I have been a tennis player and a sport person for my whole life. My knees were hurting badly for more than 10 years, trying every remedy on the market including the bilateral arthroscopy. Still on pain. Finally, I had my left TKR done in October ‘22 and three months later, in Jannuary ‘23 the right one. Honestly, it has been the best decision, ever. I do not recomend doing both at the same time. It takes several months to recover and the other leg helps a lot in that process. I do not regret at all on waiting just three months for the second replacement. Right now I am out of pain, exercising, walking and jogging regularly and even playing tennis again!!!

I had severe arthritis in both knees. I had one done at a time with no hesitation or regrets. My doctor stated he rarely did both knees at the same time. In my case, doing both knees at the same wasn't something I would have done even if it was an option. I am self employed and needed to get back on the job as soon as possible. Both knees would have made that much move difficult and I also didn't want to "slave" my family any more than necessary. In totality it was the most rewarding decision (but painful in recovery) I ever made (except marrying my beautiful wife of 40 years). It helped a lot that Medicare and my supplemental insurance picked up the tab, whoopee!!

Thank you for a very informative video. I had my left knee done last year (2023). While I also need to have my right one done, I do not think I could have had them both done at the same time. You brought up many points I never even considered. Thank you for the great info! Have a nice weekend!

I had both knees replaced, six months apart. My surgeon told me that my better knee would be helpful while rehabbing my first replacement. I couldn't be happier with my outcomes.

Very helpful information. As a person who had a single knee replacement, it seems to me that the home situation is also a consideration. In my case, I have to go up a flight of stairs to get to the main living level in my house. I think that going up 15 stairs would be very hard for the first week to 10 days and going down the stairs would be difficult for 2+ weeks. I base that on how long it took me to get to the point that I could go up the stairs normally (i.e., step up with the surgical leg), which was about 7-10 days and the longer time before I could go down stairs normally (step down with the non surgical leg), which took me a bit over two weeks. The patient would definitely have to be able to fully straighten at least one knee or have significant help with the stairs. If someone lived in a single level house it would be much easier, but getting up and down from chairs, bed, etc. would still be challenging.

Personally, I don't regret having it done one at a time. As several therapists told me when navigating steps, up with the good down with the bad. BUT having not only a few stairs to deal with, as for therapy?? I don't know how I could have done therapy having both knees done at the same time, even though I achieved my therapy goals in 5 weeks. I did ask my surgeon the bilateral knee question, and he said as a rule he doesn't recommend it, and in my case, with mitigating circumstances, he said no. Which was fine with me. So 9 months later, I had the second done. An Intersting not because he straightened the first knee/leg by only 3 millimeters it felt like a lot more to me, and it did have an effect on my gait with the other leg and hip. Anyway, appreciate the information! Ask your surgeons opinion. But personally, I'm happy I didn't have bilateral replacements.

@@jdright1157 I made the comment because I would have found it extremely difficult to navigate my house if I had both knees done. In particular I don't think I could have managed the 13 step spiral stairs up to the bedroom and primary bathroom for at least two weeks and maybe more. With just one knee done I was able to get around the house from day 1.

I was born w/one hip higher than the other, a pelvic tilt. It caused significant problems in my life, could not give birth naturally, not ride a bike, run or dance fast, Anyway I was bone-on-bone on my hip and had THR 5 yrs. ago. I injured my other knee last yr. and had a partial meniscectomy and my ortho said I had no cartilage loss in any compartment & mild arthritis. I have since injured my other knee and have full-thickness cartilage loss, it makes sense as this is my THR leg. How common is this to have a pelvic tilt? And cause all this damage?

3 m. ago I injured my knee Recent MRI shows a meniscus root tear, stress fracture, Baker's cyst, significant bone edema. Also shows full-thickness cartilage loss. My ortho says I'm not a good candidate for a root repair, and has ordered modified activity w/a walker for walking for 4-6 wks. to heal the fracture & bone edema. I feel like there may only be a TKR in my future. It's very painful and I'm on multiple doses of Tylenol ea. day. It hurt so bad I went to the ER to see if I had a DVT (neg.) I've had steroids inj. & oral- no help. THX for the great info.

I am 74 , I have to do total knee replacement surgery in a month. I have Polyneuropathy of the fine fibers - skin biopsy to diagnose a deficiency in fine nerves - Pain in ankles and calves, difficulty walking. My knee problem started a year ago. I made MRI which showed Extensive cartilaginous absence grade 4 on both sides of the medial section. The medial meniscus with an extensive tear of a degenerative nature of the body of the meniscus and the posterior horn. The meniscus has lost its shape, its borders and its signal. Small foci of subchondral bone marrow edema and large bony protrusions in the medial section. My question: is it dangerous to do the knee surgery? Do you have any experience of such a case ? Is it better not to do the surgery? Thank you very much for your answer.

I am 2.5 yrs post-TKA. The numbness has reduced in size as Dr. Rosen suggests. What bothers me more is the lack of sensation and awkwardness when kneeling. I suspect this is because the surgeon shaves off some of the under side of the knee cap to make space for a plastic "button" to fit with the other components. However, I have never been told the exact reason for the plastic piece.

I guess I’m ready…can hardly walk. Using a walker all the time. Going from the parking lot into the store isn’t feasible. I’m level 3 severity in both knees. Unfortunately, I’m 20 lbs overweight (have already lost 50 lbs) so Kaiser won’t do it. I used to go to the gym 2-3x/wk w/ personal trainer and water aerobics 3-5x/wk but bc breathlessness that turned out to be pulmonary embolisms and now I’ve lost my quad strength.I developed adrenal insufficiency from cortisone injections. Had pulmonary embolisms and DVTs in January (maybe caused by Covid-I had a very mild infection but developed Long Covid w/in a month) causing diastolic heart failure! Dropped out of my exercise routine bc I couldn’t catch my breath…pulmonologist thinks I had been throwing clots for months before it was correctly diagnosed (nope, it wasn’t “anxiety”-thanks, no thanks to Kaiser). I think I’m a mess healthwise so don’t know if they will even do knee replacements at this point.

Extensor lag!! I've watched many of your videos pre surgery. Which were helpful to set expectations of all the work involved after. Now it's 3 weeks in, and going back to review some meet me where I am. The physical therapist found the extensor lag and quad weakness. Makes more sense now. And re-watching helps get the detail of the exercise. Thanks so much, Dr. Rosen

Here is a good question: Why don’t surgeons who will need to put metal hardware in a patient have the patient go for metal allergy testing with a dermatologist/allergist BEFORE having the surgery? This will at least educate the surgeon and the patient of any adverse and potential metal allergies before making the decision to have a surgery where hardware needs to be used. This should be the gold standard for pre operative tests before these kinds of surgeries. Most people will find they are allergic to nickel, cobalt, gold and even surgical steel alloys. Titanium hardware is probably by far the best hypo allergenic hardware to use. In my opinion to help prevent metal allergy problems for orthopedic hardware surgeries.

Preparing for surgery soon. Watching lots of videos and gathering information. Your video is very well thought out and hits on all of the questions I have. I'm still fairly young, but can only walk about 1/4 mile before I hit the invisible wall that debilitates me. Pills don't make a difference, the first two injections helped for about a month each, and the third shot did nothing. MRI shows torn ligaments due to the lack of cartilage. My surgeon explained he can clean it up, but could not say how long before the lack of cartilage would create the same situation. Your video did not change my mind, but gave me confidence that I have weighed my decision appropriately. Thank you for your candor, and may your patients appreciate you.

Six weeks after partial replacement and yeah, I've a numb patch on the outside of my knee. On a personal level I'm not worried about it. If feeling comes back then great, if it doesn't then as it has no effect on my everyday life I don't give it a second thought.

10 weeks post op left knee and have the numbness on left side. Not affecting movement at all, just the "numb" sensation you described so nothing to really worry about I'd tell others. Dr Rosen your straightforward analogies are great! I'm currently reading through your book The Knee Book I bought on Amazon and these comparisons and answers in plain English are very helpful. One month post TKR here. 73 year old male here. I used a walker for around ten days and never did use a cane. The first two weeks are really rough and sleep is fleeting, only getting an hour at a time. But I definitely turned a corner at around two weeks. Now, at four weeks out, I’m at 120° and 0° flat. So much of the early pain is from all the associated trauma from the surgery and significant swelling. Back in the gym after 3-1/2 weeks and every single day is measurably better. FWIW, I already walk better than I did before the TKR, but my knee was pretty messed up for a long time before I consented to the surgery. Thanks for this very helpful video!

It hurts, but it's not hurting the knee. That being said, his recommendations are pretty tough due to pain. I'm at 4 weeks, 120⁰ and finally 0⁰ today. The bend was hard but the straightening is impossibly painful. Makes me want to vomit. My other knee was never this hard to straighten. Everyone, every limb, is different.
