I'm pretty sure that what happened by the end is that T-Samsom can't die while berserking, he will run around with zero health until the rage ends, but exiting the run just made him instantly die.

His runs are about as diverse as MCU movies at this point

I once got a TMTrainer item that anytime I lose my last soul heart- it restored it. I literally couldn't die. Thought the run was hopeless so I tried to end the run. *But my character absolutely refused to die.* It was freakin' boss mode. TMTrainer also once trapped me in an inescapable room full of infinitely multiplying enemies on another so... It is both the best item in the game, as well as at times the worst item in the game.
