About 250,000 sinus surgeries are performed in the US each year--some of which are quite necessary, and some of which might possibly be avoided. This video discusses "top 10" reasons to have sinus surgery in the opinion of Dr. Jeffrey Terrell, a professor of Otolaryngology at Michigan Medicine. I have to agree with the previous comment - Excellent presentation by the Doctor. Very educational. I came on looking for information on mucus retention cyst in the right maxillary sinus and came upon this video. I wish all doctors were this good at narrowing information down and explaining things. I hope he is teaching classes!

I had sinus surgery 6 1/2 weeks ago. I had SEPTO, FESS and SMR. The recovery was brutal, but I CAN BREATH! My sense of smell is still dulled, but I don’t care. I was completely obstructed, so I feel a great sense of relief. A side note: it took me a full 6 weeks before all of the packing and debris was cleared. Week 5 the ‘plugs’ were sticky and hard to pass through my nose. A hot shower and the NeilMed rinses do a great job helping clear it. I will continue to do these rinses probably for the rest of my life. I am a 58 yr old female. So I have a success story!

Thank you for your video. I was extremely fit 2 years ago and suddenly was struggling to breathe the the point I could barely walk a few steps before collapsing. I lost my sense of smell around the same time and was vomiting up most of my meals. I have had a constantly runny nose and polyps since then and can barely breathe resulting in sleep apnoea and other things. Recently I was at work with a splitting headache and took and aspirin and within an hour I couldn't breathe and was going into anaphalaxis. I went to the ED again and them/my GPs have always said they have no idea what it is. Pretty sure you just helped me figure out at least what it is. Thank you!!

Hey doc - question about "foreign bodies" .. i have a dental abscess about the size of an almond in my maxillary sinus that won't go away, root canal treatment and 4 rounds of antibiotics later. Would this be something that should be surgically removed? Symptoms are lots of facial pain and pressure. Waiting on an ENT referral...

I have many cysts in my nasal passage. Would I have to have surgery for this? Do even look at my sinus with a scope can I be put out for it? I don't think I could handle it. I live in Kenosh Wi. I have Medicaid insurence being I am disabled, it pays 100%.

Should and ENT always order a CT before doing surgery? Mine hasn't. He just looked up my nose with a lighted tube and don't think he went into the sinuses. I have had nasal drainage for most of my adult life and may have also been exposed to mold the past few years.

Watched his reasons not to have sinus surgery and this one and both are really helpful. Will be seeing an ENT in the coming weeks and now I feel better prepared to ask questions and know what to look for before doing anything. Saw an ENT previously and he definitely did not take a complete history and was in and out of the exam room fast. Then I had a follow-up call to schedule surgery. I said no thanks as I had a gut feeling not to do it. Glad I listened to my gut and now this video and the other one. Thanks!

Thank you Doctor. I'm having surgery for complete maxilliary Antrum opacification and terrified as a biopsy wasn't possible so straight to theatre. MRI cannot identify the white material. Please can anyone.tell how to sleep ie. sitting up?

If antibiotics or allergy medications worked we wouldn't need to resort to surgery. Thank God for ENTs. My tonsillectomy was one of the best decisions of my life and the most successful surgery. The pain was worth it

My problem is that I’m full of secretions and I can’t smell or taste. No infections but CT scan is horrible. White outs in 2 of my sinuses and mucosal thickening that is moderate which is progressing from my last scan three years ago. Will I be able to smell after this? My concern is that I have asthma and allergies and the swelling will just come back!

About 4 years ago I started to lose my smell, I then had 24/7 365 allergies. My nose would be almost always fully blocked if not just caked with mucus. I also realized I was reacting poorly to aspirin from a previous surgery in my leg and had to stop taking it. Later I started wheezing and my doctor thought it was just late onset asthma. I have dealt with it for years until I went to an ENT and he told me I needed sinus surgery. I didn't remember everything he said but I wanted to research it myself, I had no idea there was something called Triad Asthma that 100% matched what I have been going through. I couldn't be more excited for this surgery now! Thank you for your great explanation!

Did you get surgery? Any updates for us? I had surgery yesterday for a deviated septum and some other things that I can't remember today 🤔😂. Still in decent pain, but excited to breathe again!

@@danielobrien1989 the surgery was not bad at all. My ENT did not use any packing and as soon as I woke up I could breathe easily through my nose. I had a light stream of blood that I would use the exterior nose bandage to catch. However things did not end as perfectly as I hoped. Yes, I could not breathe easily through my nose and the mucus was cleared but my body still seems to make an overly large amount, I began getting pretty bad post nasal drip, still not nearly as bad as not being able to breathe out of my nose but still not fun. On top of this, and I believe because of my post nasal drip, my potential triad asthma has continued. Though it does seem to be decently lessened in intensity I am still dealing with it. In conclusion it was well worth it however in regard to specifically triad asthma I would consult your doctor directly about it before hand to see what they think. If you have any other questions lemme know!

I have adult-onset asthma, allergies, and a history of polyps that developed soon after I was diagnosed with asthma. I had surgery a year ago, and so far my sinuses are clear. However, I never had any asthma attacks from aspirin. My ENT asked me about that, and I can’t recall any issues caused by aspirin. I take Excedrin for migraines, so I hope aspirin never causes me asthma attacks.

Maybe I missed this, but what about a recently discovered large maxillary cyst in someone with sleep apnea, that has already undergone turbinate reduction sugery along with deviated septum surgery corrrection. Turbinates are growing back. Has suffered with sinusitis and fairly bad allergies for 47 years. Thank you

I had a Ct scan to recently that's how I know. I have Hypothyroidism, yearly allergies but even in the winter I have headachs. I have sleep apnea and so I use a machine nightly.

Thanks for sharing. It was great to compare and contrast the information between your two top 10 videos. I'll need to ask my ENT about the ability to remove some types of polyps (or even just one) while in office. I didn't realize that this is sometimes an appropriate option and something I'd like to learn more about.

Nice Video! Excuse me for the intrusion, I am interested in your opinion. Have you thought about - Yenamilla Sniffing Reformed (do a google search)? It is a good exclusive guide for curing nasal polyps permanently and naturally without the headache. Ive heard some interesting things about it and my friend at last got great success with it.

I'm seeing an ENT tomorrow as my ct scan showed both frontal and maxillary sinuses are under developed (right side more underdeveloped) along with calcification in the right maxillary sinus, sclerotic appearance in both maxillary sinuses (right more than left)

I’m here because I’m scheduled for surgery in the new year. After years of suffering I finally got a referral to an ENT and within minutes he confirmed chronic sinusitis. Funny thing is I was constantly dismissed as hay-fever and my asthma meds just kept being increased. Dr just responded as a matter of fact after I complained about my antihistamines not working for my sneezing, “of course it won’t. Histamine is not your problem”. I have no sense of smell. Can’t inhale through my nose. Migraines and the congestion is permanent at this point. I’m in utter agony most days.

I had ESS 3 months back, for chr. Sinusitis without polyps ,that was not responding to medical treatment. After the surgery from 5th post op day I developed fever for which no cause was found in and around sinuses ( all tests including CT, MRI , cultures ,nasal endoscopy done) fever went on for over a month. After that I developed burning pain inside my left nostril in the front part of nasal septum, again no cause was found, got second opinion from another surgeon who prescribed amytreptalin with analgesic, pain subsided after about 2 weeks, my sense of smell and taste are GONE, my surgeon says it may or may not return! Horrible experience. Anyone advised ESS should think 10 times and get second opinion about the need for this surgery before going ahead.

Dr. Terrell, you didn't mention 'deviated septum' which swells due to either allergy or some other mechanism and thus causes repetitive infections due to the inability to drain. Do you see many of those? Thank you for an informative video.

Yeah I had diagnosed (Nasal bone deviated septum)same condition And..I get lots of mucus comming from back of the throat.. It's been 2 years I've done all naturopathies shits..which none of them worked AND I'm going for surgery within 1 to 2 weeks..

This surgery did nothing for me the first couple months were ok but that was 3 years ago 😩 my sense of smell has gone down and I can’t fully breathe through my nose still

Question: What if you have sinus pressure that is ongoing - CT Scan shows no polyps or obstructions - Allergy tests show nothing that would cause inflammation etc. - No major septum deviation - Diagnosis of sinusitis twice or more per year. Usually requiring a second dose of antibiotics but still never truly relieves the sinus pressure. Continual pressure that makes you feel tired and generally sluggish. Draining will occur through the day. The pressure is there even though you can breath fine through both nostrils. Approximately 6-8 times per year, getting a zero warning out of no where loss of balance which lasts only a few seconds and then returning to normal with no further negative effects other than sometimes immediate sinus drainage directly after the loss of equilibrium balance. All of these taken meds taken daily only have a mild positive effect on head sinus pressure. • Pseudoephedrine • NSAIDs • Flonase (topical nasal cortical steroid) • Azelastine (topical nasal antihistamine) • Pantoprazole to see if there’s an acid reflux issue (nope) The only thing that seems to really make a difference in reducing the sinus head pressure is a steroid injection: 40mg Kenalog combined with 4mg Dexamethisone Or a Prednisone pack However the relief of coarse is temporary. I would almost anything to find an answer. Thanku! Myles-

Same. Had a sinus washout (as the surgery was called then) many years ago but have had almost constant issues with pain and infection and post-nasal drip before and since. The worst has been the past two years and the waiting list to be seen for an initial appointment is well over a year still. It's not fun.

@@ChrisB-cx6td inflammation is the key issue! Inflammation of the sinuses, the nasal lining and cavities. Issues with sleep apnea are another very possible key issue. I’ve been taking a 24 hour allergy pill everyday. I use Flonase or I’ll use dymista every day. If you are overweight, it’s best to get your weight down and globally get rid of inflammation everywhere not just your sinuses. That means a real change in diet. Getting rid of sugars and all the fake stuff. Look into your diet and read up on inflammation and inflammatory response issues! I know it’s not an easy simple answer but that’s what I’ve been doing and I have to say that things are improving. (Slowly) Another thing that helps is serious real consistent exercise! Lots of changes happen in the body hormonally in addition to physically with real consistent exercise. If you hate excessive, I kinda have to say too bad, do it any way and do it at least several times per week! It will literally help everything! If you have consistent head pressure, have you gone to an ENT? Have you gone to an Allergist? I would ask for a CT scan for sure.

I just had mine done today, endoscopy sinus surgery, the doctor remove polyps on my left and right nostrils, its scary at first but its not that bad. 3/10 pain and more of discomfort due to clog and bloody nose. I had to try nasal spray and netipot with saline water last few years, i got 2 different ENT to check my polyps, And now i finally decided to go surgery on my 3rd ENT doctor referred by my sister (she is a nurse in the philippines) If your polyps really affect your quality of life, GO for it! Remove it! Like mine i cannot breath on my left nostril for about 5 yrs now, i have so much discharge like plem and mucus that i had to bring soft tshirt or a lot of tissue everywhere i go. I need to sleep sideview that causes me stiff neck all the time, i felt like there are always mucus on my nose it lessen my confident. I will update here again after i got my pack removed. Maybe about 3-4days more.

Does anyone know if you can have allergic fungal sinusitis without polyps? My ENT said my CT scan was clear but I feel like I'm having an incredible allergic reaction to something. The inflammation has been so bad, I thought I'd have to go to the ER at times. Nothing eases it, and I've tried everything. I'm also wondering if an allergy to prescription medication can present like this?

I hope I don't regret having it done this past week. Doc talked me into it after a cat scan showing a deviated septum but my only problem was a headache every 2 or 3 weeks. Only 3 or 4 sinus infections in 25 years or so. Time will tell. Recovering this week.

@emotionalfish1181 headaches just as often but not as severe. Who knows if a coincidence. No post op cat scan was done as I am curious as the visual difference but I'm not counting on one proactively by the surgeon.It doesn't seem much more opened on my left side as far as my sinus rinses. When I see my primary again, I may have ask the surgeon to send the records to my primary just for kicks.

I am trying to think I shouldn't have the surgery. I saw your other video of 10 Reasons Not To Have FESS. -> You said people with allergies should not get surgery without trying to figure out allergies and then probably immunotherapy. -> In this video, your number 9 is that people with Allergies and Polyps should have surgery. I'm quite confused here. BEcause I seem to have dust mite allergies.

I’m getting sinus surgery within the next month (hopefully) after being on 3 rounds of antibiotics and my CT still showing my upper sinuses in my forehead area are completely blocked and I have nasal polyps. They will also be treating my deviated septum at the same time.

Carrie Green I’m great!!! I had surgery in February and it was an easier process than I thought. The upper sinuses in my forehead were basically solidified and my surgeon said they haven’t seen something that bad. Now I don’t have near constant headaches and I can actually breathe! I recommend the surgery if needed. Recovery isn’t fun but afterwards life is so much better.

Emily Anderson great! I get surgery next week. I can’t smell or taste and I’m looking forward to that part the most. My sinuses are also blocked plus deviated septum and a bone spur

I had a CT scan done to confirm how bad my nasal polyps was and a deviated septum. Had surgery done last July2019 by November I was breathing only thru one side but it didn't bother me until 2 months ago.So Dr. tried to get me on a new drug called Dupixent but my medical denied it cause only one side is blocked. Now he says I need surgery, about 40% will develop polyps again. It took a month to recover. I'm looking into that new implanted stent to open it up.

I have constant sinus pressure in my forehead, cheeks and eyes. I always get sinus infections though. I’m trying to find an ENT because I’ve had this pressure for 5 weeks now. I was having drainage and sore throat, ear ache and sounded like a frog. I’m on my second round of antibiotics. Did the doctor examine you in the office too with that scope? I have a deviated septum also.

I'm over here, perfect turns out my doctor was wrong.... as I hit point #10 - CRAP that's exactly what I have wrong with me haha. Guess I'm not escaping surgery afterall. 5 year old root canal has an abscess pocket and a non stop sinus infection due to this. Guess I do need surgery, very nervous never had any surgeries in my life. Anyone have any positive experience with surgery you can share? Can't put a finger on why I'm so scared... just the whole process is overwhelming me. I need to handle the tooth absess pocket too, not sure which needs to be handled first. Thanks for making this video Doc, it was helpful and appreciated.

I was told I need sinus surgery but I’m terrified to get it. The only surgery I’ve had was to remove my wisdom teeth and that’s the reason I now have sinus issues. I will get a second opinion because I can’t go through this again smh mine was for the last reason but I had a cough for 3 weeks that was coming from a post nasal drip so I’m sure all the pus drained idk

I had sinus surgery 18 years ago and am having it again. I hate surgery. This one, I agreed to because the constant infection is irritating my POTS. Both times, I've had cysts.

I desperately need help. Not sleeping at all. My nose wakes me up every 2 hours. Mostly can't breathe, sometimes I can't smell or taste anything. I feel dizzy and overly irritated for lack of sleep and headaches when I blow my nose too much. It's really getting to me,😭. Need a good nose Dr to help. I want to breathe and sleep which I have t done well the last few months.

I was told that I had Ozena, a form of sinus problems, since I was 10 years old. I had 2 sinus surgeries 30 years ego, but the infection never went away completely but improved over time. I just used ocean water to rinse my nose every day for like 10 years, and after that i used just normal saline wash. The infection still here but far less severe than when I was yong and at least no smell, but in the last 3 years I had adult onset Asthma and start having a lot of allergies. so I decide to go for sinus surgery again since the surgery is different now and might give me some relief. Will have the surgery next month but not sure if it will make a difference.

That doesn’t happen to everyone. We appreciate you trying to educate people on a possible complication of sinus surgery, but you’re also not our doctor. I advise anyone to bring up these concerns with their actual ENTs rather than a well-intentioned commenter on KZfaq.

Dr. Terrell, have you seen an increase in mucoceles developing since the beginning of COVID? Nothing to do with COVID directly but I read that mucocele can develop after trauma. I've never had sinus issues before and with the covid tests being administered by "trained" ppl (highly debatable) shoving a foreign object up my left nose multiple times I now acquired a constant 24/7 headache that I've been living with for over a year now. I've had x-rays, an MRI and a CT scan and they show a mucocele in my left sinus... The nostril they shoved that stick up... Have you seen any correlation?

I had a CT scan done & my sinuses looked clear (apart from a mucosal cyst) yet I still have the symptoms of chronic sinusitus - sleeping is a nightmare as it flares up my symptoms quite badly after around 4 to 5 hours of lying down meaning I can never get a full nights sleep - nasal rinses & an over the counter steroid spray have helped reduce my symptoms a bit but even so my quality of life is still poor & has been for around 3 years now - this video seems to suggest I am not a candidate for surgery which is quite disappointing to be honest - personally I suspect opening up my ethmoids may well help as Im fairly sure the sinuses are getting shut off due to swelling at night when I lay down - now & again when lying down I feel my sinuses popping indicating they have been blocked

I had endoscopic sinus surgery 4 days ago. It was an extremely uncomfortable procedure. The 1st night was hell. My entire face was throbbing and all my top teeth were pounding. Then there's the nose packing. I cried like a little baby when the doctor pulled it out. After the packing was removed blood flowed like a river out of both nostrils. Now, my nose is so congested it makes me get extremely anxious. I haven't slept more than a few hours at a time since the procedure. I really hope it's worth it

Had my Septoplasty and Turbinate reduction done yesterday.. I have no pain and slight swelling. The worst part has been the stuffiness and cleaning up the discharge.(like you have with a sinus infection) I have the splints in, no discomfort or pain..Today I feel fine, still dripping a little.. For the best success and lowest risk of ENS/exterior membrane damage to the Turbinates have the microdebrider procedure done, the cauterize/radiowave methods have a history of issues (have a complete understanding of each method)...I don't regret having it done..Do your research on the Surgeon and the way they reduce the Turbinates and you'll have success ..:)

What about cocaine abuse that blocks the nose and lose sense of smell, looks white inside (dead tissue?), can’t breathe, throat and ear pain, bad odour inside nose?

Number one was what I had . I had surgery yesterday . I am oozing bright red blood still and wonder if it should be less blood by now. There are also tiny to small blood clots. I do have a compromised immune system. I don’t know if I should wait it out or throw in the towel since it’s only been 13 hours since the surgery. Hopefully by the time my doctor’s office reopens ( 8 hours) I will be more decisive. So there was little to no pain for the work he had to do . It was quite extensive. The sinus was packed solid with a fungus of some sort and pseudomonas infection he literally had to scrape it all out . I had no pain post surgery but now the left side of my face ( fascial bones ) hurt and I can feel my pulse in my face . So , I am going to go back to bed and see what happens during the night .

Aaron Brown Thank you Aaron. It was discovered after he ordered an MRI. He showed me the MRI and the difference in the right side of my face and the left side. The left side was so packed he actually had to pull and scrape it all out ( he told my sister while I was in recovery) it was just a little bit more than was expected. Today is day 4 postoperative and still have a small amount of bright red clots . I won’t know what type of fungus until I see him again on the 18th of this month.

Sharon Adams yes it did happen to me. The odor and the terrible taste at the back of my throat . So I am 3 months post surgery and we ( doctor) are just going through a list of options trying to figure out how to rectify the problem. The worse part is the terrible taste in my mouth . I practice good oral hygiene ( I always have) but the taste is always there ) . If we could just find where the pseudomonas is coming from .

My son had a root canal that the roots went so deep the dentist who did the root canal ended up breaking up into his sinus cavity. So for months after the root canal he had the worst taste in his mouth and all he would smell is puss in his sinuses. We ended up getting an MRI of his head and there was a massive infection from the root canal that came in contact with his sinus cavity. The ENT had to go in and scrape the hell out of the entire sinus cavity. The infection was really bad the doctor said. I felt so bad for my son the dentist that did the root canal didn't believe my son was still having pain 3 months after the procedure. But I kept taking him to doctor after doctor till an ENT finally found the issue.

I check at least 3 of the 10 reasons. I will have to come to US to from Canada to get this surgery. The wait time here is a joke if your disease is not life threatening.

I've had chronic left ear pain and dull ache behind left eye with post nasal drip/cough for a few years. Because I didn't have a fever/ear infection I was diagnosed with eustachian tube dysfunction and didn't go to an ENT because I thought it was no big deal. Last week I had severe ear pain (when I coughed), unusual headaches from top to base of skull, neckache and vision is a little blurry. Googled symptoms and I might have sphenoid sinusitis. Antibiotics aren't resolving it and am worried I might need this surgery. I'm afraid to go to ER because of Covid19. Ugggh.... I really hope you get better soon. I Will pray for you💖

...another thing that can cause Chronic Sinusitis is Hypothyroid also known as Low Thyroid.Hypothyroid lowers the body's temperature making areas like the Sinuses more susceptible to the growth of various Infectious agents!

@@tlc6756 I have Empty Nose Syndrome, which means I cant feel air when I breathe. You need to try everything else before surgery. You can't imagine what hell is unless you have this condition. Look at your diet, its probably causing your problems.

You're right about my diet, I eat way to much sugar and because the antibiotics the doc put me on aren't doing much, I think it could be fungal. My symptoms match sphenoid sinusitis so I may need a CT and surgery to avoid something serious. I'm sorry you are dealing with this. When you say you can't take in air when you breath, what exactly do you mean? I mean is it painful or just a really abnormal feeling that you can maybe get used to over time?

I had an asthma attack after taking darvocet I sneezed and it threw me into such a severe attack. My heart stopped and I ended up on the ventilator.
