Why in case of SIADH there’s hyponatremia not hypernatremia, although patient has a high concentration of ADH which means Just a small amount of urine secreted from his body.. meaning patient doesn’t excrete sodium.. can someone correct me please..?

I wish 90% of the physicians and specialists nowadays were as thorough and educated as you (whomever created this video slideshow). I cannot begin to share how many doctors and PA's have missed the correlation bwtn low sodium levels, pituitary/hypothalamus dysfunction, plasma osmolarity, urine osmolality, and the overall symptoms indicative of probable(?) SIADH!! Keep up the amazing work!!!

Excellent explanation of a difficult concept . Thank you. A summarizing note with a quick differential diagnosis between SIDH , Dehydration and Diabetes insipidus based on serum Na , Serum osmolarity and Urine osmolarity ,would be great to help student answering MCQ in exams.

I was recently diagnosed with SIADH and a small adenoma was found in my pituitary gland, aling with an intussusception in my small bowel. My sodium level went down to 112 and had to spend 3 days in ICU. Every doctor tells me they can't believe I am alive. It's an absolutely awful feeling!

Hi where were you diagnosed? Were you treated under a nephrologist or an endo? My mom is a severe hyponatremia patient Na coming down almost below 100 couple times in past 6 years suddenly followed by vomiting. Unfortunately doctors still can't find the root cause. Did you get rid of pituitary tumors and how you feel now?

@@kimranahmed oh my goodness! Your poor mom! I was Diagnosed by my rheumatologist. The hospital where I was missed it. My rheumatologist is Dr. Stephen Soloway. He is located in Vineland, New Jersey. He is very famous for finding what is wrong, where other doctors fail. He can find it and send your mom to the doctor who can help her. People come even from Australia to see him. I did see a nephrologist who did not do much. I also saw and endocrinologist who assured me the adenoma in my pituitary is very tiny, and did not seem to be the causen in fact, I was told to not worry and surgery was not necessary. I only felt much better after the intussusception in my lower bowel resolved itself. I noticed it did so after I had to stop eating gluten, since the test for it came back me being 99.99% allergic to it. Right now my gastroenterologist is dealing with me. But if none of these doctors are helping, I promise, Dr. Soloway will find what's wrong. He has saved my life 3 times! Hope your mom feels better soon 🙏

@@76Pou yes she unfortunately suffered a lot both times spending 8-9 days in ICU and over couple weeks in the hospital. Second time we thought she aint gonna make it which just happened couple weeks ago. Her nephrologist couldn't do much either. We are takin consultation of local endo while obviously all here tests including MRI comes normal now. She's been a long term high bp patient on statins, that her only major problem

@@76Pou just stop the medication , remove the emotional stress , raise sodium and bicarbonate , fast 2 days aweek , sleep 7 hours , eat blueberry and organic fruits... thats it

I was diagnosed with SIADH a couple of years ago. I was urinating a lot especially at night so made an appointment to see my doctor. My doctor ordered a routine blood test. A couple of days later my doctor rang and wanted to see me urgently. My sodium level was very low. From the Drs I was required to go to hospital. I am now under a Endocrinologist and take Urea powder (apparently a very old medication) and my fluids are restricted. I am happy to say my sodium level has risen out of the danger zone. I still require the urea etc and regular blood tests to keep an eye on it. Your video was very helpful. Thank you.

I am in the same situation. I urinate frequently, and it's keeping me up at night. I get foot and calf cramps, but only if I forget to take my electrolytes and magnesium during the day. Any other suggestions? Looks like I'll need to see an Endocrinologist, too.

@@annedel111 Yes I was also getting calf and foot cramps. Perhaps a visit to your regular doctor to get a referral for a blood test to see what your sodium level is would be a good start. I wish you well.

Very clear explanation. CHF exacerbation is not listed as a cause, but by my understanding is both relatively common and clinically important to understand in relation to SIADH

A person with SIADH cannot have hypovolemia, dehydration or features of fluid overload. SIADH hyponatremia is Hypo osmolar Euvolemic hyponatremia. (For diagnostic/exam purposes)

You said that the decrease in serum osmolality will dilute sodium, then how about other electrolytes in the body? Will they be diluted as well(like potassium)?
