This is cool. I have been grinding my teeth for years, without knowing it. Over time my jaw began to click, and the temporalis muscle would go into an involuntary twitch. Awful. Now I am getting a splint made and I'm so excited.

Guys listen up !!!!! Im suffering from a headache since one year ago , After I spent around 10 k $ I found the pain come from that part , and After few times of massage and wearing the occlusal splint , now Im fine and I started to come back to the life again, because literally when U have a disease and u don't know the reason its painful more than the pain its self .

Me too I had headache and ear pain can't find out what is causing,then I went to dentist to check my TMJ,,, since that I wearing splint,I'm fine now,,

I had several surgeries for wrist and gallbladder and the tubes pulled my jaw muscles out of whack and no one knew what to do so an orthodontist made one for me and it hurt so much where little metal things dug in between upper molars and from closing and tap tap while he adjusted it it finally cracked bottom molar and had to have cap put on. Waste of money and never again. I got Botox on those face muscles and helped a lot but expensive and now am doing exercises by guy on here that is helping more and I do wear a soft splint at night made by someone on line as one dentist made was so hard and hurt my teeth?

The splint described in the video is not trying to move teeth. It sounds like the splint your orthodontist made was trying to move your teeth - hence the pain and metal things. A correctly made occlusal splint should fit comfortably and the way it contacts with your teeth should feel comfortable and help your muscles relax. The splint may still be hard plastic but if it is crafted correctly it won't be painful to wear. Hope that helps.

I was grinding so bad at night even with an NTI that I'd wake up and my teeth and gums would be super sore. I had to wear it during the day for a few weeks so I'd get used to it and then now I can wear it every night.

@Thilina Edirisinghe I was able to stop wearing it thankfully. My jaw hasn't been hurting so I don't think the grinding is as bad as before.

Thanks for the video. If the temporal muscles are always in spasm though and have screwed up muscle memory, would the occlusal splint be a bad idea since I'd still be putting pressure with the closing muscles? I need to deprogram my muscles.

Hey, I have been scouring the internet for any information on who I can see about my facial asymmetry and jaw. I recently saved up for braces and have had them put on however I’m not convinced my ortho knows what I am talking about when it comes to my facial assymetry. And am worried braces aren’t going to fix the problem. If I could text you or something it would mean a lot.

I have an anterior open bite only my back four teeth touch plus tmj on top of it. I am curious if this video would need to be adjusted in this case? Or would a splint still be mechanically the same effect

The answer really depends on further analysis of your situation - basically getting a CBCT scan or MRI of the joints both when your back teeth are contacting and, if you are able to bring more teeth together, when you have as many teeth together as you can. If you look around the 3:10 point of the video, the subject has an anterior open bite when the condyle (bit of the jaw that fits in the jaw joint) is fully seated, so the situation shown might be similar to yours. The appropriate treatment will depend on the full analysis of your situation, and might require a splint that has a slightly different function than what we show here. It will be influenced by what's going on in your TMJ as well as the geometry of your teeth and jaw.

What if your appliance cause your bite to be off? As in when the appliance is out, my teeth no longer meet, making chewing an issue. I have had a lower and now upper one and both have caused this. I'm getting frustrated.

That's supposed to happen, that just means your jaw joint is seating in it's proper postion, i also have a splint, when I take it out only one tooth is hitting. :)

Not quite. The splint design is slightly different as the goal in treating a disc displacement is to try to reduce the load on the condyle so the displaced disc has a chance of recovering its original position or of allowing nutrient flow to the condyle so the bone doesn't die. This animation shows a splint design that helps the condyle go to its fully seated position - which could increase the pressure on the condyle - not what you'd want if you're trying to treat TMD.

What is the most accurate test for TMJ? I had an OPG which showed no bony abnormality of the mandible and no erosions or arthropathy of the mandibular condyle but my jaw cracks constantly and I also have 24/7 TINNITUS which I believe is due to my TMJ disorder. My Doc disagrees but I beg to differ.

An OPG shows hard "tissue" i.e. bony structures, not the soft tissue of your TMJ disc. It's good that your bones are showing no abnormalities - and that is what you'd expect if you have clicking (or cracking) on opening and closing as that says your TMJ disc is still intact. However, the disc is probably partially displaced (your condyle is sort of egg-shaped with a healthy disc covering all the egg; one side of the disc can become displaced forward of the condyle). The clicking/cracking is the disc snapping into place on opening and then snapping off the condyle in closing. A more extreme displacement has all the disc forward of the condyle, but still retaining enough shape that the condyle can snap over the rear of the disc on opening and come off on closing. You might be in that state as your tinnitus suggests the condyle could be pushed back (by the fully displaced disc being in front of it) and is pressing on your ear cavities. The way to analyze the situation is to have an MRI which can pick up on the soft tissue, and ideally have two scans: one with your mouth shut, teeth together, and one with the mouth wide open - so the radiologist can see where the disc is in both those positions. Another option is to find a dentist who understands TMD and will create a splint you can wear that unloads the TMJ and, hopefully (unfortunately I don't think there's a 100% certainty) allows the disc to return to its fully seated position. The dentist might also be able to use a device called a "doppler" which is basically a sensitive microphone that can pick up on noises in the TMJ. A healthy joint will make no noise. A wearing joint will make noises that the expert can use to interpret the state of the wear. Hope that helps. @@vicvic1892 Yes, there are different types of splints depending on what is being treated. Only a small proportion of dentists would understand how to create the splint to help your TMJ - perhaps many could do it with appropriate guidance but, unfortunately, not that many have a good enough understanding of the TMJ to create the splint on their own.

I think you need to differentiate between a dentist and a dentist specializing in TMJD. I have seen regular dentists for the last 52 years and brought the clicking, popping, crunching and locking-closed to their attention as part of my history when first being examined. Not ONE of them asked me questions or gave comment to my situation. They hummed and hawed and went to work on my teeth, a dentist's specialty but in my opinion based on their responses, they knew nothing of TMJD but their ego prevented them from saying. I had one dentist say I should wear a bite guard from the drug store but I do not grind, in fact I have learned in my own research that a bite guard/nite guard or all the other names they use for the same device (including "splint" - which is not a bite guard - but an orthotic for TMJD) and a bite guard is contraindicated for TMJD and can make the situation worse! Another dentist suggested invisalign, the new braces and where braces are contraindicated for TMJD as well. So in my experience, regular dentists throw out whatever they think might work (and for a fair amount of money too) with disregard that what they offer can make things worse. People with TMJD are not to be experimented on and regular dentists are remiss and unqualified to make their suggestions for treatment without proper training and assuming they know everything. They do not. The other more common cause of TMJD that I've pieced together from my research is improper bite and the effect on the TMJ due to poor dentistry. But no dentist will ever admit to that. Not even another dentist's work that they do not even know of. It's an association of dentists that do not rat each other out. At patient expense. Unethical and immoral. Your references to "find a dentist that understands TMD" ( "TMD" is a misnomer changed from "TMJ" the other misnomer...TMJDysfunction is more accurate and is used by those who are qualified to know, dentists who specialize in treating TMJD...TMJD Dentists...there are not many and I've not seen any listing but I am having to travel to another country at great expense to see one for treatment...Canada, not a third world country, does not seem to have TMJD specialists altho regular dentists will claim to "understand" it (aren't credentials or certificates required to make that claim?) it's pathetic@@BiteFX

Again, what profession accepts one who has had "guidance" or "understanding" on a life-threatening condition such as TMJD and where BitFX admits "not that many (dentists) have good enough understanding of TMJD to even create the splint on their own - or more importantly make the correct splint/orthotic for TMJD sufferers. It can mean the difference between life and death...making the situation worse by making an improper device for TMJD such that the condition and pain continues can lead one suffering with the condition to consider taking their life and there are those that have, it's in the research on the subject. I downloaded a 12 page article on the relationship of the severe pain for TMJD sufferers, that only .05% of research funds are dedicated to TMJD research and no one knows how to treat it in general. Only a few who specialize. As it is my opinion that TMJD is caused by improper bite due to poor dentistry, this situation provides excellent job security for dentists while costing those patients who cannot live the the extreme pain, their very lives. @@BiteFX

I'm sick right now congested horribly and felt my jaw was out of whack for years but now I'm getting horrible violent vertigo feels like water tickling and stuffed in my ears. I think at night I am clenching my jaw because there is no comfortable way it sits and that is inflamming my eustachian tubes so they aren't draining. I been bed ridden for months now! Where do I start off dentist or ent dr? What kind of splint would I need?

Firstly, be aware that I'm not a doctor so I'm only communicating based on what I've heard and seen from many dentists and TMJ specialists. For sure you want to have someone checkout what is going on. You might start with a GP and tell them your concerns. If, for example, you have an ear infection that can be healed with antibiotics it would be good to have that dealt with before you run to a specialist. If there's no infection, or when the infection is cleared, then you should go to a dentist who understands TMJ issues. If in doubt, see what an ENT says too.

@@natashae5655 go to your druge store and start with a good mouth gard for sleep as soon as i put it in the tmj went away but as soon as i took it out i cant but dowen if i try it feels likea fishing line pulling apart but im keeping it in to se whut happens y dentist must of pulled my legiments when cranking on my bottome wisedome teeth but try it out se if it helps

I have scoliosis but it is not very significant. I went to a doctor and he said that you have nearly nothing, but i insisted that i want to come to the treatment for 1 course. We did exercises and it is, i think, better now. Some people say that spine alignment could be the cause of tmj. I want to ask you lets say i have 1 lvl scoliosis and if i get splint for tmj, will it get fixed and not get worse when i stop to use that splint.

@@calalkazimov6179 I've certainly heard some dentists talk about correcting scoliosis by using splints, but it's not a common specialty. It seems logical that if you have a problem in one TMJ that is producing an asymmetry in the jaw, that will influence how you hold your head, perhaps tilting to one side, and that in turn will feed down to your back so your spine curves. If the TMJ issue can be resolved, one would hope that the head posture would correct and that the scoliosis would also correct - unless there is some other cause of the scoliosis. What's more certain is that if there is asymmetry in your jaw, it likely indicates a TMJ problem and that fixing that TMJ problem can correct the asymmetry - provided the TMJ problem is fixable and hasn't advanced to a state where repair is difficult. Worth finding a TMJ specialist who can tell you what is going on with that joint.

It is not easy to fix TMJ problems. TMJ experts talk about having very specific designs to their splints/appliances to help stabilize the joints. If the appliance design/implementation is wrong, it won't help. If it's not worn when recommended to be worn, it won't help. Best to find a TMJ expert who can diagnose exactly what is going on (e.g. with the help of appropriate scans), who you believe and trust (ideally their explanation to you should make sense to you), and see what they recommend.

That's a question only our dentist can answer. They'll know what changes they're making and should be able to give you an indication of possible facial changes. In general one would hope that fixing a problem like that would improve your appearance - but you may like how you look today and find any change undesirable! Check with your dentist!

Sounds like an issue that needs to be diagnosed by a TMJ/TMD specialist. Don't think it can be explained without someone taking suitable scans and physically examining what is happening.

Whether you need a splint and what type of splint is best determined by a dentist with a good understanding of the TMJ - as I think you've already determined. I'm afraid we don't know of dentists we could recommend in Malaysia. Thailand is probably the closest. You could try the find-a-dentist functions provided by either the Dawson Academy or The Pankey Institute. I think they have wider international databases than we do.

My left side protrudes out when I open my mouth and has since I was a teenager? Would a splint help this or is that an anatomical issue?

That would suggest you have a displaced TMJ disc on your left side which prevents your left condyle moving forwards as it should when opening. The longer that has been the situation, the more difficult it may be to correct. However, only a dentist who understands your TMJ or a TMJ specialist can tell you for sure what is going on and whether it can be corrected. You'd want them to at least do a CBCT scan (advanced type of X-ray but doesn't show soft tissue, like the disc, clearly) or an MRI to be sure of the state of things. Sometimes a very specially designed splint can help but that's by no means certain, and it's not just any splint.

@@GA-op7we Some problems require teeth to be moved to be resolved. The fact that your dentist is asking for the treatment plant to be sent to him suggests he has a vision for how your problem can be resolved and wants to make sure that the orthodontist is in tune with that vision. I'd ask either or both dentist and orthodontist to explain what that vision is so you know what the plan is and confirm that they're both on the same page.

hi i have visted a tmj specialist and also went to dental college and shown my xrays of opg and tmj open and closed view , both people said to me your problem is not major and my tmj specialist suggested a hard/soft stabilization splint and dental college people said couple of jaw stretch and strengthening excercies enough to cure your problem and all you need to relax your jaw muscles i dont know what to do ,do i need to take that stabilization splint or excercises are enough to cure my problem, and my sympotoms are mild but constant there is always some little pain i am experiencing left side, what you gonna suggest me do i need to take a hard/ soft stabilization splint or excercises are enough ?

First, know that I'm not a dentist, we've just worked with many top dentists to produce these animations and videos. Over the years we've gained good insights into how the TMJ works and how it can go wrong. The dentists and specialists who have the scans and can examine you are in the best position to diagnose your condition. That said, the dentists and TMJ specialists that we work with use CBCT scans or MRIs to discern exactly what is going on in the TMJ. A 2-D X-ray image does not provide sufficient information for a confident diagnosis. Pain in your TMJ indicates that something isn't right. Exercises may relieve the symptoms - and we had a physiotherapist on our monthly webinar last week describing how she helps TMD patients in collaboration with TMJ-savvy dentists. She was clear that patients need help from both physio and dentist to bring them to full health. A stabilization splint has a chance of resolving your issue so definitely worth considering. Ideally the TMJ specialist would take a CBCT or MRI to confirm the problem, but it may be that they are confident enough from examining you that they know your problem is minor and the splint is the way to go.

Options: 1. Use our Find an Occlusally Aware Dentist page: hubspot.bitefx.com/help-me-find-occlusally-aware-dentists. We'll respond with the dentists we know in your area that we believe can help you. 2. Use similar functions provided by The Dawson Academy or The Pankey Institute. Both train dentists in the understanding and application of splints. Other institutions also do the training but I don't know for sure if they have find-a-dentist functions. These would be The Kois Center, OBI BioAesthetic Institute, and Spear Education. 3. You might search for a dentist in your area that mentions skills like: occlusal splints, TMJ or TMD, occlusal disease, or training with any of the above institutes.

I think you are saying that you are rubbing or grinding teeth together at night. A splint can help protect your teeth and might stop the grinding - if the cause of grinding is that a couple of teeth are coming together earlier than the rest. If you are grinding so much that you are shredding splints, some dentists will create splints on both upper and lower sets of teeth so if you grind you are just rubbing the two splints together - and they are designed with flat surfaces so you're just rubbing flat surface against flat surface with much reduced destructive forces.

These are all questions that your dentist needs to answer based on a good diagnosis of your current situation. For minor TMJ issues full recovery may be possible; for more severe issues it's more a case of stopping continued deterioration rather than recovering to full health. A splint is serving a different purpose than a mouth guard so the two are not interchangeable. Whether or not it is wise to box when you have a TMJ issue would again be a question for your dentist. With a TMJ problem, one of the areas of your body that is a prime target for a boxing opponent is damaged - do you want to spend much time and money to have things repaired or stabilized in that area only to be destroyed by a well-aimed punch? But you may feel that is just part of the risk of boxing.

@@BiteFX by minor do you mean just muscular non clicking and lock jaw issues? Thanks, i might just shift to wrestling (also i really meant muay thai instead of boxing). My dentist said i should stop but was just getting a second opinion about if my tmj is cured can i start again.

@@FM-dm8xj Some clicking and lock jaw issues are fixable. Sometimes you can have clicking, but it goes away, however, that can be a sign that things have got worse. Basically there's no hard-and-fast rule that says a particular set of symptoms you observe means your problem is fixable or not. Needs a TMJ-experienced dentist, with the appropriate scans, to diagnose and make that determination.

That confirms the disc in your right joint is displaced when your jaw is closed and snaps back onto the condyle when you open. However, you need to see a dentist who understands the relationships between your teeth and your TMJs to diagnose whether this is causing other problems and needs to be treated in some way.

@@fovos7359 That would be consistent with the disc being displaced and snapping back into place as your jaw opens and/or moves forward. Appropriate treatment depends on a number of factors which is why I'm saying to find a dentist who indicates they understand the connection between teeth and TMJs.

I have a tooth that shifted out of place and its my right pre molar. Could that be a sign? Take note that i had straight teeth my whole life but for some reason one tooth decides to move causing it to misalign

@@fovos7359 The tooth movement is definitely a sign. It has moved for a reason. The right dentist should be able to make the connection between tooth and TMJ issues. The sooner you get to see that person the better as things will likely continue to deteriorate unless the underlying cause is addressed. Understand. The occlusal splint should be a short-term measure. It allows your condyles (jaw bones) to settle into their fully seated position in the joints (can take a while as your muscles and TMJ disc adjust to the new position) and allows the dentist to confirm that they have the right jaw position and required tooth adjustments. Once that is done, the dentist can propose permanent changes to your teeth that negate the need for wearing a splint. One of the leading dentists who use BiteFX, Dr. Chris Toomey, has developed a technique in the last year or two in which he builds up the teeth instead of creating a splint. It's still a temporary measure but I'm sure would be more to your liking - I'm sure that's one of the reasons Dr. Toomey is having so much success with this technique. He's in Towson, MD, a suburb of Baltimore - a long shot that you're anywhere close to him.

@@fovos7359 By "healthy bite", I mean that when you close your teeth together, your condyle is fully seated in the fossa (the part of your skull that houses the jaw joint) and that all your teeth come together simultaneously. A healthy TMJ would have the condyle encapsulated by the disc in all parts of its motion; that would be stable. It's also possible for the TMJ to be stable even though the disc is displaced - that's a trickier determination the dentist has to make over a period of time. If the disc is clicking off and back on during opening and closing, then the teeth should still be coming together as they would if everything were healthy. In that case, fixing the clicking would not involve any adjustments to the bite/teeth. However, if the disc deteriorates (changes thickness or shape) because of the clicking on and off OR if the jaw:tooth geometry wasn't healthy in the first place, then a long-term solution is likely to require some changes to the teeth. Is that clearer?

Actually I’m curious about this because I too find my lower mandible has been pushed forward, when I’m at rest without my splint in it seems to want to move forward if that makes sense could the cause of this be my anterior disc with reduction or could the splint have caused this, my lower mandible feels and visually looks way more forward than ever before.

Victoria - as you've shared in other comments that you have a degenerating condyle, I'll answer assuming that's the case. We have other animations that illustrate this but hopefully you'll understand the following. Imagine your condyle is fully seated and that all your teeth are coming together. Now imagine that your condyle gradually becomes shorter. This will cause the jaw to rotate towards the condyle so your teeth only contact at the back and your front teeth separate. If you want to bring your teeth together, you have to move your jaw forward and rotate. The shorter your condyle becomes the more you have to move forward to achieve the closure.

@@Nandinijkl Price varies by dentist depending on what the dentist expects to deliver with the splint. In a survey of dentists in the US we conducted in 2016, price varied from $500 to $4,000. The lowest prices probably assumed this was a quick fix solution with later treatment charged separately, the highest prices assume complete tuning of the splint will require several visits over several months.

Splint therapy may be part of correcting an overjet issue but will not be all that is required. It may also be that no splint therapy is required. Only a TMJ/occlusally aware dentist, likely in collaboration with an orthodontist, can determine for sure.

If you have no treatment, then things will likely continue to get worse with teeth eroding more rapidly, breaking, and possibly falling out. Jaw popping usually indicates your TMJ disc is still intact and could be recoverable. Treatment will likely be involved, but worth seeing if a dentist can give you options to slow or halt the deterioration. Make sure the dentist understands the TMJ and occlusion.

Unlikely. I haven't heard any TMJ expert describe unclogging ears as an additional benefit of using a splint to improve TMD issues. You'd want an ENT doctor with a TMJ (dentist) specialist to confirm this was a viable treatment option.

That varies according to your condition and what you and your dentist agree is the goal. Sometimes the splint is used for a limited time to make sure that your jaw has found it's long-term fully seated position. That might still be two or three months. Thereafter teeth are modified to match the fully seated position. Sometimes a splint is used to protect restorations so could be a long-term option.

It would normally be used as preparation for further work from the dentist. The splint isn't the fundamental cause of the teeth seating improperly - it is demonstrating how the teeth fit when the jaw is fully seated which will happen anyway when the jaw isn't being postured forward by the muscles. The splint demonstrates how the jaw/teeth will feel when properly adjusted. Hope that makes sense.

If you're still grinding/moving your jaw at night, you're still doing damage. Far be it from me to tell a dentist he's wrong, but this goes against everything my dentist has told me. Simply putting a pad in your mouth to "protect" your teeth but not addressing why you're moving your jaw like that isn't actually doing any good. In fact, one might think you'd grind harder as there's a foreign object in your mouth. I have an appliance for night time that is supposed to suspend my jaw and keep my airway open. It doesn't just pad the teeth.

There is more than one cause for grinding your teeth, and more than one type of splint that dentists can provide. If you are grinding your teeth because of stress, for example, then a splint may not stop you grinding but can give your teeth protection - instead of cusps rubbing on cusps, you have cusps rubbing on a flat surface that spreads out the force. If you are grinding because of a premature interference - i.e. an upper and lower tooth contact before all the rest - then a properly designed splint can be the first step in confirming the problem and moving towards treatment. It may take a few months of wearing and adjusting the splint for stability to be achieved as muscles and other parts of the system adjust to the fact you no longer have a premature contact (interference). However, it sounds like your dentist has given you a splint to deal with an apnea problem. The primary goal of that splint is to keep your jaw forward and your airway open. It is not designed to stop your grinding nor deal with a premature interference. Keeping your airway open keeps you alive. Stopping you grinding helps save your teeth. Addressing your airway problem is most certainly the top priority. Hope that helps!

@@BiteFX the splint may "protect" the teeth from contacting one another but it won't stop the pressure. That's what I'm saying. In your animation, the teeth are still moving around and sure. The surfaces aren't touching but the pressure is still being exerted and at night it's immense. It's not enough to just pad the teeth.

@@diy_cat9817 A properly designed splint is doing more than padding the teeth, but if you have a splint whose main function is to keep your airway open, it likely won't have the design features for that added protection. The force that can be applied is immense but you're better having that spread across many teeth than between just a couple, which is often the case without the appliance. However, I think, as you said in your first message, you want the root cause treated, not just given a protective device that will break down over time, and I'd agree with you that that should be the goal. First make sure you continue breathing, then work on what is causing the grinding.

It's a type of splint designed by Dr. John Kois to help the jaw settle to its fully seated position. The "deprogramming" refers to helping the pterygoid muscles relax so the jaw can find that position. Why do you find it so bad? Does it not fit well? Is it inconvenient? A friend recently received a well-designed splint and the immediate relief from recurring headaches has her thrilled. Yours may not be the same situation, but I know they can be helpful when designed well.

@@HermesTrimegistos If the discomfort is because your "bite" doesn't feel right, you may need to ask your dentist to adjust the splint. Wearing a splint can allow jaw-positioning muscles to relax so the position of your jaw changes and the splint needs to be adjusted. This is quite common. If it's more because you'd prefer not to have something in your mouth, you may need to tough it out!

There's a Facebook discussion group monitored by many dentists/TMJ experts called "TMJ/TMD Discussion Group". Posting your issue there should give you some helpful feedback. They'll need more detail, though, such as: is your mouth stuck open or do you just feel the stiffness when you open your mouth? where do you feel the stiffness - in your cheeks, higher up or more internally near your ears, or in your temple muscles?
