the anaesthesia. Upper gum and lip still sensitive after 10 days of surgey. For sure I will ask people to check with you if they plan for sinus surgery.

What is the best sleeping position for someone that had a sinus and oral surgery and has facial swelling? I read that lying down on your back should be avoided and that the patient should be upright. But how can anyone sleep upright comfortably?

I had septoplasty with bilateral FESS with Turbenictomy with Ethmoidectomy under General Anaesthesia on 29.10.2020. I had no bleeding post op, just dripping. Had my first review on 5.11.2020 where the doctor also cleaned my nose. On the evening of 5.11.2020 around 7:54 pm my nose started bleeding profusely. I went to the ER, the doctor placed gauze in both my nostrils and packed it. The doctors examined my throat and informed me that there is no bleeding and everything looks fine. However the nose pack kept getting soaked. Is this normal? Also i have a flight on 11.11.2020. Will i be fit to fly by then?

I had a deviated septoplasty surgery 6 days ago. My forehead up to the top of my head is real sensitive to touch on my right side. Also when I get up in the morning my forehead hurts so bad like congestion. Is there anything wrong? My doctor said it doesn't have anything to do with what he did.

I had all three of the procedures you rattled off that your office does. I will say you were correct, I certainly have “lots of blood.” Lol I also have the plastic splints in there that are stitched in, but I can’t even feel them to be completely honest. My septum was so jacked up it was completely collapsed on the left side. My nose is actually straight now!

Great vid Doctors, thanks for helping us. I need to do my sinus surgery because I had polyps and my nose cartilage not straight this cost me lost smelling and breathing 30 to 40% only from nous. Doctor said it's easy surgery, not cost a lot of pain and bleeding only first days and you can go back to work after 2days from the surgery. Does this true? and can I ask you about sleeping can I sleep first days and showering and eat? And can I breathing first days? Thank you so much

You can definitely eat and sleep, but there will be some discomfort. When you can go back to work depends on what was done and the nature of your work. You really should just go by the recommendations of your doctor.

Very informative. Recently I got my nose endoscopy done and found my right nasal passage narrowed down because of some bone deformation or bend. Doctor further recommended a CT scan and found minor infection on left sinus ( cheek) along with obvious bone bend on the right side (but no infection on right cheek sinus). He prescribed a saline spray two puffs twice a day in both nostrils (Day & night -before bed) and suggested a minor surgery to address the root cause of bone deformation leaving the surgery decision on me, depending how I feel and evaluate my issue. he didnt prescribe any antibiotics. According to him the use of antibiotics would be a temporary solution and infection would return back anytime once the antibiotic course is completed unless the core issue has not been addressed so in other words putting me on antibiotics was simply futile. I am 9 days post op from septoplasty and incision continuously has crust even when I clean it, it seems to come right back. When I look inside it looks like the incision is not healed and basically a hole is there. What should i do? I just say my dr and it was there at the time and he said everything is fine. Should there be a hole. The suture is nowhere to be seen.

I've just had Septoplasty surgery a week ago and going back after 1 to 3 days is impossible! The meds and morphine was impairing my ability to focus and I felt very very tired for several days. Simple things like chewing and speaking caused discomfort and at times much pain. I would also NOT recommend going back too soon as this will put you into contact with germs, flus and other viruses which is not what you need so soon after surgery. Today is day 8 and I'm only now beginning to feel normal again.

@@925sterling3 Hi there. The best way to put it is that I must have gone from 20% airflow on a good day to 100% nearly all the time. It's amazing. My right nostril was completely blocked at least 70% of the day. Hot days and warm nights were the worst. Yes I snored as well. If you have a deviated septum which is causing discomfort and breathing impairment then I'd highly recommend it. The results were immediately noticeable and the day after the surgery I was overwhelmed that I was able to breath properly and take deep breaths for the first time in 15 years. Its really nowhere as bad as I could have imagined and nowhere as bad as I've read about. Just take the necessary precautions and have enough rest after the surgery. Time has flown by! I'm approaching month 3 and only have a negligible amount of swelling (10%) which no one else can notice except me. If I had to do it again I would have the surgery tomorrow again without a doubt.

@@Brent.1844 Thank you for putting your comment. I have stuffed left side for over 15 years niw and on topbof that my dentist screwed up and sent the implant into my sinus. Now that it needs to be taken I am worried. The doc mentioned about the risk of eye damages. I have time till midnight to think over and do or cancel my surgery tomorrow morning. It is strange that they told me about possible complications this late into surgery

I have a lot of bruising on my face from a pretty invasive sinus surgery. I got a septorhinoplasty and turbinate reduction surgery. I can't breathe through my nose hardly at all and have been a mouth breather since the day after surgery. I have my post op appt coming in 2 days.

Hi!! I had my surgery on October 22 .. today is October 30, the doctor removed my splints 2 on October 28 and this 2 past days my nose have been hurting so much☹️is it normal ? I did not have any pain the first week and I actually didn’t need any pain killers at all, but now I really need them for pain 😕

I had my septoplasty 2 days ago and i try to clean my nose with a medical salt water but it can not run through the other nostril because it is so congested with blood. Is that normal or should I be able to clean it properly?

@@RBLong0928 Without knowing what was done, I can't say for sure. The tearducts drain into the nose. So if anything is swollen, blocked, inflamed, etc... in the nose, things can overflow up the "pipes" and come out the tear duct openings

The Neil Med Sinuses rinse can cause the rinse to drain from the eyes if the nose is very congested, while using it. I usually stop rinsing in that nose if it occurs, until the nostril becomes less congested. I've never had the surgery though.

I’m currently entering day 3 of recovery from sinus surgery and am grateful for videos like yours. While my condition appears to be improving i plan on taking the next two days off from work. Will I be able to go to work Friday morning after I go in to get the splints/nasal packing removed? How invasive and time consuming (generally) is the post op follow up appointment?

Completely depends on the type of job you have, your pain tolerance, whether you're still on pain meds, etc... As for the follow up appointment, depends on the amount of debridement (if any) that needs to be done. Sinus surgery varies significantly in how much was done.

I doubt you'll read this but im hoping. I had septoplasty & turbinate reduction 2 days ago. My dr (in S Calif) prescribed Afrin but I couldn't pick it up til the next afternoon. I'm changing my dressing every 3 hours. Lots of blood & mucus. I have the Neil med kit & would like to rinse but I asked the dr yesterday & he said No. Beyond being completely stuffed up, the dried blood & large drips are uncomfortable. Why would he tell me not to use the rinse? It's Saturday so I can't get ahold of anyone.
