In this episode the surgeons discuss when it is appropriate to fix a broken hip and when it is appropriate to replace a broken hip. Fractures in the neck of the femur can be replaced or fixed, whereas fractures below the neck are usually fixed.

I was 50 and in good shape when I fell down 5 steps and landed on my hip. What I'd like to impress to nurses in hospitals is that breaking your hip and having a hip replacement is different to having a scheduled hip replacement due to arthritis. There is soft tissue damage which is more painful than the broken bone. I remember being flipped from the gurney by the porter onto the hospital bed with such force that I screamed out in pain and nurses came running. It was 3 days before they would give me effective pain relief: they had been giving me tylenol which did nothing. Eventually they gave me an adequate dose of hydromorphone. Even a doctor said, "we want you to be in pain", meaning I suppose that they would rather I have some feeling than to be zoned out on addictive pain meds. I'm not an addict risk. I'm not the only one who's been through this. They are so hell-bent on avoiding addiction that they don't want to give you adequate pain relief. This has to change; people in unbearable pain suffer slower recovery than people who have adequate pain relief.

I fell asleep standing up and fell and broke my hip. I fell at 4am but didn’t go to the doc til 6pm. They sent me to the ER. That’s where I found out I had a broken hip and pneumonia. I had surgery the next day. I have 2 pins in my hip. It’s been 5 wks and it still hurts. I’m so over this!

You guys rock! I love the channel and have watched loads of your videos to find answers to many different health questions I have, and you rarely disappoint. My mother in law fell on her kitchen floor earlier today and is awaiting a visit from orthopedic to determine the course of action. Trying to settle my wife's and my own nerves, I thought I'd see what 'The Docs' knew.....thank you so much for the information! Just giving advice to make sure it is titanium. In my case the doctor lied and said it was,I broke my ankle I was in a wheel chair for 4 years the doctor kept saying it was in my head. It turns out they out toxic metal in me after telling me it was titanium. It ruined my liver and everything. I got it put in at the top hospital for orthopedics in the country and the doctor was world class but he was unethical. I had to have a blood test and there are only 2 places in the country that do it. It was called Orthopedic analysis in Chicago. I live in NYC but has my blood sent there which is what most people do. iT could be the metal they used.

@@beckyholbrook7253 They told me it was titanium they lied and took money from China to put a toxic metal in me. The only way to know for sure is a blood test from orthopedics analysts and it was the top orthopedic hospital; in the country that only uses titanium

Thanks for another really good video! I especially appreciate your reference to the post-surgical mortality rate. In retrospect, a broken hip is usually referred to as 'the beginning of the end', for good reasons!

Another great and informative video Docs! This was a great explanation of the different types of fractures and associated surgeries! It also gives me reason not to fall. I don't have to worry about a ladder because of my fear of heights and I won't even climb a ladder because that's too high! 😉 I've been learning so much from you two and find all of your videos fascinating! I also commend you two for all the hard work you do on these videos, not to mention the hours you put into doctoring and surgeries, plus being husbands and/or fathers! You are truly an inspiration! That being said, one of you still has a mustache! Perhaps you filmed this video before the end of November because you told us it was for men's health month which ended November 30th! I've noticed men are still cheering you on while women are asking why you still have it. So I'll just say that you've been making videos for six years and it's hard getting used to the stache. If you like it and have decided to keep it, just say so and the women will stop asking. But maybe the more important question is does your wife like it? She's the only woman you have to answer to. It's time to fess up! If that has a different meaning in Canada, just confess, spill the beans, come clean and stop stringing us along. It's not that I hate it but I don't like it on you! No offense but I just can't get used to it since I've been watching your videos for quite a while. You may have a completely sound reason for keeping it. If that's the case, maybe in two or three years I'll get used to it! 😋 Thanks for sharing another great video! 👍

I appreciate learning why the femoral neck fracture couldn't be pinned and required a hemiarthroplasty. The surgeon just said that was the way to do it and did not explain about the blood supply situation. I also learned that at higher ages the bone takes too long to bond and heal when pinned so a replacement is better for the patient who is more elderly.

Another highly educational and valuable tutorial - you guys are skilled at presenting the subject in an understandable way for a layperson. I'm 78 and do 750 pushups and 300 reps with 50 lb. dumbbells each week, not for "body beautiful" but to reduce the opportunity for serious injury and necessary surgery in case I fall down. Keep these videos coming!

I'm 33 years old. I got around on a completely broken hip for 2 months. The sub capitol section. (I believe what your video said) After an xray i got admitted for an emergency total hip replacement. The surgeon couldnt believe it had been broken for 2 months

I am a 62yo registered nurse who still works full time in a very busy and active role. I recently had a subcapital fractured hip after tripping on the footpath at home. I had my hands full at the time and was unable to break my fall. I landed heavily on my side and immediately was unable to weightbear. Interestingly the ambulance personel did not think I had broken my hip because at that time, my leg was not shortened or externally rotated (until it became displaced when they moved me from the xray table back onto the barouche). My surgeon automatically performed a total hip replacement even though I had no previous arthritic hip symptoms. Having this fracture was a huge slap in the face for me because I felt young, fit and active and certainly not what I considered to be in the 'at risk' group. Of course since then I have had to undergo tests for osteoporsis which only showed osteopaenia and am being treated for this. I am back to walking 12 - 15km a day and back to feeling young, fit and active again. Your explaination of the different kinds of surgery offered for the different types of fractures was very informative.

I fell off a ladder onto cement block wall. Lucky for me, a neighbor walking by called an ambulance. It was so very painful trying to get me on a stretcher as I couldn't straighten my leg. Metal rod and pins. Sent home after 5 days with pain meds. The doc said I'd be moving around in a matter of days. NOT! The hardest part was trying to get out of bed. Three months later I'm at 90%. Pain still radiates down my thigh to my knee if I don't keep the leg straight when I'm bending. Still can't put left leg on a stair. I gave the ladder away! And my perpective on life is different. Oh, and I asked the doc how he accessed my hip to repair. He showed me this out-of-world spaceship table. It was cool.

I really enjoy your videos. In May 2022, I fell and fractured my femoral neck, which was repaired with ORIF. The fracture itself healed perfectly. In September, I was diagnosed with osteoporosis, which explains why I fractured. I also started developing pain, stiffness and difficulty ambulating. In December, an MRI as well as x-rays showed that I have avascular necrosis and a partially collapsed femoral head. I'll be having a THA in February. I sure wish that I would have had a hemiarthroplasty to begin with...if only I'd had a crystal!

Thank you for your time, Doctors. I had throtanterick demure fracture 4 month ago. Now I slowly do adductor abductor machine 10 lb; leg press 40 lb three time a week. Can I add leg curl/leg extension to my routine? I’m trying get back to normal. Thank you very much!!!!

I’m am 64 yrs old. I have had both hips replaced because of arthritis. But I have recently fallen 2 XS! I work out, I think this is what helped my bones take the fall.

I fell on my kitchen floor and had massive knee pain. Ambulance took me to the emergency room and the doctor there only spent maybe 2 minutes looking at my leg before I was sent home. It took 3 doctors one physical therapist and 4 weeks of not being to walk before someone x- rayed my upper leg to see that my hip was broken. My orthopedic doctor said he could have probably saved it if it was found sooner. Keep this in mind if they can’t find anything, you can’t trust doctors anymore.

I had a fracture when i was running a half marathon. 50m from the finish line. No falls nothing and im only in my late 20s! Couldn't belive it, i now have a dynamic hip screw in it. And am 2 weeks post op. Desperate to get back to running asap

I'm Cowgirl great video, I would like to share I had a dislocated left hip from a injury when I was 13 years old. I'm now 63, my left hip hurts every day I have arthritis in my left hip, I use a Rollater to help me walk. I'm so afraid to have a hip replacement because of so many bad stories of things gone wrong. I live in Southeastern Indiana. I have had 8 kids now all adults. Sometimes I hear myleft hip popping..... I do have a Orthopedic Doctor.

I had a TLHR in 09, then I had 3 screws (no plate) on 7/9/23 for a right hip fracture. Good thing I dont have 3 hips. lol. I'm 63 yo and Im not going to pass away but my life has changed. Please, if you have osteoporosis, get treatment for it.

Another brilliant, informative video. My Mom, whilst still at home, fell and broke her hip, never to return home and transitioned to LTCare. Mom’s fracture was repaired but her cognitive decline was such that she was unable to walk. I then became educated by reading the data/info at the Misericordia Hospital (Edmonton) about how many fractures occur in the elderly, how many ambulance calls are for falls, and the longevity of some of our elders post fall. Shocking! My Dad fell in LTC last year and had a total hip replacement and convalesced to walk with a walker. Dr. Paul hit the nail on the head; an ounce of prevention is applicable for all of us. You Gent’s continue to provide such great video’s, share great information in such an easy to understand way. PLUS we get to chuckle with you. THANKS AGAIN! Happy Holidays !

I am 72 years old & have had my share of painful physical incidents, related to arthritis. Disc replacement in my neck & lower back decompression surgery. By far & away, my hip pain, was the most pain I have ever experienced. It began several years ago & was treated with oral steroids, which proved effective, but in February of 2022, an x-ray revealed ''severe degenerative changes'' in my left hip as well as moderate changes in my right hip & lower back. It seemed as though, almost overnight, I lost the ability to walk, due to intense pain. I am about four months out from surgery. I still am unable to walk without a cane due to a rather severe limp, which is not painful, just exhausting. The entire ordeal has caused me to reevaluate my priorities & that quality of life is often what you make of it.

Maybe do a video on how the blood flows in the leg, as I thought it flowed through the marrow in the center or more center of the bone. If that's the case why could you not align to restore blood flow?

Approximately what age should a senior person be cognizant of fall prevention? I know this question is subjective without knowing the person health background. Are there any statistics based age for male and female in general when it comes to fall prevention? Thanks for the info. I am 73 years old and fell a month ago. I was in the hospital for almost 2 weeks. Transferred to rehab facility, with plans for discharge home after 2 weeks. My medical history is complicated. Dx: COPD, CKD /3, High B/P, Anemia, AFIB, Osteoporosis, Depression and anxiety . After watching your videos I am more concerned than ever. I did not fit into any of the time frames mentioned. I have absolutely no recall of the first week after procedure. I weighed 115-120 lbs the day of surgery. When I arrived at the rehab facility my weight was 163 lbs. A week later my weight was 142 lbs. I still have extreme swelling below my waist. Went for my follow up and was told X-rays were good and I’m to return to surgeon in a month. Yes he asked if I had any questions and/or concerns. Sadly my brain was not actively engaged and I said no. When I returned to rehabilitation center. I now nave noting but questions and concerns. Please advise me what I do now. My pain level with oxycodone remains between 4-8. What do I need to do now?

Mates Mother had a fall and broke her hip a few months ago, (94) they put plates and screws etc, all was going well she was up and about , after three weeks she left this world.

If you are an old person who has to have hip surgery get up and do the physio when you are told to. If you don't it is the beginning of the end. No excuses. You need to regain good balance so you don't fall over and be back to square one. No pain, no gain.

OMG!!!! Doc Z and Doc B or B to Z either way, there is no A to Z here. Now onto what I have floating around in my head. My mother, bless her soul, was picked up by two of my brothers when she got tangled up by the tie out to her Rotti. She fell over the side of the concrete blocks which were about 3 feet high. Not only did she break her hip, she broke her wrist and bruised up. Unfortunately, 2 of my brothers thought it was a good idea to pick her up and take her up the stairs to the front door. 5 stairs to the door, then another 7 or 8 UP to the main floor of the house BEFORE they called 911 to come assist. Well, you know how that turned out for her..... Anyway, you are SO on the ball to make sure people know NOT to move a person who has fallen from a height. Many things can go wrong. I had a hip fracture in the neck of the femur.Three screws were placed. I have pain in the hip.They say it is due to trochanteric bursitis. The mri did not show much because of the hardware. Some say that could be the screws. I'm desperate .The cortisone injection made things worse. Now I'm thinking of having the bursa and the screws removed at the same time, to play it safe.Eleven years later can the hardware be removed?

My 80 yr old dad fell and had femoral neck fx. Was fixed with pins. 4 months later you can see some protrusion and feel hardware. Not bone but hardware. It feels like it's clicking when he takes a step. Appt very soon but WHAT COULD'VE HAPPENED? He uses walker now but if he had necrosis he wouldn't be able to bear weight would he?

I did not fall, no accident X-ray showed no fracture. MRI showed fracture of femoral. I can stand and walk. Will it heal on its own?

My DAd fell and fractured his 15 year old hip replacement. somewhere.They opted for acute PT instead of the surgery. I think he should have the surgery then PT at home. he's not been active for at leas 2 years . He's 91 years old. I find it hard to believe that there's going to be bone growth I don't want him to suffer through this and then have to end up having the surgery anyway. Is it actually possible for a 91 yr old man to grow bone? merge it to repair it? it's not displaced that's why the Dr. suggested this first.

As always, I enjoy your informative videos. I made the decision not to ride my horses until I had someone living with me in case I got hurt. I am 70 years old. Five months ago, while cleaning house I stepped backwards off a kitchen step stool forgetting I was on the third rung up, I did a 180 degree turn midair trying to grab for anything to stop me from falling but went down sideways hitting the floor very hard fracturing the neck of my left femur. I had to drag myself through the house to find my phone and call 911. The emergency room surgeon performed hemiarthroplasty surgery the next morning, that evening when asked to wiggle my toes and lift my left foot towards me I could do neither. I would appreciate any information you could provide me with as to how I ended up with foot drop from the surgery. Also, will I be able to go back to riding my smooth gaited Tennessee Walking horses once I heal from all this? Thank you ahead of time. PS: I wear a neck lanyard with my phone attached now days.

Sorry to hear this. It can happen from the anesthetic or from traction on the nerve during surgery. Very rarely the nerve can be cut. If not cut it will usually get better but often will take up to a year to come back. Best of luck

So what can I do if I broke my hip 11 months ago and had a hip repair, and I'm still experiencing quite a bit of pain, and with that, my balance has been horrible, I have noticed that I have fallen a few times which before my hip fracture, I have never fallen it's like I have no balance anymore, and I'm not that old. I just turned 58 in October, what do you think ? And is there anything that can be done to help? Diona Reeves

Hi Diona. Sorry to hear this. So the best advice is to get reassessed and possibly even a second opinion. Sometimes though the hip does not get back to normal after hip fracture surgery. 11 months is a long time. Get checked out. Best of luck

I'm in a situation where the cartilage is vanishing, I don't have any fractures, so do I need a hip replacement? Or do I need to try cortisone shots first to relieve the pain and regain mobility? It's now traveling to my knee and ankle.

So hip replacement is for the disintegrating cartilage but only when the pain is no longer manageable. Sometimes cortisone can help. Chat with your doc about options. Thanks for watching and subscribing

A friend of mine had 2 hip replacements in both hips due to rheumatoid arthritis. Turns out they were plastic and one broke into pieces when a blind fell on her. They had to retrieve the pieces and replace the hip and then told her that the other hip was disintegrating and needed replacement too. I was dumbstruck as I thought these were permanent and titanium.

With posterior THR how many months do you need to maintain the 90 degree precautions? Is it safe to get into and out of a standard bathtub at 8 weeks post op?

Is it normal to have a little hip pain 6 weeks after THR? I am not taking pain meds and I still use my cane at home and or a walker in public. Some people tell me they were playing golf at 6 weeks. Am I just slow to heal? Age 66

My mother 67 age has hip ball replacement operation 5 months ago and now she has pain on hip ball sight. And sometime she is feeling sharp moving pain in leg from hip ball to knee direction. She has no other symptoms like fever, chills. Yes she has tingling in same leg foot. What will be cause of that pain? Pain comes and goes automatically. Diabetes is high around 300 but insulin is given. She has UTI and pills are running.

Hello Dr. So I am 30 years old women. I had history of spine tumor which made me wheelchair bound. And at the point my bone were so weak that I broke both hip. Dr. Fixed my right side. But my left hip bone is unfixed my bone is moved up like the 1st xray you shared. I have met couple dr. But they don't suggest me hip replacement because of my current situation (wheelchair bound) i still can stand with use of walker but time to time i feel pain on my untreated side. My last mri showed there is blood supply issue to my left hip. I wanted to know what happens long run if they don't treat my broken side ? How to prevent things getting worse? What are the options for me ?

Sorry to hear this. Your situation sounds quite complicated. If you are able to stand and walk without severe pain the risk of surgery may not be worthwhile. Hard to predict what will happen in the future but it may progress slowly. Best of luck to you

I broke my ankle I was in a wheel chair for 4 years the doctor kept saying it was in my head. It turns out they out toxic metal in me after telling me it was titanium. It ruined my liver and everything. I got it put in at the top hospital for orthopedics in the country and the doctor was world class but he was unethical. I had to have a blood test and there are only 2 places in the country that do it. It was called Orthopedic analysis in Chicago. I live in NYC but has my blood sent there which is what most people do. iT could be the metal they used.

Ummm so I’m 28 but I had a high neck fracture… my surgeon chose to fix mine with a plate and three screws…. I’m hyper mobile as it is… he said I can’t walk on it for 10 weeks… after listening to you I feel like he didn’t make the right choice….

My mother has hip replacement before 7 months ago. She had washing only revision after 2 month of operation. Now, its 7 months and she had pain in hipball and knees and thai.. doctor told me there is infection and we need to remove hip ball and implant new one. Dr told that 3rd time operation can lead low mobility. What do you think my mother can walk after 3rd operation? After Second time implementation. I am worried. Her age is 67.

Hello, I'm a 40 year old woman recovering from a femur fracture at the neck. They put 3 screws in and so far its healing good, just happened 3 weeks ago. I'm a very active person, I enjoy lots of sports with my child. I also used to be a ballet dancer and still enjoy dancing for fun, so I have great flexibility and leg strength. I know this will be a long recovery but will I be able to get back to that flexibility and strength again? Will I be able to dance again, not ballet but just for fun? Will I have any life long limitations provided it heals properly? Also what are some sex positions that can be comfortable during this time? Everything I find on the internet is for older people and men, I'm at the most confident I've ever been in my life and I'm scared this will completely change the trajectory in terms of sex.

Hi Dino. Really depends on how they fall and the rest of their health. Obviously the one coujd break their hip. The other coujd dislocate their hip or suffer a periprostheric fracture. Both could be bad. Thanks for watching and subscribing
