During just two shifts with West Midlands Ambulance Service, staff deal with a man who has been stabbed five times, a drink driver who's rolled his car and an alcoholic who refuses help.
One hour into the night shift, the control centre is already on its third cardiac arrest. The highly skilled team are administering care to a patient who has been stabbed and is bleeding heavily - the twelfth stabbing victim the service has dealt with so far that weekend.
Four miles away, another crew must stay with an elderly man long after they have treated him as they have spotted that he has run out of vital medication, and no-one else is available at this time of night to organise a refill.
A family at their wits' end and with nowhere else to turn asks the ambulance service for help with their relative who is an alcoholic and refusing medical attention, despite their heartfelt pleas. It is not immediately life threatening, but it ties up the ambulance crew for hours and is a poignant reminder for paramedic Maya of her own experiences of living alongside relatives struggling with addiction.

I was so mad at the guy who was drink driving, but then his father said "he did nothing wrong" and boy did I just get a Hell of a lot madder. Holy wow, how can you have such an attitude, that when your kid have totaled 2 cars while DUI, the first worry was to make sure he didn't get arrested or face consequences. I bet as soon as he can, that man is back on the road - with or without his license

Some people never learn. A man in a county 20 km from were I live was both drunk and high when he hit a girl with his car. He had no driver's license either. She died on the spot. He was sentenced to only a couple of years behind bars. I heard he was back driving as soon as he was out of prison.

That elderly man John is such a sweetheart, my heart was hurting for him, I know what is was to have elderly parents plus I worked in healthcare for 9 years. Some of my clients had family to look after them when I was off duty and some didn't, sometimes when my shift was over and I would leave they would get teary eyed because they didn't want to be left alone- it was heart breaking. 😢😟

**long comment sharing some of my experiences as a paramedic** I worked dispatch for my local ambulance company here in the US for a month after getting injured on a call as a paramedic. Sitting there not being able to physically HELP was light years more stressful than actually racing there & treating the patient. I did it but there is just no way I could do that job for years. I truly admire those who do! It doesn’t help that the area my company covers is 8 counties with only 3-4 large towns...the rest is all very rural & wooded. So it can take quite a long time to get to people via ground ambulance. When I was working we only had one helicopter ambulance. Thankfully 2yrs ago they added a second one. After I became an ER nurse as well as a paramedic I worked a few shifts every couple weeks on the air ambulance. Those were some of the most memorable calls I worked. Due to the area being so wooded I was extremely glad we had a couple amazing pilots who kept us safe. Even if they did scare me more than once!😂 It really helps that HERE all police, fire & other first responders are trained how to identify & Mark a landing zone for the helicopter...esp at night. Without that there would’ve been no point in us going on many calls. We’ve landed in the middle of roads, front/back yards, fields, cow pastures(poor cows)...so many different odd places & often in far smaller spaces than I thought we could fit. But that’s when u just have to trust the pilot. The helicopter I worked on was big enough that we could carry 2 patients laying down & one sitting upright if absolutely needed & weight permitting. I personally only had to do 3 patients twice. Once for 3 people & a 2yr old (so technically 4) who got caught canoeing one of the rural creeks during a freak bad storm in 50F weather. There were no roads near by but we found a clearing to land. The creek isn’t deep enough in a lot of places for a boat w/motor to pass. So we kinda HAD to take everyone at least to where ground ambulances were waiting. The mom was borderline hypothermic & the dad & his brother were seriously hypothermic. They had stayed huddled around the mom holding the baby so thankfully the baby was only mildly cold. We dropped off the mom & baby w/a ground unit & took the two men on to hospital. Everyone survived but one man & the baby ended up with pneumonia bad enough to stay in hospital for a week or so. Had they not huddled around the baby & wrapped everything they had managed to grab around them...the baby wouldn’t have made it In sure. Also had they not been able to finally get cell signal enough to say roughly were they were & had one road flare....things would’ve gone far different. Because they weren’t even EXPECTED for another 36hrs as they were doing a 2 day trip!😕😕 If they wouldve had to spend another 36hr plus the time it would take for us to find them...I doubt they all would’ve been alive. We worked a LOT of calls related to people on any of the dozen + waterways that flow thru thousands of acres of National Forest. When not on the ambulance I also volunteered with search & rescue which covered not only the waterways but also the literal hundreds of miles of hiking, horseback & ATV trails plus 21 camping areas in the local area. Plus for lost hunters or woodland explorers who go off trail & either get lost or hurt. I’ve lived in the area all my life & have hiked, rode on horseback or ATV many of the trails & canoed down all the local waterways many times from childhood onward. In search & rescue we are lucky that the local military base uses some of the National Forest to train in so they help us search by air & land. Their ability to use thermal to search by air can REALLY help. I’ve even flown with them a few times & along with others were lowered via rope & harness to help injured people until we could get the far smaller helicopter ambulance there. Wanna talk about scary! I never thought about it while doing it bc of being so focused on the patients. You HAVE to just put such emotions out of your mind & focus solely on helping those in need. It’s only AFTER that all those emotions come rushing in and hits you at once. And if the outcome isn’t all good then everything hits that much harder. Its why after such incidents with poor outcomes we have debriefings were we talk with both others who were there/a part of the incident...as well as others who weren’t. And there is always a pastor & some type of mental health professional to talk to if needed. But bad outcomes or even some really bad calls w/positive outcomes stick w/u for days, weeks, months or even years. They haunt your thoughts and even your dreams.😞 But when working u cant show those emotions to patients bc they are having an emergency & it could literally be the worst day of their lives & they are looking to those responding to be calm & help them through whatever is going on. If those there to help panic or breakdown it can actually harm the patient as they will likely panic worse and the family/friends can also panic further which could put them & others at risk. Appearing calm & thereby helping to calm others is often the best tool we can bring to any situation. That’s even more true in dispatch.

The love and compassion that the British first responders show is amazing. I've lived in the U.S. for 60 years and never had an ambulance person offer me tea! But I have heard multiple ambulance crew member intimate that they rather do something else or say they hate their job. Really? Then get the hell out! I seriously doubt that All of these Brit crews are only acting this way because a camera is there. They are mind blowingly amazing!

I wish there was a budget or a charity service for 'low-level' emergencies for the elderly or other minor things that need help but not a paramedic. the people working that can let the service know if a paramedic is needed, but if not, they can take care of the social service calls.

As an American, the concept of someone calling an ambulance and NOT wanting to go to the hospital is so odd to me. Like, I wouldn't even THINK of calling 911 unless I absolutely knew I needed to go to the hospital.

Right? We are like 99% dead before we even think about dropping $500-$1000 on JUST the ambulance ride not to mention the hospital expenses!!! So crazy how medical care is so different all over the world and there are definitely pros and cons of all different health care systems. But yeah, I wouldn't ever dream of calling an ambulance and I'd be like bleeding out and still not call. I'd rather die than live the rest of my life with that fucking nightmare Bill and harassment from debt collectors! Sad that we live in a world the technology, knowledge, and capacity to help so many more people but we let money and greed kill millions and make millions more suffer needlessly. I've lost almost all faith in humanity on 2020. Humans are cruel, selfish people.

I refuse to go these days because doctors & medical staff don’t know much about my illness & they blame it all on my weight & mental health (I have no mental health issues) why go to the ER just to be told your nuts? My pacemaker shocked them, they said I pass out for no reason. My heart was stopping🤦🏻‍♀️that’s only some of my issues. So yes I understand not wanting to go

It made me tear up. So much pain and regret in his eyes. I know what it feels like from his wife's perspective, except she was, seemingly, much smarter than I was. By the sound of it, she left the first time he laid hands on her, but I stayed with my abusive ex for many, many years, nearly a decade. While I'll never be able to bring myself to regret the relationship.. he's also the father of one of my kids..I do regret staying for as long as I did. I'm glad he was able to resolve his issues. He looked like an entirely different person in the picture at the end. Hopefully he learned his lesson and doesn't repeat old patterns. I wish him the best.

These are great videos and great series. After watching several episodes, I realize I could never be a paramedic or a nurse. I have no problem with blood and gore, broken bones sticking out of arms and legs, puke, etc., but pulling a stinky old man off a toilet in the middke of the night makes me cringe. I can't stand phlegm either. Someone coughing up a lung makes me sick. I'd have nightmares! I could never do that job. The paramedics in these programs are amazing people. I would never make it. Okay, it's a DUI, in Canada Police would have been on the scene so a breath analysis would have been taken before he left the site. In America if a person doesn’t want to go to the hospital the paramedics do not wait around for even 10 minutes let alone 4 hours! Since that guy didn’t want to go to the hospital and he wasn’t a danger to himself or others the paramedics should have left. By staying there with him your just holding up time that you could be helping other people that really need and want your help. Wasted tax payers money by waiting 4 hours for a drunk person to end up being forced to go to the hospital.

As the survivor of a head-on impaired collision, (Driver of the other vehicle was impaired, killed himself & his two passengers) I find the actions of both father and son reprehensible! What makes it worse, is that it sounds like it is the second time the son has done this, or that he admitted to at least. Not only is he putting his own life at risk, but the lives of the motoring public as well. Then his father reinforces the behaviour by springing him from the hospital, to avoid any possible repercussions, excellent example you are setting there dad!

When it comes to Mark, he’s comfortable in his situation right now. He’s in his right mind, he’s just an alcoholic and they can’t make him go to the hospital. The police can’t make him go either. He’s not committing a crime. The crew has been there for about 3 hrs as other cases are backing up. Hopefully, he will want to make the step to change his lifestyle. Self medicating never helps a situation because until Mark wants to, unfortunately he’s going to continue. He just can’t see past himself in this moment. It’s sad.

Just a bit of advice, although I don’t know if British law is same as USA, but the police would have HAS to come if the woman who owns the apartment would have called the police, or if the controller would have told the police the where wants him out.....that’s an eviction, and they have to assist. Just some problem-solving from almost 40 years of Ambulance/ER/Trauma RN! Great 👍🏽 job, though!🕊💕✝️✡️🙏♥️🙋‍♀️🌹

My heart hurts for all of them. I think the one that made me the most mad though was that young man who rolled his car driving drunk, then his dad comes and picks him up and said he did nothing wrong. He's going to kill someone. Mark broke my heart, he really needs help and he is lucky his family is there to support him. I can't remember the gentlemans name that tried to overdose 16 times but I pray for him also.

49:58 for an ambulance to be at a scene like this for hours is so bizarre. They aren’t miracle workers. Get in and get out. You can’t fix everyone’s personal problems and that’s not the job of an EMT. Medical intervention and get them to a hospital or morgue. Then move to the next scene. No wonder they are always short on help. It’s so weird.

I fully understand alcoholism is an awful addiction but to compare it to cancer is just foolish. Cancer patients WANT to live. They FIGHT to live even through it makes them so sick and weak. Alcoholics just make excuses. I've seen many get sober. It's a challenge but if you want it bad enough than you can get sober. No excuses. Soooo No I don't like alcoholism to be compare to cancer. My mom had cancer and fought with all she had and almost didn't make it. Thank goodness she won and is cancer free. I feel for them and I would like them to get help but they have to want it. Don't compare the two. They are way to different situations.

Drink driving should be prohibited globally. Cos of these lackadaisical irresponsible attitude, they are the ones who caused deaths on the roads, etc. They caused more expenses which could be channelled to more important cases/situations. It's high time that government should impose a very severe penalty on drunk drivers. 1) Suspend their driving license permanently. 2) The fine should increase to ... At least STG PDS 100K for first time offenders. For repeat offenders, the fine should be STG PDS 500K. If deaths occur to innocent victims, the drunk driver and his family to fully support the deceased family in areas of financial, housing, education, medical, insurance, everyday living expenses. The accused is to be jailed for life. If no jail sentence, then have the accused photo, details published so that everyone is aware AND if anyone saw the accused drinking, they can report him/her to the authorities. It's other peoples' lives. Innocent lives. Once gone, cannot get back. So, please consider this so that to be a law.

You cant make someone go to the hospital. You cant make them get help either. Why sit there and try and guilt them into something they don't want to do. You're wasting your time, breath, and resources. The first hour of trying, fine. But 3-4 hours? No.

The alcoholic that refused to go to the hospital, if it was here in the US the police would have put handcuffs on him and took him somewhere to get help. He's skin and bones. He only thinks about getting drunk. His liver will fail and he will die a painful death. Sad situation.

I lack all sympathy for the alcoholic who is wasting all that time. He’s a total selfish ass with no concern for anyone but himself, and in this case, the police did not act appropriately. They, too, are overstretched, but this is the fault of the system that cannot keep up with the population growth and the time wasters. They need to leave these time wasters and go to serious calls. There should be a waiver these type of patients so the truly needy patients get the help they deserve. This man has a huge family, loads of support, and there is loads of help for alcoholics, so leave him behind. All the nice music is fine, but the truth is ugly.

Philip, you can make a search your family with background checks. You just need their names, follow the program instructions, and it will tell you where your family members are, even if they live separately, in another state, it does not matter. IF you’ve tried those programs, they still won’t talk with you, that’s on them. You can make your own family with close friends, pets, etc.... DON’T EVER GIVE UP, PHILIP

The show’s Maya can pronounce her name any way she wants. Considering how much effort goes into these reality programs - including voiceovers from the crew members - it’s highly doubtful her name was mispronounced over & over & over again. Where did you get the idea there’s only ONE correct pronunciation & that yours is it? Don’t be ridiculous! 😠
