I wager it wouldn't have done that if Isaac had started at four hearts. My guess is the effect triggered, leaving Isaac at 1 heart. At that point, using or even holding an HP down would have turned it into an HP up. THEN the sound triggered, and it confused the pill that was used for the pill it would have become.

Just so you know, you can edit your options.ini file to make the announcer always say the card/pill. Its announcermode or something (0 is off, 1 is default of sometimes, 2 is always)

for people who are wondering for some of the horse pills the "I'm excited!" Speeds everybody up and makes them a little more faster than the normal pill, the "I'm drowsy" pill makes everything slow including isaacs tears.

Telepills as a horse pill, at least from my experience, seemed to respawn after I used it so It has a second activation, just like other pills.

I'm Drowsy and I'm excited actually affects rooms with enemies. Excited speeds up enemies and the player. While Drowsy slows everything down. So maybe super ones increase the effect.

Story time.When I was playing AB+ a year ago I got an achievement that said “I got a new pill” (P.S. I played on Nintendo switch so I never played rebirth.This was my first time playing and I only saw one pill which was you guessed it lemon party) so I think “lets look up the pill that I unlocked”.So with the two brain cells in my brain instead of looking up the Binding Of Isaac pills.I looked up lemon party.that was the only pill in the game that I knew.😐.
