Doctor's salaries are dropping and there is nothing we can do about it. Just what we needed today..more doom and gloom regarding physician reimbursement right!? Well, sort of! If you watch the complete video, I think you'd actually be surprised by the numbers I present, but I think you may be in the same boat as me when it comes to future numbers. Only time will tell what will really happen, but I am doing my best to remain optimistic.

I transitioned from a pre-medicine path to a different career path half way through my senior year of undergrad after working hard and making straight A's for several consecutive semesters. The main reason is because I had experienced physicians telling me about these issues and how entrenched insurance companies have become in the industry, pitting physicians against one another to see who will perform a procedure for the lower rate, then making that the standard rate of pay. It seems like possibly the worst time to go into medicine and I'm passionate about so many other things outside of medicine that I figured I'd try proposal writing and teaching English abroad. If these issues are addressed anytime soon (which I doubt) I may reconsider because I love medicine and worked hard to be competitive, I just hate what it's becoming.

I'm currently in my senior year of undergrad and I'm considering switching from premed to software engineering. I'm just curious how did you end up paying for your excess credit surcharge? Because that's one of the things that's stopping me from just immediately switching my major tbh. I love medicine (specifically diagnostic radiology) but I'm scared I won't be able to handle the stress of med school, stress of residency, finances etc. So I'm really considering switching my degree rn.

Excellent video! I am a med student and was just at a dinner last night with multiple physicians, they all complained about lowering reimbursement rates in the past few years… Many are making less than half of what they made just 10 years ago. A lot to do with Medicare and Medicaid setting extremely low reimbursement for docs meanwhile hospitals are still charging very large “facility fees”

The midlevel creep is why. Nurse Practitioners and physician assistants are being passed off as equal to physicians.

Keep making these videos. It's really interesting to see the data you find, especially on salary growth. This isn't something I hear a lot about but it's actually a better way to view your long term salary growth over time than just your compensation at the present moment. Overall, great stuff man

When speaking to my older relatives, it was hard for them to understand the countless financial pressures that pursuing a career in medicine has on those following this path. Thank you for providing data across specialties at a national level in order to bring some light to these issues. If not addressed, fewer competent people will choose a career in medicine.

He literally showed you a graph on “average physician pay raises that beat inflation” okay so basically what he is saying is that physicians were able to beat inflation by 3-4 percent and to keep in mind not all physicians are in private practice or in academic practice so it’s subjective just because you aren’t competent enough to pursue medicine doesn’t mean other people are stupid for doing so it’s a passion not a matter of being competent

The biggest take-away message from Dr. Cellini’s video is for non medical people who think he’s complaining. He’s not. He’s *concerned* where the US is headed with regard to healthcare, particularly those in the baby-boomer category. Priceless Primary care physicians need higher salaries! People with diabetes, hypertension, pneumonia, vaccine needs, mental health issues - where are they going to? They’re not going to Dr. C., and Mrs. C. Is a beautician pushing thousands of dollars of product and fakery. Maybe you should be concerned about the docs in the middle (who are extremely underpaid). They’re retiring as we speak I wish the Cellinis well - he earned every penny.

agreed. please keep making these videos doc! idk how practical it is to go into medicine anymore, especially looking at work culture and incentives to just practice your craft there. plus, lack of time in edu, lack of loans, extra 10+ years to build wealth, lifestyle to enjoy your 20's/early 30's. honest question, at what point is all this considered along with opportunity cost and it's really not worth it to go into medicine?

I am an ICU RN planned to do psych NP and having my own office in Phoenix or Denver and decided to get an MBA in finance or healthcare admin. Not willing to deal with insurance. That said, concierge medicine is a thing… inflation has destroyed RN’s salary. It’s a joke to make 60k base in most places in the country as a RN when that should be above 100k with inflation and home prices.

Thank you for this informational vid! As a doc resident in Lithuania I made about 1k€ per month in 2021. Now its no more than 900€. We are still fighting for an increase, everyone is frustrated. To be honest we've been doing that for the last 20+ years.

Damn, wish you the best. Shit is rough here in Turkey too, but on a much larger spectrum. In the end of 2020 with covid pays I got around 2k€ per month, now with huge inflamation, which is actually much higher than 100%, but the official stat is obviously false & declared as 35%, I now earn around 800€ per month! Insanely unfortunate :( Years ago Doctors here easily around 4-5k$ per month. Economy is crumbling

It is concerning for physicians as well as most sectors as I have not heard of too many receiving a +7% wage increase. Before medical school I would always tell my coworkers and employers that a 3% annual raise is not a raise just a cost of living adjustment. Hard to predict anything in this post 2020 world. Thanks for covering this topic. I know its controversial but I think it would be reasonable to cover some aspects of this such as universal healthcare, regulation from government, the role of health insurance companies in the regulatory process, and perhaps a topic such as direct primary care. There’s no simple answers to these issues.

As of lately, hardly anyone’s salary is keeping up with the inflation rate, unless you are some high level executives (but wait... their Compensation is made of more Stocks vs. salary pay...) so again, for doctors who owns assets and equity (RE and stocks) their wealth definitely soared. Okay in terms of reimbursement then yes I agree it’s not increasing but actually decreasing, and that’s sad. 😞

If only today’s physicians could have a conversation with physicians years ago who struck a deal with insurance companies re: reimbursement rates. Or maybe it was administrators who made that initial deal with the devil. 😬 I’m a PA not an MD, but there’s a trickle-down effect that I definitely feel/am aware of. I max-out at a certain salary and it just…is what it is. Unless I want to go somewhere else, but I really like my colleagues at this facility, which is a value-add I don’t want to give up. So I’m at the “acceptance” stage of grief, ha. People just don’t understand the pressure you (physicians) have, I don’t think - you get out of school in debt to the tune of a quarter of a million $$$, deal with a documentation burden that drains both your time and your soul, and then there are the select group of patients who seem to think that their healthcare should be an exchange like ordering from an extra-value menu at a fast food restaurant. We (PAs) deal with that stuff too (well, the student loan debt is a little less), but…At least for me (as a PA), I can switch specialties if/when I’m faced with burnout. For physicians, it’s like you’re trapped unless you want to put everything on hold and do another residency. I appreciate the knowledge and experience of all the docs I work with so much and I try to tell them as often as it feels appropriate, because I hope they don’t leave - I figure at least maybe some appreciation coming from somewhere is helpful, even if it doesn’t pay the bills. But…I definitely understand when physicians finally decide they’ve had it and just throw in the towel. At a certain point it’s gotta be like, “why fight the undertow.” Thank you for your videos. I’ve recently subscribed to your channel and enjoy your perspective. (I applied the take-home points from your video on the non-utility of coccyx films to patient care a couple days ago, so thanks for the medical knowledge, as well.)

@@brownmamba19 I don’t know if you’re asking me or Dr. Cellini. I’ll answer from my perspective. It’s a totally personal decision highly dependent upon the individual. A former PA colleague of mine went back to med school with the goal of becoming an emergency medicine physician. It was the right decision for her life stage and her career goals. I personally wouldn’t do it, but see above re: I like where I work. I’m happy with the amount of autonomy and the collaborative relationship I have with my physician colleagues, which is a big factor for me re: satisfaction working as a PA. The cost (not just dollars)-benefit analysis for me was always a big fat “no,” but maybe it would be different for you - do the math factoring in your years of lost wages while you’re in school, and see whether you’re going to make up that $250K debt plus lost wages with your future salary as a physician in whatever specialty you want to pursue….assuming you match into it…while also keeping in mind what Dr. Cellini has shared about the trend in physician salaries. And then there’s what’s always been the biggest sticking point for me, which is that you can’t switch specialities without doing another residency. But maybe that isn’t a factor at all for you. YMMV, you know? At the end of the day, the only person who can definitively say if it’s the right decision is you.

Hi Dr. Love your videos! I recently blacked out on a plane for at least 20 minutes after feeling like I was going to puke. When I woke up I went to er, all my labs came back negative, further testing at primary doc/Neurologists including Head MRI and EEG/EKG were normal. Yet I'm still having weird vision like floaters, headaches, and light sensitivity, and a lot of tinnitus? Any advice?! 🙏🏻

Here is advice from a patient not a doctor who also has major hypersensitivity issues to medication, has migraines, gi issues, and muscular skeletal issues as well. The neurologist treatment is limited to medication and perhaps electrical devices and that's it. So, if medication does not work, then the next path is to do alternative procedures as chiropractic, dental ( night guard and teeth cleaning), accupuncture, rolfing, and cranial manipulations that either chiropractic or osteopath doctors that actual practice real osteopath medicine perform. That's my two cents.

I think the Milwaukee lack of growth is more of a plateau that is occurring. Especially given that they are the highest paying metro for physicians at $430k average according to Becker's review and Doximity

? Wondering why Med school is sooooo costly? As an engineer, who has taught, thus seen the cost of outfitting an engineering lab, 10s of thousands, and that's for undergrads. I know it's off topic.

Lol try being a nurse in Ontario, Canada. We have an actually Bill in place capping us at 1% increase per year. No joke. Our government is metaphorically pooping on our profession, and now we're flooding out of the profession, who would've guessed it?

I agree. I'm a med student in Canada, and yes "doctor salary doesn't match inflation" but which job does? I mean, nurses, medical tech, administrative personnel, etc. Nobody had remotely increase of pay matching inflation. The different is that when you make 500 000$ a year vs 60 000$, it's harder for the second to keep up with essentiel stuff, like housing, food, transport. Can't wait for the provincial governments to pay adequately nurses.

@@stephens2663 Salary of doctors are almost the same in Canada and the US. We pay more taxes, but the salary are similar. But doctors and nurses are paid by the government from our taxes. Most doctors in Canada makes a few hundred thousands dollars a year but nurses are often overwork and underpaid. Some nurses make over 100 000 a year and some MD make over 1 000 000 year but it's not the norm. Mostly, doctors salary are similar to US but for the same procedure, in the same province (states), two doctors will be paid the same amount (sometimes there are bonus for distance and nightshift), but the "price" paid for a procedure (eg. Physical exam, radiography, skin biopsy, name it) are stated by the government. There are a small % of doctors that works in the private sector and that can make there own "price", but it's still regulated. One thing nice here is that when you follow the medical guidelines (eg a woman need a ultrasounds for vaginal bleeding during pregnancy), you don't have to ask the insurance company if you "can" do it. You just do it. But you can't ask 12 ultrasounds for one pregnancy and charge it to the government, because of our guidelines

So, what determines the salary? Looks to me like it's more whatever the market will bear over the amount of medical education or difficulty of the job. Rich people will pay a lot for plastic surgery. Young women don't have a lot of money to pay for ob/gyn services and neither do young couples with babies and young children have money for pediatrics. And the chart about the $100,000 and more disparity between men and women in medicine is shameful. I'm curious why urology is at the top of the list as opposed to other specialties.

@@FIGHTER_mlbb my personal experience has also been very similar to a doctor's journey. I chose buying real estate and a couple of daycare facilities to make my money. Guess what it took....LOANS. Guess who had to pay it back. Did I whine about it? I hated those payment but at the same time.....It allowed me to retire by 43. Life isn't easy but I won't sit here and whine while driving my nice cars or living in my nice house that most only wish for.

@@rehsa2194 it's a great comparison and the best part of it is that once my loan is paid for....I get a big raise AND once I die.....my kids make millions just selling off my properties. Try selling off a dead doctor and see how much you make on that.

I don't believe any of these numbers. When you look at different surveys of physician salaries they are all over the place even for the same specialties. The higher paying salaries are probably over 1 million dollars a year on average (ortho, plastics, urology etc).

Why are women still being paid less in 2022? Drastically less, as demonstrated in the Doximity graph. Do females gravitate towards lower paid specialties or are we still just facing a large wage gap? Overall the wage gap only exist because on average most women choose lowering paying careers and in medicine usually choose the less intensive specialty programs because woman on average care more about work/life balance then men on average who are more willing to forgo free time in the pursuit of more money/status etc.

They also tend to work less hours than men, go on leave during pregnancy and some years after, and prefer lower paying specialties because women tend to me more concerned with work life balance then men.

Stop this gender wage gap argument already, it has been disproven time and time again. Women just work less throughout the year, dont work overtime as often as men, and more men are in the workforce than women year after year.

The fact that female dominated specialties attract lower wages is a problem in itself. There is research to suggest that sectors that experience growth in female employment correlate with drops in overall wage. Women are also less likely to get promoted even if they are just as qualified, and they are more hesitant to ask for pay rises due to how they've been socialised. There is also no denying the fact that even within the same specialities there is a significant gender pay gap too. Pregnancy is a huge contributing factor. Even in 2022 pregnancy still drastically penalises women financially. Proper paid parental leave and reducing working mum stigma in the professional sphere is a must. Society has come a decent way since the 1980's but there's still a long way to go.

Let's talk about Social security cuts and all the other private payers jumping on the ban wagon cutting payments of claims, but yet they boast about how much money they reported on the S&P 500, Anthem likes to hold claims for longer than 30 days to make interest BEFORE paying the physicians for their services.

everyone's salaries are dropping, not jut doctors.....just thought you should know. And inflation affects lower classes in society more disproportionally. So I am not very sympathetic to the "plight" of doctors.

Well, you should be. We're human too, and usually hard working folks from the low-middle class. Those high salaries are for 60-80+ hr weeks after 12 yrs of education/training, and 300k+ in student loan debt.

Lol I want to know how long does it take for you to read a CT scan. Had to stay at a hospital for 8 days for an intracranial abscess, had multiple CT scans. Got a bill couple months later for over $1000 for each scan and over $200 for each time a radiologist read the scan on top of $300,000+ charge for hospital stay. And our doctor here complaining about doctor not getting enough pay. I'm glad i have insurance. I'm sure many people would have sign the DNR form if they knew how much they would have to pay to be alive these days.

The US system is completely inefficient. You get overcharged on everything… Every little supply and medication is 10 times more expensive than it needs to be, the malpractice is super high, there are tons of regulatory rules, so lots of administrative staff salaries, it’s sad, because patients like you get in the crossfire. Sorry to hear what happened to you. What percentage (would you imagine?) of that $300,000 actually went to the physicians ? I just want your opinion to know what you imagined.

Well...could be a physician in Kyiv or Aleppo. Aleppo ran out of atropine several times over the years. I wonder why? As a US physician I just can't complain. Underpaid? Yes. And yet in an extremely privileged position. If you want more money, there are many avenues to explore...like a KZfaq channel or TikTok. And of course there is always wise investing...but a "comfortable" guy knows that.

Thats kinda sad... I got all of my education on the internet for free and I make more than most Doctors. You would think after spending $300,000+ dollars on education, they'd be making more. I feel bad :/ ... Remember kids, you can learn pretty much everything on the internet for free. Use that to your advantage... I sure did.

Perhaps a go fund me site might be in order. I’m all about donating a percentile of my salary, so that I may increase funding for MDs & Wall Street investment bankers. Genuinely brilliant ,My Good MD, please don’t stop advocating, for MDs & Wall Street affluents, so that I may sleep much more peaceful, knowing that all you fellas have enough Capital,particularly for your philanthropy at the cat houses, Vegas, pro sports games etc.

@@alexmillunchick6018 PT and OT school can go up to 150,000 and yet many only make 70-80,000/year. Maybe instead of going out and buying fancy cars and mansions, they could live relatively modestly for maybe 3 years and pay off the debt. I just don't buy the victim-hood narrative.

@@FIGHTER_mlbb Well I'm sure if your smart enough to get into medical school, I would think you would do enough research, ask intelligent questions and shadow before going to med school. Medical doctors are paid very fairly for their schooling. I still think if a physician wants more money, maybe start a practice? Complaining about little money physicians get paid is disingenuous due to the fact that most Americans don't even come close to their pay. Most Americans could never get into medical school. Physicians should be proud to have gotten through medical school and the ability to affect change on a very high level, not complaining about how 200+/year isn't enough.

@@FIGHTER_mlbb I am aware that being a resident is grueling and the experience needs to change for the better. Better pay and realistic hours should be enforced. I still don't think after all that schooling that physicians should be paid even more. They are paid very well for what they do. If a physician isn't happy with the money they are earning, maybe it's the job because more money isn't going to solve the problem, it's the career. I think a lot of people want to become physicians for the wrong reasons. Wealth, prestige, respect, power come in mind but yet they try and tell themselves that they really just wanted to help others and the money doesn't matter. In that case,why soo many boutique medical practices focusing on servicing rich, upper class people? Why soo few primary care? IL tell you why, $$$$$$$$$ and greed.

I have yet to see a doctor driving a 10 year old POS car. You guys run around in cars that start at 90k and live in super nice home yet you complain about your pay?!!?? WTF. Does the term greed many anything to you? All your money comes from fellow humans that are having medical issues and you make more in one hour than most make in 4 hours. Quit crying

@@rehsa2194 my PCP drives a 5 year old car and she's still working to pay her loans off. She lives in the nicer city where home prices start at 700k, all 3 of her children go to private Catholic school and 1 of the girls is in ballet and the other 2 are in dance and football. She's not rich but she's not hurting. I see her every 2 weeks and she's worth every penny.

@@johngatsby1473 I hear you, but I don't know your PCPs personal story. They mightve gone to college/med school 20-30 yrs ago when costs were a minor fraction of what they are now. I talk to older docs who left medical school with only 20k or 50k in debt. Their parents mightve helped alot, as many physicians are 2nd generation physicians whose parents helped them pay for school. Also, a comfortable living should be expected from someone working as a physician. The concern is the current cost being paid by younger aspiring docs and recent grads.
