Hi @Whispering eagle, sometimes when it seems like you have every pain that the video talks about it is because the inflammation has gotten a bit out of hand. Once we start treatment on a problem we gradually get rid of all of the pains and get down to the original source. So although it may seem like you have all of those problems you probably have one major problem that is creating enough inflammation to confuse the issue. If you tend to ignore the problem and let it go on for sometime that could be the case.

@@Biomechanicphysicaltherapy thanks i first thought well i have cancer. Iam a bit of a hyper condriac due to watching my brother as a child having his lung removed in 64. I fell off a ladder onto a log last year and although i had some earlier long time back and arthritis spondalitis. I felt like i was ok but sore. But now i feel pains everywhere. Hips knees ,muscles, pings. That seem to change places. Ive never been to a Chiropractor. When i was young i developed an exstream phobia watching my brother always in the hospital. I because of exspense got ignored a bit. Not my parents fault they had alot to deal with my brothers slitus unversus, and keritans disease. Very few videos address phobias. I sleep now sitting up gives me some relief. Trying to build courage. Thank you for the response

This video gave me so much hope! I have 24/7 pain and am about to start traditional physical therapy -- but don't have a lot of hope they will know what you do. I have Medicare and supplemental HMO insurance. I started in November with an SI Joint problem with pain - but it varied with some good days, some bad. I was seeing a chiropractor and he would help from time to time. Then suddenly the SI Joint pain disappeared and it was ALL hip. He worked on it, but it only seemed to irritate it. I have a spot in my buttocks that hurts even when I take NSAIDs. I think it's the first one you are talking about - that is is actually the SI Joint. There is only one person here in my area who does MET, but charges over $300 just for a consult to determine what is wrong. It would probably be worth it to just fly to VA and see you all! :)

Hi @Mary-Chris Hines i’m glad we were able to give you some hope! Yes it does sound like SI but we could definitely help you sort it out. That is a very expensive evaluation! Our cash price is $110! We do except Medicare if you wanted to know what your benefits would be you could certainly call our office and they will check with your insurance company and let you know the nitty-gritty. We have had patients fly in for a week or two at a time to see us when nothing else would work, so you are not alone in that idea! Come on down!

@@Biomechanicphysicaltherapy Although I have Medicare, I also have the supplemental, and that is only HMO, so won't cover you. However, that's okay. I am seriously considering coming over! I'll make it a vacation as I used to live in Washington DC and Baltimore so can do some visiting. I'll call when I work it out.

After listening to this and reading comments, I’m surprised nobody has mentioned my symptoms. Walking and turning from side to side in bed, and especially walking down the stairs is painful. But it doesn’t hurt when I wake up until about an hr. Then the only thing that alleviates the pain completely is to walk while bent over almost at a 90 degree angle. There is a spot on my spine that is tender to touch. 2 chiropractors have helped a bit temporarily. At least one alleviated my nerve ache down my leg. But after 6 weeks, I’m so discouraged. 2 VA drs diagnosed me. One said bursitis and one said a muscle spasm. I’m waiting for a cat scan before they send me to therapy.

I can't seem to diagnose my problem either. Best way to describe my pain is if you sit cross-legged and then try to lean back. She briefly touched on that in the hip osteoarthritis one, but I really want something in more detail.

Thank you for the video. Yesterday I had pain in my left hip just before bedtime and was unable to sleep on either side without provoking the pain response. The pain was deep in the hip was only reproduced when externally rotating the left leg/thigh or by opening the thigh outward while in bed in fashion similar to the clam shell exercise movement. The pain is best described as a sharp quick jolt that lasts a mere second and then disappears until provoked by the movement patterns mentioned above. Today this pain is gone but the left hip felt tight this afternoon when putting on a sock. That tasj requires the leg to be crossed and placed over the opposing knee similar to the Indian style sitting position. After the sock was put in and I uncrossed and lowered the left leg my left hip and it's surrounding tissue felt sore and stiff for several minutes. About me: I'm a runner with a lean fit body. My thighs, glutes, core, and legs are generally strong. However, I have never had great flexibility. How do my symptoms fall into this range of diagnosis that you outlined in the video?

Hi it’s always hard to diagnose without actually looking at someone but it sounds to me like your sacroiliac joint could’ve been out of line that often gives a sharp pain and makes you unable to sit in the tailors‘s position which is what you were talking about with the leg crossed over the knee. It can go back in line by itself or someone can put it back for you. You don’t mention if there was any incident that occurred before hand so I’m guessing it just started up out of the blue.

@@Biomechanicphysicaltherapy Thank you for your reply. Regarding your question approximately 4-6 weeks ago I woke up one morning with this terrible pain on the lateral side of my left hip. It was not the deep sharp pain I experienced yesterday with the external rotation. In the earlier incident the pain was closer to the surface of the skin and was soft tissue related. Palpating along, just above, and just behind the Greater Trochanter revealed tenderness in the soft tissue. I presume there was an inflamed bursa along with some aggravation of the localized tendons or muscles that pass along the Trochanter. The odd thing is that I did not have these symptoms of tenderness, inflammation, and pain before I went to sleep that night 4-6 weeks ago. I do sleep predominantly on my Left side. Additionally, my latex mattress topper is worn out and has not been providing me with an adequate level support and cushioning. Even my healthy right hip feels discomfort after a couple of hours sleeping on my right side. So perhaps this played a role in the development of my hip pain 4-6 weeks ago. This weekend I finally ordered a new 3" talalay latex topper so mattress comfort issue will soon be resolved. Yesterday's onset of sharp pain in the deep left hip still remains a mystery. I never felt such a pain before. For the past few weeks my hip was progressively improving in range of motion and a general reduction in stiffness. Then, after a couple hours sitting at my computer, I experience the sharp deep hip pain when I got up to walk around. Only one movement pattern seemed to provoke the pain, external hip rotation. When I went to bed last night the pain cropped up again after a couple hours sleeping on the affected left hip which is my preferred sleeping position. When I rolled over to my right side the sharp pain in the left hip counterintuitively flared up again when I ever so slightly shifted or adjusted my body position. Frustrated, I eventually ended up sleeping on my back with my knees propped up by a pillow. While in this position I played around with my left hip and found that if I rotated my left leg externally that triggered a jolt of sharp deep in my hip. But here's the INTERESTING thing. When I squeezed my glute muscles to engage them and then rotated my left leg...no pain occured in the left hip. I kept doing this for maybe 15-20 reps and afterwards there was no pain in the left hip even when I externally rotated the leg without consciously squeezing the glutes. Despite this breakthrough I stayed in the supine position for the remainder of the night to avoid aggravating the left hip again. Since I woke up this morning there has been any sharp pain but the left hip does feel a bit stiffer than usual. To say the least I don't know what exactly is going on. Any input or insights you may be able to add no matter how speculative would be most appreciated. Btw, since the original onset of left hip tenderness and pain occured I have been steadfastly doing exercises to build strength in my hips with a focus on abduction.

The IT Band or Tendinitis or Bursitis seems to be the best fit outof of those diagnoses. Although Sciatic Nerve pain is a possibility instead.

Hi :) is there a way to test for femoral neck stress fracture without getting mri? I have really sharp pain on initial foot contact with every weight bearing activity. Hip flexors are completly painless when acitivated as well as my adductors....

Thank you for a very thorough run down of the various issues that affect one's hip and potential treatment options! I understand the the most reliable means of assessing a problem is in person, but would like to ask a question, if I may. I've developed (past 6 months) an intense pain in one hip that only presents when in the position shown @ 6:19 of your video, or when sitting with a leg crossed (ankle resting just above opposing knee). I'm daily stretching to try to relieve the issue but progress is painfully slow. Any advice on what it may be, or the best stretches to overcome this? Thank you again for taking the time to make these videos to help the masses, it's greatly appreciated!

Hi @jeff. It could be the osteoarthritis which I am discussing in that part of the video except that you are not saying you are having pain that is the other hallmarks of osteoarthritis. One of the things that sometimes happens is there is malalignment of the head of the femur in the hip socket. This happens when your ilium is slightly out of line. If you can find a practitioner in your area who does muscle energy technique they can check your alignment in the sacroiliac joint and see if that is the problem. This is usually a physical therapist with specific training and so may be hard to find and also some chiropractors are good at sacroiliac joint but not all. The other thing that I would use to diagnose if it is a sacroiliac joint problem of alignment of the ilium problem is this ; if you sit with the soles of your feet together and your knees out to the sides is one knee resting higher than the other?. If so that can be a problem of the head of the femur not resting correctly in the hip socket which could be due to the ilium not being in its optimal position. if you can’t find someone near you you can do a telemedicine visit with us. I think you nailed it because when I do as you say, soles together & legs resting outwards, the left (problem hip) stays higher & presents with mild pain. Hmmm. Not sure about that one. Perhaps a hernia? I recently got had a guy who was diagnosed with a labral tear but ended up with a hernia so I’m guessing labral tear versus hernia.

@L Tom sometimes you have more than one thing going on and sometimes the whole darn thing hurts if your problem has been going on for a long time and multiple things end up getting inflamed. As you progress through physical therapy done properly each of those areas would resolve and you finally get to the underlying thing that caused it all. Regardless, a good therapist will handle each one of those things and look for the biomechanical problem beneath it. The biomechanical problem beneath it is critical to solving the problem. If you just treat the symptoms you may not be able to get to all of those issues. You cannot exercise this problem away. A lot of physical therapists just use exercise. This will not work for you.

I get pain getting out of my car, and the pain feels in my groin area, then it seems to move to my upper thigh, like something just hit it. Hurts especially in the evening after an active day. I'll falter when I'm getting up. Would this be that impingement you were talking about? Will an MRI see that?

Hi, yes, it does sound like it could be hip impingement, but it could also be SI joint which changes the way the femur moves in the hip socket, which can give you a similar kind of pain. an x-ray or other imaging will be able to show impingement but it does not show if the SI joint is out out of alignment. So if your x-ray or MRI is negative, then you may want to pursue the SI angle.

interesting list on the oa for hip.. I do a lot of running.. around 1hr/day normally.. but this time last year I got the worst hip pain after too many fast runs at around 10mph pace ... started and soreness on the inside of hip.. eventually moved more towards front.. and went super deep and almost felt it was digestive tract pain. Pain recently flared up again.. I do wonder if it might be start of oa in the hip... maybe ... pain generally goes away if i stop running for awhile... but comes back and always same side. Strange thing is.... as sore as it is.. I can normally run on it... I could probably even run fast on it and ignore it... until I stop and then later I'd be in really really bad pain. Strange one...

First things first check your SI alignment. It is the simplest thing that can cause hip pain. And if your alignments out and you continue running it could eventually cause OA.

yes ,well i have none of the above. my internal groin socket pops,about 3 times a day now. extremely painful when it happens. but no other pain.

OK. Does the pop feel like it’s on the front of your body? It could be your psoas muscle popping across a bone. In which case your sacroiliac joint is out of alignment.

Hello, No, it is not usually perfectly symmetric, but yes, both can hurt at the same time. The most common time I see both is when it is muscular like Ilitotibial band (ITB) syndrome or piriformis syndrome. Which are often very related to mechanics and alignment.

Not too sure. Pain at the top of my right hip is regular but fairly mild, sitting down position is OK. But more severe pain which also moves across to a more central location when standing up. Once I have stood up the pain goes back to mild. Getting out of bed is problematical with the standing up type pain inflicting a reap pain jab as described above.

Hello sounds like SI joint sacrum probably a little twisted and the pain across the top of the hip is muscular pain from some muscles being overworked due to the strain of the hip bones being slightly out of line. I would definitely find a practitioner who does muscle energy technique And get that sorted out you will be very happy. If it's been going on long time, it will take a bit of maintenance and you may find yourself and PT kind of regularly to keep yourself but PT takes less than an hour, most people think it's worth it to keep things in line. Plus, remember the long-term consequences of potentially deteriorating a joint and then having arthritic pain later so I think it's worth it.

I went to a physical therapist and they told me my glute muscles are weak. I don't disagree with them. They are super weak and it causes pain, but I have been working them out for two months and at first it got better and the pain went away except during rotations, but I could walk straight without pain, but then it randomly came back even though I kept working out my glutes. The pain is right over the hip joint, but also travels back to the upper buttock sometimes(where you said the SI pain is). Sometimes it is more of a tightness in the joint, but other times it is like a very sharp jolt. Like one wrong step and a ton of pain occurs, otherwise it is like mini jolts every time I take a step. But for me the pain isn't when sitting or lifting. It is from movement, so idk if that means it can't be SI. I just feel so lost. I would go back to PT, but I feel like they are just going to tell me my glutes are weak, but while that is true, it doesn't seem like strengthening them is helping in the long run.

Hello @KamiltheCamel Your experience with the physical therapist is one of my biggest pet peeves! To get to the bottom of what your problem is the first thing that needs to happen is a look at your mechanics and your alignment. It might also be helpful to know if you have any arthritis in your hip joint. The pain you’re describing can be from pain within the joint due to arthritis or due to something like a labral tear it could also be from malalignment of the SI which causes pain in the hip when the ileum is not oriented properly with the femur. So it would be really great if you could find a therapist who is interested in mechanics. I don’t know where you live but I’m guessing you are too far away to visit one of our clinics. I believe that the most important and first thing that should be addressed is pain and mechanics once that is handled then you can work on strength. Unfortunately it is a very common tactic for physical therapists to rely on exercise alone. It’s possible that we might be able to help you with a telehealth visit if you cannot find anyone in your area. Not all physical therapists are the same so although I do agree with you that it does not sound appropriate to go back to the person that you were already seeing - but maybe there’s another therapist in your area that could help. @@Biomechanicphysicaltherapy Yeah the whole time I felt very lost. Like, I am going to you over 12 times and not once can you explain what is actually going on. The only real tests they did was testing my hip strength and they found it was really weak. They also saw that I had pronation and a flat foot and helped me have better walking technique, but when there is a lot of pain, I can't really walk correctly. The other really weird thing is the pain comes and goes for NO reason, so I feel so out of control. Like I don't even know what I am doing right or wrong. I am in Washington State, so I am not sure if my insurance would cover a telehealth visit from out of state. If you know anything about that and it does, then I would love to call. Otherwise, if you have any recommendations for the Seattle area, that would be great. Thank you for taking the time to respond.

@@moodz8111 Lamoy Moodie. It turned out I have Ankylosing spondylitis. Basically I have an autoimmune form of arthritis that mainly affects the hips and SI joint. Literally all they had to do was take an X-ray, but I am young, so they never did. If you don't have an X-ray yet, GET ONE. At the very least you can rule out arthritis and you know it is something less serious.

Virginia Beach? I live here. The orthopaedics told me that I got FAI but he never explain he just told me my hips is going bad I need to get a MRI with dye. I just research what is FAI and found out by my self. I might check out your place.

I had a car accident about 23hrs ago. Could stand on my left leg initially but everyone kept making me sit down on the floor until we were picked up 1,5hrs later. I was in a sitting position during the 30 min drive to a clinic where I was carried onto a bed coz now my pain was extreme at the hip when putting weight on the leg or even dangling it. Physical examination was difficult coz of the pain so I got Diazapem & Pethadine to knock me out. Radiologist wasn't available so I may only go in for x-rays today. Pain was initially in the left buttock (Deep) but now sharp and extremely weak on lifting leg from any position. I'm concerned how long it will take me to recover and whether it's just best to recover at home with visits by a Physiotherapist. I'm in Africa.

@Delan Nyandoro If you were initially able to bear weight and then not able to bear weight later it may not be broken. You will of course be able to see from the x-ray. Sometimes the adrenaline will cover up pain however. If the x-ray shows no fracture but the SI and the pubic symphysis are not in line that is something that the physiotherapist could help with immediately and the doctors would not be able to help with. It may be muscular. Take a picture of your x-ray & let us know!

Great video. I get pain in one hip while I’m walking that lasts for a few days or a week or two then goes away for similar or longer stretches of time often in conjunction with similar(ish) pain in my big toe joint,the MTP. Doesn’t stop me walking the dog 1or2 hours a day. I’m a male 55yrs. Is gout a possibility? Hello from Australia 🖐🏻

Hello @pudding7074 Gout in the hip is not very common. If you have gout in your great toe that causes you to limp I could imagine it might cause hip pain. If I were to see you in my clinic I would also be ruling out the SI joint and low back. I have seen patients who have no back pain, but have knee or forefoot or heel pain that is coming from their back. I would use muscle energy technique to evaluate and treat as I have not found other physical therapy approaches have helped with those type of cases. I think there are great manual therapists in Australia, call around to the local PT places and ask for Muscle Energy Technique and manual therapy.

hey, i didnt really relate to any of these, im 19, have had pain in my general right hip area for as long as i can remember, even when i was like 12, it'd only start hurting if i've walked a lot that day, or worked out/did physical things, it doesnt hurt immediatly, but several hours later, at night when im sitting at my desk for example, im not exactly sure where the pain is but its somewhat most obvious in the upper pelvic bone, the one that sticks out at the front of the body, the pain is worse if i lay down on my back with my knees bent, and is unnoticable when i squat, any help would be greatly appreciated as i cant afford a doctor right now

@rad stf It sounds like you have some thing that was not covered in the video. I’m saying this based on the age that you say you are and when your pain started. It’s called a slipped capital femoral epiphysis. Basically due to high impact activity or growing too fast or weighing too much for your joints to take It causes a slip of the growth plate and a slight change in the architecture of your hip. Have you had an x-ray? That could help answer the question.

I sometimes have intense pain in front, or the groin area when standing from a seated position on both sides of my pelvic area. My first few steps are particularly painful, then it goes away completely after a minute or so. Sitting with my knees elevated above my pelvis, or leaning forward a lot while seated exacerbates the condition when I do finally rise. I am searching for helpful information as to the medical reason.

Happy to help - Sounds like femoroacetabular impingement - could be some arthritis. Usually IR deficit too. Core and hip strength and some specific stretching without causing the pinch is the treatment. Sounds like a more chronic case for it to be B and severe, maybe lumbar compression making it worse. A good mechanical, manual physical therapist would be able to help!

I have Ehlers-Danlos syndrome and dislocate my hips regularly. However I dislocated my hip and got it back in, but I've had severe pain, unable to bear much weight, weakness, swelling, and the pain is going into my lower back, along with extreme tenderness of my hip that goes into my thigh. This is not normal for me at all. I'm worried it could be a hip fracture

Oh yes it could be a fracture but it could be something else out of line. Unfortunately you’re probably gonna have to get an x-ray to figure that out. The other problem is that with Ehlers-Danlos and the things that happen are unpredictable. There are other things that can be wrong that would prevent weight-bearing if that makes you feel any better for now!

@@lifewithkirsten7670 yes that does often indicate fracture but you can have bone pain down to you like even if your SI joint is out. And my longest Ehlers Danlos patient always had bone pain from her SI. She did end up having a fused. Very successful surgery.

I wish that I could find an answer online for my hip pain. I'm 34, I've only had this pain for a couple months, it's only in the right hip. If I sit cross-legged and lean back that's where the pain is. Or if I bend my leg in front of me and try to push my knee downward

@@angelr9096 When the SI joint is out of alignment it can cause bone pain down the leg from the hip. Find someone who does muscle energy technique for sacroiliac and get them to put you in alignment. Chiropractors are not as good at SI joint as physical therapists who know muscle energy.

@Kyhree Armstead Physical therapy is usually covered by your insurance with a co-pay of $20-$40 per visit. Here at BioMechanic, if you don’t have insurance or want to go out of network your visit can range anywhere from a free 15 minute screening or for a full visit it can cost between $50 and $110 a visit.

@BrightonMusic A problem in the hip could cause pain going down to the ankle especially if you have some bony malalignment but it could also be a sciatica type pain. Definitely would benefit from an experience professional having a look at it and telling you which one it is.

I point to the pain and they make a diagnosis from that description without any physical movements to determine the real cause of my pain that you have so well described.

@@brightonmusic You could try a physical therapist who specializes in bio mechanics, some osteopathic physician‘s (that’s DO not MD) might be able to help you figure it out. There are other physical therapy places besides us who do bio-mechanic physical therapy look for a tagline something like we go to the source of the problem not the symptoms. We all seem to use a similar kind of tagline. Hope this helps and good luck! You could always take a vacation to Virginia Beach or Leesburg Virginia and see us!

Pain on the surface on the outside or lateral to the hip. Sore when you try to massage it. Can also involve itself with the IT band and be tight all the way down to your knee.

I used to do running, one day i got sharp pain in my left hip after running. I visited the doctor and they gave me pain killers i ended up quitting athletics but every now an then when i exert my self or run the pain comes back. It has been over a year and the doctor says theres nothing wrong, i haven't done an xray yet. Btw Iam 15 as of writing this can you please share your thoughts on this.

Often times when the alignment is off you cannot see anything wrong on the x-ray. It sounds like your hip bone moved and therefore the leg is no longer in proper alignment in the socket. When your hip bone moves out of position it can give you pain low and to the side on your back and in the socket and down the leg. See if you can find a muscle energy technique physical therapist in your area. If we’re not too far come and visit us and we can fix it. One of the other problems that young people have if they are overweight is they can have a shifting of the growth plate. But that you would see on x-ray.

You could have more than one thing going on. But the other thing I’ve discovered. When the inflammation or problem has gone on for a long time, the whole darn thing hurts. So as soon as you get under treatment and start getting the pain and the inflammation handled, the pain will start to get more specific from where it actually started as. Maybe you can remember which pain it was when it first started? Or you could try to find a good Physical Therapist with an MLS cutting edge Laser to decrease your inflammation and help focalize it.

Yea, Sometimes it’s hard. An in person diagnosis would be the best. You can see an orthopedic MDor a physical therapist with a direct access license to diagnose.

60 years old is just sort of a hallmark for how long the problem has probably been going on and how much it has deteriorated. An x-ray would be a better indication than your age, but you know how the studies will correlate age with deterioration and that's just what the studies show.
