I was born with two of my discs in the spine being misaligned, always had pain. Whenever i tried deadlifting, it also hurt like hell. All my physical therapists always talked sbout strengthening the "small muscles" in my back to reduce the pain. And sure stretching etc reduced it for a few days. Finally in january i started doing SS, did the deads and they hurt like hell for the first few weeks in my lower back then it disapeared completly. Been pain free for nearly 8 months. It dawned on me why that was the case: The big muscles in the back where just weak. And if the big muscles are weak, training the smaller ones and stretching etc is irrellevant. Now these stretching movements might be something i'll have to incorporate when i'm in my 50s (i'm 32), but as of now ut's completly gone. And i went from pulling a plate and half, to pull 4 plates in these months. Pain free. Strength is key.

My closest friend is a PT and is somewhat concerned about my powerlifting routine. “You are going to hurt yourself, you’re not young anymore and I wouldn’t never recommend this for anyone over 50 especially you having back and shoulder issues .” Well both shoulders were repaired over 3 years ago and my back pain is gone. I took the leap, a big chance and I’ll be damned……my body looks and feels so much stronger now. He for some reason can’t except it. I’m 56, feeling amazing and stronger than ever. Thanks guys.

A lot of PTs forget that they're purpose is to help repair and assess moderate injury. Not prevent injury. Anyone doing anything will at some point get an injury of some kind, why use that as an excuse to be weak

A licensed plumber that I hired wasn't as skilled as I thought he was, and he messed some things up at my home. So should I simply conclude that all plumbers are worthless frauds? There are PTs who know what they are doing, and there are PTs who don't know what they are doing. Just as there are plumbers who know what they are doing, and plumbers who don't know what they are doing. There are teachers out there who are passionate about what they teach and who are invested 100% in each of their students. There are teachers out there who could care less about any of their students. There are "levels" to everything in life, my friend.

The overall concept is that strength training is PT, and the subject begins at the level at which they are capable. Most PTs don’t know what this means. Some do.

No, PTs only get you to a certain point and ice you but I’m a fitness trainer and took the principles for my injuries they taught me and continued on my own. Mainstream doctors wanted to give me a spinal fusion ! I’m proud to say that I said no

The late Jerry Pournelle, who had 3 ph.ds (Psychology, Political Science, and Aeronautical Engineering) formulated his "Iron Law of Bureaucracy" which simply says, when any institution begins, there is usually a certain percentage of members who actually want to perform the stated aims of the organization, but over time, membership increasingly becomes purely focused on increasing the institution's wealth, prestige, and power, even at the expense of original aims.

The key reason for this is that the types who are able to create are very limited. When the organization grows to the point of becoming public or let's say above 100, 250 employees the starters, entrepreneurs and creators get bored or kicked out. Then what happens is managers take over. Managers only hire people they like and who they can control. Which are more managers of lesser quality. So the entire institution becomes obsessed with containment instead of evolution/growth like you say. Obsessed with staying relevant while the world is moving on. Also look up company lifecycle it's a thing.

6 months of physical therapy didn’t do shit for me after a major car accident. The last month I restarted my LP only adding the McGill big 3 and I’m significantly better. Not 100% but getting there

Before ranting I want to clarify that most are good people trying to help others. That's admirable - Of course some of it works and changes lives. I'm focusing on what doesn't I'm a physical therapist and have realized that so much of the field is outdated and/or just wrong. It truely sickens me. Much like docters, many don't think for themselves. Especially when it comes to rehab. There is barely any progresion. It's okay to do silly band excersices when you just had shoulder surgery. But I was tought to make clients do this for several weeks. I of course made them do harder thisngs when they could, but it felt kinda "wrong", because i was tough to do the same bullshit forever and that other excersices would be dangerous. Now that I've been licensed(or whatever it's called) for 9 years, I'm doing whatever actually works. I'm MUCH worse at finding whatever small singular muscle might be causing the problem, than my peers, but it doesn't really matter. The approach to rehab and progression stays the same. I'm spending way less time in the clinic and more in the gym. Because it works better!

even if intent isn't there, it's still legally fraud. also, if progress isn't shown in restorative care, insurances may not reimburse and patients may get a bill. it's on the provider as to whether they make the patient pay.

The Dr. as high priest has been around for a long time. In the 1930s, it was common for women about to give birth to be prevented from doing so until the Dr. was able to be there. I only have anecdotal evidence though.

People within incidental back pain under a specific movement should have imaging done first. They want to send me to PT... when it should be reserved for people that have constant pain. Imaging and a chiropractor. I went to one session and they showed me simple stretches that I already know.

This episode was hard to get through, and not because of you guys but the subject. Back in 08' I had a match in UCE and my opponent Stevenson Keith had me in side mount and I bucked up to shrimp out and he came down on my toes pinning my foot. I won the fight via RNC but realized getting dressed I couldn't put my shoe on because of swelling. Two weeks went by and I kept reinjuring it so I went to a doctor who rolled his eyes and assured me it wasn't broken because there was no way I'd be working on it. I told him I've had spranged ankles before and this was different and I'm paying cash anyway so let's just do the x-ray. He comes back in and says I told you everything is fine just rest it a little longer. And on my way put the door my dumb no PhD having ass turns and takes one glance at my x-ray and sees two bones that should be one. I asked "what's that?" pointing to it. He puts on his glasses and says, "oh! Looks like you broke the end of your fibula off." Thanks doc you're worth every penny. Fast forward to 2013 I'm in my forend Dane's guard. He gets wrist control and puts his left shin across my stomach with his right leg next to my left and I know he's gonna try a scissor sweep so I plant my weight down but him being as strong as he is popped my knee. The whole gym heard it and I could not bend my leg now. I made an appointment with the Black Hills Orthopedic Spine Center. It's the size of a university surely they can help me out. I get the MRI and before I leave I tell the secretary I'm not allowed to have a phone at work. It's a fireable offense and would only be available week days after 3pm. She says sure thing. And two weeks pass and I don't hear back. Mean while I'm still hobbling around on a leg that doesn't bend working full time at a saw mill. I call them and ask what's the issue? They say they'll get back to me. Monday I have a message left at 9am. I call back, the doctor is out, I say I can only talk after 3pm, she says okay. Tuesday I have a message left at 9am. I call back, the doc is gone, I tell the lady I'm only available after 3pm, she says understood. Wednesday I have a message at 9am. I call back, the doc is out, but this time I wait on hold for 45 minutes to talk to someone in authority. And I mean wait because it's the black Hills and I may lose service driving home. I finally get whom I'm told is an overseer. I explain that it's been three weeks since my MRI and I've told them four times now about my availability and I would like to know why my knee doesn't bend. And she got very upset at me and told me that my doctor is a great man. So great in fact he does all this charity work for kids and this cause and that and I haven't been diagnosed and my pleas ignored for three weeks because he's such a good man and I'm practically selfish to what five minutes to tell me what in the fuck is wrong with my leg. My suggested I be transferred to a doctor that isn't such a humanitarian and can take five minutes from saving children to tell me why my leg no longer bends. And was told they'd get back to me. I started having nightmares about running in there and punching doctors and performing double leg take downs on nurses. And one week later they did call after 3 to let me know my MRI results were given to a sports doctor because apparently I wasn't assigned a sports doctor to begin with despite this being a BJJ related injury in the first place. And my new doctor would get in touch with me in another week. Finally after five weeks with a leg that would not bend i got my results of which I had to take the day off work because they couldn't tell me over the phone because phone conversations aren't four hundred dollars. It turned out I actually broke the knee cap itself and had some other fun chips and tears to all the connective parts inside a knee that make it function. And the swelling was inside pushing out in all directions keeping me from moving it but because I'd been working on it for so long I made it worse and that was pretty much the end of my MMA career. I apologize profusely over the length of this and I'm sure there are some grammar errors but I can not sum up quickly the amount of times the medical community has fucked me over because of arrogance and just plain lack of caring. If you read this Mr. Rippetoe be kind, I'm one of the lovers. Anyway nine years later I'm happy to say I'm squatting 405lb because of you guys.

BTW, surgeon's office said no consideration of surgery unless/until lower limb numbness or more serious symptoms. That's all good.

Hard to find a PT that you get for the whole hour I found one but I had trial and error. I basically picked their brain till I got what I needed. Regular doctors suck I go to a functional Medicine /naturopath doctor but insurance doesn’t pay for. But I’m healthier for it. Health is more important then money

DPT enters chat…There’s definitely some truth to this statement and without getting into the insurance and healthcare bureaucracy aspect I will tell you this. As a whole, Physical Therapists are considered “movement experts” upon graduation and receiving their “Doctorate of Physical Therapy”. That said, without my instruction and the instruction of a few other classmates with a strength training and fitness background, our class would have graduated without knowing how to properly squat, deadlift, and press. Hell, most of our professors were in shock when we used these movements in a physical therapy setting. One professor didn’t even know what a hip hinge was, probably still doesn’t. In addition, the way most insurance based out patient physical therapy clinics are set up most clinicians are seeing between 15-20 patients per day. This means they are double and triple booked outside of initial evaluations. How would you feel if you went to your dentist, lawyer, physician and they were seeing multiple people at the same time? This fact alone make me think that physical therapy is much more of a fraud than its not. At best it’s watered down group exercise. As mentioned in the video, cash-based practitioners are a huge part of the solution. Help patients find the root cause of their problem, fix the problem, educate them in a 1:1 basis each time they are seen so their injury doesn’t happen again. This happens by transitioning them from “physical therapy” to a proven program like starting strength so they can continue to become stronger, more resilient, and more useful human beings as they age. In the traditional model, once PT is done, most patients go back to their sedentary ways and the cycle repeats itself.

Peds OT here! This! My PT was like: look you need to transition to a trainer, theres no point in coming because you have the tools and groundwork. ACL has stood strong since with max A transfers from the floor🎉
