This footage from inside the cockpit of an Air Niugini Boeing 737 plane shows the moment it crashed into the Pacific in Papua New Guinea, last year September, killing one person. It was filmed by an alarmed aircraft maintenance engineer on his iPhone.
Thirty-four passengers and 12 crew members were later rescued by local boats and US Navy divers, with six passengers seriously injured.

Lost in "translation" by a couple down below I see. I picked up on the humour in your comment The KZfaq subtitle/closed captions option wasn't available on this particular clip, which made the printed cockpit sounds overlay even more likely to confuse some viewers. If it had been available, I wonder if it might have displayed something like this: "sing raid", along with "hide smoke". This might be a great warning delivery for a group of weed/pot entrepreneurial flatmates though, for if the police are about to go crashing through the door to their apartment looking for the crops and stashes 😁 I recall one particular rate of descent warning annunciator installed on the Boeing 747-400 simulator which, instead of annunciating "don't sink", would sound to many (pilots included) like, "don't think". The clarity of these warning system annunciators appear to have improved somewhat over the years.

CVR recordings showed they ignored the warnings on previous approaches to Pohnpei and Chuuk. Reason: They intentionally flew in low to ensure early touchdown since the runways on those islands are very short. Bear in mind, those airports don't have ILS but only RNAV-approach (GPS-based) where there is only an imaginary glideslope, not a precise one. "Glideslope" warning can thus be ignored as long as the runway & PAPI is in sight. Which was NOT the case here. So the core mistake was: Not going around, when there was no runway visibility at minimums. It's suicidal to continue a non-precision approach when you neither see the runway nore PAPI lights.

Experienced pilots will know that Glideslope and Sink rate warning means that the aircraft is dangerously descending.if is alarm comes on approach,you can go around.and if this happens on descending,you should immediately add full power to the thrust leavers and start a positive rate of climb and stabilise at maybe 5000Ft.

Experienced pilots will execute the go-around instructions on the airport's chart and climb to the altitude and the heading in the chart, not "maybe 5000ft". Also take into account that the engines might take several seconds to spool up and deliver full power while a microburst might suddenly make the plane go down several hundred feet in seconds. The power of the downburst can actually exceed aircraft climb capabilities.

This is what you get ("flights low") when people who have no clue what they're talking about post KZfaq videos. I don't know about anyone else. But when the AP announced "minimums," I didn't see anything that remotely looked like the runway environment - no approach lights, no PAPI, nothing. So a go around would have been mandatory.

@drymeen
@drymeen 8 ай бұрын
Minimums is a callout from the airplane pulled from the data the crew setup in the plane's computer. It is specific to each airport's runway, airplane category and landing procedure and it tells the pilot they're closing in on the "Decision Altitude" (DA) which is the altitude limit at which they're supposed to go around if they don't have visual contact with the runway. The callout in itself is absolutely normal and happens on every airliner landing, usually you can hear the captain say "continue" meaning they're gonna continue the procedure and complete the landing. The issue here is that the pilots didn't call for a go around and decided to stick it anyway with 0 visibility while not paying attention to their instruments and trying to manually land the plane in conditions where only automatic landing procedures are allowed. The crash is due to several human errors and overall reckless and terrible piloting.

At minimums, unless the runway is confirmed to be in sight, the approach is stabilized and on glidepath, the go around is mandatory. There are no further decisions to be made at this point other than to go around. The only exception would be if there is not enough fuel remaining for another approach.

WHY did this aircraft go in? despite all the automated warnings under the sun regarding their glide slope situation on the ramp? And (apparently) not even getting a visual on the threshold at minimums to boot? This appears to have been totally controllable. Considering the conditions and circumstances atthe time, throttling up for a go around (even before 150) would have been the most appeopriate option to take.

Correct me if I’m wrong but if you can’t see the runway at minimums you need to go around. I think you would maybe need to auto land after the missed approach and once you are back on final.
@robhawk6463
@robhawk6463 8 ай бұрын
This appears to be a case of pilot error. The auto-pilot disconnect warning horn sounds indicating the pilot disconnected the auto-pilot and they receive a “glide slope” warning shortly after. Then they receive a sink rate warning as they go further below the glide slope. If you look at the instruments you can see the raw glide slope information and see the airplane going further below gs. It looks like the pilot disconnected the A/P and tried to “duck under” the gs in an attempt to see the runway when transitioning from instruments to outside looking at the runway for landing. Warnings were ignored, probably due to task saturation trying to visually acquire the landing surface. I didn’t see any sequence flasher or runway lights. Possible that runway didn’t have them but instrument runways in the US are required certain types of lighting to accommodate certain types of instrument approaches. This is out of country so I’m not familiar with their requirements. Just my .02.

Apparently they were intentionally went below the profile because of the short runway and that happened everytime they approach a short runway, which i never understand. I mean, the company must had done a proving flight before opening routes to that airport and it was proved the aircraft will be able to stop normally when flown on the right glidepath and then they make performance table for that runway for all condition, so why you have to doubt that data and start going on your own "the right and true" way, that i will never understand.

Chuuk airport has no ILS, only RNAV approach (GPS-based). Meaning: There is no precise glideslope, and final approach has to be flown manually with runway & PAPI lights in sight. They initially came in high (according to the CVR, the co-pilot says "three white, one red" = too high). The captain therefore deactivated the autopilot and "over-corrected" the descent rate. Problem: At 600 feet, suddenly a rain cloud appeared between the plane and runway. They lost sight of the runway and should have IMMEDIATELY started a go-around. Instead, the captain stubbornly insisted on continuing the approach, hoping for the raincloud to disappear soon. Which didn't happen. He basically flew blindly into the water.

So many moments here where they made Fatal errors that any trained or slightly knowledged Pilot or Hobbyist pilot knows. 1. No acknowledgement or mention of the fact they reached Minimums ie decision height, the height at which you are REQUIRED to say either 'Continue' if you can SEE the runway, or 'Go around' if you cannot SEE the runway. This alone should deem the crash to be 100% the fault of the idiots at the yoke here. 2. Radio altimiter ticking down, glide slope alarm, no visual below minimums, no pilot on earth has ever deemed these to be OK conditions for any Category of ILS or any landing system approach. A zero visibility approach REQUIRES that the ILS is functioning without fault, it's obviously retarded to have your only system of direction be malfunctioning yet be sat there ok with it. 3. Pilot monitoring didn't think that '100' is too low (even though 100 feet in most parts of earth is at the level of Trees, buildings, and ground), but then when he sees the fog closing he then thinks it's too low, Reaction time of a sloth.

@Nameisnotimportant
@Nameisnotimportant 2 ай бұрын
It doesn’t seem to terrifying until you hear how many warnings were going off. “Glide slope” warning (not “Flight’s low” like in those subtitles indicates) is a warning that indicates that you’re below the normal glide path. This can be very dangerous especially in low visibility situations like here in this video. “Minimums” is a warning that informs you that you have reached your MDH ( minimum decision height). At that point your approach must be stabilized and you must visually see the runway. If you don’t see the runway or your approach isn’t stable enough then you have to initiate go around immediately and without hesitation. Not doing so strictly violates standard operating procedures. Those procedures are there to ensure the safety of all people on board and they are there for a reason. In this case pilots ignored all of the warnings and continued descending despite (most likely) not seeing the runway at all below their MDH. And that’s unfortunately why they crashed.
