First off, a national study was conducted a while back about siren use, and on average sirens only save about forty five seconds... something to think about. Second, the caller requested a silent approach. Third, this is more than a siren usage video. Put your whacker instincts away for one minute and concentrate on the actual emergency. The thought process of what should be done when you get to the scene... great video, lots of good information to think about.

people ive been a paramedic for almost three years now,theres diffrent protocalls everywhere,just stop being rude,in some states you have to use sirens know matter what call,some states,you have to keep the sirens off(use them if someone is not getting out of the way)if you are going to a school

@TRIXSTER03 That actually depends. Where I work sometimes there in not a Medic available so sometimes if the patient has the signs and symptoms of an acute coronary episode we will go hot the facility without a monitor to confirm everything. The only time I have gone hot to the hospital with a paramedic aboard is for a cardiac arrest and stroke.

Like I said, you can transport at cardiac pt hot. By protocol standards this is technically the correct mode of transport. There is some research being conducted in regards to using audible and visual warning devices and the effect that it may have on a cardiac pt. According to Mosby's Paramedic Textbook 3rd Edition, there is a passage that suggests not to use these emergency warning devices because of negative effect that can occur in regards to a cardiac pt. @Kheldarren I dont know if you actually watched the video. There is siren use. This was brought up since many people might question why request an ambulance at all if you dont want lights and sirens. The suggestion regarding students was made to bring to light possible reasons for this. Thanks for the comment.

@rsizemore12 FDNY has a noise ordinance in place so that is why for the siren usage on and off.

good video where i work as a firefighter/emt our county standard operating procedue states that if we recieve a medical or fire response and even if requested non code it is mandatory we respond code on all fire or medical call i think its a good idea because granted if you have an emergency and you request non code response it makes some question the nature of the emergency so as county police we alwas respond code liability thing

@nyqs81-It's also different if you live in the U.S. I beleive in most states and county's within those states, you have EMT-A's EMT-B's and then Paramedics. In Canada, we have paramedics. Starting with BLS ---->ALS--->Critical Care/ CCP---> then Infant Transport/ ITT. I guess we all have different protocols.

@nyqs81--Like I said, although it is technically, a code 3 run, most paramedics go routinely because the lights and sirens can raise the HR/ BP of the cardiac pt. Even if it is an AMI, many still paramedics choose to go routine Also, I guess it's different where you live, but most cardiac pt. should be place on an ECG while enroute. If you live in the U.S. it's different I guess. Each state/ county has there own local protocols.

Like I said, by book, it says to go code 3. However, protocols differ from country to country. Our protocols over here are "guidlines." As paramedics we can use our heads a little bit. Also even if they are a cardiac pt, that doesn't mean that they are not oriented/ alert. Pt with chest pain may still infact have the mental capacity to understand things. What I am saying is more street knowledge. Read Mosby's paramedic textbook. There is a passge in their regarding what I am saying.

code 3 is different fore different places. if youve noticed fdny does the siren on siren off. i dont think its right to critisize these medics by saying it was an unsafe response bc from what i saw it was a very safe response, they used the siren when needed if there was no one around they didnt use it and if they knew they were coming up somewhere they used. I really liked this video especially with the talking in the background. good job

Also, if the pt was unstable then code 3 would be best. But if overall, they are stable then using no L&S may ultimately be of benefit to them.

Of course you could upgrade to code 3 if the pt was very unstable or their condition shows signs of deterioration. Just because you may not agree with something, doesn't mean it is not true.

Like I said, it depends on the patients situation and if the pt is very unstable. Most hosptitals where I live are only 6-8 minutes away anyways. But my statement is general street knowledge.

lol, I love how people don't understand code 3 driving and transport of the cardiac pt. When transporting a pt c/o C/P, audbile/ visual warning devices should not be used even though these pt may be RTC. One reason for this, is because the sound of the siren and the sight of the strobes/ beacons can raise the HR/ BP of the C/P pt ultimately increasing their stress levels and increasing the workload of the heart. Increasing the stress level of the C/P pt, is not good patient care.

Also, what does the NY State certification have to do with anything? I never mentioned anything about NY State protocols. I was referring more to street knowledge and research. Hence, why I wrote the previous comment regarding transport of the cardiac pt.

In a city, I agree with you, but where I work our company covers over 500 square miles, with a minimum of a 30 minute transport to the hospital, I can save 5 to 10 minutes in my transport time which as we all know time is heart/brain/lung tissue.

i disagree. it may slightly raise the Pt's heart rate or BP but TIME IS MUSCLE and i doubt the severe chest pain Pt is thinking about the lights. Plus, if you're waiting at red lights or stuck in traffic it could take you 45 minutes to get to an emergency room as opposed to 15 minutes code 3. Nitro doesn't always open a total coronary occlusion and your patient may die en route.

Yes, but I believe that I mentioned earlier, most hospitals where I live are within 6-8 minutes on a routine drive. Also, keep in mind that driving code 3, does not necessarily mean that the time to get to the ER/ CATH lab, will be minimized. Going faster doesn't always mean you will get to your destination any faster. The pt may not be thinking of the lights/ siren however, they are proven to create a sort of alarm reaction/ stress response in people.

@rudeboystu69 Yes, most protocols at the paramedic level do allow for NTG administration based on patient condition, complaints and assessment. I bet in the casino it is hard to get a better picture when responding. R u in Las Vegas? Next time I go to the Expo maybe we could meet.

I do have experience actually. Calming measures may work. Like I said, by the book, C/P patients are required to transport code 3. However, sometimes, due to street knowledge we transport routine. It was also published in either Mosby's paramedic textbook or Brady EMT text, which also stated what i'm saying. Also, I didn't say all patients should not be transported code 3, I was referring more to C/P patients. In addition, I said the sight of the lights and sound of the siren places stress on

Maybe if you have some experience and have the ability to calm a nervous patient and expertly and succinctly explain what is going on then then you can decrease their stress level. By your rationale, no patient should be transported code 3 because it could increase their HR and BP. Everyone is stressed to some degree when they are in the back of an ambulance. Also, I don't understand how the sight of flashing lights from inside the ambulance would place undue stress on a patient.

In an emergency yes, by all means burn up the sirens. But in this case as was stated, not much pre arrival is given, the callers requested a silent approach, and it was at a school where there is already a nurse there. Also it lessens your respectability when you resort to cursing and name calling in a conversation. But I've given you all the time I'm prepared to waste today. Awwww, isn't that cute. It's like it's trying to pretend like it knows what it's talking about. So cute, and watching the news? All by yourself? I'm so proud of you :) Haha, a busy city street? No. Not by a long shot is any of those streets busy. More over, how about thinking of the patient; what is the issue going to be, what medication am I going to need,... any true responder's though process should never dwell on 'How much siren noise can I make?'

You're not using a siren on an unknown chest pains call because the students might get excited?!? You're fired and please never go into EMS work again because you obviously have poor judgement. This turned out fine, but you had no way to know that en route. I hope no one like you ever responds to an emergency I am in.
