I had PRP injections 13 months ago. Happy to say I’m pain free. I had arthritis in my hips and each hip received an injection. I also had a sprained ankle with torn ligaments/plantar fasciitis. Injections in my feet 9 months ago and I’m back hiking and running. This doctor explains PRP exactly like my problems. Important to know that the injections improve over time. I was disappointed after the first 2 weeks and then my doc reminded me that the relief from the injections would take 1 month. That’s exactly what happened. I’m sold on PRP! No drugs and it’s your own cells working!

I am 68 years old and 4 years ago I had 2 torn miniscus and a bone spur in my left knee and med to advanced arthritis in both knees! The orthopeadic surgeon wanted to operate on my left knee snd give me steroid type whatever for my arthritis. I had stem cells from a delivered placenta (so no moral issue) and my own blood plasma spun and put into a shot for each knee. It worked fast and after 4 years, my knees STILL feel like new!

@@snakeshiet Honestly, the next morning I was a lot better! But, I would say to get to really good and feeling cured, probably 4-6 weeks. But, I could tell right away it was good, they say the stem cells go after inflammation and the inflammation was miles better pretty fast. It cured the two torn miniscus too! The nurse told me that they have had people cured of a torn miniscus by using just their own spun blood plasma! I'm in NJ, not sure how the stem cell situation is in all the other states.

My son had small knee surgery. Waited 2 weeks then I paid 850 for the PRP. His healing time wascut in half. He was back to 100% in sports by month 4.

PRP cost me over $1,000 and it didn't give me any relief. Stem Cell injections cost me $2,000 and gave me instant relief that lasted over 2 years. So I paid for another $2,000 to receive shots in my shoulders and back. Instant relief! It's worth it to pay more money and get relief rather than paying cheaper without results!

Wow, that's pretty cheap for stem cells! The cost difference between PRP and stem cells are often 4-10x the cost. Which is why in many situations, PRP is more economical and a better starting point. If you find providers that can do stem cells for cheap, then yes, it might make sense!

The honest truth, a combination of mesenchymal stem cells (particularly expanded high number) and PRP is far superior to PRP and long-lasting, what stops people from going for the therapy is the cost and are not covered by the insurance companies, cells are the building blocks and plasma has numerous positive proteins and growth factors

Excellent video Jeffrey, the stem cells combined with PRP for large joints give more prolonged and sustainable results. Small joints PRP does not work, so as you said it depends on the clinical case. Adult mesenchymal stem cells are safe but less potent than the pluripotent ones but the latter ones can carry a small risk of cancer-teratoma. I am confident all regenerative medicine products have a positive effect but PRP is the cheaper practical option. I have been doing expanded stem cells for 5 years and PRP for 10 years with good effect. I use one-off PRP but trying higher dose to reduce the costs of multiple unaffordable sessions

there is a lot about joints but what about muscle pathology? For those that had a previous history of large muscle tear that scarred and eventually healed but with poor result, is stem cell any effective and what type of stem cell treatment would help for muscle remodeling/regeneration? Also for these treatments, how many do you recommend if cost is usually an issue?

I am 62yrs old and had NStride APS injection (1 step higher than PRP) on one knee and it worked quite well on pain relief but the problem I was bow-legged on 1 leg and the bone-on-bone on medial compartment of my knee caused my leg to be unstable & give-way when walking so 10 months after the APS injection, I had a proximal fibular osteotomy to straighten my leg and arthroscopic meniscectomy to trim my torn meniscus. It has been 3 months now and I am happy with the result

Great comparison. I've undergone PRP and amnionic stem cell injections and there was no comparison. Amnionic was far superior at my age; our stem cells get tired following chronic dis-ease and aging I would love to know where you're located as I'm researching for a friend who is not a candidate for knee replacement due to health conditions and would love to see him be able to have relief and regeneration. Be careful with amniotic injections. No live cells. No independent research. Lots of fraud in that area.

I'm 73 years old, used to be an athlete, so wear and tear is an issue, and my knees are a train wreck. I've called twice to make sure, and my Medicare Advantage insurance company says PRP injections are covered. I might be too far-gone to fool with it, but I'm excited about the prospect of delaying knee replacement for a few years ... maybe forever. I tried Synvisc and Monovisc, and both worked for 6 to 9 months, but I'm hoping for greater, more long-lasting relief. X-rays tomorrow, then I'll see the specialist in a couple of weeks to see if I'm a candidate Thanks for all your very informative videos on the subject!.

I couldn't afford prp injection into my arm with a torn tendon, but my Dr said we can try a blood injection and wow my arm healed in 3mo and the pain was gone. I had been trying to heal that arm for over a year with no success and that blood injection healed it.

I’m in Texas and paid $650 for PRP about 2 weeks ago for my cracked cartilage in knee. I’ll be watching the results. Thus far I feel like the aching from PRP is somewhat still lingering. Sometimes I can’t tell from the original pain vs aches and my body adjusting to the PRP. My orthopedic told me the aches should go away after a week so I was a bit confused. I originally got the injury from a sprained knee from a kick(I do kickboxing). So whenever my weight shifts around from one side to the other when boxing, I’ll take note of what exactly happens during my shadow boxing with that cracked cartilage and if the pain lessens over time from the PRP. I’m 28, almost 29

Hi! Thanks for your video. Very helpful! I've had 2 root tears on my meniscus...one in my right knee, which ended up coming out after repair (my fault, NOT my Ortho's), so I had a subsequent surgery. After all that, i was having the after trauma arthritis flare up, so i went through PRP injections, physical therapy and bracing and stem wave and Im happy to say that knee is doing quite well. A few months later, my left knee decided it wanted in on the action and I had root tear there. :( I've had 2 PRP injections and am doing stem wave now with minimal results so far. Which is causing me to consider Stem Cell. I know this video is about a year old and in terms of technology and research,it could be considered a little out of date...maybe? (NOT meant as a criticism....please know that!) From listening to this, it seems your opinion is that Stem Cells aren't really better than PRP...is that what I'm gathering?! The most important aspects of Dr.Pong’s presentation is the study comparing PRP vs BMAC for knee osteoarthritis, however the results can be disputed and are disputed by some of the most respected Regen Med experts. However, it is a great starting point and should always be offered as 1st line biologics for most patients. Dr. pong however, is relatively new to the world of biologics showing his lack of understanding of cell biology proper nomenclature. BMAC is not stem cell therapy, is a bone marrow concentrate that consists of numerous types of different cell populations in which mesenchymal and endothelial progenitor stem cells constitute less than 1% of the oral cell population. In other word, it is a treatment which is 99% stem cell free. The proper term is BMAC ( bone marrow aspirate concertante) or just Cell Therapy. The ubiquitous use of the “Stem Cell Therapy” is very deceiving specially when dealing with real stem cell research.

Thanks for your comment. This entire field is very tricky. We need more people like yourself who understands that orthobiologic and cellular therapy is not even "regenerative medicine" and that "stem cells" are in fact the incorrect nomenclature. Even Arnold Caplan prefers the term mesenchymal signaling cells. But alas, the general population has not caught on to that -- even members within the 'regenerative medicine' community has not caught on to that. I am in fact aware of everything you just conveyed, but it's hard to condense all that information into a short video and dive into the nuances. Academics and clinicians want to tease out the nuances. Patients and viewers on youtube just want to know if these treatments work (regardless of what you call it). The key point of the video is to inform people of the science. That not all "stem cells" are "stem cells". And that there may be a less invasive option (ie. PRP) that can potentially offer similar results. You know very well there are bad actors out there who are selling snake oil and advertising everything as stem cells. That's what I'm trying to combat. Ultimately, it seems like all of these orthobiologics are still better than what our current health care system will cover and we just need more data to help get these treatments covered for our patients.

Good presentation, the treatments i have done ,they did help some but within a few weeks things returned as before. The treatments do not cure arthritis, they do not restore cushion so bone on bone is still an issue. I am now scheduling knee surgery .

Umbilical tissue/cells seems to be the superior treatment. But you have to go to Mexico to get it.

Umbilical cord msc stem cells are more potent than your own bone marrow derived cells..however they are not available in most countries because the big pharma controls regulation

Thank you for this video doc! I have a question, you stated that Mesenchymal stem cells cannot regenerate into cartilage cells since they are not pluripotent but according to the Mayo Clinic they can differentiate into cartilage cells because they are still multipotent. Is this true?

Thanks for your comment, the answer is fairly technical. MSCs can be induced into pluripotent stem cells in vitro (ie. in a lab). However, this has NOT been possible in vivo (ie. in the body). Because of this, many researchers have sought to reclassify the mesenchymal stem cell name to remove the "stem cell" component. One proposed name is "medicinal signaling cell" to keep the MSC nomenclature. The medicinal signaling cell name also is more accurate in terms of what the MSC is actually doing in the body.

I had my first stem cell injection today. They also had a machine that expanded and contracted my knee. I will be going in twice a week to use this machine. No pain from the injection. I will post updates as my therapy progresses.

@@coltenkerlek7944 I got my 3rd and final injection yesterday. My knee is better 2 months down the road. Dr. told me in 6 months to a year for the healing to stop, with a 6 year life of the injections. I can stand for 3 hours without much discomfort. Walking is getting better. Still have my good and bad days.

That's interesting. PRP on the face doesn't seem that effective so I sort of wrote it off, but then I recently heard that PRP injections to the ovaries helps some women with fertility challenges. Perhaps PRP actually introduces some of your own VSELs from your peripheral blood into a specific area? The act of spinning the blood is basically telling your cells that a trauma just occurred and they need to get to work. So your cells then look for the closest issue to repair, or in the case of VSELs, possibly even regenerate?

thank you for explaining the regenerative therapy so well. I have a partial high grade PCL tear and my knee feels unstable. I am hopeful to try stem cell therapy to correct this problem so i can get back to full function and play sports as i was before. The orthopedic surgeon that i recently saw told me stem cell therapy speeds the healing but will not correct the torn ligament and that i will need surgery if i want best result. Any comments about umbilical cord stem cell therapy being more potent, what is a good center that provides this service. I want to try this less invasive method only if high chance to work. I see it works for arthritis, but not sure about torn ligament. thanks

I’m hoping to participate in Dr James Voos study. MRI to evaluate before and after stem cell, from BM. 2 year study. I’m 6 months post op RTKA, would like to save my other knee from the same fate. If I don’t end up a study participant I’ll pay out of pocket for PRP. Any thoughts about how to find a reliable physician to trust giving PRP? Seems like so many out to make a buck, makes me nervous.

Your best bet is to find a sports medicine physician who does the injection under ultrasound guidance. Easiest way to find someone who does this is to search for a "primary care sports medicine" physician. We are also called non-operative orthopedists. I have not come across many orthopedic surgeons who do these injections under ultrasound guidance. If you are ever told it doesn't matter, that would be a big red flag. Surgeons are trained to operate, which they do unbelievably well! But they may not have the training to do injections under ultrasound guidance. If you are lucky enough to be around an academic institution, pretty much every academic institution will have someone who does PRP injections regularly and under imaging guidance.

In your experience, do insurance cover prp/stem cell treatment if sports med doctor submits the claim and conservative/physical therapy has failed? i have just researched that muscle fibers have limited stem cell. So when they get exhausted due to chronic injuries or trauma, the body cannot produce more?

If you live close to Mexico they use a Drug called Bedinvetmab the Brandname is i think Librela. This is a Drug for Horses and Dogs but i got years of all of this what he talks up there and had pain pain pain, after the first Injection i was the first time in years painfree. This are some monoclonal antibody Cells from China and they work. I mean they w o r k! I know you have to go a long way to decide to take Horsedrugs in Mexico, but i feel sooo good. I had 2 Inj. in 4 weeks and now its more then 12 months that i didnt need any NSAIDs at all. I wont recomend Librela here because i am not a Docter but it helped me a lot and i want to share this. In the US its not labeled for human use!!! One shot in Juarez is 80 bucks. I paid 150 for both shots.

May i ask, What do u think is the best injection for me, I had a sports injury in my heel, 3 years ago, healed completely, then 1 year ago, got reawakened cause of over exercise, Right now there is a constant sense of pressure in my heel area (similar to plantar fasciitis, but paradoxically, no pain first thing in the morning) I do exercise regularly and it helps .. Does it matter what type of arthritis the patient has for PRP treatment? There are so many types, with many being autoimmune conditions. I hope it does not make the autoimmune reactions more.

Yes -- almost all of these studies are looking at osteoarthritis. PRP has not been well studied for conditions such as rheumatoid arthritis. Thanks for your question.

Very helpful. Could you comment if avoiding any food in your opinion slows inflammation for osteoarthritis. I gave up sugar and most carbs (no honey molasses etc) and cut out most of carbs (except berries) and felt no real difference. I’m on day 130 of this eating plan.

Yes definitely. Anti-inflammatory diets help! Great example would be the Mediterranean diet. Diet is super important. Another example is turmeric supplementation. Turmeric has been shown in randomized controlled trials to help symptoms related to osteoarthritis. Turmeric has natural anti-inflammatory properties which we believe help control the inflammation related to arthritis.

You should compare umbilical cord mesenchymal stem cells that gets administered in shots of 50 million cells each. That should be compared to PRP and see if the results will be the same. I don't think so.

My problem now is I need my more arthritic right knee done. I didn’t do both 9 yrs ago, but definitely should have. Finances played a big part in that. But I still want to give stem cell atty before total knee arthroplasty! Thing is I’m 61 yrs old now & my stem cells are 9 yrs older than last stem cell procedure. I’m very interested in using allogenic MSC’s from umbilical cord. It is being done in this country and out of the US. Thing is I want a definite answer about safety, efficacy, and how viable and the amount ! Not to mention I don’t think there’s a gold standard protocol that is used. It’s hard navigating this info ! I’ve heard it’s very successful and problems would only come from placenta stem cells…, rejection, cancers, ect …. Can and will you advise ???

Had two PRP injections for my worn meniscus but they did not seem to help at all. Why did you not cover stem cells derived from umbilical cords? These treatments are not allowed in the US but those people who have traveled overseas for the treatment have had great success.

Your correct bc after 9 yrs my stem cell procedure worked & still going strong ! But X-rays do not show increased joint space. Not true bout not working if bone in bone … but then Drs use that term very loosely imo.

Hi doc, can PRP help treat chondromalacia patella grade 1? I’m 25 years old. I heard that they can combine it with MSC, will this help me better? Can they inject hyaluronic acid too like the 3 of it together or one after the other with some time gap between each one of the therapies? What is the best option for me? My knee hurts under the patella and cracks+crepitus when I straighten it or raise out of sitting position. I can’t go up or down stairs. Can this be fixed with these therapies? Also I do physical therapy but I need to do everything to save my knees until it’s not too late bc I really need to return to active lifestyle.

Yes PRP can help treat grade 1 chondromalacia patella. There is no evidence of combining PRP and MSC. I do recommend interval HA injections as well for maintenance therapy. You will need to discuss with your health care provider how to individualize treatment for yourself. Best of luck!

Hi DR peng.. two questions. are these prp and stem cell injection treatments available in Australia? And is it an option for people with rheumatoid arthritis?... I recently had a MRI scan on my knee. Finding small knee joint effusion, near full thickness cartilage loss at lateral aspect of medial femoral condole with defect of 8mm..prominent osseous oedema of medial femoral condole is evident likely reactive....everything else is normal....so I don't know exactly what I'm dealing with..the GP said I have lost cartilage.....I'm seeing a specialist in two days time..but I don't understand what I even have .I mean I don't think it is arthritis .... based on the information I have given I'd appreciate your feedback thank you

I had a Synvic injection in one one and it was useless. I felt better for about 2 weeks and now it’s as bad as it was before the injection. Both knees need fixed. Great video. I’m 5 months out of partial menisectomy of my right medial meniscus. 30% removed. I’m feeling knee pain on the medial side and in the back of the knee. I’m worse off after surgery than before. I’m so depressed. I know I haven’t aggressively rehabilitated with PT and my quadriceps strength on ipsilateral side is much weaker than opposite. Should I consider these treatments now or really ramp up PT? If yes, which one PRP or Stem Cells?

I know this is about knee therapies, but I have arthritis in my wrist for years now and tried many things including PRP that didn't work. Is there any chance to get better results with stem cells? I have given up on many things that used to give me pleasure in life because the arthritis felt like such a handicap I'd just feel frustrated instead of enjoying myself. I feel like my life has been put on hold since this condition has started affecting me, I'm focusing on my work at the moment waiting for some miracle that might not happen to get back to my hobbies one day hopefully...

If your willing to look to a natural therapy, try grating ginger & make a poultice, wrap it over the wrist, then wrap plastic wrap over it to keep in place, leave it on overnight. It draws out inflammation. Keep doing it until pain is gone.
