It’s a little of both. Tom Maden the actor who plays Jake is 6 foot. Carlson Young who plays Brooke is 5’2.

And yes it is worse to take out the knife. If someone is stabbed or has something lodged in their body like that be it a knife or something else you would never remove it. It needs to be removed by someone like a nurse or a doctor or maybe a paramedic. But preferably in a hospital.
