The first month after total knee replacement surgery is hard and extremely important. With the Covid-19 pandemic many patients, including my own, have had alterations in their post-operative course. I have summarized some common issues that many patients face while recovering from total knee replacement surgery. (Disclaimer: please discuss issues with your own surgical team to make sure that the information is correct for you and your treatment plan.)

I’m post-op day 12 Total Knee Arthroplasty. The information is so helpful. I’m so grateful that I found this site. It answered every questions I have about what I’m going through now. This is the most painful procedure I have ever went through. This procedure is done as out-patient now. I’m a nurse for 50 yrs and working full time until this surgery. I’m doing out-pt PT twice a week and home exercises three times a day. I’m doing everything on my own post-op day 7. My family just prep the meals for me. The pain is getting better. Thank you again for Dr Adam. You have eased a lot of my worries and anxieties.

YOU GAVE ME A WAKE CALL. I AM AT 4 WEEKS AND STILL ON A WALKER AND FLEXION IS 88 DEGREES. EXTENSION IS 4 DEGREES. MY PT SAYS I AM DOING OK AND ON TRACK. I DONT THINK SO. I AM 66 AND HAVE BEEN ATHLETIC MY WHOLE LIFE. I WATCHED THIS VID LAST NIGHT. I AM ON A MISSION NOW. I AM HITTING IT SUPER HARD TODAY AND WILL UNTIL I GET TO WERE I NEED TO BE.

Excellent video! I have watched numerous knee vids since my TKN 3 weeks ago, but this one explained things either not covered before, or so much clearer, and the why behind it. Thank you so much for taking the time to make this video. It has inspired me to give more effort to my exercises.

I had knee replacement surgery and know people who have also had TKR and not one was at 90 degrees at week one. Please do not feel defeated give yourself up to 4 weeks then yes after that it should only get better. Depending on your age try to get to at least 110. Walk a decent amount and massage that scar daily!

I left the hospital after an overnight stay at 60 degrees. How on earth does anyone leave with 90?

Not sure how but I did. Went home at 90, got to 110 at two weeks. I realize it wasn’t a typical result. I did a lot of pre-op therapy & exercises to prepare so maybe that was a big part. I’m sure luck was a part of it, too, but I also did a lot of work.

Hi I’m just joining this I am one month and one week postop and I’m just getting to 98° I believe that everyone is different and everyone heals at their own pace so I take this with a grain of salt I Dupri appreciate the information because there’s nothing written or guidance on what to expect that I had access to before with the progression after knee replacement so I am grateful to Dr. Rosen for the information. I will continue to look at the videos for help and support and I will reach out to my surgeon as well. Thank you

Thank you for your good clear helpful advice much appreciate your videos very reassuring going well. Not giving up am hopeful will be all good have to work through exercises pace if need & be patient. Only had TKR month ago bit of a vicious cycle if overdo moving around learn to rest more. Think going to hydrotherapy soon will be wonderful.

As usual, I find your videos helpful. Thank you for being direct about importance of the first 6 weeks. I have not been a good exerciser before knee replacement, but I am committed now. I am just under 3 weeks and measured 108.5 at my last PT session. Grateful.

I know this is a couple of years old, but thanks for all the helpful videos Dr. Rosen. I just had a TKR 4 weeks ago. My doctor was a little more on the cautious side compared to you when it came to flexing my knee that first week. They really wanted me to take it easy for the most part that first 7 days. It was OK for me to walk, but they didn't want me bending my knee too much. As a matter of fact, when I went in for my 2 week checkup and showed them I had about 105 degrees of flexion, they told me to stop right there and slow it down. They were worried I would pop open the suture line.

Good information. I definitely needed strong painkillers longer than 3 days though. 3 weeks maybe. Particularly at night.

Dr R great and helpful videos. I had Right Knee TKR on July 8th 2021 so it is now 22nd Day!! Yes I am making progress but struggling with pain vs exercises and have 92 bend 4 degree straight leg and am struggling with getting Range of Motion metrics per Dr targets.......105,110 ....120. I am 69 and 260 lbs 6' and had previous knee bend issues from soccer/life injuries.

I have a TKR scheduled next month and I appreciate your video’s immensely. While I have confidence in my surgeon you explain things in a way that he doesn’t have time to do. The visual aspect is also very helpful. I am curious about doing “too much” after surgery comment I saw. When I had meniscus surgery I immediately started walking every hour and returned to work without device assistance in 4 days, because I had too. For my TKR I just want to be sure I push myself to restore function for comfort and mild-moderate recreation. Thank you

Really helpful. I am 8 days pay op with PKR and this is crystal clear. I like the 'on the hour' guidance. Yes the pain is a bummer but I am taking relief and trying to get on and work through the pain. Sarah

Thanks Dr. Rosen for your video. I'm in week 3 of TKR, range of motion is good so far but I still have some apprehensions. You helped provide some comfort that many of these things may be normal (clicking sounds, numbness on outside of knee). You have some great advice that I will follow and hope that others that are at this site do the same

Omg Dr. Rosen, thank you so much for opening my eyes and overall knowledge about how crucial the first 6 weeks are in getting my Range of Motion….because that is the only time that you have to get it; and therefore, we must do our crucial flattening and bending exercises that you showed us in previous videos!!! No one informed me of this. Not the doctor or the physical therapist that have come to my house. The PT that I have doesn’t even bother to do hands on, she just has me do the same exercises that I have on the handout that was given to me. So I do not understand what her purpose is. Shouldn’t she be trying to help me stretch me out and do more hands on?

Im 9 days out. The pain is so much worse than Tylenol! Just the ride to and from PT puts me at pain level of 8. Everyone is SO worried about people getting hooked on pain pills.

I was on Percocet after my surgery. 2 and a half weeks i5 developed constipation. Once that passed I quit those. Four weeks in 120° of bend, do all the PT you can at home, ice, elevate, good luck.

I agree Natalie, my surgeon was great he said we will ensure your comfortable for as long as you need to recover and get through PT, Narcotics have a purpose in surgery’s like knee Replacements he said and like this doctor said it’s one of the most painful orthopedic surgeries they carry out, so why would you expect Tylenol to control the pain along with anti inflammatory medications, just bear this in mind my surgeon said one of the main reasons patients do not progress with PT is poor pain control which then runs the risk of not reaching your ROM goals and by restricting adequate pain medication can result in greater problems down the line. It’s all down to how understanding and realistic your surgeon is and knowing from experience how best to ensure each patient has the best experience and results from a massive life changing surgery, those who are left in pain and expected to have the same results unfortunately are left to suffer so much.

Opioids the first 3/4 days??? Let’s talk after you have a Total Knee done? This pain is insane. I’m doing all the interventions too.

Thanks for your good videos--very helpful. I've watched many of them and just today the one on the first month. I had TKR just short of two weeks and am wondering why flex is so painful. I'm stuck at 60 degrees (according to my PT), even though i had good flex prior to surgery and have pretty strong quads from working out before surgery. (No problem with straightening--at 0 degrees). Should I push through the pain every day to try adding a degree of flex or could something else be happening?

Night pain. Ugh. I had 120 degree articulation at four weeks, and I have been diligent with my therapy; during the day, I'm mostly comfortable, but as soon as I lie down at night, within ten minutes my knee starts to throb and keeps me from sleeping. Using my iWatch sleep app, I've averaged 3.2 hours of sleep for the past five weeks of postop. This SUCKS most verily, and I'm throwing everything I can at it: Ibuprofen, Acetaminophen, lots of ice, cbd ointments/creams/edibles, cryotherapy, Melatonin, Agawagandha, oxycodone, tramadol, even dilaudid, which barely blunted the pain while turning my head into a boiled cabbage . . . and all opiates cause me additional insomnia, so I got off those within the first two weeks. It's very frustrating AND after joining a FB TKR group, I discovered that my experience is pretty common. The answer? Be patient . . . not my strong suit. My new meme is that pic of the fat kitten with the caption "I Hate Everything."

I am 7 weeks Post op. I have severe skin sensitivity in my lower leg starting approximately 3 weeks ago. I had Dry needling during 1 physical therapy session. Since then, the skin has been so sensitive, it hurts to wears anything other than shorts. Unfortunately, it is Winter now in Virginia. I went to see my surgeon today, she did not seem to ever have heard of this reaction.

8 days postop, watching lots of videos. Your presentation and information get high marks, but… Watch a few videos on camera angles, lighting and background. If I wasn’t looking up your nostrils, I was looking at your ceiling fan because your camera angle is shooting up. You’re very easy on the eyes and your pace, eye contact and elocution are great, just work on the technical elements. Thanks for the good advice!

Hi Dr Rosen, I had TKR nearly three months ago. I can bend my knee over 90 degrees but can’t straighten my leg properly. Do you think by doing straightening exercise I still have time to straighten my leg? Could you please advise. Thanks

I am have total knee replacement scheduled in 5 weeks. I still have full range of motion on bad knee (135) and hope to regain most it after surgery. I have one concern thought, I am an avid wilderness canoeist on my trips, we run rapids which requires kneeling to lower the center of gravity. My surgeon told me that kneeling will be difficult after surgery and many patients are not able to do it. He added, it was not because of the mechanical limitation of the prosthetic but rather to some pain associated with funny uncomfortable feelings when doing it. My canoes all foam kneeling pads for comfort in them. What is your opinion on kneeling? I have a rowing machine would it be a good device to use for regaining full range of motion? Your videos are just great and sure motivated me to push myself in the first six weeks. Thank You.

We all have candida auris and it sets up in any wound including surgical sites causing increased pain. Run a swab on any area of your patients skin and send it to one of the labs that can preform the c auris test and run one on your body you will quickly see I'm correct

I agree it doesn't help much but for specific few weeks that amt of tylenol won't damage your liver

thank you for explaining some of these... I'm arriving at 4 weeks... and the PT and the surgeon didn't tell me about the clicking or the numbness.... I appreciate that information. Although I've been working my knee... at 4 weeks... I still can't stand the pain of 90 degrees... and you're talking about even further by one month.... oh no... will I ever be able to achieve that range of motion? I was a fitness instructor before this surgery... and terrified I won't be able to teach again.

keep working at it with your therapist. If you are not progressing be sure to follow up with your surgeon to find out what options are available or get a second opinion.

Debbie, You got this! Do your best and your previous fitness instructor experience should definitely give you great advantages! I wish you luck. 😃 Believe in yourself! Encourage yourself! Everyone heals differently. I’m on day 7 and I’m going thru total torture, traumatic pain that hinders me being able to do the very critical daily exercises so I’m extremely concerned. I wish us both luck and success! ☺️

Total knee but due to bad fall, not years of pain...I cannot seem to walk without walker...much of it is fear of falling and much instability

Very interesting . Do you think that if the surgery is successful, low impact dancing is possible after knee replacement...in your experience ? In other words - If I get 120 degrees of knee flexion and up after knee replacement will I be able to dance ?

If my surgeon put my knee replacement in at a 7 degree ankle due to how severe the arthritis was before replacement. Can that ankle be changed if it's cause severe stiffness and pain? Thanks!

hard to say, was the femur put in at 7, the tibia, or is the knee angle 7? Probably best to seek out a second opinion in your area where they can examine your knee and your xrays.

I am in a bit of a quandary. I'm scheduled for Nov. TKA. But after seeking a second opinion by a different surgeon, he is advocating a arthroscopy instead because of my age I'm 60. Now I don't know what to do.. my pain level is about a 3 to 4 on a scale of 1 to 10.. I have a annoying limp, I don't know if with the arthroscopy the limp will go away. Some advice would be highly appreciated.

Hard to say without an exam and seeing your films. Typically an arthritic knee does not get better with a scope. And if your arthritis is mild patients don't always have the best outcome with a total knee. If you have an unstable meniscus tear with catching and locking and other treatments have not worked the scope can help some of the symptoms.

What did you decide to do? I'm 69 and have a torn meniscus and fluid on my knee. After my MRI I was told that I have advanced arthritis and significant cartilage loss. When I saw the orthopedic surgeon for the first time he prescribed PT. I asked about the meniscus tear. He told me they'd take care of it when they do the TKR. That was the first mention of TKR. I guess he knew something I didn't My thought is it won't get better on its own, and I'd rather do it sooner, rather than later, while I still have one good leg (it has arthritis and meniscus tears, also). I asked the PA if I was just prolonging the inevitable since I don't see the knee getting better on its own. She also mentioned that if I wait, another year or so, I will be older, and as we age, we don't always recover as quickly. I'd rather do one procedure than do one minor one when I know a major one is looming, but that's my choice. Each person has to decide for themselves. Mine is scheduled for 10-26-22.

If you are wondering what a knee sleeve is? It is a knee brace. They are made of different materials (wetsuit like or an elastic nylon). It can be simpler for some as opposed to wrapping it with an elastic bandage such as an ace wrap.

I had total knee replacement about two weeks ago.strength is good, but bending not so much. The actual water proof bandage that covers the incision won’t let me bend good at all. Plus every time I exercise, my knee turns like purple. Thanks for any advice.

Lewis I had a knee replacement 6 weeks ago. I was super active prior and it took me 4 weeks to get my knee to bend at 90 degrees. I was walking w a cane a mile the first 2 weeks then without a cane. I feel movement and massaging the scar area after the 3rd week was super helpful. Don’t get discouraged!😀
