Today I review 5 practice questions that cover various topics and I apply the SFAREAFI acronym as applicable.
The ASWB Clinical Exam, or the Association of Social Work Boards Clinical Exam, is a test that was made to determine whether or not an individual's level of knowledge of social work in a clinical setting is sufficient. You will have four hours to complete 170 multiple choice questions, including 20 pre-test questions, that test your knowledge in Human Behavior, Diversity Issues, Diagnosis and Assessment, Psychotherapy, Communication, Therapeutic Relationships, Professional Ethics, Clinical Supervision, Practice Evaluation, Service Delivery, and Clinical Practice and Management. To register, you must have at least a Master's degree from a social work program, and two years of clinical social work experience.

@@mofesolaadeola8611 I used Dawn Apgar's study book and Phil Luttrell's videos. Twice I practiced taking the test without food or drink at the time of the day I was scheduled to take it. I sat for the 4 hours in my rv (no distractions), because I knew sitting still and concentrating that long would be the problem.

Just passed my test! Someone told me about your videos on KZfaq! I think what saved me in passing the test was remembering the safety, feelings, assessment section when it came to application questions. I'm a community organizer so this test was definitely hard because I wasn't really taught clinical stuff. Thank you!!!

Hello Phil!!! Hey I been rocking and rolling with you for about 2-3 years or might have been during the pandemic. I told my exam April 2021 and missed it by 8 points but I taking it next week December 2022. I declare by Faith I will PASS!!! It has been a long journey since 2008 of making attempts. I refuse to quit but I claim surely next week will be my last time taking it. I love how you provide the motivation, inspiration, and break down questions. I came up with a way to remember the Application Steps. I will put in a separate comment... Lastly @PhillipLuttrell I need you to get READY to Announce my Name on you December video!!!lol

This way of breaking down questions is helpful. I failed my clinical 3 times and need AND WILL pass it this time around. How do i enroll in some tutoring. I love how encouraging and supportive you are.

First of all, thank you so much for these amazing videos. They are extremely helpful in preparing for this exam. Secondly, the thing I find frustrating about ASWB exam questions is that they're completely based on social work theory which does not always fit within real world application of social work and clinical work, especially in multidisciplinary settings. I've been in the field for 15 years and am just now about to take my clinical exam, and sometimes to me there will be a clear answer of what would be the correct action in a situation, but then I'll get the question wrong because it doesn't fit the theory. I guess it sometimes just doesn't feel like this exam is really testing applicable knowledge and understanding of social work. Which is the long way to say thank you for translating what we should be prioritizing in these questions!

A bit confused on # 5. Feelings comes before Assess and the question states "AFTER completing the assessment;" so even if it is not explicitly stated in the question, wouldn't the SW have already addressed feelings? Wouldn't the next step be to refer?

I got number 2 and 3 wrong. #3 I was torn between A&D because I didn't think we needed to refer right then but thanks for breaking that down. #2 was kind of tricky as well cause I was thinking of schizophrenia or delusional features instead of mood disorder

I'm still confused about this one because other questions I've seen that are similar don't have us referring the client. I'm still trying to wrap my head around when it's okay to keep and need to refer out!

Question ✋ For the last question (the 50 year old client) I am a little confused with why we have to go up on the acronym on feelings. The question says that we completed an assessment wouldn't you go to "refer?" instead of "feelings?" any help is greatly appreciated! cheers

So confused! Someone please help with number 3. EVERY time there is problematic transference in the early stages of therapy, we need to refer out? Then how the heck does anyone treat people with severe borderline who instantly overvalue the therapeutic relationship? Are we just going to pass these clients around from agency to agency forever?

Hi @phillipluttrell. Can you explain to me why the answer to question #1 would be solution focused therapy and not task-centered therapy? My understanding is that solution focused therapy looks at the strengths of the client, whereas task-centered looks at resolving the prominent presenting problems. Thanks!

Task centered therapy requires the client to be a bit more independent and do the work on their own outside of therapy. It would be challenging for this client to do this work on their own at the time since he seems to be very depressed.

Solution focused focused on clients strengths and problem solving (low self esteem)and task centered focuses on specific tasks the client needs to do to solve problems (steps needed to take to blank blank blank…)task centered would be useful if we are helping the homeless man find a job or something

I apologize for not getting back to you until now as things have been busy on my end. The acronym is a tool and not the 100% law. It does not always go in order because the meaning of the answer choice is more important than the order of the acronym.

I just typed a long comment and lost it all so I will try my best to recall what my concern was. I had difficulties with number 3 here as D was the first answer I ruled out. This is only the first real therapy session with this client, the boundaries of this relationship may not yet be so clear to the client. I would fear that by referring to someone else would cause the client to feel shame or guilt over their natural feelings. I also would fear they would feel abandoned and not feel like they could trust a therapist again to be honest with them and trust that they would still be there for them. It just seems hasty to me because we are assuming the client would not be able to handle the truth and that their feelings are already so strong that they would not be able to let them go and still get benefit from continuing with this therapist. I would think the therapist wants to model honest and clear communication and working together for a solution. If it were me I would feel like I would first need to clarify the boundaries with the client and talk to them about it to really get a feel as to whether they really need to be referred to someone else. I worry about making a client feel abandoned. It seems like you would first want to try to come up with a solution and if that doesn't work, then refer. I know this could waste their time if it doesn't end up working and it keeps them from building rapport sooner with another therapist but I have had clients I thought might be better served by another therapist and was told that I should try anything before I abandon them because that is what is ethical. I can see how my situation might be different since it has to do with MY feelings and the first situation is based on the CLIENT's feelings, but I feel there is woo much assuming on the therapists part that leads to the referral. I'm just sharing because this is how I would think about this question and if I got this wrong, I may not do so well on the exam. But maybe the answer is D because there was no answer that included telling the client that dinner is inappropriate and explaining the boundaries and checking with the client to see if they would be able to move on and continue to work together, then if it didn't work, the referral would be made? I mean, if this were an option for an answer, could that be correct? I'm just thinking that many clients are attracted to their therapist and is it only because they dont share this information that they continue to work with said therapist, and often it still benefits them? If this is the case, it seems this client is being punished for being honest.

For the 50 year old male experiencing grief, the vignette states that an assessment has already been done, wouldn't that indicate that feelings had already been addressed as well since feelings should be addressed before assessment? I also believe group therapy is the indicated therapy for grief based on empirical evidence, therefore, shouldn't refer to grief group be the answer? Thoughts?

For me, what helped was thinking about what I do next after the BPSA, which would be the treatment plan. Plus, when there are many things going on with a client, you always want to prioritize what they want to work on!

Like others have said can you explain #3 better? Other similar questions have not resulted in referring out... in fact you have used the logic that if the social worker can address with client without referring out or seeking supervision that is usually BEST thing to do FIRST in those situations. Then those things could be done if it was the NEXT thing to do. Hopefully these rationales align with ASWB but its all rather confusing. I ethically question all these online tutoring services for the exams if their "product" isn't actually preparing for the exam rationale. I guess if they can confuse enough social workers they will get people to buy more study sessions and give more money...

Of course :) My first recommendation would be to NEVER EVER EVER EVER compare questions to each other, as each one is different. My second recommendation is to start viewing FIRST/NEXT/BEST/MOST questions as the same as that ONE word does not change how you answer the question. In the question the social worker attempts to set boundaries, yet the client oversteps them and asks the social worker on a date. Therefore, appropriate therapeutic boundaries are not possible and as social workers we do not need to subject ourselves to us uncomfortable situations. As for the rationales they align with the Code of Ethics, but they are created by me so they are not 100% carbon copies of the ASWB exam. I can understand your stance and I am not challenging it as it is your view. All I can say is that I have helped over 2,000 social workers PASS their exams and continue to provide FREE content for those that are unable to financially rock with me.

Some of your answers are incorrect, such as the one that asked who the co-worker should go to after hearing gossip. Three separate social work professors said the answer would be to go directly to the person, not to a supervisor first.

I would encourage you to refer to the Code of Ethics because it states "when feasible," which means that if there is a barrier such as discomfort, then you would not HAVE TO go to a coworker. GOOD LUCK on taking your exam :)
