Just a quick clarification - for the MAP/GA correlation, I assume we use the corrected age? So, if an infant is born at 28 weeks, but is now 4 weeks old, we'd be hoping for a MAP >32, not 28, correct? Thanks for any info!

Hello Laura- excellent point. I probably should have mentioned this! Again- there really isn't that much scientific data out there on what numbers we should be using- but the corrected gestational age is a pretty good 'rough' number to start with. Thanks so much for bringing this up!

So message is BP lower than 30 to be consider as hypotensive neonate. In FT baby MAP is 50 to 60mm/hg BP will be lower in lower gestational baby in comparison with FT baby. Interesting discussion and clear message.

Hello Dr. Parmar- yes- definitely the smaller the baby the lower the BP. Nobody really sure what a 'normal' BP is- because a normal blood pressure implies an infant getting the cardiac output they need. So if an infant has good perfusion, good urinary output and not acidotic then that BP should be ok. Because we're keeping such tiny babies alive now, generally that could happen in these tiny ones below 30. Gestational age as the starting point for the mean BP is OK- we just need to be aware that every baby is different and an adequate BP may be lower than that.

As a top NICU training expert, can you give me more information about guided circulatory monitoring (including blood pressure monitoring methods, non-invasive cardiac function, and ultrasound monitoring of perfusion)? I think that this is very important to manage preterm babies in the NICU. FROM CHINA

Hello Nadiah! This is an excellent question and not really understood. This is current thought: In IDMs, the calcium in the fetus is generally on high side. (Not really sure why- Calcium transported through placenta by active transport, but PTH doesn't cross placenta).Maybe secondary to all growth IDMs go through as a results of elevated IGF?? The elevated calcium suppresses the baby's own PTH. So after baby is born, its PTH is low, causing calcium to drop. If you check PTH level, it will be inappropriately normal or even low, given the low Calcium level in the baby. (Ie in a normal person, if your calcium is low, the parathyroid gland 'sees' this and releases more PTH which then increases Calcium). This is only transient because eventually the PTH level creeps up and calcium is normalized. Does this make sense?!
