Special grade just means that they can conquer a nation if left unchecked. The special grade curse here would be able to conquer a nation. So would Sukuna. So they would be both special grade since they both can do that.

During the finger bearer fight Sukuna refused to help and was forced by Yuji to switch, they explained it later when Sukuna tricked Yuji into the binding vow. not only that but Sukuna also consume a finger to increase his strength and pull out Yuji heart to stop him from coming back because Sukuna thought Yuji would be affraid of death.
