I think the problem is that he took the kids from her. Keeping the kids from their mom and grandmother (who raised Dwayne) is nasty work. I thought the ex-wife meant that Dwayne enjoyed the attention that the transitioning child brought him and is living through the child that way. And I don't have anything to say about his relationship with Gabrielle or the "break" baby.

But their gender can be changed as they please. Dwayne’s daughter/son gender is female but his sex will forever be male.

Not only that but to add salt to the wound, there was probably a fear of the police possibly killing her husband.
