Trigeminal Neuralgia is a pain condition that involves the three branches of the trigeminal Nerve. The trigeminal nerve primarily carries sensory information between the face and scalp, and brain. Sufferers often describe a shock-like, stabbing or shooting pain on one side of their face. This pain can last anywhere between half a second and a few minutes. I was diagnosed in 2020 with an arterial venous malformation on my right side on my head. Headaches that year continued to get worse but they cleared the AVM. Fast forward Dec 2021 a sharp electric pain started on that same right side but it didnt last so I didnt think of it again. Jan 2022 it hit like lightning, I have prior heart failure & it along with high BP sent me into the ER. It took 5 day to calm everything down to leave this as the last thing to be unofficially diagnosed by a Rhumalolgist until I can get a Neurologist to do so. Thank you for such an informative video!

Thank you Dr. I was diagnosed with TN. After your explanation, I will pursue treatment with a headache clinitian to see if the cause is dure to the 3 cervical vertebraes afecting my brain stem

I don't have pain all the time like when I used have Migraines lived on Migrazone like candy. Some days it's like fireworks in my right frontal.lobe behind the right eye. Its been known to trigger a seizure. Today at least 100 shocks 1 was very bad I lost my ability to stand. And the one I just had I wanted to crush my skull. The shock lasted about 2 seconds the pain was residual for over 10 minutes. I sweat on my skull during this and I am so disoriented I can't think or talk when it's this bad. The regular ones are extreme pain and very frequent but not much residual pain. It sometimes triggers a buzzing feeling that can last the whole keeping me mentally on edge. This video was helpful as I don't have a diagnosis as the Dr's and neurologist have been focus on seizure and blackout and amnesiac events. I also rarely sleep because of this. I've been calling them lightning bolts in the brain. I will call my insurance and ask if they have a headache clinician in network.

It's great that you've been working together with your doctor and neurologist to address the seizures, blackouts and amnesia so they can determine if there's any underlying conditions. I was originally diagnosed with Bells Palsy although my face didn't drop like you see traditionally there was a reduction in movement. Dr recently changed diagnosis to Trigeminal Nuralgia and in the process for getting referrals for neurologist and MRI. I had been seeing a physiotherapist which has been a huge help but I keep having flare-ups. It's hard to explain to people that one side of my face feels numb and like it's on fire at the same time. I sometimes feel like my neck wants to click and my jaw clicks too. Feels like this is going to be a long road to recovery.

Hi Kai, I'm so sorry to hear you've had to experience this. It's great that you've been working with a neurologist and getting investigations such as MRI's done. Those symptoms you describe (numbness + burning) are quite typical of Trigeminal Neuralgia, we've have many patients describe something very similar. You may additionally benefit from seeing a headache clinician who has experience in treating trigeminal neuralgia.

Get Neuro mag. For this condition. Called magnesium. L threonate. It's wonderful for pain anxiety and insomnia. Great stuff.

These videos are incredibly helpful and informative. I have been struggling with pain for so long that I have basically given up on getting treatment for it. I have seen neurologists in the past, and I really have felt heartbroken and angry at the lack of treatment that I have received. I have been describing this pain in the exact same way for years. I have had to just learn to accept that I have to live with it, but it has become so unbearable that I can’t function now. I’m very hurt by the way I have been dismissed by doctors for years while I continue to fall apart. I don’t feel like I can attack this anymore. I just don’t know what to do. If I just go to an emergency room, will they help me? I am just not feeling well enough to tolerate it anymore.

Lack of knowledge and cost are very frustrating. I got from dental work and it has caused me peripheral neuropathy affected my sleep cognitive functions tremors. My face doesn’t hurt so much. My face is numb. I have pain and shooting pain to my body and it has affected my life severely.

I have a strong feeling that I have this. The pain is unbearable & it seems to happen more often when i've exhausted myself. I've had injuries to my face in the past & I wonder if that's how I may have got this condition (though not formally diagnosed yet). To me it feels like chilli powder on an open wound inside my head & it's always the right side of my face.

I would love to know how you treat the brain stem? I have TN and ON. I had the nerve ablation, it has not helped. And I need help. Where can I get more info about theft treatment your are referring to, so that I can discuss with my neurologist.

Hi Shaban, I'm sorry to hear you've been having it for nearly 4 years now. Neurologists normally aren't trained in assessing the cervical vertebrae, but maybe talking to him about the possibility of a hypersensitive brainstem could be a good way to initiate that conversation. Best of luck!

Hi doc , one more question to you first why trigeminal nerve get compressed by artery or veins ..?? And second many asymptomatic people have vascualr contact as seen in regular study mri for other disease and whereas some have symptoms , What is reason for that ???

Hi Doc, thank you for making this video. Very informative. I have TN left side and it felt like it progressed in 6 months, now- my neuro said i have occipital neuralgia too. :-( my pain usually starts at the back of my neck (left side) then radiates to left side of face then to the back of my head(left). will this be gone doc without going through invasive surgery?

Hi Abby, i apologise for the late response. Yes it absolutely can, we've treated many, many patients with TN and either cured or vastly reduced their symptoms through non-operative procedures. Have you had your brainstem assessed in the past for central sensitisation?

I have atypical TN which is much different from TN, I don't have triggers or responses to brushing/poking/chewing etc, mine just comes about randomly and it doesn't last minutes it last hours. It can start off as a toothache feeling in one tooth and spread to all my teeth usually the upper jaw, nose, cheek, around my eye and temple and behind my ear. Pain can be 2-4 usually with some attacks reaching 8 9 10 on the scale. Usually last 1-2 hours with bad ones being up to 4 or 5. Carbamazepine seems to help the level of pain but doesn't stop it completely.

i have had both occipital neuralgia which was at the base on the skull and neck and also trigemincal neuralgia. The occipital has been fixed but i still have tn mostly on my right side. Maybe the base of skull pain is the same as what i had?

Hi doc, may I ask a question? I have toothache on the left side but when my tooth starts to hurt, my left side headache will start as well and sometimes even my left ear hurts. I don't experience pain when brushing my teeth, eating, or chewing. I am drinking medicine like painkillers like paracetamol or mefenamic acid. It can relieve the pain but eventually the pain will come back. I have no idea which should I go, dentist or neurologist? What kind of medicine should I take?

Hi Baryonyxx, thanks for your question. The symptoms your describing sound like you may have an issue related to the trigeminal nerve- the Trigeminal nerve supplies structures such as the gum, teeth, jaw and face. I recommend discussing these symptoms with a GP and neurologist- they'd be able to recommend any necessary medication and you may also benefit from seeing a headache clinician.

I have TN and 1 1)2 years ago I injected Vit B12 under my cheekbone on the affected side. It went into remission for 1 1/2 years. This subcutaneous injection of 0 75 ml should be done by a doctor

@@andredegefa5765 I've done it 4 times now, whenever It flares up. I also shove a tegretol pill up into the corner back part of my gums, as close to the nerve as I can get it instead of swallowing it all..I think it gets absorbed a bit through the mucosa and targets the nerves there a bit better.

@@sabinecohen8369 Thanks! I have been with this horrible disease since 2018. I quit my Engineering job too. Now, taking 900 mg Gabapentin and 1500 Trileptal. I will talk to my Dr about the injection and see what he gonna say. Of course I take vit b compex, D3, C & Magnesium supplements too but no luck so far.

@@andredegefa5765 I had to give it to myself because this is not a normal spot to inject and if you inject into a blood vessel you could stroke something. But I've heard people who take it subcutaneously properly by doc helps them

My temple nerve swells up and is in pain only after my teeth on my right I brush or mess with, haven’t been to the dentist in a long time, it’s caused probably by dental reasons? If I get a check up and get the teeth fixed will it go away?

I had this extreme pain in my right side of my upper jaw and in my face for a long time and it was ruining my everyday in my life and if you want to get rid of it get a great medication from your doctor Lamotrigine and it will take all the pain in a short time hopefully and it cured me and you will thank the doctor everytime you visit him

Hi Phillip and Tiffany, that's a great question. When you've been taking a certain type of medication consistently for a long time, your body can develop resistance to it. Usually when this happens, people may have the dosage increased, however, after a while the same thing may happen where the body develops resistance again to the new dosage.

Great question! Hormones can often play a role in headache and migraine conditions of all types (of with TN is one). Changing hormone levels can result in changing responses in the brain, even if those changes in hormone levels are actually perfectly natural. This happens in the same way that alcohol and caffeine can often affect headaches and migraines. The brain is essentially overreacting to the input.

Don't know but it's interesting after menopause women in their 50s who never had trigeminal neuralgia suddenly develop it. Is that when estrogen drops off?

Hi Huss, thanks for your question. There aren't any hacks or quick fixes found that resolve this condition. Things such as appropriate pain medication and heat/cold packs can help to reduce the pain/symptoms but unfortunately does not resolve the root cause of the problem.
