If it had a simple hole on top it would prevent that, which is probably what would have been done nowadays after rigorous tests.

suction would keep it there, and a baby wouldnt be strong enough to grab it if they even knew that that was the cause since they are, yknow babies. it wouldnt take long for the baby to suffocate and thats what made it very very dangerous, since there was little to no time for a parent that didnt have a 24/7 watch on their kid (which to be fair they should at least when the kid is awake, but even 15 minutes looking away couldve resulted in this) to act.

Babies are basically actively suicidal in the way they act, and something pushed on you can form a suction force that keeps it there; I remember managing to do that and having trouble getting it off when I was like 10, let alone an infant.

Yeah I use to do that thing where you put I cup around your mouth and suction it to your face, what I presume happened to the babies here. However, the thing about that is it only stays on for as long as you hold your breath, meaning the second they pass out it would just pop off. I really don’t understand how this happened.

Not sure how long the second kid had the pokeball on their face since the father was able to remove it before harm was done, but the first and third... they must have been rather unsupervised with the toys.
