Season 1 Episode 9 "DNR"
John Henry Giles (Harry Lennix), a legendary jazz musician with ALS, is brought in to be treated by Foreman for pneumonia. House's attempt to prove that he doesn't really have ALS causes John Henry to suffer respiratory failure. House intubates him in violation of his DNR and tries to keep him on life support using a legal technicality. Cameron notices a blood clot, which is removed, and MRI reveals an Arteriovenous malformation which is operated on restoring his ability to walk. Meanwhile, Foreman receives a lucrative job offer from John Henry's doctor, his former mentor.

Violating a DNR is not at all chaotic good. It is a direct violation of the patient's freedom. It is highly immoral and anyone who does that IRL deserves to be found guilty of battery, to go to prison, and to have their medical credentials revoked.

@@person8987 There are plenty of reasons to go to a hospital, such as better chances at survival. However, a DNR is a evidence of a clear decision by the person with one that they would rather die than be intubated, etc., which is a decision that is that person's right to make. If someone has a DNR and their heart or breathing or whatever has stopped you leave them alone -- that's what DNR means: Do Not Resuscitate. I would note that I do not have a DNR and would absolutely go to a hospital to try to save my life, but people who feel differently are entitled to have their wishes respected.

@@MrDGoldDragon In that case, I don't think patients should be allowed to ask doctors to DNR. Euthanasia is not legal in most if not all places, so DNR should be treated the same way imo.

Foreman is right on this one ethically speaking if the patient was having cardiac arrest and is unresponsive. If he was then they wouldn’t intervene. But there is a difference between DNR and DNI (do not intubate). So House is also right because this patient had a compromised airway. Basically there needs to be a clear conversation with patients about what kind of measures they would want taken in different situations. Idk

Yeah. It was weird to say DNR when the patient was alive, also, his condition was caused by "malpracticed" so not intubating is akin to murder.

From what I hear DNRs work very differently in the real world than they do on TV. If there's even a shred of doubt from anybody present doctors will often go against DNRs because standing by and watching somebody die is far worse than resuscitating somebody against their will. One is an accident, the other is gross negligence resulting in a patient's death. In this case House was dead on - the patient had a DNR because they were worried about long term degenerative sickness, not because they were worried about their doctors giving him medicine that might kill them.

Ethics are weird. They are annoying to deal with because something like 90% of all cases lie within their grey area where there's room for interpretation. But oh well...

There's something I never understood from that episode ... "A do-not-resuscitate order, or DNR order, is a medical order written by a doctor. It instructs health care providers not to do cardiopulmonary resuscitation (CPR) if a patient's breathing stops or if the patient's heart stops beating." This os the definition of a DNR. However, in this case, his breathing and his heart didn't stop, he had trouble breathing but his stats were at 70, House did not reanimate him, he did not do CPR, he eased his breathing by intubating, so it shouldn't be assimilated to a DNR, should it?

@mehazad rahman yes its true, thats his background story. His family is with the church and he ran away from all that. So he has some conflicts, when things doesnt go right he questions if its his fault for running away

That’s actually a good question. If someone is dnr but isn’t dead yet and still breathing, why would they just give up and let him die? I thought it was supposed to be only once someone flatlines. Not oh.. he’s about to die. Hands off. Oh well

They literally said, he doesn't want to be intubated and has a legal paper saying so. I'm no expert but I assume people who sign a dnr just don't want to be potatoes

That's pretty much exactly what that means. If someone has a DNR, it means that if they start to die, just let them die. Having taken a medical class, if someone signs a paper saying they don't want to be resuscitated, you can get in a lot of trouble if you violate that

@@gerstein03 not in this case. DNI (Do Not Intubate) is a separate thing from DNR (Do not Resuscitate). If the patient agreed to one without the other, the lawsuit if they let him die would’ve been magnitudes larger than if they saved him, seeing as one is medical malpractice.

The guy was DNR, they can intubate him no problem. DNR only applies to a stopped heart. (No CPR) but for intubation he would need a DNI.

It's a DNR. He has the right to ask not to be resuscitated and House was technically in the wrong for doing it. He only justified it because he believed it had nothing to do with the original reason the patient signed the DNR.

The guy was DNR. Not DNI, until later it was brought up he signed a legal document saying so. BUT, in this case, I think House was right to save the patient because the diagnosis was wrong and the medication was wrong. It doesn’t mean that whatever the guy had, he wouldn't make a full recovery. So, as such, it would be morally wrong to not to save the patient since the dnr requirements weren't kicked in and it could be easily fixed or he could truly die later.

Can we just all take the time to appreciate house here? In any other intubation scene in this show, it takes any doctor at least 15-20 seconds to do it House comes in and before foreman has a chance finish saying yOu CaNt Do ThIs.. he's done and successfully intubated the guy. (5.5 seconds btw)

My poor coosin, came to my house to spend the afternoon together, suddenly she started to say very depressing things, and then she was hallucinating, we took her to the hospital, and they told us after a week, that she has problems with her thyroid, my poor girl, she was traumatized from the monsters she was seeing!

I'm not a lawyer but doesn't a DNR apply when someone has stopped breathing/heart beating? Not for example choking, allergic reaction, arm chopped off during industrial accident so bleeding to death? Are things different state side?

Level End Baddie The patient in this case stopped breathing hence why House had him intubated to help get oxygen into his body. DNR, Do Not Resuscitate means you can’t use simple efforts such as mouth-to-mouth breathing and pressing on the chest Electric shock to restart the heart Breathing tubes to open the airway Medicines

"You assaulted that man." The framing for this episode is so weird. It wants this to be one of those House moments where he does something clearly wrong, but no matter how the episode frames it; unless the viewer has a history in medicine, they're not going to see this as a House moment. They're going to see this as House doing what is objectively the right thing - even though that's not the case. It's like that nonsensical scene from The Incredibles with the guy that tried to kill himself.
