Very nice video! Very well explained, congratulations. I wanted to ask you if the trust in this case is carried out with a push movement on the facet joint or with a rotation movement towards the opposite side. Many thanks for your time and video. Greetings from Spain

Pier Carlo Agostinelli thank u for the question!! Glad u like the video. The dominant hand is pushing, the non-dominant hand is pulling into rotation. The combined force produces a down glide on the treated side to maintain facet apposition, and a gapping on the opposite side that we believe generates the negative internal pressure that actually causes the cavitation.

@@bodyworkdojo Many thanks for your explanations. Cervical manipulations are very complex techniques and require a lot of precision and good biomechanics by the therapist. I have attended many courses but I must say that in a few minutes you have solved many of my doubts. You now have a new subscriber for your channel :)

@@anthonypaganin5500 thanks for the questions. Any time you twist the cervical spine, whether you are mobilizing, stretching or manipulating there is going to be some stress on the artery. The key is to never perform your manip at end range and do not overthrust. Always use combined motion to lock the facets. You cannot avoid artery stress, but you can minimize it greatly with good technique.

Great video. Do you think that having too much upper cervical rotation could have been the reason, when some patients have been paralyzed or died after neck adjustment? And I see a lot of comments asking more videos like this, but I guess you haven't been able to do those yet?

Thank you. Yes, I have been teaching a lot but haven't had time to make videos, but I hope to start again soon. As far as I know, there aren't any verified incidences of paralyzation after cervical manipulation. Dissecting the carotid artery is the primary concern. However, the manipulation does not likely dissect the artery, but rather the individual is already having dissection which is causing their neck pain, gets a manipulation, and then is found to have this underlying problem. If deaths have occurred it is likely for this reason.

Thanks for the video! It's one of the fewer comprehensive videos about manipulation on KZfaq, helped a lot. Quick question, as you mentioned the dominant hand performs a Downglide and non-dominant hand performs an upglide, is the motion of force basically facing each other? E.g if treating the right side you downglide towards their left shoulder with one hand, and then upglide up towards their right eye with other hand. And so the main thrusting action is still just a downglide rather than a little twist from locking position?

Thanks for the awesome comment. Basically, yes. The downglide hand creates most of the lock. The other hand provides the rotation. But most of the thrust is into the downglide so as to avoid wrenching the neck into excessive rotation. If done wrong the patient will feel jamming on that side, so the rotation must be included to allow dissipation of force. That said, it is a combined motion using both hands (bimanual).

I totally agree with you with what you said about the cavitation mattering. Do you happen to know what this so-called literature is out there cuz I hear clinicians bring this up all the time but I bet 99% of them couldnt even give me the article on it

Hey Mr. Cooper, My Chiropractor says doing a neck adjustment while sitting would really hurt so he doesn’t ever do that. I would think it might feel good if I had a bad headache. Would it really hurt? And btw I happen to crack my fingers all the time. Is this going to weaken my hand grip? Thanks..

I take it you mean you’re sitting not the chiropractor. Sitting adjustments are similar to supine. The primary difference is the weight of the head is resting on the neck so some people experience more discomfort. But it depends on the manipulation being performed. And no, cracking your knuckles will not weaken your grip, however, compulsively cracking a joint can stretch the ligaments, which has the potential to make the joint less stable.

It also depends if the chiro is trained in that area. Some of my pts can't relax so doing sitting adjustments really helps!

you ara a great teacher, , new sub here! I feel more comfortable manipulating people with short necks, but actually my girfriend is skinny and has a long neck, do you have a particular technique for long necks? any pointers will be helpful. She´s become more untrusty due to losing the lock and not getting the adjustment, plus she gets tense and while i think i got the lock, I didn´t have it in the first place. So it´s a vicious circle. (I might go to the zoo and practice with a giraffe first, jk)

Hey Paul. Physios generally dislike the term "adjustment" because physios generally don't believe in the subluxation theory. I, on the other hand, this it's a gentle term that resonates with patients. Sounds better than "manipulation."

There’s actually a very large number of Chiro’s who also believe the same as me. Look up evidence based Chiro’s if you’d like to learn more about them. But I had no idea this term adjustment was so ....offensive. I never associated it with subluxation theory. I feel like a renegade saying it now hahaha (just kidding).

How would you adapt this setup if the patient has a neck spasm and can not move their neck?

Thanks for asking. It helps to address the spasm first with soft tissue work, heat, and whatever else you can before attempting the HVLA. Sometimes the HVLA knocks out the spasm. Manipulate in the less painful direction. Build the barrier slowly and take your time. It may take a few minutes to set up a good manip when spasm is present. Hope that helps.

Osteo is much more holistic. Manipulations are one of the many techniques we use (mostly by structural therapist).

In literature says if there is myelopathy, you should stop but i learned from courses for manuplation techniques and instructure said if there's myelopathy sign, then you should examine the patients neck, bend forward + rotation, pain icreases = disc hernia ; Bend Backward + Rotation, pain icreases = Stenosis (spondylosis, thick lig.flavum, facet cysct etc.) and he said basically you can perform manipulation away from the pain source if there is no low muscle weaknes lower than 3/5 in examination, what do you think about it ? according to literature there are cases of incidence between myelopathy and manuplation series

Thank for the question Omer. Appreciate you watching the video. If there are signs of myelopathy I would consider that a strict contraindication to manipulation and most other mobilization of the neck. Myelopathy is typically a surgical situation and if not dealt with properly can result in paralysis.

By "downglide" and "upglide" do you mean translation? Not sure what you're referring to there.. thanks!

An upglide is a pull technique that glides the superior facet superiorly, and a downglide is a push technique that glides the superior facet inferiorly. Up and down glides both induce segmental rotation. Translation would theoretically be induced by lateral glide or a PA. 👍

Up/downglides induce rotation and osteokinematic rotation produces up on one side and down on the other. They occur together.

Your not a chiro & don’t adjust everybody’s neck. Ahem.. not all chiropractors adjust everyone the same. Motion palpation is extraordinarily specific & thrust is special & direct.
