Why fracture boots fail to heal plantar plate sprains in runners

A plantar plate sprain is actually one of the worst possible injuries for an injured runner. They can be notoriously difficult to heal quickly if you don't do everything right.
Plantar plate sprains also have a tendency to evolve into a chronic nagging pain that just keeps you off of running and training for months. Even worse, if you don't treat it correctly, the plantar plate ligament can get so weak and degenerated from all of that chronic inflammation that you even get a hammer toe.
Unfortunately, one commonly recommended treatment for plantar plate sprains is a fracture walking boot.
Even though fracture walking boots are a mainstay of treatment, many runners will be horribly disappointed to find out how long it really takes to heal a plantar plate sprain if relying solely on the boot for the healing process.
This video by Dr. Christopher Segler talks about why fracture walking boots fail to heal plantar plate sprains in runners.

The production value and sophistication of the graphics in these videos are insane. So helpful for non-medical people to understand these topics. I've watched +50 other videos on the same topics, nothing comes close to this in clearly being able to understand what is going on with your foot problems. I am not friends with or related to the Doc. The guy deserves a lot of credit for how well these videos are made.

This is so true. I had a plantar plate tear. I was in a walking boot for 3 months which actually made my case worse. I eventually had foot surgery to repair the tear.

I love this video. I was diagnosed with a "high grade" partial plate tear last week and the doc said I need a boot for 6-8 weeks, even though I was past the point of significant pain. About two days into wearing the boot, I started to wonder how effective the boot would actually be and whether all of the impending atrophy would do more harm than good. From my experience wearing the boot, you hit the nail on the head - it really is not a perfect solution to immobilize the plantar plate. You are still putting weight on the area and there is still toe movement.

Hello,Thanks for your sharing! May I ask how you are feeling now? Hope you had already got much improvement! I had the mild partial PP and am using the pad & taping, as well as wearing a rocker bottom shoe, now 8 weeks passed, and I am improving although it is slow, but I know it won't heal within weeks, but could be months.

Fully recovered and able to run daily now, in regular shoes and without taping. Depending on the severity, it can take a long time to heal. For me the critical part was a solid 8 week stretch of stiff rocker shoes and tape at all times. The key is to minimize pressure on the plate as well as any type of toe flexion. Much to my surprise, it worked. Hang in there!@@frankh8821
