Today we're tackling the taboo topic of what could go wrong when having dermal fillers injected.

Dermal fillers have become popular during recent years for their use in famous lip transformations (we're looking at you, Kylie!)
More and more people are turning to the injectable for a bit of help looking lovelier, and today I'm talking about the risks you should bear in mind before deciding to go ahead with the treatment.

Notice how barely any other youtube cosmetic surgeon goes into this detail of caution. 'Slight bruising...' is about as much as they warn you.

You're a terrific doctor. Thank you, Dr. Tonks! These kinds of videos are so so important. I too find it horrifying that an unqualified (...less anatomically or physiologically "expert") injector is popping filler into patients, as if it's no biggie. While hugely unlikely, the worst-case scenarios are events that patients should be made aware of. Thank you!

I have that deepish "corner" between my forehead and the nose and some wrinkling there. My doctor, who is also a surgeon, says she will never incject a filler in this area because it is way too risky. I asked another doctor if this spot could be fixed and she said- 'yeah, no problem at all', without any warning about this area. Never contacted her again.

I always watch your videos and appreciate your honesty, I've had lip fillers done and I've never been told there was a risk of blindbess 😩 I was looking to have a top up of 0.5ml in the vermillion border of my lips soon is there a possibility of blindness even with just that amount? @@MsTammi125 I had my temples, cheeks and undereyes filled. Everything went beautifully, and it was over a year ago now. No one could tell anything was done. Id definitely do it again. I went to a board certified plastic surgeon for my treatment and it was well worth the cost. Nothing was over done. I had one syringe of Radiess split between my temples and one per cheek, then something else..the name escapes me now..but it was a hyaluronic acid filler I think usually used for lips..i had basically nothing in my test troughs..this helped avoid over filling and causing puffing. I also got dysport. Im 39 and everyone thinks I'm 27! Not saying this is thencaee for everyone..but my results were amazing. My swelling was completely gone in 2 weeks.

@@amandaiosua9269 Hi I'm so happy I saw this comment. I'm 39 too, and I have an appointment next week for face fillers (first time). I'll cancel because I think Id rather have a surgeon do it instead of the nurse who doesn't do as much injections (at least not at this clinic). I'm sure there are fantastic nurse injectors but I would feel safer with a doctor specializing in faces. I'm glad it all went well for you

Would like here the risks of Botox! I want Botox in my jaw due to teeth clenching, it gives me jaw ache and headaches and I run the risk of damaging teeth. Years of clenching has also widened my face in that area, so the slimming effect would be nice too. However the research on lowering bone density in the jaw is enough to put me off.

I’m 20 years old and I really wanna define my jawline and chin. I’m really thin but my face looks round because of my jaw and chin and I wanna give it a slim defined v-shape. Also my face doesn’t look even, it’s not really noticeable in real life but in pictures I just can’t get both sides of my face to look the same, like my left side looks more fuller and a little more wonky and lopsided than my right side, and my front profile is a big no because it looks like I have fat around and under my jaw and chin, and my chin just looks so weird. Can getting fillers in my jaw and chin fix that and give me a more symmetrical slim v-shape face? And what about the risks of blindness? That terrifies me.

I had a Juvaderm injection on Monday at 1:00 pm, and by evening at had severe joint pain that continued into the next day, and got worse with a headache and nausea. It seemed like an unfavorable immune response. I am better after 36 hours of feeling sick. I have Celiac Disease which is an autoimmune disease. I want to have another injection because one side needs more. Do you think this is unwise due to my first reaction? I had no swelling or bruising at the site of the injection, My negative side effect was in my body.

Thanks for this video! My only complication from dermal fillers (that I had just once) was that a small white line of scar tissue filled in my mouth corner a bit. It's not that noticeable but it's there and I worry about getting any more because what if it does somethng worse. Someone else said that must have been an injector error - like where the needle was put. I don't know. It developed quite a while after the filler was done.

Hi, is there any risk by using hyaluron pen only - though with the same fillers inside? Why I ask is because it’s a really very low depth in the skin, so I would guess zero risk especially the arterial ones.

I recently visited my Dermatologist and explained her the needs of wanting to diminish my under eye dark circles, and instead of fillers , she recommended me PRP injections . What’s your opinion on it and have you ever performed this procedure ? Thank you

I did nose filler 3months ago but it seems the redness on my nose doesn't go away! The doctor who did the procedure said maybe i have mild rosacea which i really dont have before so he gave me mirvaso but still the redness doesn't go away so he advised me to do laser. So i did nd yag laser this month but still waiting for best result to go awY the broken capillaries on my nose. Is it possible that my redness on my nose cause by filler? Because im thinking now to dissolve it maybe thats the cause of my redness on my nose since its really uncomfortable to me. And it bother me so much.

amazing video as always. Is still possible to inject permanent or semi permanent fillers? I thought bioalcamid or those metacrilate derives were not in use anymore.

Alberto Leguina yes you can! For example Radiesse is calcium hydroxyapatite and I use it sometimes however it cannot be dissolved, it breaks down over 18-36 months. Some people still use Artecoll which is a permanent filler. Scares me!

D B totally agree! It’s really the manufacturers issue, because when we started using filler it would be common to use maybe 1-2ml per year and now it’s common to use 10-20ml so the business model doesn’t really make sense

After almost 3 weeks of fillers my lumps on my nasolabial area are still there. I went to my after 2 weeks follow up and injector did not seem concerned at all. They are not totally visible but I feel them, there are 3 on the same side of the mouth. I am planning to wait 1 more week and if they do not go down I will let her know. I am afraid she will charge for any correction and she is not cheap.

Dear Dr Tonks, I am having filler in my cheeks and year through next week and am terribly worried about the risk of blindness. My doctor has assured me that it wouldn't really be a risk as he chooses to inject directly against the bone, under the muscle, and that problems of that sort occur when injecting superficially and with a cannula. What do you think? Looking forward to your opinion! Xx

a8n7c the odds are most definitely on your side! Risk of blindness is extremely, infinitesimally small. Yes in theory you can still have this problem when injecting on the bone, but look at the numbers, we are talking less than 200 people out of millions of injections. The most important thing is not to panic because if you panic, your vision starts to go blurry at the edges as you slip into almost fainting, then you panic more, you tell your doctor and then THEY start panicking 😂😂😂 it can get ridiculous.

D B absolutely I agree, when I was teaching I found many of the dentists to be very good at a basic level as they are used to handling a needle and used to working on a micro scale.

Dear Sarah, thanks so much for your very useful reply and kind words! My doctor has 20+ years of experience in the field so I am not too worried about his skills. Do you agree that the best way to treat tear through would be to inject a thicker filler under the muscle as well as a thinner one superficially? My doctor told me he would use Voluma for under the muscle, but I don't know whether he will us another one for the upper layers of the skin. Thanks to your videos, I know more about the techniques which is immensely useful, and I really wish that more people would do some research before jumping to fillers without being properly informed.

a8n7c it really depends on the person but yes using two different types of product often works quite well!! But you can’t really say without having a look!

Hello doctor,I would like please to know from your experience how to handle difficult patients? happily they are very rare but sometimes it's enough to make me rethink my carreer.lately,one patient refused to pay just because he found that full upper lip doesn't suit him and prefer the before.i made hyaluronidase to dissolve and lost my time and money .

I’ve always thought complications even when administered by the best will still occur. Basic anatomy and physiology would dictate that. Isn’t that correct? The amount of variables would say you will have patients with complications at some point. Being educated and knowledgeable about how to treat issues is the best way to handle these situations.

I agree completely Kenneth! The more experienced the pracitioner is the better the outcomes are for the patient on the rare occassion that something goes a bit awry.

I almost died of laughter when you said, I just have my heart in my mouth! :D My nose is my biggest problem. It makes me look Indian, now I am so terrified! Also, are these risks associated with Botox as well or is it simply related to the placement of something under the skin? Before, you mentioned that threads are boring? What about the barbed ones? Is there no risk of damage to the blood vessels? Even just the placement with a cannula?

JabawakeeBewy no it’s just filler that causes these problems, something particulate. Threads don’t cause problems in the same way, but you can get damage to blood vessels, nerves and salivary glands by directly lacerating them if you’re not careful.

You did not mention the accumulation of water on the filler and overly fill the area,? You did not mention how poking a nerve can also retract the face. Making it look like you had a stroke, you did. Or mention the under eyes thing skin

This video IS scary, and I'm so glad you did it. Question: Does the risk of blindness only apply to fillers (and not Botox, e.g.)? You may have said and I didn't catch it, and if so, sorry. I would have thought the risk for blindness with fillers would be greater under the eyes and not forehead and nose! That is why we need you, o great one, to tell us things we did not know we needed but absolutely do! If I ever thought of going on price alone (I have), I have definitely had my mind changed. Thank you again! 🙏 😘

Old and Still Here hello! No it doesn’t happen with Botox, it’s particulate things only and Botox is mixed with saline. The reason is the blood vessels in the forehead and nose are more closely linked to those in the back of the eye than the vessels in the rest of the face 😘

I had dysport on my 11's and it caused me super blurry vision where I had trouble seeing at times. When the dysport wore off in 6 months my sight returned to normal. Never again!

My doctor put in a filler in my under eye area 2,5 years ago. I can still feel slight bumps where the filler was placed. Could this be scar tissue that has build there? Is it something to worry about?

I have several procedures done and have done very well. Over the last 3 years of this journey I have seen a huge improvement in my skin. Alot of the fine lines, creppyness, and wrinkles have greatly disappeared. I'm 65 and am very happy with the results. I haven't wanted to go under the knife and to age gracefully but this has gave me a new lease on myself. Yes I have worried about ill effects but I have full confidence in my nurse that is fully trained. Love your channel, watching you have made me understand what procedures I can have done to improve my shape of my face. So lately had my chin and jowls worked on. Looks amazing. Next year I am going to get threading done instead of injectables in my jawline.

@Element Air I've had all kinds of skin procedures but I thought that the PRP with needleling the best to build collagen in my skin. I saw an improvement after a month. And has remained good since I had it done 2 yrs ago. Want to do it again soon. As for injectables, I've had my lips, cheeks, jowls, nasal folds, and my forehead 11 done. It has been over 2 yrs. I feel that also helped with building collagen but supporting the skin structure has been huge. I'm very happy with everything.

Recently I had cheek fillers, I saw the nurse pour a blue liquid into a dish, have never seen this before during a procedure, what is this blue liquid ? I'm having discomfort in the left cheek 8 weeks later

I’m wracking my brain trying to think what a blue liquid could possible be and I’m drawing a blank. Was it definitely blue? Chlorhexidine can be pink and betadine is brown, definitely not one of those?

@@DrSarahTonks it was definitely blue & I think she injected into me she also grabbed my cheek after the filler went in and dug her fingers deep in under my cheekbone it was so painful and still really sore my skin is unusually dry and wrinkled, I'm worried about what she has done to me

I have a butt chin that I want to get filled... I'm a female so I really hate it. It's also getting more visible the older I get. But I'm an illustrator, so going blind would be next level devastating. What's the risk factor for chins?

I am using Retin A and Metro gel for over 30 years. Do not do the math scary. Injectables along the way. Just getting ready for a “tune up” with injections. Your advice is priceless and I thank you. If you get a chance watch “Vivian tries” she tests products that have been advertised on TV and does an excellent show. I would like to know more about anti age products. Thank you, I am in US Dinah

Leis over 30 years ago Late 30’s for acne around my mouth, For me the best The new gel type is not so irritating Every time I stop skin shows age I may stop for a day or two if out in bright sun Every night in wrinkle areas after waiting 15-20 minuets after water washing Wait then heavy moisturizer Magic and over time cost effective Best

I wish I had listened to this BEFORE I got it. I got Juvederm Voluma on my cheeks yesterday, and I am freaking out about the bumps! What if it becomes infection? What if my face deforms? What if I get granuloma?! So much can happen!

@@janereaction4799 get it dissolved because it will move around and you will end up needing more and more. Get your filler dissolved because filler stretches your skin and you will end up needing more and more. I think some doctor said that it can cause you lose of collagen by dissolving so I recommend taking collagen and never getting filler again.

I am about to have it ! I mean fillers because with botox combined with hyalouronic on the forehead and around eyes area I have side effect an inflamation which caused headache and made my eyes very "heavy" to open! After this video I really think If I should go or not! Most of my friends at around 60 are doing it with no problem and I really hoped it would work for me ok! But I must present at en event in 8 days after thw filling and I wonder that in case something happens I want have time to do something about it and recover!

Will cheek fillers stretch your skin and make your skin sag faster I’m 21 and thinking of cheek fillers for a more contoured look but scared it might stretch out my skin in long term

Hm I think fillers are injected in such small portions to the face (except maybe in the lips from the proportions) I asume that even if your weight is changing slightly all the time 3 or 5 kg up and down it will do more harm than the fillers. The fillers also make your skin produce more own collagen (as I understand) so it is even less the chance. that fillers are the reason for sagging skin. Sorry for my english :) I am from Germany.

re: facial symmetry. I used to watch this show on Canadian TV called 'Style By Jury'. [They would have people vote on the makeover ideas for participants.] They had a PSurgeon assess one participant's face, saying she had a facial asymmetry and one cheek was hypo-developed vs the other. What happens if you’ve had radiesse and you have issues, how do you dissolve it?

You cant dissolve it you have to wait for it to go. If there is a problem like vessel occlusion then firm massage can be used, hyalase which will affect the surrounding tissues, and of course not using it in an area you will have a problem in in the first place

@@DrSarahTonks it was done 3 weeks ago n the swelling only started last week. It blew up making my hand twice the size. That was 2 weeks after I had filler. I never experienced swelling until 2 weeks later.

@@lifewithjimmy4389 it’s odd for it to come this long after the initial treatment and I would be thinking either infection or some kind of inflammatory process if you have been otherwise sick such as Covid vaccine, food poisoning etc
