There are so many problems with this, the side effects cancel the potential benefits. Most people with schizophrenia have other health issues that this drug would only compound. As a caretaker, it is so disheartening that this is the best there is to offer. It breaks my heart that my daughter is dealing with massive weight gain and debilitating fatigue to achieve only a moderate level of relief.

Imagine this. My doctor say there is a lot of medication for this but to see you have to take it for months. Im like 3 years in this and never felt healed and still they dont give quick solution they say people with your condition sometimes need 10 years of trying to find succsess ( can you imagine )

Seems like that is the case with these type of mds. I had a childhood friend with treatment resistant depression and anxiety and did not respond to alot of phy mds and not even shock therapy long term. Finally she was put on the strongest ant phycotic md and unfortunately ended up gaining 100 pounds and is diabetic and has extremely high blood pressure and heart rate putting her at risk for heart attack and her legs can’t support her extra weight anymore. She says her depression is better and all . But seems like she got rid of one problem and got other problems just as bad

I was many years on clozapine. But it was two years ago changed to a combination on olanzapine and quetiapine. This combination works for me by similar effectiveness as clozapine did.

Hello due to using clozapine for almost 2 years, I can’t sleep anymore unless I’m taking the medicine at night. And at the same time I’m getting these side effects. I believe I’ve a problem in neuro transmitter. Is there a way to reverse this and get back normal and natural sleep without this? If so, then what medication and therapy is required

Clozapine is almost the most powerfull sedative drug so it means it lowers your brainwawes to the pointyour brain using like 40percent not 100 percent of brain. So if you do it for a long time . Neurons acept the fact is has nothing to do with creating those chemicals of natural sedation we get so you have to not take it for months and eat healthy and get sun and all the shit helthy people do to make thwm grow but its not like a pill you take it and you like im fine . Its literally 4 months of sleepless nights and painsand all the shit and then maybie it will recover . People dont know how these drugs affect long term brains fuction to recover . Its not eorst than benodiazepines but its like if you make your brain stopcreating soething you will hve lackof that for months without solution . ( there is many natural herb stuff concentrates to help sleep ) Best one is melatonin spray . 5mgs and you sleep good asf But still you stop you are fucked again. ( drug shit you cant fix )

if you want to stop maybe you can talk to your doctor about mirtazapine for sleep and personally i had no side effects,unlike the ablify i,m using together with mirtazapine i,m getting nausea,headaches,yawning,flu like symptoms etc

Is Olanzapine the 2nd best antipsychotic? I read it is almost as good as clozapine but with less side effects. Is this correct?

Olanzapine has lower sedation but has bigher need to eat food and weigt gain is insane and you lose erection

The best medication is the one that the patient complains about the least. This sounds like an awful med clozaril olanzapine isn't nice i took that for a good amount of time. Definitely heavy sedation olanzapine can forget about waking up and you'll wake up groggy. I didn't like it and my dr said it was best for what I have. It bloody isn't you know aha.i always felt like basically most of my problems in adult life are the affects of crap theve put me on. I started to help them pick what you know better results!

Dr your videos are wonderful! A little off topic, but I have had severe chronic insomnia for 2 years. I have tried everything and nothing has worked but 3mg of Lunesta. My Dr said I can also try mirtazipine 7.5 mg. Question to you sir which is safer with few side effects to be on long term? The exhaustion has been awful, .Not sure if Lunesta is safe long term God Bless you

There was a recent study that looked at the best medications for insomnia from a tolerability and efficacy standpoint. Lunesta was one of the medications listed, it does have some issues as it is potentially abusable and falls into the benzodiazepine category of medication. I would say tis not good option if it’s something that is intended to be a long-term treatment. Things like sleep hygiene and CBTI are good to include if you haven’t explored them, I believe I have two videos on sleep hygiene. The other option that did well in the study was as a treatment for insomnia with few side effects and no risk for addiction was Lemborexant which is an orexin 1 and 2 receptor blocker. I think I will do a video on these medications next week since it’s a new concept. Hope this helps www.medscape.com/viewarticle/977318

Did your insomnia every go away? I currently have had super bad insomnia since February, i went 4 days without sleeping, my sleeping problems came out of nowhere and the only thing that seems to help me sleep is seroquel 200mg, I have tried ambien, trazedone, and they haven’t worked, the only thing that works is seroquel. I haven’t been able to sleep not one time since February without seroquel, and currently now doctors want me to taper off seroquel without a solution because I developed paranoia. I never been diagnosed with schitzophrenia and only anxiety and major depressive disorder. I didn’t have a traumatic event that caused the sudden insomnia but it’s been complete hell. Now that I’m tapering off the seroquel, the furthest I’ve gotten is taking the seroquel every other night and sometimes take 2 day gaps in between even though it feels horrible because I’ll go that entire time without sleeping at all. Worst part is the docs don’t have a solution to help my sleeping they just want me off the seroquel to help my paranoia. So my question is did your insomnia ever go away? Did anything help for you?

I have an interesting situation. I used to be exclusively on clozapine and came off it completely because of the constipation and took amisulpride instead. Couldn't sleep. Now I am on a low dose of 25mg of clozapine in conjunction with promethazine. Can sleep up to 12 hours daily. No constipation.

I tried that but made my left lip spasm and I could hardly walk on 10mg, was a shame as I heard good things about it, back on sodium valproate as I couldn't tolerate lithium either

Hey. Doctor i know this is irrilevent but i just want to say that in one of your videos you mentioned that if someone has treatment resistant depression and they fail all medications the last thing could be ect,however i just want to say that some people with severe extraordinary treatment resistant depression might fail ect as well and dont benefit from it. In that case when someone doesn't benefit from ect or tms there is a last option called vagus nerve stimulation that could help people who are highly treatment resestant depression and failed ect or tms or even ketamine infusion.

The entire premise is off! Clozapine is an atypical (newer Antipsychotic) not "an older/more effective" psychotropic! Your whole intro is a contradiction

@@Northside-tc5ex Clozapine is not new at all … it was first released in the 1960’s and was banned for many years, decades in fact due to the agranulocytosis deaths linked to it. It was reinstated in 1989 as a drug to treat treatment resistant schizophrenia. It has horrid side effects too, some are irreversible and in most cases it’s a last resort treatment option.

It's possible but not expected. If you get a baseline EKG and dose the medication appropriately the risk is low. It will prolong QTc in a dose dependent manner.

The best antipsychotic I the world is...walking. writing. Reading & telling to yourself the truth...that life can be brutal. The people that expect happiness are the most upset. How can you be happy on heavy drugs? Get some perspective by at least being honest with yourself if you cannot be honest with near & dear because they are part of the problem. Your symptoms are defending you against unpleasant fact that some families, partners, friends are not loving at all, and in fact behave quite hatefully...in the form of neglect & other abuses.

Are u kidding me? Clozapine may work wonders for some, but how can you in your right mind say íts the best antipsychotic? Its got more serious side effects than other meds and the continous blood work is a hindrance for anyone, let along getting a psychotic patient to comply. You failed to mention, as expected coming from a psychiatrist, the long term brain damage and shortened lifespan that all these antipsychotics cause! With all due respect, what happened to the vow, "Do no harm"? Instead of fluffing over these horrid side effects like its no big deal, tell your patients about the brain damage they cause. Then let them decide if they want to risk their life with the medication. And if you are ignorant about the shortened lifespan of 20-25 years, and the brain damage, just google search it or search here on u tube! I am talking from personal experience as my son now has permanent brain damage for no other reason, but all the antipsychótics for 25 years. And to add to that, he was never psychotic until on these meds. Was put on for anxiety! If you have any compassion and a conscious, you would use your medical knowledge to help patients withdraw from these antipsychotics and hopefully they can recoup some of the intellectual functioning they gradually lose. Many patients need help and guidance in reducing instead of being convinced that antipsychotics are the answer, which dont help the majority anyway. The cure is worst than the disease!

Exactly, this video is highly concerning on so many aspects. I was on it and the weight gain was colossal and it can’t be reversed. I saw so many others suffer the same fate as myself. Greatest antipsychotic in the world … why do they NEED to try at least two others before being able to prescribe it by law. Why does it have such a high death toll and serious side effect rate. It is known to be one of the most dangerous drugs that can be prescribed. Myocarditis, seizures, immune system depletion, insulin resistance, syncope, bradycardia, constipation that’s so bad people have died … the list goes on. It’s a last resort treatment !!! This drug can and does KILL PEOPLE. It killed so many it was taken off the market in the 1960’s and only became available again in 1989 with extreme restrictions that needed to be met before prescribing it was allowed. The enthusiasm thus psychiatrist shows for Clozapine is scary. The next worst is Olanzapine as it works in a similar manner, but it is a bit safer.

@@rosettebattista6121 because he knows we are speaking the truth, that’s why. You are right, it’s shameful he won’t reply. To get off Clozapine I needed a 6 week hospital stay so they could taper me off it very slowly and I needed ECG’s every day to monitor my heart. Even coming off it can be deadly as the heart attack risk is high if not done correctly and monitored. One needs to ask is there big pharma kickbacks involved where he’s getting paid to prescribe and promote Clozapine. He wouldn’t be the first or last doctor to get paid by big pharma in such a manner. I had a good friend die of a bowel obstruction starting from severe constipation from this drug. She reported being in pain and was ignored, thought to be attention seeking. It was horrible to watch her being in such agony. After her passing away I swore that I’d be open and honest to others about how dangerous this drug can be and it should be a very last resort and monitored carefully. If any side effects are persistent or slowly getting worse the discussion of ceasing it under medical supervision is wise. Never ever do it without medical supervision or by going cold turkey as the heart attack/failure risk is very high.

I just don’t want people stopping their medications without medical supervision as the risks are very real. It could take a long time slowly tapering doses back but at least it’s done properly to reduce risks. It also allows for managing symptoms that may get worse in the short term and add new treatment options in with medical guidance.
